Abstract:China has transitioned from being one of the fastest-growing populations to among the most rapidly aging countries worldwide. In particular, the population of oldest-old individuals, those aged 80+, is projected to quadruple by 2050. The oldest-old represent a uniquely important group—they have high demand for personal assistance and the highest healthcare costs of any age group. Understanding trends in disability and longevity among the oldest-old—that is, whether successive generations are living longer and with less disability—is of great importance for policy and planning purposes. We utilized data from successive birth cohorts (n = 20,520) of the Chinese oldest-old born 10 years apart (the earlier cohort was interviewed in 1998 and the later cohort in 2008). Disability was defined as needing personal assistance in performing one or more of five essential activities (bathing, transferring, dressing, eating, and toileting) or being incontinent. Participants were followed for age-specific disability transitions and mortality (in 2000 and 2002 for the earlier cohort and 2011 and 2014 for the later cohort), which were then used to generate microsimulation-based multistate life tables to estimate partial life expectancy (LE) and disability-free LE (DFLE), stratified by sex and age groups (octogenarians, nonagenarians, and centenarians). We additionally explored sociodemographic heterogeneity in LE and DFLE by urban/rural residence and educational attainment. More recently born Chinese octogenarians (born 1919–1928) had a longer partial LE between ages 80 and 89 than octogenarians born 1909–1918, and octogenarian women experienced an increase in partial DFLE of 0.32 years (P = 0.004) across the two birth cohorts. Although no increases in partial LE were observed among nonagenarians or centenarians, partial DFLE increased across birth cohorts, with a gain of 0.41 years (P < 0.001) among nonagenarians and 0.07 years (P = 0.050) among centenarians. Subgroup analyses revealed that gains in partial LE and DFLE primarily occurred among the urban resident population. Successive generations of China’s oldest-old are living with less disability as a whole, and LE is expanding among octogenarians. However, we found a widening urban-rural disparity in longevity and disability, highlighting the need to improve policies to alleviate health inequality throughout the population.

Abstract:This study examined the possible underestimation and age-trajectories of mortality at oldest-old ages in China’s 2000 and 2010 censuses. By linking logit-transformed conditional probabilities of dying from 13 countries with the highest data quality in the world, this study found that many Chinese provinces had underestimations of mortality at oldest-old ages when a relatively lenient criterion was applied. When a relatively strict criterion was applied, most provinces had a 30% or more underestimation in the probability of dying. We also investigated age trajectories of death rates after age 80 in these two censuses by applying the Kannisto model. Results showed that the age trajectories were distorted in most provinces after age 95. Overall, eastern-coastal provinces had higher data quality — in terms of low underestimation rates and less distorted age trajectories — whereas western China had provinces with problematic data. Females had greater rates of underestimation yet less distorted age-trajectories than males; and the 2010 census had greater rates of underestimation yet less distorted age-trajectories than the 2000 census. We conclude that appropriate adjustments with simultaneous applications of the Kannisto model are needed for direct estimates of mortality at oldest-old ages in the 2000 and 2010 censuses for China and for its provinces.

