Abstract:China has transitioned from being one of the fastest-growing populations to among the most rapidly aging countries worldwide. In particular, the population of oldest-old individuals, those aged 80+, is projected to quadruple by 2050. The oldest-old represent a uniquely important group—they have high demand for personal assistance and the highest healthcare costs of any age group. Understanding trends in disability and longevity among the oldest-old—that is, whether successive generations are living longer and with less disability—is of great importance for policy and planning purposes. We utilized data from successive birth cohorts (n = 20,520) of the Chinese oldest-old born 10 years apart (the earlier cohort was interviewed in 1998 and the later cohort in 2008). Disability was defined as needing personal assistance in performing one or more of five essential activities (bathing, transferring, dressing, eating, and toileting) or being incontinent. Participants were followed for age-specific disability transitions and mortality (in 2000 and 2002 for the earlier cohort and 2011 and 2014 for the later cohort), which were then used to generate microsimulation-based multistate life tables to estimate partial life expectancy (LE) and disability-free LE (DFLE), stratified by sex and age groups (octogenarians, nonagenarians, and centenarians). We additionally explored sociodemographic heterogeneity in LE and DFLE by urban/rural residence and educational attainment. More recently born Chinese octogenarians (born 1919–1928) had a longer partial LE between ages 80 and 89 than octogenarians born 1909–1918, and octogenarian women experienced an increase in partial DFLE of 0.32 years (P = 0.004) across the two birth cohorts. Although no increases in partial LE were observed among nonagenarians or centenarians, partial DFLE increased across birth cohorts, with a gain of 0.41 years (P < 0.001) among nonagenarians and 0.07 years (P = 0.050) among centenarians. Subgroup analyses revealed that gains in partial LE and DFLE primarily occurred among the urban resident population. Successive generations of China’s oldest-old are living with less disability as a whole, and LE is expanding among octogenarians. However, we found a widening urban-rural disparity in longevity and disability, highlighting the need to improve policies to alleviate health inequality throughout the population.

Abstract:BACKGROUND:The oldest-old (those aged ≥80 years) are the most rapidly growing age group globally, and are most in need of health care and assistance. We aimed to assess changes in mortality, disability in activities of daily living, and physical and cognitive functioning among oldest-old individuals between 1998 and 2008.METHODS:We used data from the Chinese Longitudinal Healthy Longevity Study. Three pairs of cohorts aged 80-89 years, 90-99 years, and 100-105 years (in total, 19 528 oldest-old participants) were examined; the two cohorts in each pair were born 10 years apart, with the same age at the time of the assessment in the 1998 and 2008 surveys. Four health outcomes were investigated: annual death rate, Activities of Daily Living (ADL), physical performance in three tests and cognitive function measured by Mini-Mental State Examination (MMSE). We used different tests and multivariate regression analyses to examine the cohort differences.FINDINGS:Controlling for various confounding factors, we noted that annual mortality among oldest-old individuals was substantially reduced between 0·2% and 1·3% in 1998-2008 compared with individuals of the same age born 10 years previously, and that disability according to activities of daily living had significantly reduced annually between 0·8% and 2·8%. However, cognitive impairment in the later cohorts increased annually between 0·7% and 2·2% and objective physical performance capacity (standing up from a chair, picking up a book from the floor, and turning around 360°) decreased anually between 0·4% and 3·8%. We also noted that female mortality was substantially lower than male mortality among the oldest-old, but that women's functional capacities in activities of daily living, cognition, and physical performance were worse than their male counterparts.INTERPRETATION:Advances in medications, lifestyle, and socioeconomics might compress activities of daily living disability, that is, benefits of success, but lifespan extension might expand disability of physical and cognitive functioning as more frail, elderly individuals survive with health problems, that is, costs of success.FUNDING:National Natural Science Foundation of China, National Institute on Aging/National Institutes of Health, United Nations Funds for Population Activities.

Abstract:Objectives This study was performed to describe the health status of Chinese oldest-old individuals aged ≥80 years from 1998 to 2014 and explore differences in their health status based on demographic characteristics and socioeconomic status. Methods Data were obtained from the seven waves of the Chinese Longitudinal Healthy Longevity Survey. The Mini Mental State Examination, Katz Index of Independence in Activities of Daily Living, and chair stand test were used to examine cognitive and physical function. Logistic regression was used to evaluate the changes in health outcomes. Results Regression results showed increasing cognitive impairment trends and decreasing disability trends, indicating that the oldest-old individuals had worse body function in more recent years. Older people and women had a weaker health status. Rural elders had better physical performance but worse cognitive function. Participants with an education had better overall functioning. Individuals with severe disability had the shortest survival time among the study population. Conclusions From 1998 to 2014, the health status of oldest-old individuals was relatively stable in China. These findings indicate the need for specific and continuing attention to the oldest-old population to improve their health condition.

