Electrocardiographic changes in angiographically documented occlusion of the dominant left circumflex artery with acute inferior myocardial infarction

Abstract:Objective To analyze electrocardiographic changes in 19 cases angiographically documented occlusion of the dominant left circumflex artery in the setting of acute myocardial infarction(AMI). Methods Retrospectively analysis of electrocardiographic changes of AMI from Sep. 2000 to Dec. 2005. Twenty five cases were determined dominant left circumflex as infarct-related artery(IRA), among them 19 cases were admitted in 12 hours and 12 or 18 leads electrocardiography could be found. Results Two cases had Non-ST-segment elevation myocardial infarction, both of LCX blood flow restored to TIMI 3. Among 17 cases with inferior ST-segment elevation myocardial infarction(STEMI), 15 had posterior myocardial infarction 12 cases had ST elevation Ⅲ>Ⅱ and ST depression aVL>I, 4 of 19 had complete atrioventricular block(AVB) or high degree AVB. Two of 19 lead RV4 ST segment elevation. ST depression was seen in 14 cases of lead V4R and in 11 cases of lead aVR. Conclusions Electrocardiographic changes in AMI which occlusion of the dominant LCX were hard to differentiate from right coronary artery, inferior and posterior ST elevation and ST depression in lead RV4 and aVR may be its important feature.
