Abstract:The unduly rapid growth of drug expenses in China at present exceeds the countrys economic development and patients endurance, turning into one of the central social issues. The paper gives a detailed analysis of the causes of the problem, which can be attributed both to the field of production and the process of consumption. It also puts forward appropriate countermeasures from the perspective of the structure of the drug industry, hospital management, drug prices, doctors supervision and the medical insurance system.

Abstract:At present, China’s 5.6% GDP for health care has not reached to the point that is beyond the bearing ability of the national economy. However, why the overly expensive health care expenditures have been complained by the public throughout the country? The paper thought that lacking of risk pooling of high medical expenditure, which resulted from the incomplete health insurance, brought forth the overly expensive health care. How to deal with the relationship between the market and the government, increase the government’s capital collecting during financing system reform in order to ensure stability and fairness, widen the cover of health insurance, enhance the risk pooling, and advance the medical expenditure control method should be a key during health insurance reform.

Abstract:Objective To investigate the factors influencing the expenses of the medical insurance inpatients so as to provide basis for the control of irrational medical expenditure.Methods The cases involving the first 10 systematic diseases in order of inpatients who admitted to a grade A tertiary hospital from 2003 to 2006 were enrolled this study.Their hospital expenditure were analyzed using the descriptive analysis and multi-factor analysis on affecting factors.Results The expenses of the health insurance inpatients were affected by various factors such as age,sex,hospitalized days,operation or not,complication or not,and so on.Conclusions In the context of the rational control of the controllable factors and construction of the health service networks based upon the community health care clinics,the elderly patients,the patients with chronic diseases or advanced tumors would benefit from the improved medical condition.In addition,it is helpful to shortened the average hospitalization days,make best use of the health resources,control medical expenditure,and lighten the patients' economic burden.

Abstract:With the development of income per capita,high personal medical shows the wider of disease risk,which is resulted from the increasing medical payment and imperfect risk pooling.The former that is related with the excess delivery of advance medical technology and drug has been a dominating reason since 1980.And the imperfect risk pooling means the imperfect social medical insurance,of which the government should take charge.However,influenced by the reform policies of the economic systems,similar to all the transitional economies reducing the government role in healthcare financing,China has put an increasing emphasis on the payroll tax-based social insurance system and out-pockets payment,moving away from a predominantly general revenue-based financing system under the old command economy.In a conclusion,the Chinese Government should make clear the financing function,and replace fee-for-service(FFS) payment according to the distorted pricing system with aggregated pre-payment for patients covered by social insurance to mitigate the incentive for cost escalation.Meanwhile,the target of the new medical reform that should resolve the problem of high personal medical expenditure is demanding and perfecting the social medical insurance with wider risk pooling.

Abstract:Objective To probe into the health expenditure and medical expenditure of Chinese people in rural and urban areas as objective references to improve the funding strategy of China's health care system. Methods Statistics annuals from 1999 to 2007 were called into play to analyze the status quo and trends of both expenditures. Results Definite proportion of funds raised for rural and urban health expenditure has grown by 200%;a distinct gap exists between the growth of per-capita medical expenditure in rural and urban residents (152. 86% in urban and 172. 77% in rural areas);medical expenditure maintains a steady growth along with rising per-capita income, yet the health consumption remains at a low level in general, especially among rural residents;since 2003, the income elasticity of the medical expenditure among rural residents keeps rising, narrowing the relative gap with urban residents. Conclusion It is recommended to push up the income of rural residents with greater resources, and to encourage the trend of marginal social medical expenditure. In addition, it is necessary to upgrade medical services, expand medical service coverage, and encourage reasonable consumption of medical services by both rural and urban residents;and to focus the role of cultural factors for greater awareness of the people in health investment and self-healthcare.

Abstract:Since the reform and the adoption of opening up policy, two extraordinary problems have appeared in the rapid development of China's health care: the medical cost has risen too fast; and doctors' integrity has been worsening. In the previous studies, the first issue was interpreted largely from the aspect of "cost-driven", while the latter one from the angle of "environment-induced". With the cases of six large state-owned general hospitals as our study cases, we have investigated the transformation of hospital organizational structure in the assignment of decision-making right, performance evaluation and stimulation policies, analyzed the effect of this change respectively from the views of hospitals, hospital staff, patients and the third party payers, and accounted for the two problems mentioned above from the aspect of change in the organizational formation of hospitals. Furthermore, this paper discusses the policy implications of the conclusion made in this article.

Abstract:In the past 26 years,China's THE grew fast,and the government consented would reach 5% of GDP.However,in the process of rapid soaring of health expenditure,of which the government should shoulder re- sponsibility,it is the consumers who bear the brunt,rather than the government,so it is reasonable that the public blame for overly expensive medical services.When we resolve these problems by regulating the responsibilities of the customer,the government and the society,we should pay more attention that the proportion of net revenue to gross revenue of the hospitals is about 1:5～7,so financing the hospitals must consider the concept of“net revenue”, which is very important to control the health expenditure.Without regard to this relationship,any dispute about finan- cing hospitals or customers makes no sense,as they all belong to the game among the departments.

