Expression and clinical significance of sCD40 in patients with B-cell malignancies

Abstract:Objective To assess the serum sCD40 level and its significance in patients with B-cell malignancies. Methods Seventy-nine patients of B-cell malignancies were enrolled,including 23 cases of B-cell acute lymphocytic leukemia(B-ALL),24 cases of multiple myeloma(MM),and 32 cases of B-cell non-Hodgkin's lymphoma(B-NHL).Flow cytometry was used to detect surface CD40 expression in tumor B-cells.Level of serum sCD40 expression was assessed by enzyme-linked immunosorbent assay(ELISA) before and after chemotherapy,and compared with 30 cases of normal control.Results Mature B cells percentage,expression of CD40 in mature B cells,and fluorescence intensity of CD40 expression of mature B cells had no significant difference between these three groups of patients(P> 0.05),however,significant differences(P<0.05) were found in plasma cells percentage,expression of CD40 in plasma cells,and fluorescence intensity of expression of CD40 plasma cells.In the latter two items,MM(plasma cells) was significantly higher than B-ALL(naive B cells).After treatment, serum sCD40 level was decreased significantly in B-ALL,MM,B-NHL patients with complete remission when compared with pre-treatment level(P<0.05),and was reduced to normal level in B-ALL and B-NHL patients.Serum sCD40 level in noncomplete remission(partial response + no remission) B-ALL,MM,B-NHL patients was not significantly different from the pre-treatment level(P>0.05),and was significantly higher than normal controls.Conclusions CD40 is expressed on surface of mature tumor B cells.CD40 expression could be detected on cell surface of naive B cells in B-ALL and MM.Serum sCD40 level could be the reference to reflect the treatment effect of B-ALL,B-NHL,MM.
