Abstract:Blood pressure is one of the basic physiological parameters of human physiology. Frequent and repeated measurement of blood pressure along with recording of environmental or other physiological parameters when measuring blood pressure may reveal important cardiovascular risk factors that can predict occurrence of cardiovascular events. Currently, wearable non-invasive blood pressure measurement technology has attracted much research attention. Several different technical routes have been proposed to solve the challenge between portability or continuity of measurement methods and medical level accuracy of measurement results. The accuracy of blood pressure measurement technology based on auscultation and oscillography has been clinically verified, while majority of other technical routes are being explored at laboratory or multi-center clinical demonstration stage. Normally, Blood pressure measurement based on oscillographic method outside the hospital can only be measured at intervals. There is a need to develop techniques for frequent and high-precision blood pressure measurement under natural conditions outside the hospital. In this paper, we discussed the current status of blood pressure measurement technology and development trends of blood pressure measurement technology in different scenarios. We focuses on the key technical challenges and the latest advances in the study of miniaturization devices based on oscillographic method at wrist and PTT related method at finger positions as well as technology processes. This study is of great significance to the application of high frequency blood pressure measurement technology.

Abstract:Hypertension is a common cause of cardiovascular diseases, closely associated with the high mortality and disability rates of cardiovascular diseases such as stroke and coronary heart disease. Therefore, developing a comfortable and sustainable device for monitoring human pulse signals holds practical significance for the prevention and treatment of hypertension and cardiovascular diseases. PVDF flexible pressure sensors possess the characteristics of high sensitivity, good flexibility, and strong biocompatibility, thereby demonstrating extensive application potential in areas such as health monitoring, wearable devices, and electronic skins. This paper focuses on the development of a modified piezoelectric polymer and its application in an intelligent blood pressure monitoring system, demonstrating its outstanding performance and feasibility through a series of experiments. This research provides innovative material choices for the development of intelligent medical devices and offers beneficial guidance for the design and application of future intelligent health monitoring systems.

Abstract:In this work, non-invasive continuous blood pressure sensors in a recent decade have been reviewed to motivate the development of health monitoring devices for future biomedical engineering applications. Firstly, the fundamentals in blood pressure measurement of arterial pulse wave analysis and pulse transit time (PTT) techniques are explained as the basis blood pressure demodulation . Secondly, a brief history of blood pressure instrumentation is provided which reveals the significant interest in this instrumentation. A validation protocol for non-invasive blood pressure instruments is, thirdly, summarized as a guideline for future innovation. This work, consequently, describes the principles and utilizations of electrocardiographic (ECG) electrodes, electronic sensors, fiber optic sensors, photoplethysmographic (PPG) sensors, and ultrasound systems in blood pressure measurement. Ultimately, the advantages, limitations, and prospects of each sensor are discussed which would eventually pave ways to the development of novel blood pressure measuring instruments for future practical applications.

An Alignment-Free Sensing Module for Noninvasive Radial Artery Blood Pressure Measurement

Abstract:Sensor–artery alignment has always been a significant problem in arterial tonometry devices and prevents their application to wearable continuous blood pressure (BP) monitoring. Traditional solutions are to use a complex servo system to search for the best measurement position or to use an inefficient pressure sensor array. In this study, a novel solid–liquid mixture pressure sensing module is proposed. A flexible film with unique liquid-filled structures greatly reduces the pulse measurement error caused by sensor misplacement. The ideal measuring location was defined as −2.5 to 2.5 mm from the center of the module and the pressure variation was within 5.4%, which is available in the real application. Even at a distance of ±4 mm from the module center, the pressure decays by 23.7%, and its dynamic waveform is maintained. In addition, the sensing module is also endowed with the capability of measuring the pulse wave transmit time as a complementary method for BP measuring. The capability of the developed alignment-free sensing module in BP measurement was been validated. Twenty subjects were selected for the BP measurement experiment, which followed IEEE standards. The experimental results showed that the mean error of SBP is −4.26 mmHg with a standard deviation of 7.0 mmHg, and the mean error of DBP is 2.98 mmHg with a standard deviation of 5.07 mmHg. The device is expected to provide a new solution for wearable continuous BP monitoring.