Abstract:BackgroundThe increasing cost on healthcare exposes China’s healthcare budgets and system to financial crisis. To control the excessive growth of healthcare expenditure, China’s healthcare reforms emphasize the control of the global budget for healthcare, which leads to the release of relevant policy and a series of cost-control actions implemented by different hospitals. This work aims to identify the effects brought by the cost-control policy and actions via surveying and analysing feedback from clinicians.MethodsQuestionnaires on the cost-control policy and actions were designed for surveying 110 clinicians in hospitals from different regions of China. The data on the implementation of the cost-control actions and doctors’ feedback on these actions were analysed using descriptive statistics. Pearson’s chi-squared tests were performed to detect associations between doctors’ opinions and specific cost-control actions. A value of p < 0.05 was considered statistically significant. Association relationships between doctors’ opinions and cost-control actions were modelled into network models, and key factors were identified in a multi-variate framework. Last, we visualized our resultant data using a network model, and further multi-variate analysis was performed.ResultsThere were three main findings. (1) The cost-control policy has been widely implemented in the sampled hospitals in different regions of China, with more than 80% of those surveyed acknowledging that their hospitals take actions of reducing average prescription fees for outpatients, drug costs, and in-hospitalization durations. (2) Most doctors have a negative view of some cost-control actions; this is mainly due to concerns about the effects of these actions on the doctors’ own healthcare performance and patient satisfaction. (3) Cost-control actions that had a significant impact on doctors’ performance included limiting average prescription fees for outpatients and limiting the use of examinations/drugs/surgeries. Decreased patient satisfaction was associated with fewer admissions of critically ill patients, reduced use of brand-name drugs, and increased total costs to patients due to increased frequencies of visits to the hospitals.ConclusionsCost-control actions implemented in hospitals in response to the government’s policy to reduce its national healthcare budget affect both doctors and patients in several ways. Moreover, the cost-control policy and actions can be improved.

Main drivers of health expenditure growth in China: a decomposition analysis

Abstract:Background: In past two decades, health expenditure in China grew at a rate of 11.6% per year, which is much faster than the growth of the country's economy (9.9% per year). As cost containment is a key aspect of China's new health system reform agenda, this study aims to identify the main drivers of past growth so that cost containment policies are focussed in the right areas. Method: The analysis covered the period 1993-2012. To understand the drivers of past growth during this period, Das Gupta's decomposition method was used to decompose the changes in health expenditure by disease into five main components that include population growth, population ageing, disease prevalence rate, expenditure per case of disease, and excess health price inflation. Demographic data on population size and age-composition were obtained from the Department of Economic and Social Affairs of the United Nations. Age- and disease- specific expenditure and prevalence rates by age and disease were extracted from China's National Health Accounts studies and Global Burden of Disease 2013 studies of the Institute for Health Metrics and Evaluation, respectively. Results: Growth in health expenditure in China was mainly driven by a rapid increase in real expenditure per prevalent case, which contributed 8.4 percentage points of the 11.6% annual average growth. Excess health price inflation and population growth contributed 1.3 and 1.3% respectively. The effect of population ageing was relatively small, contributing 0.8% per year. However, reductions in disease prevalence rates reduced the growth rate by 0.3 percentage points. Conclusion: Future policy in optimising growth in health expenditure in China should address growth in expenditure per prevalent case. This is especially so for neoplasms, and for circulatory and respiratory disease. And a focus on effective interventions to reduce the prevalence of disease in the country will ensure that changing disease rates do not lead to a higher growth in future health expenditure; Measures should be taken to strengthen the capacity of health personnel in grass-roots facilities and to establish an effective referral system, so as to reduce the growth in expenditure per case of disease and to ensure that excess health price inflation does not grow out of control.

Abstract:To study the medical expenditure of insured patients with cancer and its influence factors, to analyze the utilization of pharmaceuticals, treatment-seeking behavior and the relationship between the medical expenditure and payment patterns of medical insurance. This study involved 5351 cancer cases in 3 municipalities (Beijing, Shanghai, Chongqing) and 5 province-capital cities (Shenyang, Fuzhou, Jinan,Zhengzhou, Xining).The actual claim data of their medical expenditure and medical care utilization in 2008 were collected. Descriptive analysis and multivariate linear regression analysis were applied. 1) Outpatient's annual medical expenditure per head was US$802 and inpatient's annual medical expenditure per head was US$6200. Although the basic medical insurance (BMI) fund covered about 70% of the expenditure, the patients' economic burden is still heavy. 2) Western medicine cost accounted for 44% to 60% of the inpatients' medical expenditure in various cities. Cytotoxic drug usage accounted for over 80% of the anti-tumor drugs. 3) The proportion of the patients who sought outpatient and inpatient treatment in the third level hospitals was 81% and 74 %, respectively. 4) Multivariate linear regression analysis showed that the main influence factors of inpatients' medical expenditure include medical insurance payment pattern, hospital level, category of cancer, gender, length of stay in hospital, and medical insurance type. When all factors other than payment pattern were set to be control parameters, inpatient's annual medical expenditure per head under flat rate payment was US$348 lower than that under fee-for service. Medical expenditure causes heavy burden for cancer patients and the BMI fund. Flat rate payment has proved to be more effective in expenditure control compared with fee-for service. To keep the fund safe and running effectively, it is necessary to adjust present payment patterns and set rational payment standard to encourage medical care providers to control the expenditure actively.

