Abstract:Objective To observe the clinical efficacy of the routine drug combination with nitroglycerin for coronary heart disease angina pectoris.MethodTotal 75 patients were randomly divided into the treatment group(25 cases of high-dose group and 25 cases of small-dose group)and the control group(25 cases).The control group was given conventional therapy,and the treatment group received nitrog lycerin on the basis of conventional therapy.ResultsThe total effective rate of treating angina pectoris and ECG were better than the control group.After treatment,compared with the control group,liver,kidney functions and the three conventional testing in the treatment group showed no significant differences.ConclusionThe combined therapy is more effective,and with no serious side effects.
Long-term Observation of Nitroglycerine-Induced Coronary Artery Vasodilatation in Transplanted Heart

Abstract:Objective To further clarify the long-term impact of nitroglycerine on the changes of coronary artery vasodilatation after heart transplantation. Method The quantitative coronary angiography (QCA) was employed to assess the diameters of left anterior descending coronary arteries before and after intra-coronary nitroglycerine injection and to evaluate the dilatation of coronary artery response to nitroglycerine 1～10 years after heart transplantation. Meanwhile, the correlation between gender or age of the donor and the dilatation of coronary artery induced by intra-coronary injection of nitroglycerine was analyzed. Results The magnitude of coronary artery vasodilatation response to nitroglycerine tend to decrease over the time after transplantation, but there is no statistical significance. The gender of donor implicated in the dilatation of coronary artery response to nitroglycerine in short period post transplantation, the coronary artery from the female donor was more obviously dilated by nitroglycerine than that from heart of the male donor during 1～4 years after heart transplantation. However, the age of the donor has no significant influence to the coronary artery vasodilatation. Conclusion The ability of coronary artery vasodilatation response to nitroglycerine is not damaged over long term after heart transplantation. The coronary artery from the heart of female donor was more obviously dilated by nitroglycerine in short term.
Double-Blind, Multicenter, Active-Controlled, Randomized Clinical Trial to Assess the Safety and Efficacy of Orally Administered Nicorandil in Patients with Stable Angina Pectoris in China

Abstract:BACKGROUND:The efficacy and safety of nicorandil were evaluated in Chinese patients with stable angina pectoris (AP) in a double-blind, multicenter, active-controlled, randomized clinical trial.METHODS AND RESULTS:After a 2-week washout period, 232 patients with stable AP were randomized to receive either nicorandil (5 mg tid; 115 patients) or isosorbide mononitrate (ISMN: 20 mg bid; 117 patients) for 2 weeks. Exercise capacity, number of weekly anginal attacks, nitroglycerin (NTG) consumption, and safety were evaluated. Nicorandil and ISMN significantly prolonged the time to 1 mm ST-segment depression in an exercise tolerance test. Both drugs improved the total exercise time and the time to onset of chest pain. There was no significant difference between the 2 groups. Nicorandil significantly decreased the number of anginal attacks and NTG consumption. ISMN decreased the number of anginal attacks significantly; however, there was no significance in NTG consumption, and the ratio of anginal attack reduction was at least 50% was significantly higher with nicorandil. Nicorandil was well tolerated and there was no safety profile difference compared with ISMN. Thus, nicorandil may have equivalent or better antianginal effect than ISMN.CONCLUSIONS:Nicorandil is beneficial as treatment for AP.
EFFECT OF ANGIOTENSIN II RECEPTOR ANTAGONIST AND ENDOTHELIN RECEPTOR ANTAGONIST ON NITROGLYCERIN TOLERANCE IN RATS