In fact, we acknowledge this in our paper, and conclude that “further studies need to include chronic diseases”. Li and Harmer also point out that we did not assess the magnitude of rural–urban differences, and Tomohiro Morita and colleagues describe our rural–urban dichotomy as “too simplistic”. We agree that an investigation of rural–urban differences is important, which is why we adjusted for rural–urban residence as a covariate in our study. We would like to point out that it might not be appropriate to divide the data into rural and urban subsamples, because it would cut the sample size by about half, and make the subsample data not fully compatible between 1998 and 2008 because of rural–urban migration. Morita and colleagues correctly point out that the Global Burden of Disease Study showed China can be divided into five areas, and suggest we consider this. However, besides the fact that the division of data would largely reduce the statistical power because of a much smaller sample size, there are wide disparities within each of the five areas, so we believe such analysis would be unhelpful. Adjustment for rural and urban residence plus education, although imperfect, captures most socioeconomic disparities. We disagree with Li and Harmer's comment that our ascribing of greater longevity or lower mortality among the oldest-old to benefits of success is “problematic because no valid measure of health status exists for this population”. The standard activities of daily living and objective tests of physical performance and cognitive function are valid measures of health status strongly associated with survival. We believe Li and Harmer's comment that “the costs of success hypothesis might not be tenable because of the lack of key measures, such as non-communicable disease” is invalid, because: the costs of success is an interpretation of the decline in average scores of physical performance and cognitive function as the survival of more frail, elderly individuals with health problems, rather than a hypothesis to be tested; and, a lack of non-communicable disease data does not prevent analysis of changes in the health status of physical and cognitive abilities. We agree with Morita and colleagues that “the information about family members who support an oldest-old individual is of concern”; however, we did adjust for marital status (ie, living with or without spouse), and thus the effects of family member support are controlled for, to some extent. Abstract:BACKGROUND:The oldest-old (those aged ≥80 years) are the most rapidly growing age group globally, and are most in need of health care and assistance. We aimed to assess changes in mortality, disability in activities of daily living, and physical and cognitive functioning among oldest-old individuals between 1998 and 2008.METHODS:We used data from the Chinese Longitudinal Healthy Longevity Study. Three pairs of cohorts aged 80-89 years, 90-99 years, and 100-105 years (in total, 19 528 oldest-old participants) were examined; the two cohorts in each pair were born 10 years apart, with the same age at the time of the assessment in the 1998 and 2008 surveys. Four health outcomes were investigated: annual death rate, Activities of Daily Living (ADL), physical performance in three tests and cognitive function measured by Mini-Mental State Examination (MMSE). We used different tests and multivariate regression analyses to examine the cohort differences.FINDINGS:Controlling for various confounding factors, we noted that annual mortality among oldest-old individuals was substantially reduced between 0·2% and 1·3% in 1998-2008 compared with individuals of the same age born 10 years previously, and that disability according to activities of daily living had significantly reduced annually between 0·8% and 2·8%. However, cognitive impairment in the later cohorts increased annually between 0·7% and 2·2% and objective physical performance capacity (standing up from a chair, picking up a book from the floor, and turning around 360°) decreased anually between 0·4% and 3·8%. We also noted that female mortality was substantially lower than male mortality among the oldest-old, but that women's functional capacities in activities of daily living, cognition, and physical performance were worse than their male counterparts.INTERPRETATION:Advances in medications, lifestyle, and socioeconomics might compress activities of daily living disability, that is, benefits of success, but lifespan extension might expand disability of physical and cognitive functioning as more frail, elderly individuals survive with health problems, that is, costs of success.FUNDING:National Natural Science Foundation of China, National Institute on Aging/National Institutes of Health, United Nations Funds for Population Activities.

Abstract:OBJECTIVE:This study aims to identify distinctive cognitive trajectories jointly with mortality probabilities and to explore factors related to the particular trajectories of cognitive ageing in China.METHOD:6842 individuals aged 80 years and above from 7 waves of the Chinese Longitudinal Healthy Longevity Survey were assessed with the Mini-Mental State Examination for up to 16 years. A group-based trajectory model was used to jointly estimate cognitive ageing and mortality trajectories; and to explore the factors related to membership of the trajectory groups.RESULTS:A four-group model best fit the data. For all groups, the cognitive function declined with age according to different rates. Group 4, 3, 2, and 1 showed slow (prevalence 52.8%), moderate (31.1%), progressive (12.6%) and rapid (3.5%) cognitive decline, respectively. Mortality probability trajectories followed a hierarchy in consistence with cognitive trajectories approximately. Females, illiteracy, and those born in rural areas were less likely to belong to the most favorable trajectory group.CONCLUSIONS:The heterogeneity of cognitive ageing was identified among Chinese oldest-old. Childhood socioeconomic status, especially education, was associated with the rate of cognitive decline.

Abstract:Abstract Determinants of mortality may differ depending on context. This study uses a nationally representative sample of persons aged 60 and over in China to determine if socioeconomic factors, early life conditions, community characteristics, biological and physical functioning, and disease burden predict four-year all-cause mortality. We employed a series of Weibull hazard models based on exact survival time to predict mortality. We find that current education and place of residence, physical functioning, uncontrolled hypertension, diabetes, cancer, a high level of systemic inflammation, and poor kidney functioning are strong predictors of mortality among older Chinese. We did not find linkages between early-life experiences or community infrastructure to mortality at older ages. Results from this study highlight the value of incorporating biological and performance measurements and the importance of social and historical context in studying old-age mortality.

Abstract:Although an inverse relationship between socioeconomic status (SES) and mortality has been well documented for many populations throughout the world, it remains unclear whether this relationship holds true for the oldest old. Most notably, some scholars have suggested that the relationship may disappear at the oldest ages. Using data from the 1998, 2000, and 2002 waves of the Chinese Longitudinal Healthy Longevity Survey, this study examined the relationship between SES and mortality among the oldest old (80 years and older) population in China. The results show the continuing prevalence of SES differentials in mortality—higher SES is significantly associated with lower mortality risks—among the oldest old in China. The authors further show that the relationship holds regardless of how the oldest old are operationalized (as 80 years and older, 90 years and older, or 100 years and older).