Abstract:Background In the context of rapid economic and social development, there has been a continuous intensification of population aging, transformation of disease patterns, and wide application of new medical technologies. As a result, health expenditures in various countries have sharply soared. How to utilize limited medical resources to maximize the improvement of health levels has become a hot and challenging issue related to the well-being of all humanity. The relevant indicators of total health expenditure play a crucial role in monitoring and evaluating the fairness of health financing and health security in the region. Objective This study explores the changes in the main expenses that constitute China’s total health expenditure and uses indicators related to health expenditure to observe the changes and future development trends of China’s health expenditure. Based on this, the utilization of China’s health expenditure is monitored to identify possible problems, and thereby targeted suggestions for promoting the development of China’s health and wellness cause are put forward. Methods Based on the comparison of previous literature, this paper analyzes the changes and future development trends in China’s health expenditure by using the relevant indicators of China’s health expenditure through the structural variation analysis method and the gray prediction model. Results The results show that the scale of government, social, and out-of-pocket health expenditures has continuously expanded, with social health expenditures becoming the main funding source for total health expenditures. The burden of medical expenditures on individuals has been further reduced. In the institutional method of total health expenditures, hospital expenditures account for about 60% of the total and are the main component. The expenditures of health administration and medical insurance management institutions are the main driving force behind the growth of total health expenditures. However, the proportion of health expenditures in China’s GDP is relatively low, so more investment is needed in the healthcare sector, and the burden of individual medical expenses also needs to be continuously reduced. Discussion In the future, China should further increase its investment in the medical and health sector. Specifically, the government should persist in investing in fundamental medical and health services. Simultaneously, efforts should be made to establish a scientific cost control mechanism for pharmaceuticals and broaden financing channels for healthcare, such as accelerating the development of commercial health insurance.

Abstract:Objectives This study aimed to quantify increases in the medical expenditures of public hospitals associated with changes in service use and prices, which could inform policy efforts to curb the future growth of hospital medical expenditures. Design Nationwide and provincial data regarding service volume, service price and intensity of public hospitals’ outpatient and inpatient care from 2008 to 2018 were extracted from the China Health Statistical Yearbooks, and population size data were obtained from the 2019 China Statistical Yearbook. Methods A decomposition analysis was performed to measure the relative effects of changes in service use (volume or its subcomponent factors) and service price and intensity on the increase in the inpatient and outpatient total medical expenditures of public hospitals from 2008 to 2018. Results After adjusting for price inflation, the total medical expenditure of public hospitals increased by approximately threefold from 2008 to 2018. During this period, the increase in service volume was associated with 67.4% of the observed increase in the total medical expenditures in the inpatient sector and 57.2% of the observed increase in the total medical expenditures in the outpatient sector. Most of the service volume effect is due to an increase in the hospital utilisation rate. The growth in the utilisation rate was associated with 73.7% of the observed growth in the total medical expenditures in the inpatient sector and 60.3% of the observed growth in the total medical expenditures in the outpatient sector. Conclusion Service use, rather than price, appears to be the major driver of increases in medical expenditures in Chinese hospitals. An important policy implication for China and other countries with similar drivers is that the effect of controlling price and intensity growth on containing medical costs could be limited and controlling service utilisation growth could be essential.

Abstract:Objective: Analysis on the composition of high hospitalization expenses and explores the influencing factors so as to put forward some specific measures and suggestions.Methods: Selected 811 inpatient cases with expensive costs whose total hospitalization expense of indi-vidual patients is over RMB100, 000 at a 3A hospital of Shanghai in 2014.Data is analyzed with SPSS 19.0.Results:Drugs and medical material costs are the highest in high hospitalization expenses.The influencing factors are including gender, medical payment, and classification of disea-ses, department, hospital days and curative effect.Conclusion: By lowering drugs and material costs, strengthen health education of focus groups, monitoring priority diseases and departments, reducing the average hospitalization days, improving medical standards, perfecting clinical pathways, exploring DRGs, the hospital can effectively control the high inpatients.