Abstract:OBJECTIVE:To investigate whether angiotensin II receptor antagonist and endothelin receptor antagonist can improve the nitroglycerin (Nit) tolerance in vivo.METHODS:Twenty-four rats were divided into 4 groups (n = 6, each): Control group, Nitroglycerin (Nit) group, Nit + bosentan group and Nit + losartan group. Nitroglycerin tolerance was induced by 2-day treatment of (10 mg x kg(-1) nitroglycerin patch(0.05 mg/h). Angiotensin receptor antagonist losartan(10 mg x kg(-1) x d(-1)) and endothelin receptor antagonist bosentan (100 mg x kg(-1) x d(-1)) were given by gavage for 2 days respectively.RESULTS:The least hypotensive response to sodium nitroprusside (SNP) was observed in Nit group. The effective percentages of hypotensive response to SNP were increased in both Nit + losartan group and Nit + bosentan group compared with Nit group [(31.95 +/- 4.45) % vs (21.00 +/- 3.69) %, P < 0.01 and (33.18 +/- 6.16) % vs (21.00 +/- 3.69) %, P < 0.01, respectively]. The maximal vessel relaxation induced by SNP was the same in 4 different groups but the highest EC50 (concentration which produces 50% of the maximal response to SNP) was found in tolerant group[(34 +/- 10) nmol/L, P < 0.01]. The ET-1 amounts in plasma and vascular tissue were markedly increased by 54% and 60% in Nit group compared with those in control group( P < 0.01). The ET-1 amounts in plasma and vascular tissue were decreased by 30% and 37% in Nit + losartan group compared with those in Nit group (P < 0.01).CONCLUSION:Endothelin receptor antagonist and angiotensin II receptor antagonist could prevent against the Nit tolerance.
Effect of Metallothionein on Tolerance of Nitroglycerin in Rats

Abstract:AIM: To assess whether metallothionein (Met) could improve the nitroglycerin tolerance in vivo; METHODS: Nitrate tolerance was induced by 2-d treatment of nitroglycerin (Nit) patch (0.05 mg.h(-1)). Endogenous Met production was induced by pretreatment of ZnCl2 and coadministration of intravenous Met (15 mg kg(-1).d(-1)) 2 d with Nit in tolerant rats. The induction of Met production was confirmed by the assay of liver and plasma Met levels. RESULTS: ZnCl2 induced large amount of endogenous Met production in liver and plasma in Nit + ZnCl2 group than that in control group, (89+/-4) mu g/g tissue vs (11.0+/-2.4) mu g/ g tissue in liver, P < 0.01, and in plasma (85 +/- 6) mu g .L-1 vs (71+/-6) mu g.L-1, P < 0.01. There was no significant difference in plasma Met levels in Nit and control groups [(75 +/- 6) mu g.L-1 vs (71 +/- 6) mu g. L-1, P > 0.05]. The endogenous Met production enhanced the hypotensive response in Nit + ZnCl2 group [(15.7+/-0.8) kPa vs (11.5 +/- 0.6) kPa, n = 6, P < 0.05]. The maximal vessel relaxation induced by sodium nitroprusside (SNP) was the same in 4 different groups but the highest EC50(concentration which produces 50 % of the maximal response to SNP) was found in tolerant group [(42 +/- 9) nmol.L-1, P < 0.01]. CONCLUSION: Exogenous Met or induction of endogenous Met production antagonize the development of Nit tolerance.
Nitrite Modulates Aminoglycoside Tolerance by Inhibiting Cytochrome Heme-Copper Oxidase in Bacteria

Abstract:As a bacteriostatic agent, nitrite has been used in food preservation for centuries. When used in combination with antibiotics, nitrite is reported to work either cooperatively or antagonistically. However, the mechanism underlying these effects remains largely unknown. Here we show that nitrite mediates tolerance to aminoglycosides in both Gram-negative and Gram-positive bacteria, but has little interaction with other types of antibiotics. Nitrite directly and mainly inhibits cytochrome heme-copper oxidases (HCOs), and by doing so, the membrane potential is compromised, blocking uptake of aminoglycosides. In contrast, reduced respiration (oxygen consumption rate) resulting from nitrite inhibition is not critical for aminoglycoside tolerance. While our data indicate that nitrite is a promising antimicrobial agent targeting HCOs, cautions should be taken when used with other antibiotics, aminoglycosides in particular.
Prevention of Nitroglycerin Tolerance in Vitro by T0156, a Selective Phosphodiesterase Type 5 Inhibitor

