Abstract:The prevalence of allergic rhinitis and asthma increased over the last few decades.Current therapeutics is mainly confined to symptom controlling by pharmacological treatment, with only a small proportion of patients under the treatment of allergen-specific immunotherapy.Recently, based on the development of molecular biology, immunology, genetics and the related sciences, basic and clinical studies have been conducted successfully in new therapeutics of allergic airway diseases, including genetic recombinant vaccine, anti-IgE antibody, anti-cytokine, interference in gene expression of transcription factors, antisense therapy and RNAi, in order to explore novel methods and approaches for blocking the initiation of disease.This review summarized the research advances in new therapeutics for allergic rhinitis and asthma.

Abstract:Summary Asthma and allergic rhinitis frequently coexist in the same patient. There is a similarity and variation as well as potential relationship between asthma and allergic rhinitis. There is an increasing evidence to suggest a major involvement of airway epithelial cells in the pathogenesis of asthma and allergic rhinitis. The present review describes the importance of the airway epithelial cell in the development of allergic airway diseases, its role as the primary airway defense against exposure of the airway and lung to inflammatory stimuli and antigens and as an important player through activation of epithelial Toll-like receptors (TLRs) to provide an important link between innate immunity and allergic disease. Additionally, airway epithelial cells can act as inflammatory promoters capable of directing dendritic cells (DCs) towards a T helper 2 (Th2) response, and as active producers of several inflammatory/anti-inflammatory mediators. It is hypothesized that airway epithelial cells may play as both inflammatory initiator and immuno-pathological feedback regulation between allergic rhinitis and asthma via release of systemic inflammatory mediators. Thus, airway epithelial cells may be valuable therapeutic targets for discovery and development of new drugs and/or new therapeutic strategies to treat asthma and allergic rhinitis.

Abstract:Alveolus is the basic unit of gas exchange with type Ⅰ and type Ⅱ alveolar epithelial cells covered on its surface.With the rapid development of the global industry,the quality of air and environment has been deteriorating gradually in recent years,leading to a series of lung diseases because of the damage of alveolar epithelial cells.Emerging reports suggest that alveolar epithelial cells play an important role in lung cancer formation and metastasis,pulmonary fibrosis and other diseases.This paper describes the progress of alveolar epithelial cells of type Ⅰ and type Ⅱ,including their structure and function,isolation and purification,plasticity and the role in pulmonary diseases,to provide a reference in the research and treatment of lung diseases.

Abstract:In the pathogenesis of bronchial asthma(asthma), different individuals have different phenotypes and endotypes.Individualized or precision therapy for different phenotypes and endotypes is a therapeutic trend at present and even in the future.Epithelial cytokines are important initiators and promoters of various subtypes of asthma, especially allergic asthma.A better understanding of the endotype of epithelial cytokines and how they affect the airway immune response may have important implications for individualized therapy of asthma in the future.We provide an overview of the characteristics, functions and current research progress in the treatment of asthma of three typical epithelial cytokines.

Abstract:<p>Bronchial asthma is a heterogeneous disease with complex pathological mechanisms representing different phenotypes, including severe asthma. The airway epithelium is a major site of complex pathological changes in severe asthma due, in part, to activation of inflammatory and immune mechanisms in response to noxious agents. Current imaging procedures are unable to accurately measure epithelial and airway remodeling. Damage of airway epithelial cells occurs is linked to specific phenotypes and endotypes which provides an opportunity for the identification of biomarkers reflecting epithelial, and airway, remodeling. Identification of patients with more severe epithelial disruption using biomarkers may also provide personalized therapeutic opportunities and/or markers of successful therapeutic intervention. Here, we review the evidence for ongoing epithelial cell dysregulation in the pathogenesis of asthma, the sentinel role of the airway epithelium and how understanding these molecular mechanisms provides the basis for the identification of candidate biomarkers for asthma prediction, prevention, diagnosis, treatment and monitoring.</p>

Abstract:Activation of the epidermal growth factor receptor (EGFR) pathway is involved in the pathogenesis of asthma. Although decades of intensive research have focused on the role of EGFR in asthma, the specific mechanisms and pathways of EGFR signaling remain unclear. Various reports have indicated that inhibition of EGFR improves the pathological features in asthma models. However, extending these experimental findings to clinical applications is difficult. Several measures can be adopted to promote clinical application of EGFR inhibitors. This review focuses on the role of EGFR in the pathogenesis of asthma and the development of a potentially novel therapeutic target for asthma.