Abstract:Objectives: To investigate healthiness of survival and quality of death among oldest-old Chinese. Methods: Grade of Membership (GoM) method is applied to fulfill our goals using a nationwide longitudinal survey in China. Results: GoM method generates six pure types/profiles for healthiness of survival and five profiles/types for quality of death. The authors combine these 11 profiles into 4 groups. On average, a Chinese oldest old from 1998 to 2000 had 48% probability of experiencing healthy survival, with 30% experiencing unhealthy survival, 11% having nonsuffering death, and 11% having suffering death. Similar memberships of dying with nonsuffering conditions are found across ages among the decedents. Men have a higher probability of being in healthy survival and nonsuffering death as compared to women. Marriage, high social connections, nonsmoking, and regular exercise are important contributors to healthy survival and quality of death. Discussion: It is possible to live to ages 100 and beyond without much suffering.

Abstract:Based on 6,444 deceased respondents ages 80 to 105 years from the first three waves of the Chinese Longitudinal Healthy Longevity Survey, the authors use multilevel modeling to examine how community development, individual sociodemographic characteristics, health conditions, and health resources affect place of death. Results show that 92% of Chinese oldest old die at home, with 7% dying in hospitals and 1% at institutions. Analyses indicate that residents from relatively developed communities in China tend to have a higher chance of hospital and/or institutional deaths; individuals with higher socioeconomic status (SES) and worsening health are more likely to experience hospital deaths; those who have pension and/or public and/or collective free medical services are more likely to die in hospitals and/or institutions. The authors also propose a theory about place of death consisting of three evolutionary stages, which might explain the disparity in patterns of place of death in different societies.

Abstract:Objectives This study was performed to describe the health status of Chinese oldest-old individuals aged ≥80 years from 1998 to 2014 and explore differences in their health status based on demographic characteristics and socioeconomic status. Methods Data were obtained from the seven waves of the Chinese Longitudinal Healthy Longevity Survey. The Mini Mental State Examination, Katz Index of Independence in Activities of Daily Living, and chair stand test were used to examine cognitive and physical function. Logistic regression was used to evaluate the changes in health outcomes. Results Regression results showed increasing cognitive impairment trends and decreasing disability trends, indicating that the oldest-old individuals had worse body function in more recent years. Older people and women had a weaker health status. Rural elders had better physical performance but worse cognitive function. Participants with an education had better overall functioning. Individuals with severe disability had the shortest survival time among the study population. Conclusions From 1998 to 2014, the health status of oldest-old individuals was relatively stable in China. These findings indicate the need for specific and continuing attention to the oldest-old population to improve their health condition.

Abstract:This paper investigates historical changes in both single-year-of-age adult mortality rates and variation of the single-year mortality rates around expected values within age intervals over the past two centuries in 15 developed countries. We apply an integrated hierarchical age-period-cohort—variance function regression model to data from the human mortality database. We find increasing variation of the single-year rates within broader age intervals over the life course for all countries, but the increasing variation slows down at age 90 and then increases again after age 100 for some countries; the variation significantly declined across cohorts born after the early 20th century; and the variation continuously declined over much of the last two centuries but has substantially increased since 1980. Our further analysis finds the recent increases in mortality variation are not due to increasing proportions of older adults in the population, trends in mortality rates, or disproportionate delays in deaths from degenerative and man-made diseases, but rather due to increasing variations in young and middle-age adults.

Abstract:OBJECTIVES:To investigate the association between tea consumption and mortality in the oldest-old Chinese.DESIGN:Population-based longitudinal data from the Chinese Longitudinal Healthy Longevity Survey (CLHLS) were analyzed using a Cox semiparametric proportional hazard model.SETTING:Six hundred thirty-one randomly selected counties and cities of China's 22 provinces.PARTICIPANTS:Individuals aged 80 and older (N = 9,093) who provided complete data in the baseline survey (1998).MEASUREMENTS:Self-reported current frequency of tea drinking and past frequency at approximately age 60 were ascertained at baseline survey; a follow-up survey was conducted 2000, 2002, and 2005.RESULTS:In the oldest-old Chinese, tea consumption was associated with lower risk of mortality after adjusting for demographic characteristics, socioeconomic status, health practices, and health status. Compared with non-tea drinkers, the adjusted hazard ratio (HR) was 0.90 (95% confidence interval (CI) = 0.84-0.96) for daily tea drinkers (at the baseline survey, 1998) and 1.00 (95% CI = 1.01-1.07) for occasional tea drinkers (P for linear trend .003). Similar results were found when tea drinking status at age 60 was used in the analysis. Further analysis showed that subjects who reported frequent tea drinking at age 60 and at the baseline survey had a 10% lower risk of mortality than subjects who reported infrequent tea drinking at age 60 and at the baseline survey (HR = 0.90, 95% CI = 0.84-0.97).CONCLUSION:Tea consumption is associated with lower risk of mortality in the oldest-old Chinese.

