EVALUATING THE STUNNING MYOCARDIUM AND VIABLE MYOCARDIUM IN INTRACORONARY DELIVERY OF BONE MARROW STEM CELLS IN DOGS BY PET/CT

Abstract:Objective:To evaluate stunning myocardium and viable myocardium in intracoronary delivery of bone marrow stem cells in dogs by PET/CT myocardial perfusion imaging(MPI) and myocardial metabolism imaging(MMI).Methods:20 dogs were divided into opration group and control group,15 dogs were in opration group,5 dogs were in control group.Opration group was divided into A,B,C groups according time of blocking the blood.Acute myocardial infarction(AMI) models were made by ligation of lower 1/3 of left anterior descending branch of coronary artery(LAD) in dogs,and released the branch after 30min、60min and 90min,followed by electrocardiogram(ECG),blood biochemical examination,MPI,MMI,pathology and immunohistochemical analysis respectively to evaluate AMI model and stunning myocardium in opration group.Bone marrow stem cells were grafted into each dog of opration group heart's AMI area through intracoronary way.MPI and MMI were performed in all dogs of A,B,C group in 6 weeks and 10 weeks after graft.The blood stream was not blocked in control group,but observation of ECG,blood biochemical examination,pathology and immunohistochemical analysis were performed in all dogs.Results:1.One dog died during surgery in B group.The other 14 dogs in the ECG after ligation 24～48 hours,the myocardium zymogram of vein blood serum after ligation 9～12 hours,and MPI after ligation 2 days,they all accorded with the change of AMI.Result of ECG,blood biochemical examination and MPI were normal in control group.2.Viable myocardial segments of segmental myocardial perfusion defects were found in MPI/ MMI in A,B and C groups with 11/12(92%),8/11(73%)and 5/15(33%) respectively 2～3 days after opration.There are different degree of available myocardium in region of ischemic and infarct by electron microscope.3.Changes in MPI/ MMI were found between after 5～7 days and 10 weeks in segmental viable myocardium in surgery group(P<0.01);however,no changes in MPI/MMI were found between after 5～7 days and 6 weeks in segmental no viable myocardium.No changes in MPI/MMI were found between after 6 weeks and 10 weeks in all segmental myocardial perfusion defects(P>0.05).And no changes in MPI/MMI were found among after 5～7 days,6 weeks and 10 weeks in control group.Conclusion:MPI/MMI are available method to evaluate AMI model.MPI/MMI not only can confirm part of infarction,area and degree,but also detect stunning myocardium after myocardial infarction and reflect metabolic change after transplantation.