Abstract:The idea has been popular for a long time that Th1/Th2 imbalance is the major cause of many diseases. However,the Th1/Th2 paradigm has encountered increasing challenge since the discovery of a novel subset of Th cells,Th17.Th17 cells secrete a series of cytokines(IL-17A ~ F,IL -21 and IL -22),which is quite different from those produced by Thl and Th2 cells.It is now generally accepted that Th17/IL-17A plays a pivotal role in autoimmune and host defense.Although first discovered in autoimmune diseases,emerging studies begin to explore the way in which Th17/IL-17A acts in chronic inflammatory airway diseases,such as asthma and chronic obstructive pulmanary disease.In this review,we will summarize the differentiation and function of Th17,and introduce the progress in the correlation between Th17/IL-17A and chronic inflammatory airway diseases.Further elucidating the mechanism of Th17/IL-17A-related pathophysiological changes will contribute to prevention and treatment of chronic inflammatory airway diseases.
Airway remodelling in asthma and the epithelium: on the edge of a new era

Abstract:Asthma is a chronic, heterogeneous disease of the airways, often characterised by structural changes known collectively as airway remodelling. In response to environmental insults, including pathogens, allergens and pollutants, the epithelium can initiate remodelling via an inflammatory cascade involving a variety of mediators that have downstream effects on both structural and immune cells. These mediators include the epithelial cytokines thymic stromal lymphopoietin, interleukin (IL)-33 and IL-25, which facilitate airway remodelling through cross-talk between epithelial cells and fibroblasts, and between mast cells and airway smooth muscle cells, as well as through signalling with immune cells such as macrophages. The epithelium can also initiate airway remodelling independently of inflammation in response to the mechanical stress present during bronchoconstriction. Furthermore, genetic and epigenetic alterations to epithelial components are believed to influence remodelling. Here, we review recent advances in our understanding of the roles of the epithelium and epithelial cytokines in driving airway remodelling, facilitated by developments in genetic sequencing and imaging techniques. We also explore how new and existing therapeutics that target the epithelium and epithelial cytokines could modify airway remodelling.

Abstract:Bronchial asthma is one of the main diseases endangering children's health and the number of patients with asthma is continuously growing these years.Airway inflammation and airway remodeling are the two main pathological characteristics,while the latter is an important factor effecting asthmatic curative effect.Currently glucocorticoid is the most widely used anti-asthma drugs and is the first choice for the prevention of airway remodeling.Early treatment with glucocorticoid and cysteinyl leukotriene receptor antagonist may help to prevent airway remodeling,but could not reverse airway remodeling that already formed.Early reveal and intervene of airway remodeling are the keys to prevention and treatment of asthma in children.

Abstract:Asthma is an inflammatory disease of the airway wall that leads to bronchial hyper-reactivity and airway obstruction, caused by inflammation, mucus hyper-production and airway wall remodelling. Central to pathogenesis, Th2 and Th17 lymphocytes of the adaptive immune system control many aspects of the disease by producing cytokines such as IL-4, IL-5, IL-13, and IL-17. In addition, many cells of the innate immune system such as mast cells, basophils, neutrophils, eosinophils, dendritic cells (DCs), and innate lymphoid cells (ILCs) play an important role in the initiation or maintenance of disease. Epithelial cells are ever more implicated in disease pathogenesis, as they are able to sense exposure to pathogens via pattern recognition receptors (PRRs) and can activate DCs. This review article will deal with the role of cytokines that are considered essential controllers of the inflammatory, immune and regenerative response to allergens, viruses and environmental pollutants. Emerging Th2 cytokines such as thymic stromal lymphopoietin, GM-CSF, IL-1, IL-33, IL-25 mediate the crosstalk between epithelial cells, DCs, and ILCs. Understanding the crosstalk between structural cells, innate and adaptive immune cells that is mediated by cytokines provides important mechanistic insights into how asthma develops and perpetuates itself. It could also provide the framework on which we will select new therapeutic strategies that prevent exacerbations and alter the natural course of the disease.

