Abstract:As the material basis of clinical examination, in -vitro diagnostic reagent could directly affect the timeliness and accuracy of clinical examination report if it was not uses properly. This article discusses about how to strengthen the management for the temperature monitoring, effect management, inventory management and cost-benefit analysis of in-vitro diagnostic reagents. An information management system that provides methods for fine management of in-vitro diagnostic reagents has been developed.

Abstract:Clinical trials are an important method for evaluating the safety and efficacy of in vitro diagnostic reagents, and are a key basis for product registration review and approval. In order to strengthen the management of clinical trials of in vitro diagnostic reagents, the National Medical Products Administration and relevant departments have formulated a series of regulations at the regulatory level, and require applicants and clinical trial institutions to establish a quality management system for clinical trials of in vitro diagnostic reagents. Medical laboratory is the main department and implementer of in vitro diagnostic reagent clinical trials in medical institutions. In recent years, with the rapid development of the in vitro diagnostic industry, the clinical trial projects of in vitro diagnostic reagents conducted by medical laboratory have been increasing day by day. However, there are currently few discussions on the clinical trial of in vitro diagnostic reagents from the perspective of researchers. Therefore, this article summarizes the characteristics of clinical trials of in vitro diagnostic reagents, analyzes the problems and difficulties in conducting clinical trials of in vitro diagnostic reagents in current medical laboratories, and introduces the laboratory's experience in management; to provide reference for medical testing laboratories that have not yet conducted or have already conducted clinical trials of in vitro diagnostic reagents, in order to improve the quality and efficiency of clinical trials.

Abstract:With the development of medical examination technology, a real-time and efficient information management system of In-vitro Diagnostic Reagents (IVD Reagents) is urgently needed in the hospital. This article introduces the information management system of IVD Reagents in Huashan Hospital of Fudan University with the supply chain management mode from reagent ordering to the using. Abstract:As the intravenous drug supply department,the center of pharmacy intravenous admixture service(PIVAS) not only helps to avoid bacteria and insoluble particulate pollution during preparation in the general environment,but also promotes the rational clinical use of drugs by avoiding drug incompatibility.Centralized preparation and supply for intravenous drug has become an important measure to guarantee the rational use of drugs and to avoid infusion reactions for medical institutions.In our country,PIVAS is still not common.However,with the continuous establishment of hospital PIVAS,PIVAS management will become more standardized.In this paper,the PIVAS work and charging fees in the Third Xiangya Hospital and some other hospitals were discussed to demonstrate the domestic PIVAS development.

Abstract:Nanotechnology‐basedin vitro diagnostics (nanoIVDs) are widely studied for disease diagnostics with promising sensitivity, specificity, and convenience. However, it is still a major challenge for both regulatory authorities and scientific researchers to accelerate the clinical translation of such an innovative technology. Herein, this perspective discussed the benefits and challenges of nanoIVDs as well as the administration considerations on the distinguish features of nanoIVDs by regulatory authorities, so as to further achieve the evaluation, translation, and application of nanoIVD products.

Abstract:The European Directive 98/79/EC on in-vitro diagnostic medical devices (IVD) stipulates the marketing and post marketing surveillance of IVD in the European Economic Area. In cases of incidents and field corrective actions related to IVD, manufacturers have to inform the responsible Competent Authorities. In Germany, the Federal Institute for Drugs and Medical Devices (BfArM) is the responsible Competent Authority for most IVD. Only a small subset of IVD for immune hematological and infectiological testing, as well as tissue typing specified in Annex II of the Directive, is under the responsibility of the Paul-Ehrlich-Institute (PEI). Until the end of 2005 the BfArM received a total of 653 notifications regarding IVD. From these 48 related to IVD used for diagnostics in transfusion medicine and tissue typing which are subject of this study. The investigated products included reagents (tests, calibrators and controls) used for immune hematological analysis as far as they are not under the responsibility of the PEI, analyzing instruments used for immune hematological testing, liquid handling systems for sample preparation and a small subset of analysing instruments and reagents for tissue typing. Most of the reports originated from manufacturers (n=27, 56.3%) and Competent Authorities (n=20; 41.7%) whereas other sources played only minor roles. Product failures were confirmed by the investigations of the manufacturers in 36 (75%) of the reported incidents. Reagents were frequently subject of manufacturing errors and biological contaminations, whereas analysing instruments were typically affected by software errors. In consequence, corrective actions were performed in the large majority of cases (n=37; 77.1%). These included customer information, product recalls, changes in production or quality management and software upgrades (the latter predominantly in cases of failures of analysing instruments). Our data suggest that the governmental system for marketing surveillance is an established tool to ensure product safety for IVD used in transfusion medicine.

