Abstract:The skin of preterm infants is a delicate organ with critical structural and functional differences as compared to term born infants. Unique features contribute to an increased susceptibility to injury, infection, thermal instability, and water loss. During rapid, often accelerated adaption of the physical barrier function of preterm skin, a parallel and mutual development of host skin immunity and skin microbiome seem to be crucial for skin homeostasis. Recent advances in molecular biology have enabled researchers to gain a deeper understanding of the microbial community composition of preterm skin and the important relationship with microbiome composition of other body sites. Nevertheless, several questions remain to be answered, including niche factors and environmental influences on skin maturation. In line with that, evidence-based guidelines on skin care practice in preterm infants are missing. This review articles aims to provide an overview of the current knowledge of preterm infant skin development including immune and barrier function, host-microbial interactions, and potential clinical implications. Abstract:Atopic dermatitis(AD) is a common chronic inflammatory skin disease.It has complex etiological factors which associated with the interaction between patients and environment.Skin barrier is a mechanic and immune barrier,which plays key roles in preventing penetration of microbial products and allergens,as well as in immune surveillance and homeostasis.The pathogenesis of AD concerns the destruction of the structural integrity and defects of the functions of the skin barrier.This article provides an overview of the changes of components which maintain the functions of skin barrier in AD.
Application Research of Community Voluntary Nursing Service in Puerperal Women

Abstract:Objective To explore application effects of community voluntary nursing service in puerperal women. Methods A total of 128 natural labored puerperal women were selected as experimental group ( routine nursing education and voluntary nursing service) from January to June 2014, and another 128 natural labored puerperal women were selected as control group (routine nursing education) from January to June 2013. Eight weeks after interventions, we compared the puerperal mastery of nursing skills and nursing satisfaction, and incidence of complications. Results In the experimental group, puerperal women had higher mastery rate than that of the control group in nursing skills such as perineal care, feeding baby skills, postpartum setting-up exercises, copping with cracked nipple, bath, diaper change, and umbilical nursing (P<0. 05). The nursing satisfaction was 100. 0% in the experimental group compared with 89. 8% in the control group (χ2 =15. 236, P<0. 01). The incidence of complications in the experimental group was lower than that in the control group (P<0.01). Conclusions Community voluntary nursing service can enhance nursing skills of puerperal women, reduce the incidence of complications and enhance nursing satisfaction of puerperal women.

Abstract:Background: Inadequate skin care may increase morbidity in preterm infants. Skin care practices that support skin maturation have barely been investigated. Objectives: To investigate the effect of sunflower seed oil (SSO) on skin barrier development in low-birth-weight premature infants. Methods: 22 preterm infants (<48 h after birth, 1,500-2,500 g) were randomized into group C (control) and group SSO, receiving daily SSO application during the first 10 postnatal days, followed by no intervention. Transepidermal water loss (TEWL), stratum corneum hydration (SCH), skin pH and sebum were measured <48 h after birth and on postnatal days 5, 11 and 21 on the forehead, abdomen, thigh and buttock. Results: Skin pH decreased, while sebum remained stable in both groups. In group C, TEWL remained stable; in group SSO, TEWL increased significantly on the abdomen, leg and buttock until day 11, followed by a decrease after SSO application had been stopped. Abdomen SCH remained stable in group C, but continuously decreased in group SSO until day 21. Conclusion: SSO application may retard postnatal skin barrier maturation in preterm infants.
Essential components of an educational program for implementing skin-to-skin contact for preterm infants in intensive care units: an integrative literature review

Abstract:Background: Globally, prematurity is the primary factor behind the mortality of children under the age of 5 years, resulting in approximately 1 million children dying annually. The World Health Organization (WHO) recommends Skin-to-Skin Contact (SSC) as part of routine care for preterm infants. Evidence shows that SSC reduces mortality, possibly by improving thermoregulation, facilitating the earlier initiation of breastfeeding and reducing the risk of nosocomial infection. An educational program for implementing SSC has been demonstrated to enhance the knowledge and practice of parents and nurses in intensive care units. This study, the first of its kind in the North West Province (NWP), aims to identify the essential components of an educational program for implementing SSC for premature infants in intensive care units. Objective: This paper presents an integrative literature review that critically synthesizes research-based literature on essential components of an educational program for implementing SSC for preterm infants in intensive care units. Methods: A comprehensive search of electronic databases, such as CINAHL, MEDLINE, PsycINFO, ProQuest and Health Source: Nursing/Academic Edition and Health Source-Consumer Edition, was conducted using different keywords and references lists from the bibliography. Results: Twelve articles relevant to this review were identified, read and synthesized to answer the research question. Three essential components emerged from the findings of this review, namely (1) the necessity of policy and role players for implementing SSC, (2) the availability of education and training, and (3) counseling and support for parents of preterm infants. Conclusions: The outcomes of this study have the potential to facilitate the implementation and expansion of SSC in intensive care units. This could aid program implementers, policymakers, and researchers to implement and scale up this important tool in intensive care units.