Abstract:With the implementation of the new policy for coronary stent centralized volume purchasing in China, the blood pressure sensor at the tip of the catheter, as one of the essential medical instruments for the diagnosis and treatment of coronary artery disease, will meet the new development opportunity of the industry, a number of medical device companies will actively participate in the development and registration of the catheter tip blood pressure sensor. As an invasive blood pressure sensor, the catheter tip blood pressure sensor should meet the current effective industry standard YY 0781-2010, however, there are many problems when using YY 0781-2010 as a blood pressure sensor because of the difference of product structure and working mode. In this paper, the problems about "Operation Manual", "electrical performance" and "safety requirement" in the course of carrying out YY 0781- 2010 with the blood pressure sensor on the tip of catheter are discussed and analyzed in detail, hope to provide some inspiration for more research and development enterprises of blood pressure sensors on the tip of catheters and inspectors of medical device testing institutions, also hope to be able to contribute to the high-quality development of blood pressure sensor industry at the tip of the Catheter.

Abstract:Hypertension is the leading cause of morbidity and mortality worldwide. Hypertension mostly accompanies no symptoms, and therefore blood pressure (BP) measurement is the only way for early recognition and timely treatment. Methods for BP measurement have a long history of development and improvement. Invasive method via arterial cannulation was first proven possible in the 1800's. Subsequent scientific progress led to the development of the auscultatory method, also known as Korotkoff' sound, and the oscillometric method, which enabled clinically available BP measurement. However, hypertension management status is still poor. Globally, less than half of adults are aware of their hypertension diagnosis, and only one-third of them being treated are under control. Novel methods are actively investigated thanks to technological advances such as sensors and machine learning in addition to the clinical needs for easier and more convenient BP measurement. Each method adopts different technologies with its own specific advantages and disadvantages. Promises of novel methods include comprehensive information on out-of-office BP capturing dynamic short-term and long-term fluctuations. However, there are still pitfalls such as the need for regular calibration since most novel methods capture relative BP changes rather than an absolute value. In addition, there is growing concern on their accuracy and precision as conventional validation protocols are inappropriate for cuffless continuous methods. In this article, we provide a comprehensive overview of the past and present of BP measurement methods. Novel and emerging technologies are also introduced with respect to their potential applications and limitations.

Abstract:Wearable cardiac monitoring devices can provide uninterrupted monitoring of cardiac activities over a long period of time. They have developed rapidly in recent years in terms of convenience, comfort, and intelligence. Aided by the development of sensor and materials technology, big data and artificial intelligence, wearable cardiac monitoring can become a crucial basis for novel medical models in the future. Herein, the basic concepts and representative devices of wearable cardiac monitoring are first introduced. Subsequently, its core technologies and the latest representative research progress in physiology signal sensing, signal quality enhancement, and signal reliability are systematically reviewed. Finally, an insight and outlook on the future development trends and challenges of wearable cardiac monitoring are discussed.

Abstract:This paper proposes a novel sensing system for noninvasive and continuous estimation of blood pressure (BP), which is based on a custom designed wearable 120 GHz Doppler radar sensor. With radar signals detected from brachial artery at the elbow, the moving trajectory of the pulses can be accurately demodulated with micrometer accuracy, and thus the reflective pulse transit time (RPTT) can be obtained for the prediction model to estimate blood pressure. Clinical trials have been launched with invasive blood pressure as reference. The experimental results show a great performance of the designed wearable radar sensor in the detection of weak pulse signatures. Compared to the ground truth of the invasive blood pressure, the estimated blood pressures by radar have mean errors below 3 mmHg for most subjects. The results reveal the possibility for the proposed radar-based BP estimation technique to be employed inside and outside the clinic, such as applications in the vast internet-of-things (loT) and in-home health monitoring.

Abstract:Non-invasive blood pressure (NIBP) assessment via constrained wearable devices aided by intelligent algorithms has led to tremendous advancements in healthcare. However, existing NIBP assessment methods generally impose hard constraints on human behavior, resulting in lower accuracy of blood pressure assessments. Therefore, for unconstrained ambulatory blood pressure monitoring, we propose a textile-based sensor with a multistage structure that acquires human ballistocardiogram (BCG) signals. Using nylon fabric as a substrate, a multistage structure consisting of sensitive micro-cilia and nylon supports ensures that the sensor maintains high sensitivity (0.533 V/kPa) even under static pressure (0–7 kPa) equal to the self-weight of the human body. The BCG waves acquired by the sensor have excellent agreement with the cardiac vibration wave obtained with laser vibrometry (displacement precision of 0.5 pm). Benefiting from the accurate acquisition of BCG, the time interval between each characteristic point of BCG waves is independently used as the characteristic parameter. A blood pressure assessment model via a back-propagation neural network is proposed to realize an unconstrained blood pressure assessment. For unconstrained measurements of diastolic pressure (DP) and systolic pressure (SP), the mean absolute error and standard deviation of error were 3.90 ± 4.79 mmHg and 4.62 ± 6.00 mmHg, respectively; this result meets the accuracy criteria of the American Association for the Advancement of Medical Instrumentation for Class II medical. We propose a new approach for unconstrained ambulatory blood pressure monitoring that shows great potential in home, hospital, and personalized medicine.