Abstract:Asthma is a phenotypically heterogeneous chronic disease of the airways . Studies have found that neutrophils are crucial to airway inflammation in acute asthma , persistent asthma , particularly in asthma of poor response to glucocorticoid treatment . The role of neutrophils in development of bronchial asthma is complex , as they can release a potent source of cytokines and inflammatory mediators participating in asthma . Differing from eosinophilic inflammatory asthma , neutrophilic inflammatory asthma is not depend on helper T ( Th)2 cells, but may be related to Th1 and Th17 cells.This review highlights the role of neutrophils in the development of asthma , and the treatment of neutrophilic asthma with biological agents and novel small molecules .

Abstract:Asthma is a very common respiratory condition with a worldwide prevalence predicted to increase. There are significant differences in airway epithelial responses in asthma that are of particular interest during exacerbations. Preventing exacerbations is a primary aim when treating asthma because they often necessitate unscheduled healthcare visits and hospitalizations and are a significant cause of morbidity and mortality. The most common cause of asthma exacerbations is a respiratory virus infection, of which the most likely type is rhinovirus infection. This article focuses on the role played by the epithelium in orchestrating the innate immune responses to respiratory virus infection. Recent studies show impaired bronchial epithelial cell innate antiviral immune responses, as well as augmentation of a pro-Th2 response characterized by the epithelial-derived cytokines IL-25 and IL-33, crucial in maintaining the Th2 cytokine response to virus infection in asthma. A better understanding of the mechanisms of these abnormal immune responses has the potential to lead to the development of novel therapeutic targets for virus-induced exacerbations. The aim of this article is to highlight current knowledge regarding the role of viruses and immune modulation in the asthmatic epithelium and to discuss exciting areas for future research and novel treatments.

Abstract:Human lungs enable efficient gas exchange, and form an interface with the environment which depends on mucosal immunity for protection against infectious agents. Tightly controlled interactions between structural and immune cells are required to maintain lung homeostasis. Here, we use single cell transcriptomics to chart the cellular landscape of upper and lower airways and lung parenchyma in health. We report location-dependent airway epithelial cell states, and a novel subset of tissue-resident memory T cells. In lower airways of asthma patients, mucous cell hyperplasia is shown to stem from a novel mucous ciliated cell state, as well as goblet cell hyperplasia. We report presence of pathogenic effector Th2 cells in asthma, and find evidence for type-2 cytokines in maintaining the altered epithelial cell states. Unbiased analysis of cell-cell interactions identify a shift from airway structural cell communication in health to a Th2-dominated interactome in asthma.

Abstract:The airway epithelium is now considered to be central to the orchestration of pulmonary inflammatory and immune responses, and is also key to tissue remodelling. It acts as the first barrier in the defence against a wide range of inhaled challenges, and is critically involved in the regulation of both innate and adaptive immune responses to these challenges. Recent progress in our understanding of the developmental regulation of this tissue, the differentiation pathways, recognition of pathogens and antimicrobial responses is now exploited to help understand how epithelial cell function and dysfunction contributes to the pathogenesis of a variety of inflammatory lung diseases. Herein, advances in our knowledge of the biology of airway epithelium, as well as its role and (dys)function in asthma, chronic obstructive pulmonary fibrosis and cystic fibrosis will be discussed.

