Implications of inferior ST-segment depression in patients with anterior acute myocardial infarction

Abstract:Objective The aim of this study is to assess the values of ST changes in the various electrocardiographic leads during anterior acute myocardial infarction(AMI)in predicting the location of the LAD coronary artery occlusion.Methods 87 patients with anterior AMI underwent selective coronary angiography within 1 month of hospitalization and were divided into two groups according to the culprit lesion:56 patients had a LAD artery lesion proximal to the first septal or diagonal branches (Group P);and in 31 it was distal to both the first septal and diagonal branches (Group D).The admission 12-lead electrocardiograms were evaluated for ST-segment deviation amplitude in each lead at 0 08 second after the J-point.Result ①The incidence of ST-segment elevation≥1 mm in lead I and aVL was significantly higher in Group P than Group D (I:50% vs 35%,aVL:61% vs 42%,P0 01);②The incidence of ST-segment depression≥1 mm in leads Ⅱ,Ⅲ,and aVF was significantly higher in Group P than Group D(Ⅱ:79% vs 29%,Ⅲ:86% vs 19%,aVF:75% vs 45%,P0 01);③Sensitivity and specificity of ST-segment depression in lead Ⅲ were greater than that of ST-segment elevation for predicting proximal LAD occlusion (86% vs 61%,81% vs 58%, P0 01) Conclusion Analyses of ST-segment depression in inferior leads,especially in lead Ⅲ,are useful for predicting proximal LAD occlusion in patients with anterior AMI.