Abstract:Background: Adults with cognitive impairment are prone to living alone in large numbers but receive relatively little attention. This study aimed to evaluate whether living alone with cognitive impairment was associated with a higher burden of functional disability but lack of informal care. Methods: 982 observations of adults living alone with cognitive impairment and 50,695 observations of adults living with others and with normal cognition were identified from 4 waves (2011/2012, 2013, 2015, and 2018) of the China Health and Retirement Longitudinal Study (CHARLS). A matched comparator was selected using propensity score matching (1:2). Functional disability included disability in Activities of Daily Living (ADL), Instrumental Activities of Daily Living (IADL), and mobility. The time of receiving informal care was measured in monthly hours. Results: Adults living alone with cognitive impairment demonstrated significantly higher odds ratio of ADL disability (OR = 1.59, 95 % CI: 1.30, 1.95), IADL disability (OR = 1.19, 95 % CI: 1.00, 1.44), mobility disability (OR = 1.38, 95 % CI: 1.12, 1.70), but received fewer hours of informal care (beta = -127.7 h per month, standard error = 25.83, P < 0.001), compared to the adults living with others and with normal cognition. Conclusions: This study highlights the high burden of functional disability but low coverage of informal care among Chinese adults living alone with cognitive impairment and calls for more resources to be allocated to this vulnerable subpopulation to improve the functional health and to increase the provision of long-term care services.

In fact, we acknowledge this in our paper, and conclude that “further studies need to include chronic diseases”. Li and Harmer also point out that we did not assess the magnitude of rural–urban differences, and Tomohiro Morita and colleagues describe our rural–urban dichotomy as “too simplistic”. We agree that an investigation of rural–urban differences is important, which is why we adjusted for rural–urban residence as a covariate in our study. We would like to point out that it might not be appropriate to divide the data into rural and urban subsamples, because it would cut the sample size by about half, and make the subsample data not fully compatible between 1998 and 2008 because of rural–urban migration. Morita and colleagues correctly point out that the Global Burden of Disease Study showed China can be divided into five areas, and suggest we consider this. However, besides the fact that the division of data would largely reduce the statistical power because of a much smaller sample size, there are wide disparities within each of the five areas, so we believe such analysis would be unhelpful. Adjustment for rural and urban residence plus education, although imperfect, captures most socioeconomic disparities. We disagree with Li and Harmer's comment that our ascribing of greater longevity or lower mortality among the oldest-old to benefits of success is “problematic because no valid measure of health status exists for this population”. The standard activities of daily living and objective tests of physical performance and cognitive function are valid measures of health status strongly associated with survival. We believe Li and Harmer's comment that “the costs of success hypothesis might not be tenable because of the lack of key measures, such as non-communicable disease” is invalid, because: the costs of success is an interpretation of the decline in average scores of physical performance and cognitive function as the survival of more frail, elderly individuals with health problems, rather than a hypothesis to be tested; and, a lack of non-communicable disease data does not prevent analysis of changes in the health status of physical and cognitive abilities. We agree with Morita and colleagues that “the information about family members who support an oldest-old individual is of concern”; however, we did adjust for marital status (ie, living with or without spouse), and thus the effects of family member support are controlled for, to some extent. Abstract:Objective: To describe the characteristics of oldest-old Chinese with disability and their adult-child caregivers, and the extent to which these characteristics were associated with caregiver burden. Methods: The study was based on 168 pairs of disabled oldest-old adults and their adult-child caregivers, derived from the Chinese Longitudinal Healthy Longevity Survey. Descriptive analyses of care recipients' and caregivers' characteristics were conducted respectively, in reference to caregiver burden. Statistically significant characteristics identified in these bivariate analyses were then jointly evaluated in multiple linear regression models with caregiver burden as the outcome. Results: Care recipients positive emotion status [(beta =-0.227 (-0.412,-0.042)], multiple chronic disease [(beta = 0.513 (0.081, 0.945)], and caregivers spent more caregiving time [(beta = 0.225 (0.061, 0.389)] were main factors associated with caregiver burden. Conclusion: Adult-children caregivers perceived heavier burden if care recipients had low positive emotions, had multiple chronic diseases, and caregivers spent more time caregiving. Abstract:Background: Physical performance impairment and disability are common in the elderly. As human longevity increases, maintaining their independent living ability and healthy physical function are meaningful and necessary. This study aimed to investigate the extent of the effect of exercise duration and its cessation on disability and physical performance of older Chinese adults during their last 6-yr before death. Methods: Deceased older populations from the Chinese Longitudinal Healthy Longevity Study interviewed during 1998-2014and died during 2005-2014 were included to be divided into long-term non-exercising group (NN), non-exercising to exercise group (NY), exercise to non-exercising group (YN), or long-term exercise group (YY). Disability and physical performance were measured by using the activities of daily living (ADL) scale and three objective performance-based tests every three years. And the differences of four groups were compared using logistic regression analyses. Findings 7384 participants were included(NN: 3708; NY: 1017; YN: 1622; and YY: 1037). Compared with NN group, YY group had lower risk of having moderate (odds ratio [OR]=0·48, 95% confidence interval [CI] 0·30–0·77; p =0·02) and severe (OR=0·13, 95% CI 0·09–1·89; p <0·0001) disability after adjustment for the various confounding factors; YN group had higher risk to have moderate disability (OR=1·68, 95% CI 1·28–2·25; p <0·0001), but no significant difference in objective physical performance. Interpretation: Exercise may be beneficial in decreasing disability risk and physical performance impairment in older adults before death. However, these benefits may be lost upon the cessation of exercise. Abstract:Aim:This article examines the prevalence of functional disability as well as sociodemographic correlations among Chinese older adults based on the most recent China National Sample Survey on Disability in 2006.Methods:Functional disability was defined as lost motor function or limited movements and activities according to the International Classification of Function, Disability and Health, and physicians made the diagnosis of disability. Utilizing stratified, multiphase and cluster probability sampling design, a nationally representative sample of the non-institutionalized elderly Chinese population was studied. Factors associated with functional disability were determined using a logistic regression model.Results:The prevalence of functional disability was 8.1%. Major causes were vascular disease, osteoarthritis and accidents. In the non-institutionalized elders, increased age, rural residence, less education, lower income, unemployment, being unmarried, living in simple housing, and living with others or non-homeowners emerged as factors associated with functional disability in men and women.Conclusion:To face the challenge of the rising prevalence of functional disability, prevention strategies should focus on reducing the incidences of chronic disease and improving socioeconomic status of older adults.