Abstract:The efficacy of nitroglycerin as a vasodilator is limited by tolerance, which develops shortly after treatment begins. The present study aims to examine whether T0156, a newly developed potent and selective inhibitor of phosphodiesterase type 5 (PDE5), could attenuate the tolerance to nitroglycerin on rat aortas. Rat aortic rings were suspended in organ bath for the measurement of changes in isometric tension and nitrate tolerance was acutely induced by preceding exposure for 90 min to 30 microM nitroglycerin. Concentration-response curves to nitroglycerin were obtained on aortic rings pre-contracted with phenylephrine. Pre-exposure of rings with or without endothelium to nitroglycerin reduced the relaxations to nitroglycerin. The tissue levels of cyclic GMP were measured by enzyme immunoassay kit. Treatment with T0156 inhibited and prevented the reduced relaxation and cyclic GMP levels in response to nitroglycerin in tolerant rings. In contrast, nitroglycerin-induced tolerance was unaffected by cilostazol (PDE3 inhibitor) and rolipram (PDE4 inhibitor). Finally, incubation of aortic rings with thromboxane prostanoid receptor antagonist, cyclooxygenase inhibitor, or endothelin ET(A) receptor antagonist did not inhibit the development of tolerance. The present results suggest that nitroglycerin tolerance may involve an increased activity of PDE5 but not PDE3 or PDE4 isoforms in vascular smooth muscle cells since T0156 prevents the development of tolerance. Thromboxane A(2), cyclooxygenase (COX)-dependent prostaglandins and endothelin 1 play little role in the acute induction of nitroglycerin tolerance.
The Effect of Losartan Treatment on Nitroglycerin Tolerance in Rats

Abstract:Objective Nitroglycerin (NTG)and other nitrovasodilators have been widely used to treat congenital heart disease,primary cardiomyopathy and congestive heart failure in pediatrics. However,during the long\|term administration,the hemodynamic and anti\|ischemic efficacy of NTG is rapidly attenuated by the development of tolerance.This study aimed at investigating whether angiotensin Ⅱreceptor antagonist losartan could improve NTG tolerance in vivo. Methods Losartan potassium was purchased from Trademark of EI,Du Pont de Nemours and Company,Delaware,USA.Endothelin I (ET\|I) radioimmunoassay reagent was purchased from Phoenix Pharmaceuticals Inc. NTG patch (16 mg/patch) was purchased from Zhuhai Schwarz Pharma Company Limited. Trasylol was made by Bayer AG, Germany UK Subsidiary.Male Wistar rats weighing 250\|300 g were provided by Experimental Animal Center of Peking University. A total of 28 rats were divided into 4 groups.(1) Control group( n =10):No drugs were used.(2)NTG group( n =6):NTG tolerance animal model was prepared according to Munzel T and Urmlia, et al. A region between scapulae was shaved and NTG patch (0.05mg/h)was applied to the skin. The treatment period started between 8 AM and 10 AM and NTG patch was changed each morning for the ensuing 2 days. (3)losartan group( n =6):losartan 10 mg/(kg·d) was given by gavage for 2 days. (4)NTG plus losartan group( n =6):losartan 10 mg/(kg·d) was given by gavage for 2 days and simultaneously NTG patch treatment was given the same as NTG group. Results Hypotensive responses in the 4 groups before sodium nitroprusside (SNP) showed no significant difference. Hypotensive responses after SNP 20 ug/kg, iv were different among 4 groups. The percentage of hypotensive response to SNP was decreased in NTG group(21±4)% compared with that in control group(34±4)%, P 0.01. The hypotensive response was enhanced in NTG plus losartan group (31±5)% compared with that in NTG group, P 0.01.The maximal vessel relaxation induced by SNP was the same in 4 different groups but the highest EC 50 (concentration which produces 50% of the maximal response to SNP) was found in NTG group [(33.89±10.86)nmol/l, P 0.01].The ET\|1 amounts in plasma and vascular tissue were markedly increased by 54% and 60% in NTG group compared with those in control group ( P 0.01).In contrast to NTG proup the ET\|1 amounts in plasma and vascular tissue were decreased by 30% and 37% in NTG plus losartan group ( P 0.01).Conclusion The results of the study supported that angiotensin Ⅱ receptor antagonist losartan seemed to be a possible candidate medicine to prevent from NTG tolerance in vivo.
Toleration of compound nitrendipine tablets in healthy volunteers after a single oral administration

Abstract:Objective To assess the toleration of compound nitrendipine tablet in healthy volunteers after a single oral taking. Methods Forty volunteers were enrolled and randomly divided into 5 groups (8 subjects in each group) , including 0.2,1, 2, 3 and 4 tablets of compound nitrendipine tablet. The charges of blood pressure, heart rate, clinical symptoms, electrocardiography and laboratory test were observed before and after dosing. Results There were decreased significantly on the mean seated systolic and diastolic blood pressure as well as on the mean seated heart rate after single oral taking 0. 2,1,2,3 and 4 tablets of compound nitredipine; the antihypertensive effect was occurred 1 hour and sustained over 6-8 hours after dosing (expect the group of 0. 2 tablet). The changes of blood pressure and heart rate were compared with among the 5 groups, the former was no significant difference, but there was significant difference in the latter( especially at the group of 4 tablets). There were no observation on the postural hypotension, the serious adverse reactions and changes of the laboratory examinations. Conclusion The tolerance of compound nitrendipine is good in healthy volunteers.