Abstract:Various skin defects resulting from all kinds of acute and chronic injury factors, such as mechanical injuries, burns, chronic ulcer and body tumor resection that is very common.Compared to the traditional methods, the advent of skin tissue engineering provides an ideal and promising way for the repair of these skin defects.In the past decades, several kinds of products have been developed and presented good clinical effects.However, some basic science problems emerged and blocked the traditional process of some tissue-engineered skin products.This paper aims to present the critical problems of skin tissue engineering, offering some thought of the related researchers and readers.

Abstract:Objectives: this study aims to explore neonatal nurses' experiences of facilitating early bonding between mothers and premature babies. Method: a descriptive qualitative approach was adopted, using focus group discussions with 13 participants from four referral hospitals in a major city in a developing country. This was followed by in-depth interviews with three participants. Results: three main themes emerged, highlighting the barriers and facilitators to early bonding between mothers and preterm infants. The study also explored care practices aimed at facilitating early bonding within the social environment of preterm infants, involving mothers, families, nurses and hospitals. Conclusions: the barriers and facilitators identified inform the development of intervention strategies for neonatal nurses to promote early bonding. Optimal early bonding requires enhanced nurse management skills, adequate hospital infrastructure and involvement of the patient's family. These findings contribute to the advancement of health and nursing knowledge.

Abstract:The multiple protective functions of the skin derive from the interactions between epithelial skin and immune cells as well as the commensal microbiota. Developed in the last trimester of intrauterine life, the skin barrier adapts dynamically after birth. Specific differences in the structure and physiology have been disclosed between infant and adult skin. The stratum corneum of infants is thinner and structured by thicker corneocytes with a more anisotropic surface in comparison to adult skin. Lower levels of the natural moisturizing factor and its constituents, together with the increased protease activity in the epidermis result in dry baby skin and ongoing adaptation of the desquamation to the extrauterine environment. Infant epidermis is characterized by an accelerated proliferation rate and clinically competent permeability barrier in term neonates, despite the higher baseline values of transepidermal water loss in infants. The skin surface of newborns is less acidic, which could increase susceptibility to diaper and atopic dermatitis. Immediately after birth, skin is colonized by commensal bacteria – a process dependent on the mode of delivery and of major importance for the maturation of the immune system. Skin bacterial diversity and dysbiosis have been related to different pathology such as atopic and seborrheic dermatitis. This paper focuses on the ongoing structural, functional, and biochemical adaptation of the human skin barrier after birth. We discuss the interactions on the "skin barrier/ microbiota/ immune system" axis and their role in the development of competent functional integrity of the epidermal barrier.

Abstract:The provision of breast milk and breastfeeding is recommended for preterm infants. Multidisciplinary cooperation is required to promote breastfeeding. Nurses play important roles in breastfeeding preterm infants. First, nurses are strong advocates to help families be aware of the significance of breastfeeding. Second, nurses are educators providing technical and emotional support to pregnant and lying-in women. Third, nurses are coordinators in a multidisciplinary team. Nurses’ unawareness of the importance of breastfeeding, knowledge deficit, and unsupportive lactation policy are barriers to breastfeeding promotion for preterm infants. It is therefore suggested that hospital administrators provide appropriate breastfeeding policies, set up posts, and ensure nurses’ responsibilities in breastfeeding. Additionally, tailored training should be strengthened to improve nurses’ knowledge and skills and thereby enable them to fully exert their roles in the breastfeeding promotion of preterm infants.