Abstract:Background: Disability is an important problem in aging societies globally. However, the research findings of the prevalence of disability have been inconsistent. This study aims to estimate the prevalence of disability and its influencing factors among the Chinese older population from 1979 to 31 July 2021. Methods: A systematic review and meta-analysis were conducted using both international (PubMed, Web of Science, CBMdisc, PsycINFO, the Cochrane Library, and EMBASE) and Chinese (CNKI, CQVIP, and WanFang) databases. Meta-analysis was performed using a random-effects model to account for heterogeneity. Subgroup analyses were also done. Results: The pooled prevalence of disability across all 97 studies was 26.2% (95% CI: 23.7–28.6%). The estimates varied according to the types of activities of daily living (ADL), gender, age, and region. Studies based on the identification of cases by using the complete ADL scale showed a higher prevalence than those using the basic ADL scale. The prevalence was slightly higher among female older individuals than among male older individuals. The highest rates were seen in older individuals aged 80 years or older. Elders in central China, southwest China, and northwest China were more likely to be BADL-disabled. Conclusion: Prevalence of disability among the Chinese older population is high, around 26%. Using standardized diagnostic systems to correctly estimate the prevalence of disability would be helpful for public health professionals in China.

Abstract:type="main"> China has the world's largest oldest-old population, but information on trends in late-life disability is lacking. We use data from the Chinese Longitudinal Healthy Longevity Survey for 1998 to 2008 to determine whether prevalence of limitations with physical functions and daily activities has changed recently among the Chinese population aged 80 to 105 and, if so, to investigate the factors associated with the change. We find that prevalence of need for assistance with activities of daily living and inability to independently conduct instrumental activities of daily living declined substantially. Males did not experience improvement in ability to carry out underlying physical functions over the same period, but females did. Variables associated with trends in one or more of these outcomes were adequacy of medical care as a child, childhood hunger, father's occupation in agriculture, main occupation before age 60 in agriculture, adequacy of current medical care, and body weight.
A New Method for Correcting Underestimation of Disabled Life Expectancy and an Application to the Chinese Oldest-Old

