Abstract:Objective To explore the effect and evaluate the feasibility of new labor standards applied in clinical practice .Methods Retrospective observational study was conducted to analyze the clinical data of singleton full-term pregnant women in control group who were selected from January to December 2011 in the First Affiliated Hospital of Xi' an Jiaotong University (1527 cases including trial delivery in 547 cases with Friedman labor management) .And the study was also conducted among singleton full-term pregnant women in observation group who were selected from August 2014 to July 2015 (2642 cases including trial delivery in 1461 cases with new labor standards) .Two groups were compared in terms of delivery mode ,indications for cesarean section , trial-delivery results and maternal and neonatal outcomes .Results The cesarean delivery rate of the observation group was remarkably lower than that of the control group (χ2 = 180 .50 ,P < 0 .01) .The top six cesarean section indications of the control group were fetal distress ,abnormal labor stage ,pregnancy induced hypertension (PIH) ,scar uterus ,oligohydramnios ,and elder primipara in order .In the observation group the top six cesarean section indications were fetal distress ,scar uterus ,abnormal labor stage ,PIH ,breech presentation and oligohydramnios in turn .The rate of scar uterus in the observation group was significantly higher than that in the control group(χ2 = 77 .17 ,P < 0 .01) .For pregnant women undergoing vaginal trial-delivery ,the success rate in the observation group was significantly higher than that in the control group (χ2 = 39 .65 , P < 0 .01) ,and the rate of cesarean section because of abnormal first labor stage in the observation group was significant lower than that in the control group (χ2 =48 .63 ,P < 0 .05) .The rate of cesarean section because of abnormal second labor stage had no significant difference between two groups (χ2 = 1 .41 ,P > 0 .05) .For cases with ultimately successful spontaneous delivery ,the rate of postpartum hemorrhage had no significant difference between two groups (P > 0 .05) ,and the neonatal asphyxia rate of the observation group was lower than that of the control group (χ2 = 4 .70 ,P < 0 .05 ) .In the cesarean delivery cases who failed to vaginal trial-delivery , there was no significant difference between two groups in the rate of postpartum hemorrhage or in the rate of neonatal asphyxia (P > 0 .05) . Conclusion New labor standards applied to clinical guidance can promote vaginal delivery and reduce cesarean rate ,and the incidence of adverse maternal and fetal outcomes will not increase .