Abstract:A scientific supervision system enhances vigorous and standardized development of emerging things. Artificial intelligence-based software as a medical device (AI-Based SaMD) is an important product in the health field enabled by artificial intelligence (AI). As AI develops further, its unique black box algorithm and independent learning ability have posed major supervision challenges. AI-Based SaMD supervision needs to keep pace with the times and a more scientific supervision strategy is urgently required to minimize the adverse events of AI-Based SaMD and their risk impact. This article summarizes the current status of supervision systems and supporting resources of AI-Based SaMD in China and abroad considering the difficulties in algorithm change management, quality control, and safety traceability. In addition, we explore the problems and challenges of AI-Based SaMD in China. Furthermore, we suggest that the AI-Based SaMD supervision and support systems should be improved in China to overcome the disadvantages of post-market supervision.

Abstract:In vitro diagnostics (IVD) refers to in vitro tests of biological specimens and contributes to two‐thirds of clinical diagnosis. There are two major components to consider for the construction of devices for IVD, including: i) capture and recognition; and ii) detection methods. Herein, the principle, design, and application of microdevices regarding these two routes in recent progress for IVD are shown. Microarrays, microfluidics, and nanodevices are reviewed for capture and recognition. Mass spectrometry, fluorescence, electrochemistry, and so on are summarized as detection methods. The roles of materials in devices toward diverse applications are elucidated with the aims of designing materials for high‐performance device toward precision medicine and initiating the development of advanced diagnostic tools involving various biomarkers.

Abstract:In vitro diagnosis (IVD) is an important source of clinical diagnostic information, and provides an important decision basis for disease prevention, diagnosis and treatment. IVD is a necessary tool for promoting graded diagnosis and treatment, realizing precision medicine, constructing a "Healthy China" and responding to major public health emergencies. Combining the great progress made in the development of in vitro diagnostics in China and the shortcomings and weaknesses faced by it, this article analyzed the demand for IVD, policy support, technical and industrial development trends, and the ways to accelerate the industrialization development, aiming to promote the development and improvement of IVD in China.

Abstract:In vitro diagnosis (IVD) has become a hot topic in laboratory research and achievement transformation. However, due to the high cost, and time-consuming and complex operation of traditional technologies, some new technologies are being introduced into IVD, to solve the existing problems. As a result, IVD has begun to develop toward point-of-care testing (POCT), a subdivision field of IVD. The pandemic has made governments and health institutions realize the urgency of accelerating the development of POCT. Microfluidic paper-based analytical devices (μPADs), a low-cost, high-efficiency, and easy-to-operate detection platform, have played a significant role in advancing the development of IVD. μPADs are composed of paper as the core material, certain unique substances as reagents for processing the paper, and sensing devices, as auxiliary equipment. The published reviews on the same topic lack a comprehensive and systematic introduction to μPAD classification and research progress in IVD segmentation. In this paper, we first briefly introduce the origin of μPADs and their role in promoting IVD, in the introduction section. Then, processing and detection methods for μPADs are summarized, and the innovative achievements of μPADs in IVD are reviewed. Finally, we discuss and prospect the upgrade and improvement directions of μPADs, in terms of portability, sensitivity, and automation, to help researchers clarify the progress and overcome the difficulties in subsequent μPAD research.

