Clinical evaluation of non-invasive central aortic pressure measurement and the effect of different antihypertensive drugs on central aortic pressure in hypertension

Abstract:Objectives(1)To evaluate the consistency and reproducibility of measuring CAP by non-invasive technique and invasive catheterization,preliminary analysis the relationship of CAP and brachial arterial pressure.(2)To investigate the influence of different antihypertensive drugs on central and brachial blood pressures in essential hypertension subjects. Methods(1)23 patients(14 males and 9 females) were recruited,aged 62.2±10.6 years.CAP were determined by invasive catheterization and non-invasion BPro~(TM)6000 arterial pulse wave analyzer,respectively,respectively,and brachial arterial pressure were determined by E-cuff blood pressure monitor.To compare the three valus of brachial arterial pressure,CAP by non-invasive technique and invasive catheterization.(2)45 patients with mild to moderate essential hypertension were respectively given amlodipine besylate tablets(23 cases)and metoprolol succinate sunstained-release tablets(22 cases).At least 4 weeks later,to measure cSBP, AI,heart rate by BPro~(TM)6000 and bSBP by E-cuff blood pressure monitor for the cases(40cases) whose blood pressure comply with a normal standards,and then compare them. Results(1) No significant difference was found between invasive cSBP and non-invasion cSBP[the difference was(1.6±3.7) mm Hg,P>0.05],with high correlation(r=0.973,P<0.01).(2)After antihypertensive therapy,bSBP was similar between the two group(P>0.05),in which amlodipine group was(128.7±9.5) mm Hg and metoprolol group was(127.7±10.6) mm Hg;cSBP of amlodipine group was(118.9±8.7) mm Hg and metoprolol group was(121.3±10.7) mm Hg.bSBP-cSBP in each group was significant(P<0.05). Conclusions(1) Non-invasive cSBP measurement is in good agreement with invasive cSBP,bSBP was greater than cSBP, so brachial blood pressure could not accurately reflect CAP. (2) Although the two antihypertensive drugs can have similar effects on brachial blood pressure,amlodipine decreases CAP and AI more significantly than metoprolol.