Abstract:Background and Aims Emerging evidence has raised an obesity paradox in observational studies of body mass index (BMI) and health among the oldest-old (aged >= 80 years), as an inverse relationship of BMI with mortality was reported. This study was to investigate the causal associations of BMI, waist circumference (WC), or both with mortality in the oldest-old people in China.Methods A total of 5306 community-based oldest-old (mean age 90.6 years) were enrolled in the Chinese Longitudinal Healthy Longevity Survey (CLHLS) between 1998 and 2018. Genetic risk scores were constructed from 58 single-nucleotide polymorphisms (SNPs) associated with BMI and 49 SNPs associated with WC to subsequently derive causal estimates for Mendelian randomization (MR) models. One-sample linear MR along with non-linear MR analyses were performed to explore the associations of genetically predicted BMI, WC, and their joint effect with all-cause mortality, cardiovascular disease (CVD) mortality, and non-CVD mortality.Results During 24 337 person-years of follow-up, 3766 deaths were documented. In observational analyses, higher BMI and WC were both associated with decreased mortality risk [hazard ratio (HR) 0.963, 95% confidence interval (CI) 0.955-0.971 for a 1-kg/m2 increment of BMI and HR 0.971 (95% CI 0.950-0.993) for each 5 cm increase of WC]. Linear MR models indicated that each 1 kg/m2 increase in genetically predicted BMI was monotonically associated with a 4.5% decrease in all-cause mortality risk [HR 0.955 (95% CI 0.928-0.983)]. Non-linear curves showed the lowest mortality risk at the BMI of around 28.0 kg/m2, suggesting that optimal BMI for the oldest-old may be around overweight or mild obesity. Positive monotonic causal associations were observed between WC and all-cause mortality [HR 1.108 (95% CI 1.036-1.185) per 5 cm increase], CVD mortality [HR 1.193 (95% CI 1.064-1.337)], and non-CVD mortality [HR 1.110 (95% CI 1.016-1.212)]. The joint effect analyses indicated that the lowest risk was observed among those with higher BMI and lower WC.Conclusions Among the oldest-old, opposite causal associations of BMI and WC with mortality were observed, and a body figure with higher BMI and lower WC could substantially decrease the mortality risk. Guidelines for the weight management should be cautiously designed and implemented among the oldest-old people, considering distinct roles of BMI and WC. Abstract:Unique data from a 1998 healthy longevity baseline survey provide demographic, socio‐economic, and health characteristics of the oldest old, aged 80–105, in China. This subpopu‐lation is growing rapidly and is likely to need extensive social and health services. A large majority of Chinese oldest old live with their children and rely mainly on children for financial support and care. Most Chinese oldest old had no or very little education. Ability to function independently in daily living declines rapidly and self‐rated health declines moderately across the oldest old ages. As compared to their urban counterparts, the rural oldest old have far less pension support, are significantly less educated, and are more likely to be widowed and to rely on children for support. Apart from higher rates of survival, the female oldest old in China are far more disadvantaged than the male oldest old.

Abstract:Abstract With the rapid socioeconomic development and the increasing average life expectancy, China has shifted into an era of longevity. One significant feature of this era is the increasing size of Chinese centenarians and their health status are attracting increasing attention from academic communities. By using Chinese Longitudinal Healthy Longevity Survey (CLHLS), this paper employs Cox model to pinpoint the key influencing factors of mortality risk of Chinese centenarians. Our findings turn out that compared with the other elderly population, the mortality-risk determinants among centenarians are unique. The most important risk of the latter comes from their objective health status. The worse the health situation, the higher mortality risk is. Meanwhile, this rate is less affected by social and economic conditions. However, gender and smoking habit all play parts in the mortality risk of centenarians. This study comprehensively understands the key factors of the mortality risk of Chinese centenarians, which is of great significance for reducing centenarians’ mortality risks and enhancing their health and well-beings.
A New Method for Correcting Underestimation of Disabled Life Expectancy and an Application to the Chinese Oldest-Old

Abstract:This article demonstrates that disabled life expectancies that are based on conventional multistate life-table methods are significantly underestimated because of the assumption of no changes in functional status between age x and death. We present a new method to correct the bias and apply it to data from a longitudinal survey of about 9, 000 oldest-old Chinese aged 80-105 collected in 1998 and 2000. In our application, the age trajectories of disability (activities of daily living-ADL), status-specific death rates, and the probabilities of transitions between ADL states of the oldest-old were investigated for the first time in a developing country. In this article, we report estimates of bias-corrected disabled and active life expectancies of the Chinese oldest-old and demonstrate patterns of large differences associated with initial status, gender, and advances in ages. Using combined information on ADL disabilities and length of having been bedridden before dying, we analyze gender and age patterns of the extent of morbidity before dying among the oldest-old and their implications for debates on the hypothesis of compression of morbidity.