Abstract:AIM To explore safe and effective dosage in clinical practice through discussing the safety of com- pound nitrendipine tablet for healthy volunteers.METHODS After seven-days successive administration (a compound nitrendipine tablet=nitrendipine 5 mg+atenolol 10 mg;4 tablets,qd×7 d) 9 healthy volunteers were observed.The observation items included BP,HR and electrocardiogram,blood and urine routine examination,liver and renal function and adverse drug reaction.RESULTS Their BP and HR were decreased.There was not any difference in the laboratory results between before and after administration(P>0.05),and there was not any severe adverse drug event.CONCLU- SION Healthy volunteers show good tolerance after taking multiple dose of compound nitrendipine tablet.HR should be monitored by electrocardiogram.
Use of nitrates in ischemic heart disease

Abstract:The organic nitrates have remarkably diverse actions that are or should be beneficial in patients with ischemic heart disease. These drugs are effective in all the important ischemic syndromes. Preliminary data in patients with acute infarction suggest that the drugs may be truly cardioprotective, resulting in improved mortality. This review has not discussed the role of nitrates in congestive heart failure or LV dysfunction, a subject of great importance. The nitrates are useful adjunctive agents in these syndromes, and the two VeHfT trials support the concept that long-term nitrate administration, in conjunction with hydralazine, may favorably alter the natural history of heart failure. This cardioprotective effect is similar to that suggested for the post-MI patient. The data are not strong enough for definitive conclusions at this time. The clinical benefits of nitrates in decreasing subjective (angina) and objective indices of ischemia in stable and unstable angina, as well as limited data in asymptomatic myocardial ischemia, are unequivocal and are as favorable as those for beta blockers or calcium antagonists. Tolerance is an important problem that unfavorably influences the potential benefits of nitrate therapy. I believe that this problem can be avoided with well-designed dosing regimens. Current research into endothelial biology in health and disease has further supported a physiologic role for the organic nitrates in patients with ischemic heart disease. The nitrate-platelet story, while controversial, is promising and offers another positive rationale for nitrate administration. The concept of nitrates replenishing disordered EDRF release or action is an exciting one. Physicians should feel fortunate to have such a remarkable group of drugs available for their patients.

Recently it has been recognized that nitrates also have intrinsic antiplatelet effects, which would add further rationale for the use of nitrates in cardiovascular disease. Abstract:Dietary nitrate which mainly comes from green leafy vegetables, is absorbed into blood circulation by the intestinal mucosa. Parotid gland is an important organ for transporting nitrate. Nitrate in blood is taken up by sialin, a nitrate transporter and concentrated in salivary glands and secreted into saliva. The salivary nitrate is partially reduced to nitrite and nitric oxide by oral bacteria, and then salivary nitrate and nitrite return into blood circulation with swallowing and intestinal mucosal absorption. As a non-classic source of nitric oxide, nitrate-nitrite-nitric oxide pathway plays an important role on physiological and pathological conditions, especially on the condition of hypoxia and ischemia. These functions include body protection, such as gastrointestinal tract, cardiovascular system, anti-inflammation, regulation of glucose/lipid metabolism, improvement of sport ability, maintaining gut microbiome hemostasis, and alleviating senility. The traditional view on nitrate as a harmful substance to human body has been proved to be lack of scientific evidence. With further research and application, as a pioneer from the mouth to the whole body, nitrate is expected to play a crucial part in human health, and prevention and treatment of systemic diseases.
Identification of the Enzymatic Mechanism of Nitroglycerin Bioactivation.