Abstract:Objective To explore the effect of family-centered care on parents' ability caring for premature infants.Methods Totally 66 premature infants admitted from January to December 2015 were divided into a control group and an observation group chronologically,with 33 in each group.The control group received routine admission health education,hospitalization nursing and diacharge guidance,while the observation group additionally received family-centered care,which included the primary nurses guided infants' parents relevant nursing knowlege and skills when visiting their baby each week during hospitalization,and arranged parents to live in the family ward before discharge,then the parents learned and undertook the main daily nursing after the guidance and demostration of the primary nurses.Results The caring skill score of the parents in the observation group at discharge was significantly higher than that of the control group,and the incidence of diaper rash was significantly lower than that of the control group within the first month after discharge (P＜0.05 for both).Conclusion Parents' participation in the care of the premature infants during hospitalization,can help the parents achieve more caring knowledge and skills,improve their caring skills,and make sure they would be the primary caregivers of the premature infants.

Abstract:Journal of Human Lactation, Ahead of Print. Breastfeeding offers significant health benefits for both mothers and infants, particularly preterm infants, where it serves as a therapeutic strategy to reduce mortality and morbidities. However, breastfeeding practices are threatened globally by societal norms and systemic barriers at both micro and macro levels. This paper explores the complex interplay of these barriers and facilitators, focusing on the Neonatal Intensive Care Unit (NICU) setting. Preterm infants face specific feeding challenges due to immature physiological functions, yet evidence supports that strategies like cue-based feeding and individualized care can enhance feeding success and health outcomes. For mothers, initiating and maintaining lactation after preterm birth is critical but challenging, with early lactation support and achieving sufficient milk volume being key predictors of success. Partner support significantly influences lactation outcomes, although more inclusive research is needed for diverse family structures. In the NICU, a multidisciplinary approach to lactation care is vital, emphasizing the need for experienced staff and family-centered practices. NICU design, policies promoting parental presence, and adherence to Baby-friendly Hospital Initiative guidelines further support breastfeeding. This paper aims to provide directives for local breastfeeding policies through an integrated approach, considering societal attitudes and healthcare practices. The findings advocate for improved lactation support in NICUs, inclusive language and policies, and further research into diverse familial and gender roles in breastfeeding.

Abstract:Clinical jaundice is present in the majority of the newborns, while bilirubin encephalopathy is one of the most serious complications.Preterm birth is high risk factors of bilirubin encephalopathy.Premature infants may develop bilirubin-induced central nervous system(CNS)injury even at total serum bilirubin(TSB)levels conventionally considered to be non-neurotoxic, and less frequently show the typical abnormal neuromotor clinical signs.Preterm birth is one of the risk factors of bilirubin encephalopathy.The potential for bilirubin-induced neurotoxicity in the premature neonate remains a clinical concern, while efforts to improve current clinical assessment and early diagnosis are certainly needed.Recently research on the molecular and cellular mechanism of bilirubin neurotoxicity and the clinical manifestations of bilirubin-induced CNS injury in the preterm neonate has made great progress, and it has been found that laboratory index and neuroimaging techniques are closely associated with early diagnosis of bilirubin-induced neurotoxicity in the preterm neonate. Pilot study on effect of preventing skin tears with prevention protocol based on guideline

Abstract:Objective To explore the effect of preventing skin tears with prevention protocol based on guideline. Methods According to International Guideline of Prevention Skin Tears to discuss and make prevention protocol, including prediction risk people, prevention fall and fall from bed with auxiliary means, wearing long sleeves clothes, using emollient on skin twice a day, providing nutrients and water, prevention friction and shear force with movement skills, totally six comprehensive measures. Wound care team trained backbone nurses the measures and backbone nurses trained clinical nurses. Before and after intervention 5 month and 17 month, every patient with age more than 18 years and hospitalized more than 24 hours was checked skin from head to toes in order to observe and compare skin tears incidence and practicable rate of prevention measures by cross-sectional survey.Results Before intervention, skin tears incidence was 0.77%(12/1 558), and skin tears incidence after intervention 5 month and 17 month was 0.16%(3/1 777) and 0.22%(4/1 825) ( P<0.05) . Practicable rate of 6 measures in patients with risk and with skin tears after intervention 5 month and 17 month was increased than that before intervention which was unsatisfactory (P>0.05).Conclusions Preven-tion skin tears protocol can reduce skin tears incidence, but practicable rate need be improved. In future nurses training, guidance and management bedside should be strengthened in order to enhance practicable rate of prevention measures.
Changes in skin barrier over the first four days of life: a cross-sectional study