Abstract:This article demonstrates that disabled life expectancies that are based on conventional multistate life-table methods are significantly underestimated because of the assumption of no changes in functional status between age x and death. We present a new method to correct the bias and apply it to data from a longitudinal survey of about 9, 000 oldest-old Chinese aged 80-105 collected in 1998 and 2000. In our application, the age trajectories of disability (activities of daily living-ADL), status-specific death rates, and the probabilities of transitions between ADL states of the oldest-old were investigated for the first time in a developing country. In this article, we report estimates of bias-corrected disabled and active life expectancies of the Chinese oldest-old and demonstrate patterns of large differences associated with initial status, gender, and advances in ages. Using combined information on ADL disabilities and length of having been bedridden before dying, we analyze gender and age patterns of the extent of morbidity before dying among the oldest-old and their implications for debates on the hypothesis of compression of morbidity.
Exercise, disability and physical functioning in older Chinese adults during their last six years of life: a cohort study

Abstract:Abstract Background Little is known about the potential effects of exercise behaviour on disability and physical functioning (PF) in late-life. This study aimed to investigate whether exercise behaviour with four different long term exercise status influences disability and PF in older Chinese adults during their last six years of life. Methods Secondary data analyses were performed using participants (≥ 65 years) from the Chinese Longitudinal Healthy Longevity Study. Exercise behaviour was collected every three years during their last six years of life. 4265 participants were enrolled and divided into four groups, including long-term non-exercising (NN), non-exercising to exercise (NY), exercise to non-exercising (YN), and long-term exercising (YY). Disability was measured based on the Katz Index of Activities of Daily Living scale and divided into three disability levels. PF was included as chair-rise, pick-up book and turn-around. Risks burden in disability and PF among four groups were compared using logistic regression analyses. Results Compared with NN, the risk of developing severe disability was attenuated in YY (OR 0.11, 95%CI 0.06 to 0.21) and attenuated in NY (OR 0.27, 95%CI 0.17 to 0.44) and increased in YN (OR 1.36, 95%CI 1.08 to 1.72). Moderate disability risk was increased in YN (OR 1.85, 95%CI 1.26 to 2.71) only. The risk of developing physical dysfunction ( chair-rise, pick-up book, and turning around 360°) was decreased in YY and NY, and did not have significant difference in YN. Conclusions EP were associated with disability and PF in older Chinese adults during their late-life. Late-life exercise behaviour is still beneficial. Our findings suggest that it is meaning to encourage older people doing EP at late-life in China.

Abstract:Abstract Background The aim of this study was to examine the trends in impairment regarding activities of daily living (ADL), physical performance, and cognitive function among the oldest-old (those aged 80 and older) in China between 1998 and 2014. Methods We used data on 34,297 oldest-old individuals from the seven waves of the Chinese Longitudinal Healthy Longevity Study. We estimated age, period, and cohort effects on the prevalence of self-reported ADL impairment, tested physical performance and cognitive function impairment using the age–period–cohort model. Results Regarding age, the prevalence of ADL, physical performance, and cognitive function impairment were highest in the centenarians, but they did not increase with age in this population. Among the literate subgroup, the prevalence of cognitive impairment increased more rapidly with age than that in the illiterate subgroup. Regarding period, the prevalence of self-reported and tested physical impairment slowly increased between 1998 and 2014, but cognitive impairment remained stable. Regarding cohort, ADL impairment continuously decreased. However, physical and cognitive impairment remained stable after a brief decline in the early birth cohorts. Conclusions The results suggest that the age effect is still the most obvious effect regarding several types of functional impairment. The likelihood of a younger person experiencing functional impairment may not change significantly, but ADL is likely to be amenable to improvement resulting from improved medical and social care. Therefore, increased care for the oldest-old may considerably improve their quality of life, particularly regarding their basic ADL.

Abstract:Objectives: This study evaluates trends in disability-free life expectancy (DFLE) of Chinese adults aged 60 and older. Methods: Based on two national disability surveys in 1987 and 2006 as well as mortality data from World Population Prospects, the Sullivan method was used to calculate DFLE. Results: DFLE increased from 13.0 to 13.9 years at age 60 and from 1.2 to 1.5 years at age 90 in1987 and 2006, respectively. The proportion of DFLE increased after age 75. The proportion of DLEx with severe disability decreased while that of least disability increased. Onset of disability was delayed from 0.3 years to 4.7 years across disability types. Conclusions: Trends in DFLE by age and severity of disability as well as the delayed onset of disability provide evidence for the compression of morbidity among the oldest old in China. But a similar trend was not found among the young-old.

