Evaluation of Effect of Induction Chemotherapy Followed by Radiotherapy in Patients with Locoregionally Advanced Nasopharyngeal Carcinoma

Abstract:Purpose To re-evaluate the effect of induction chemotherapy (ICT) followed by radiotherapy (RT) for locoregionally advanced nasopharyngeal carcinoma (NPC) through long-term follow-up in a matched cohort study. Methods One hundred twenty-seven NPC patients stage Ⅲ and Ⅳa (Fuzhou staging 1992) were treated with ICT followed by definitive RT (ICT/RT group). This group was matched with a group of 127 patients treated with RT alone (RT group) in the same year. Matched characteristics were T stage,N stage,gender,age,and pathological type. Similar dose and technique of RT was administered to both groups. Cisplatin-based ICT was given for one cycle in 71 cases (56%),two cycles in 50 cases (39%) and three cycles in 6 cases (5%). Median follow-up time in alive patients was 83 mo (range 11mo~101mo). Results The 5-year overall survival (OS) was 64.6% and 52.2% in ICT/RT and RT group,respectively (P=0.045),and the 5-year distant metastasis-free survival was 86.7% and 71.7% respectively in the two groups (P=0.003). The benefit of OS in the ICT/RT group attribute to decreasing distant metastasis. No significant difference in relapse-free survival was seen in the two groups,which may be related to a proportion of 56% patients received one cycle of ICT. Conclusions The addition of cisplatin-based ICT to RT improves distant metastasis-free survival and overall survival in patients with locoregionally advanced NPC. It provides evidence for selection of this treatment modality for clinic.
