The protective strategies of superior petrosal vein in trigeminal nerve microvascular decompression operation

Abstract:Objective To investigate the significance of superior petrosal vein protection in trigeminal neuralgia surgery,analyze the surgical classification,explore the protective strategies of superior petrosal vein in trigeminal nerve microvascular decompression and enhance surgical safety.Methods A retrospective study was performed to analyze the clinical data of 456cases with microvascular decompression in our hospital.In these cases,the superior petrosal vein were classified into four types by its number and its positional relationship with trigeminal nerve.Based on the surgical practice,the influence of superior petrosal vein on exposure and decompression process was evaluated.The security and difficulty of operation was analyzed.The protective strategies were summed up according to the different four types of superior petrosal vein.Results The microvascular decompression surgery for trigeminal neuralgia was a safe and effective method for trigeminal neuralgia.In 55%of cases,superior petrosal vein could cause difficulty to the exposure of tregeminal.By changing the opening range of CPA arachnoid and the optical project perimete of microscope,in most cases the full trigeminal nerve roots could be exposed satisfactorily.In the cases of early stage,21patients got superior petrosal vein cut off,2patients got cerebellar infarction and 1patient died.Conclusion The protection of superior petrosal vein is significant in trigeminal nerve microvascular decompression operation.To protect superior petrosal vein will observably reduce the incidence of postoperative complications.