PTT Based Continuous Time Non-Invasive Blood Pressure System.

Abstract:This paper introduces the design and implementation of non-invasive, continuous time, cuff-less, blood pressure (BP) system based on pulse transmission time delay (PTT). The wearable BP device processing and monitoring both are performed using Smartphone. The PPG sensor and the integrated ECG sensor are wireless connected to smartphone. An adaptive threshold peak detection algorithm is proposed and tested to enhance the measurement accuracy. The proposed BP system achieves a systolic BP (SBP):- 1.36±7.51 mmHg and diastolic BP (DBP):- 2.44±3.49 mmHg. A new 4-LED PPG sensor is also introduced which can save 30% power consumption and reduce the interference of motion artifact (MA).

Abstract:Objective. To review the progress of research on photoplethysmography- (PPG-) based cuffless continuous blood pressure monitoring technologies and prospect the challenges that need to be addressed in the future. Methods. Using Web of Science and PubMed as search engines, the literature on cuffless continuous blood pressure studies using PPG signals in the recent five years were searched. Results. Based on the retrieved literature, this paper describes the available open datasets, commonly used signal preprocessing methods, and model evaluation criteria. Early researches employed multisite PPG signals to calculate pulse wave velocity or time and predicted blood pressure by a simple linear equation. Later, extensive researches were dedicated to mine the features of PPG signals related to blood pressure and regressed blood pressure by machine learning models. Most recently, many researches have emerged to experiment with complex deep learning models for blood pressure prediction with the raw PPG signal as input. Conclusion. This paper summarized the methods in the retrieved literature, provided insight into the artificial intelligence algorithms employed in the literature, and concluded with a discussion of the challenges and opportunities for the development of cuffless continuous blood pressure monitoring technologies.

Abstract:Hypertensive patients account for about 16% to 37% of the global population, and about 9.4 million people die each year from hypertension and its complications. Blood pressure is an important indicator for diagnosing hypertension. Currently, blood pressure measurement methods are mainly based on mercury sphygmomanometers in hospitals or electronic sphygmomanometers at home. However, people’s blood pressure changes with time, and using only the blood pressure value at the current moment to judge hypertension may cause misdiagnosis. Continuous blood pressure measurement can monitor sudden increases in blood pressure, and can also provide physicians with long-term continuous blood pressure changes as a diagnostic reference. In this article, we design an artificial intelligence-enhanced blood pressure monitoring wristband. The wristband’s sensors are based on piezoelectric nanogenerators, with a high signal-to-noise ratio of 29.7 dB. Through the transformer deep learning model, the wristband can predict blood pressure readings, and the loss value is lower than 4 mmHg. By wearing this blood pressure monitoring wristband, we realized three days of continuous blood pressure monitoring of the subjects. The blood pressure monitoring wristband is lightweight, has profound significance for the prevention and treatment of hypertension, and has wide application prospects in medical, military, aerospace and other fields.

Abstract:Cardiovascular disease is one of the leading causes of death worldwide. Long-term and real-time monitoring of cardiovascular indicators is required to detect abnormalities and conduct early intervention in time. To this end, the development of flexible wearable/implantable sensors for real-time monitoring of various vital signs has aroused extensive interest among researchers. Among the different kinds of sensors, mechanical sensors can reflect the direct information of pressure fluctuations in the cardiovascular system with the advantages of high sensitivity and suitable flexibility. Herein, we first introduce the recent advances of four kinds of mechanical sensors for cardiovascular system monitoring, based on capacitive, piezoresistive, piezoelectric, and triboelectric principles. Then, the physio-mechanical mechanisms in the cardiovascular system and their monitoring are described, including pulse wave, blood pressure, heart rhythm, endocardial pressure, etc. Finally, we emphasize the importance of real-time physiological monitoring in the treatment of cardiovascular disease and discuss its challenges in clinical translation.
Wearable Skin-Like Optoelectronic Systems with Suppression of Motion Artifacts for Cuff-Less Continuous Blood Pressure Monitor