Abstract:Type 2 inflammation is a complex immune response and primary mechanism for several common allergic diseases including allergic rhinitis, allergic asthma, atopic dermatitis, and chronic rhinosinusitis with nasal polyps. It is the predominant type of immune response against helminths to prevent their tissue infiltration and induce their expulsion. Recent studies suggest that epithelial barrier dysfunction contributes to the development of type 2 inflammation in asthma, which may partly explain the increasing prevalence of asthma in China and around the globe. The epithelial barrier hypothesis has recently been proposed and has received great interest from the scientific community. The development of leaky epithelial barriers leads to microbial dysbiosis and the translocation of bacteria to inter- and sub-epithelial areas and the development of epithelial tissue inflammation. Accordingly, preventing the impairment and promoting the restoration of a deteriorated airway epithelial barrier represents a promising strategy for the treatment of asthma. This review introduces the interaction between type 2 inflammation and the airway epithelial barrier in asthma, the structure and molecular composition of the airway epithelial barrier, and the assessment of epithelial barrier integrity. The role of airway epithelial barrier disruption in the pathogenesis of asthma will be discussed. In addition, the possible mechanisms underlying the airway epithelial barrier dysfunction induced by allergens and environmental pollutants, and current treatments to restore the airway epithelial barrier are reviewed.

Abstract:Bronchial asthma is an immunological disease that there are a variety of immune cells and inflammatory cytokines involved in the pathogenesis. In the initial and maintenance phase of the immune process,dendritic cells play a key role in allergen identification,uptake and presenting,CD4+ T helper cell differentiation and activation as well as airway hypersensitivity and immune tolerance and so on. The therapy of the disease by intervening the function of dendritic cells has become the hot issue for instructing the clinical medication.

Abstract:Objective. Dysregulation of epithelial-mesenchymal transition (EMT) in the airway epithelium is associated with airway remodeling and the progression of pulmonary fibrosis. Many treatments have been shown to inhibit airway remodeling and pulmonary fibrosis progression in asthma and chronic obstructive pulmonary disease (COPD) by regulating EMT and have few side effects. This review aimed to describe the development of airway remodeling through the EMT pathway, as well as the potential therapeutic targets in these pathways. Furthermore, this study aimed to review the current research on drugs to treat airway remodeling and their effects on the EMT pathway. Findings. The dysregulation of EMT was associated with airway remodeling in various respiratory diseases. The cytokines released during inflammation may induce EMT and subsequent airway remodeling. Various drugs, including herbal formulations, specific herbal compounds, cytokines, amino acid or protein inhibitors, microRNAs, and vitamins, may suppress airway remodeling by inhibiting EMT-related pathways.

Abstract:Airway hyperresponsiveness (AHR) is a key clinical feature of asthma. The presence of AHR in people with asthma provides the substrate for bronchoconstriction in response to numerous diverse stimuli, contributing to airflow limitation and symptoms including breathlessness, wheeze and chest tightness. Dysfunctional airway smooth muscle (ASM) significantly contributes to AHR and is displayed as increased sensitivity to direct pharmacological bronchoconstrictor stimuli, such as inhaled histamine and methacholine (direct AHR), or to endogenous mediators released by activated airway cells such as mast cells (indirect AHR). Research in in vivo human models has shown that the disrupted airway epithelium plays an important role in driving inflammation that mediates indirect AHR in asthma, through the release of cytokines such as TSLP and IL-33. These cytokines upregulate type 2 cytokines promoting airway eosinophilia and induce the release of bronchoconstrictor mediators from mast cells such as histamine, prostaglandin D<sub>2</sub> and cysteine leukotrienes. While bronchoconstriction is largely due to ASM constriction, airway structural changes termed 'remodelling', likely mediated in part by epithelial-derived mediators, also lead to airflow obstruction and may enhance AHR. In this review, we outline the current knowledge of the role of the airway epithelium in AHR in asthma and its implications on the wider disease. Increased understanding of airway epithelial biology may contribute to better treatment options, particularly in precision medicine.