Abstract:Background: We aimed to evaluate the trajectory of skin barrier properties in full-term newborns during the first four days after birth. Methods: Based on the MKNFOAD cohort (NCT02889081), transepidermal water loss (TEWL), stratum corneum hydration (SCH), skin pH, and sebum content at five anatomical sites (cheek, forehead, volar forearm, abdomen, and dorsal lower leg) were examined once within 96 h after birth in 384 full-term infants. Multivariable linear regression analysis was performed to assess variations in these skin barrier parameters with age adjusted for gestational age, neonate's sex, parents' allergy history, delivery mode, amniotic fluid characteristics, and birth weight. The regression coefficient (ß) and 95% confidence interval were reported. Results: We analyzed a total of 384 neonates including 198 (51.6%) boys. TEWL values remained stable and showed no significant association with age (days). pH values exhibited a declining trend with age (p for trend <0.001). Both SCH values and sebum content grew with age (p for trend <0.001). Conclusion: During the first four days after birth, the skin TEWL remained stable, pH decreased, and the SCH and sebum content increased over time. These findings provide insights into the neonatal skin physiological development at the beginning of life. Impact: From birth to 96 h, TEWL was stable, pH showed a steep decline, SCH and sebum content increased. This study provides the first evidence of skin adaptation in the newborn due to changes in utero to after birth in the first 4 days of life in an Asian population. These findings will provide a new theoretical basis for neonatal skin physiology and clinical strategies for guiding newborn skin care.

Global Variation in Skin Injures and Skincare Practices in Extremely Preterm Infants

Abstract:Background Globally,are skincare practices and skin injuries in extremely preterm infants comparable?This study describes skin injuries,variation in skincare practices and investigates any association between them.Methods A web-based survey was conducted between February 2019 and August 2021.Quantifying skin injuries and describing skincare practices in extremely preterm infants were the main outcomes.The association between skin injuries and skincare practices was established using binary multivariable logistic regression adjusted for regions.Results Responses from 848 neonatal intensive care units,representing all geographic regions and income status groups were received.Diaper dermatitis(331/840,39％)and medical adhesive-related skin injuries(319/838,38％)were the most common injuries.Following a local skincare guideline reduced skin injuries[medical adhesive-related injuries:adjusted odds ratios(aOR)=0.63,95％confidence interval(CI)=0.45-0.88;perineal injuries:aOR=0.66,95％CI=0.45-0.96;local skin infections:OR=0.41,95％CI=0.26-0.65;chemical burns:OR=0.46,95％CI=0.26-0.83;thermal burns:OR=0.51,95％CI=0.27-0.96].Performing skin assessments at least every four hours reduced skin injuries(abrasion:aOR=0.48,95％CI=0.33-0.67;pressure:aOR=0.51,95％CI=0.34-0.78;diaper dermatitis:aOR=0.71,95％CI=0.51-0.99;perineal:aOR=0.52,95％CI=0.36-0.75).Regional and resource settings-based variations in skin injuries and skincare practices were observed.Conclusions Skin injuries were common in extremely preterm infants.Consistency in practice and improved surveillance appears to reduce the occurrence of these injuries.Better evidence regarding optimal practices is needed to reduce skin injuries and minimize practice variations.

Abstract:With the development and progress of medical technology, the survival rate of premature and low-birth-weight infants has increased, as has the incidence of a variety of neonatal diseases, such as hypoxic–ischemic encephalopathy, intraventricular hemorrhage, bronchopulmonary dysplasia, necrotizing enterocolitis, and retinopathy of prematurity. These diseases cause severe health conditions with poor prognoses, and existing control methods are ineffective for such diseases. Stem cells are a special type of cells with self-renewal and differentiation potential, and their mechanisms mainly include anti-inflammatory and anti-apoptotic properties, reducing oxidative stress, and boosting regeneration. Their paracrine effects can affect the microenvironment in which they survive, thereby affecting the biological characteristics of other cells. Due to their unique abilities, stem cells have been used in treating various diseases. Therefore, stem cell therapy may open up the possibility of treating such neonatal diseases. This review summarizes the research progress on stem cells and exosomes derived from stem cells in neonatal refractory diseases to provide new insights for most researchers and clinicians regarding future treatments. In addition, the current challenges and perspectives in stem cell therapy are discussed.