Abstract:According to the statistics of the World Health Organization, an estimated 17.9 million people die from cardiovascular diseases each year, representing 31% of all global deaths. Continuous non-invasive arterial pressure (CNAP) is essential for the management of cardiovascular diseases. However, it is difficult to achieve long-term CNAP monitoring with the daily use of current devices due to irritation of the skin as well as the lack of motion artifacts suppression. Here, we report a high-performance skin-like optoelectronic system integrated with ultra-thin flexible circuits to monitor CNAP. We introduce a theoretical model via the virtual work principle for predicting the precise blood pressure and suppressing motion artifacts, and propose optical difference in the frequency domain for stable optical measurements in terms of skin-like devices. We compare the results with the blood pressure acquired by invasive (intra-arterial) blood pressure monitoring for >1500 min in total on 44 subjects in an intensive care unit. The maximum absolute errors of diastolic and systolic blood pressure were 7/+/- 10 mm Hg, respectively, in immobilized, and +/- 10/+/- 14 mm Hg, respectively, in walking scenarios. These strategies provide advanced blood pressure monitoring techniques, which would directly address an unmet clinical need or daily use for a highly vulnerable population.

Abstract:High blood pressure (BP) or hypertension is the single most crucial adjustable risk factor for cardiovascular diseases (CVDs) and monitoring the arterial blood pressure (ABP) is an efficient way to detect and control the prevalence of the cardiovascular health of patients. Therefore, monitoring the regulation of BP during patients' daily life plays a critical role in the ambulatory setting and the latest mobile health technology. In recent years, many studies have been conducted to explore the feasibility and performance of such techniques in the health care system. The ultimate aim of these studies is to find and develop an alternative to conventional BP monitoring by using cuff-less, easy-to-use, fast, and cost-effective devices for controlling and lowering the physical harm of CVDs to the human body. However, most of the current studies are at the prototype phase and face a range of issues and challenges to meet clinical standards. This review focuses on the description and analysis of the latest continuous and cuff-less methods along with their key challenges and barriers. Particularly, most advanced and standard technologies including pulse transit time (PTT), ultrasound, pulse arrival time (PAT), and machine learning are investigated. The accuracy, portability, and comfort of use of these technologies, and the ability to integrate to the wearable healthcare system are discussed. Finally, the future directions for further study are suggested.

Patent landscape review of non-invasive medical sensors for continuous monitoring of blood pressure and their validation in critical care practice

Abstract:Objectives: Continuous non-invasive monitoring of blood pressure is one of the main factors in ensuring the safety of the patient's condition in anesthesiology, intensive care, surgery, and other areas of medicine. The purpose of this work was to analyze the current patent situation and identify directions and trends in the application of non-invasive medical sensors for continuous blood pressure monitoring, with a focus on clinical experience in critical care and validation thereof. Materials and methods: The research results reflect data collected up to September 30, 2022. Patent databases, Google Scholar, the Lens database, Pubmed, Scopus databases were used to search for patent and clinical information. Results: An analysis of the patent landscape indicates a significant increase in interest in the development of non-invasive devices for continuous blood pressure monitoring and their implementation in medical practice, especially in the last 10 years. The key players in the intellectual property market are the following companies: Cnsystems Medizintechnik; Sotera Wireless INC; Tensys Medical INC; Healthstats Int Pte LTD; Edwards Lifesciences Corp, among others. Systematization of data from validation and clinical studies in critical care practice on patients with various pathological conditions and ages, including children and newborns, revealed that a number of non-invasive medical sensor technologies are quite accurate and comparable to the "gold standard" continuous invasive blood pressure monitoring. They are approved by the FDA for medical applications and certified according to ISO 81060-2, ISO 81060-3, and ISO/TS 81060-5. Unregistered and uncertified medical sensors require further clinical trials. Conclusion: Non-invasive medical sensors for continuous blood pressure monitoring do not replace, but complement, existing methods of regular blood pressure measurement, and it is expected to see more of these technologies broadly implemented in the practice in the near future.
Thin, soft, wearable system for continuous wireless monitoring of artery blood pressure

Abstract:Abstract Continuous monitoring of arterial blood pressure (BP) outside of a clinical setting is crucial for preventing and diagnosing hypertension related diseases. However, current continuous BP monitoring instruments suffer from either bulky systems or poor user-device interfacial performance, hampering their applications in continuous BP monitoring. Here, we report a thin, soft, miniaturized system (TSMS) that combines a conformal piezoelectric sensor array, an active pressure adaptation unit, a signal processing module, and an advanced machine learning method, to allow real wearable, continuous wireless monitoring of ambulatory artery BP. By optimizing the materials selection, control/sampling strategy, and system integration, the TSMS exhibits improved interfacial performance while maintaining Grade A level measurement accuracy. Initial trials on 87 volunteers and clinical tracking of two hypertension individuals prove the capability of the TSMS as a reliable BP measurement product, and its feasibility and practical usability in precise BP control and personalized diagnosis schemes development.
