Health service demand and utilization of peasants who lose territory in the course of urbanization

Abstract:Aim: To investigate the status of health demand of peasants lost territory and the health manpower in the country. Methods: A total of 3 300 peasants lost territory aged 15 years and above in the Luji Village, Pantang Countryside of Xuzhou South-city exploitation District were selected with representative sampling; meanwhile, the condition of township health centers and village clinics of Xuzhou were investigated from 1998 to 2004. The data were from the Year Book of Health of Xuzhou. Self-made questionnaire was adopted for investigation by in-door interview, including general condition (age, sex, occupation, educational degree and so on), self-assessment of health (self-consideration of health, health condition compared with that of the last year, knowing having some chronic disease or not), experience of health examination within recently two years, hospitalization within one year, treatment in two weeks, wish of receiving medical treatment, health service demand, simple physical examination and so on. The standard for self-assessment of health: better than the condition of the last year was regarded as excellent, worst than that as bad. The information about health manpower, bed, amount of clinical service and hospitalization, average days of hospitalization, bed usage rate, bed turnover rate in the township health centers and village clinics were collected and statistically analyzed. Results: Among 3 300 sent out questionnaires, 3 134 qualified ones were retrieved with the effective rate of 94.97%. (1) 209 (6.67%) farmers lost territory thought their health status was bad, 211 persons (6.73%) thought health status was worse than the last year and 616 persons (19.66%) clearly knew having chronic disease. (2) Among interviewees, 259 persons (8.26%) had done the health examination within two years. (3) Twenty-five persons (0.80%) had experience of hospitalization; There were 24 persons (7.66%), who should be in hospital but not including 23 ones because of the high cost and 1 person because of the inconvenient traffic. (4) There were 11 persons (0.35%) seeing the doctor in 2 weeks including 7 persons having received medical treatment and 4 persons not received medical treatment for expensive drug cost. (5) Half of interviewees had the health service demand for health cultivation , other kinds health service demand was less than 10% (6) More than 50% of the farmers lost territory thought drug and diagnosis and treatment cost was too expensive. (7) 113 small town health centers were investigated, amount of health manpower was 12 042 and amount of per day per capita diagnosis and treatment was 1.7 person times. There were 3 211 village health rooms and amount of health manpower was 3 211 and amount of per day per capita diagnosis and treatment was 4,6 person times. Conclsion: Health service demand and utilization among peasants lost territory show "three high and five low": Drug price is very high, cost of diagnosis and treatment is very expensive and demand of health examination is high; Rate of understanding chronic disease is low, rate of visiting the doctor is low, rate of receiving medical treatment is low, hospitalization rate is low and healthy life style is low. The medical support for peasants lost territory should be strengthened from the medical service ensure system to decrease the phenomenon of lacking of health resource and health service. utilization and illogicality construction of health manpower.