Abstract:This study aims to investigate the age, period, and cohort effects on trends in activities of daily living (ADL) disability among Chinese older adults; and to explore these three temporal effects on gender and residence disparities in disability. We utilized multiple cross-sectional waves of the Chinese Longitudinal Healthy Longevity Survey data (1998–2018), including 89,511 participants aged above 65 years old. Our measurement of disability is the number of ADL items (dressing, bathing, indoor transferring, toileting, eating, and continence) participants can’t perform independently. Hierarchical age-period-cohort cross-classified random effects models were conducted to investigate age, period and cohort trends in ADL disability. Results showed that ADL disability increased with age at an increasing rate. A V-shaped cohort trend and a fluctuated period trend were identified. Females and urban residents were associated with more ADL limitations. When age increased, the gender and residence gaps in disability further increased. The cohort-based gender and residence inequalities in ADL limitations converged with successive cohorts. The period-based residence gap in ADL limitations diverged throughout the 20-year period, while the corresponding period-based change in gender disparity was not significant. These findings suggested that age, period, and cohort had different and independent effects on ADL disability among Chinese older adults. The age effect on trends in ADL is stronger compared to period and cohort effects. The gender and residence disparities in disability increased with age and decreased with successive cohorts. These patterns might help inform healthcare planning and the priorities for medical resource allocation accordingly.

Abstract:Based on unique data from a sample of nearly 9,000 people ages 80 to 105 interviewed in 22 provinces in 1998, we found that gender differentials in educational attainment among the Chinese oldest old are enormous: Many more women are illiterate. Oldest old women are more likely to be widowed and economically dependent, much less likely to have pensions, and thus more likely to live with their children and rely on children for financial support and care. The female oldest old in China are also seriously disadvantaged in activities of daily living, physical performance, cognitive function, and self-reported health, as compared with their male counterparts; these gender differences are more marked with advancing age. The large gender differentials among the Chinese oldest old need serious attention from society and government, and any old-age insurance and service programs to be developed or reformed must benefit older women and men equally.

Abstract:This study is based on survey data from a sample of 8,805 elders aged 80–105 in 22 provinces of China. Rural oldest old are significantly more active in daily living than urban oldest old; adaptation is perhaps the major explanation of this difference. Female oldest old are seriously disadvantaged in functional capacities and self–reported health as compared with their male counterparts. Percentages of being active in daily living, having good physical performance, normal cognitive function and well–being drop dramatically from age 80–84 to 100–105. Proportion reporting satisfaction in current life, however, remains almost constant from age 80–84 to 90–94 and declines slightly afterwards. This may suggest that being more positive in self–feeling of life is one of the secrets of longevity.

Abstract:Unique data from a 1998 healthy longevity baseline survey provide demographic, socio‐economic, and health characteristics of the oldest old, aged 80–105, in China. This subpopu‐lation is growing rapidly and is likely to need extensive social and health services. A large majority of Chinese oldest old live with their children and rely mainly on children for financial support and care. Most Chinese oldest old had no or very little education. Ability to function independently in daily living declines rapidly and self‐rated health declines moderately across the oldest old ages. As compared to their urban counterparts, the rural oldest old have far less pension support, are significantly less educated, and are more likely to be widowed and to rely on children for support. Apart from higher rates of survival, the female oldest old in China are far more disadvantaged than the male oldest old.

Abstract:Background: Self-care disability among older adults is a global public health issue. However, it lacks the up-to-date information based on nationally representative, more comprehesive data in China. Methods: Using China's 2020 population census data, this paper provides a macro-analysis of the prevalence and socio-demographic characteristics of self-care disability among older adults. Results: 25.5 million older adults aged 60 and over participated in the health status survey, of which 48.2% were male, and 51.8% were female. We find that the prevalence of self-care disability among older adults aged 60 and above in China is 2.34%, and the older the population, the higher the prevalence. A higher prevalence was reported by female older adults, rural older adults, and older adults in western China. Single (never married) and widowed older adults are at higher risk of self-care disability. Compared to 2010, the prevalence of self-care disability among older adults decreased. However, the urban-rural difference still exists. Self-care disabled older adults rely mainly on family members for livelihood and mainly cohabitate with them. While pension is an essential source of livelihood for urban older adults with self-care disability, fewer rural self-care disabled older adults rely on pension. Conclusion: The prevalence of self-care disability among older adults aged 60 and over in China is low and has decreased compared to 2010. Older adults with self-care disability are not a homogeneous group, and they have apparent socio-demographic disparities and regional differences. The Chinese government should continue to reduce inequalities between urban and rural areas, especially in pension and long-term care systems.