Abstract:Objective To Statistically and retrospectively analyze in vitro diagnostic device testing from 2006 to 2009 in Beijing Institute of Medical Device Testing.Methods The changes from 2006 to 2009,such as that of the quantity of the tested devices,of the quantity of in vitro diagnostic device and the constituent ratio,were calculated and analyzed.Results The quantity of the tested devices was from 1719 to 2336,and the one of in vitro diagnostic device was from 128 to 166 in the four years.Conclusion The results and analysis can lay a foundation for the operation of Beijing Institute of Medical Device Testing.

Abstract:In vitro diagnostics (IVD) are essential in healthcare systems for detection of diseases, conditions, or infections. Affordable and sensitive methods remain a challenge in the development of IVD. In this article, we give an overview of paper-based fluorogenic devices, an emerging cost-effective analytical method for IVD applications. The paper-based devices are inexpensive, sensitive, selective, user-friendly and equipment-free. Furthermore, the fluorescent detection on paper-based devices has recently attracted enormous attentions due to its high sensitivity and selectivity. We summarize and compare various fluorescent materials that are used in the paper-based fluorogenic devices, including fluorescent dyes, quantum dots, metal nanoclusters, upconversion nanoparticles, and carbon dots. We review a wide range of IVD applications of the paper-based fluorogenic devices, e.g., detection of nucleic acids, proteins, cells, and so on. It may open an avenue to improve the global healthcare systems in the developing countries at point of care settings and in emergency situations.

TrAC Trends in Analytical Chemistry

Abstract:ELISA technology has received great attention in in vitro diagnostics (IVD) due to its high specificity, sensitivity and high throughput. However, the traditional ELISA technology is tedious, lacks of multiplexing detection capabilities, and is not easy to implement in point-of-care (POC) settings. The emergence and development of magnetic bead technology, paper-based platform and nanomaterials have attracted researchers in various fields to improve and design new ELISA derived technologies. Recently, ELISA has been creatively used to integrate with these analytical techniques to realize high sensitivity, specificity, repeatability, low cost and multiplexing detection. However, the applications of various ELISA derived technologies in IVD are rarely reviewed, where is the thrust of this review. In this review, we first introduced the history and principle related to ELISA technology, then we classify and introduce various ELISA derived technologies with an emphasis, and finally we discuss the current challenges and prospects of ELISA technology.

Abstract:This paper is a description of the ICSH guidance for internal quality control (IQC) policy for blood cell counters. It follows from and links to a separate ICSH review for such policies and practices. The ICSH has gathered information regarding the current state of practice through review of published guidance from regulatory bodies, a questionnaire to six major cell counter manufacturers and a survey issued to 191 diagnostic laboratories in four countries (China, the Republic of Ireland, Spain, and the United Kingdom) on their IQC practice and approach to the use of commercial IQC materials. This has revealed diversity both in guidance and in practice around the world. There is diversity in guidance from regulatory organizations in regard to IQC methods each recommends, clinical levels to use and frequency to run commercial controls, and finally recommended sources of commercial control materials. The diversity in practice among clinical laboratories spans the areas of IQC methods used, derivation of target values, and action limits used with commercial control materials, and frequency of running commercial controls materials. These findings and their implications for IQC Practice are addressed in this guidance document, which proposes a harmonized approach to address the issues faced by diagnostic laboratories.

From these 115 related to IVD for analysis of the infection status (reagents, control materials, calibrators, culture media and analyzing equipment). Most of the reports originated from manufacturers (57.4 %), while other sources of reports played only minor roles. Product failures of test reagents, control materials, calibrators as well as culture media were frequently caused by manufacturing errors and biological contamination. Analyzing equipment was typically affected by software malfunction. Through the investigations of the manufacturers product failures were confirmed in most cases and consequently corrective actions were performed in the large majority of incidents. The corrective actions frequently included customer information, product recalls, changes in the production process and/or the quality management or software upgrades for the analyzing equipment. Our data suggest that the existing system for post marketing surveillance is an important tool to ensure product safety of IVD even though it should be further optimized in the future.
