We agree with the importance of delivering tumoricidal drug exposure (concentration x time) to both the tumor and adjacent brain tissue to test the concept of the “sink effect.” However, we believe that the drug exposure gradient should be greatest within the tumor and least in brain histologically devoid of tumor (i.e., normal cortex). Based on our ‘*C-methotrexate data (cited in our article), barrier modification with a 2.0-mold intracarotid infusion of mannitol resulted in approximately 1.5 times more l4C-methotrexate drug exposure (pdgm x 70 minutes) to cortex than to tumor [4, 5) . The paper on neurotoxicity [63 cited by Dr Watts did not test methotrexate, although a previous study reported little acute effect of the drug on dog brain [73. However, there is ample clinical evidence that methotrexate IS neurotoxic, especially after radiation therapy. Attempting to increase exposure of tumor to a neurotoxic agent by creating a downhill concentration gradient, from tumor-free cortex to tumor, appears to be a prescription for both leukoencephalopathy and tumor recurrence; it violates the concept underlying the therapeutic index. It is generally accepted that results from animal experimentation provide the basis for human clinical trials. Following this tenet, we have performed experiments on an animal model and attempted to extrapolate our conclusions to human studies. Of course, any model is not expected to be universally applicable and animal studies approximate the clinical setting to varying degrees. In our view, it is not ethical to begin human trials without adequate experimental data supporting both the safety and usefulness of such treatment. Recently, Neuwelt et aJ reported 38 patients who were treated with barrier modification and three chemotherapeutic agents: methotrexate, cytoxan, and procarbazine (81. The study was not controlled, but the authors believe the chemotherapy was efficacious. Of note IS that seizures and exacerbation of preexisting neurological deficits accompanied the treatment in more than half of the patients, and were permanent in 8%. We do not know if these patients would agree that this is “modest toxicity.”