Abstract:Nitroglycerin (glyceryl trinitrate, GTN), originally manufactured by Alfred Nobel, has been used to treat angina and heart failure for over 130 years. However, the molecular mechanism of GTN biotransformation has remained a mystery and it is not well understood why "tolerance" (i.e., loss of clinical efficacy) manifests over time. Here we purify a nitrate reductase that specifically catalyzes the formation of 1,2-glyceryl dinitrate and nitrite from GTN, leading to production of cGMP and relaxation of vascular smooth muscle both in vitro and in vivo, and we identify it as mitochondrial aldehyde dehydrogenase (mtALDH). We also show that mtALDH is inhibited in blood vessels made tolerant by GTN. These results demonstrate that the biotransformation of GTN occurs predominantly in mitochondria through a novel reductase action of mtALDH and suggest that nitrite is an obligate intermediate in generation of NO bioactivity. The data also indicate that attenuated biotransformation of GTN by mtALDH underlies the induction of nitrate tolerance. More generally, our studies provide new insights into subcellular processing of NO metabolites and suggest new approaches to generating NO bioactivity and overcoming nitrate tolerance.

Abstract:Nitric oxide (NO) is generated endogenously by NO synthases to regulate a number of physiological processes including cardiovascular and metabolic functions. A decrease in the production and bioavailability of NO is a hallmark of many major chronic diseases including hypertension, ischaemia-reperfusion injury, atherosclerosis and diabetes. This NO deficiency is mainly caused by dysfunctional NO synthases and increased scavenging of NO by the formation of reactive oxygen species. Inorganic nitrate and nitrite are emerging as substrates for in vivo NO synthase-independent formation of NO bioactivity. These anions are oxidation products of endogenous NO generation and are also present in the diet, with green leafy vegetables having a high nitrate content. The effects of nitrate and nitrite are diverse and include vasodilatation, improved endothelial function, enhanced mitochondrial efficiency and reduced generation of reactive oxygen species. Administration of nitrate or nitrite in animal models of cardiovascular disease shows promising results, and clinical trials are currently ongoing to investigate the therapeutic potential of nitrate and nitrite in hypertension, pulmonary hypertension, peripheral artery disease and myocardial infarction. In addition, the nutritional aspects of the nitrate-nitrite-NO pathway are interesting as diets suggested to protect against cardiovascular disease, such as the Mediterranean diet, are especially high in nitrate. Here, we discuss the potential therapeutic opportunities for nitrate and nitrite in prevention and treatment of cardiovascular and metabolic diseases.

Abstract:Phosphoinositide 3-kinase (PI3K)/Akt plays a pivotal role in the vascular response. The present study is to determine whether PI3K/Akt pathway in vascular smooth muscle cells is involved in nitroglycerin (NTG) tolerance and the underlying mechanism. Nitrate tolerance of porcine coronary arteries in vitro was induced by incubation of NTG (10-5 M) for 24 h. Nitrate tolerance in vivo was obtained by subcutaneous injection of mice with NTG (20 mg kg-1, tid, 3 days) and the aortas were used. Protein levels of total and phosphorylated Akt, forkhead box protein O1 (FoxO1), and cGMP-dependent protein kinase (PKG) were determined by western blot analysis. Isometric vessel tension was recorded by organ chamber technique. PKG mRNA was determined by real-time PCR. The cellular translocation of FoxO1 was observed by immunofluorescence. Reactive oxygen species (ROS) level was measured by DHE staining. The vascular relaxation to NTG was significantly inhibited in in vivo and in vitro NTG tolerant arteries. Meanwhile, the protein level of phosphorylated Akt at Ser473 was increased in the tolerant arteries. The attenuated relaxation and the augmented Akt-p were ameliorated by LY294002, a specific inhibitor of PI3K. The protein and mRNA expression of PKG were significantly down-regulated in NTG tolerant arteries, which were reversed by LY294002. The level of phosphorylated FoxO1 at Ser256 and its translocation from the nucleus to the cytosol were both increased in NTG tolerance and were also inhibited by LY294002. ROS production was significantly increased in NTG tolerant arteries, which was not be affected by LY294002 but inhibited by N-acetyl-L-cysteine. In conclusion, the present study suggests that PI3K/Akt in vascular smooth muscle is involved in the development of NTG tolerance via inhibiting PKG transcription and the effect is mediated by FoxO1.

Abstract:Chronic morphine which is commonly used to treat severe and chronic pain leads to analgesia tolerance. According to the research towards mechanism of tolerance, people demonstrated that NMDA/NO cascade might contribute to the development of morphine antinociceptive tolerance. NO is enzymatically formed from the unique precursor L-arginine by nitric oxide synthase. Expression of nitric oxide synthase, especially neuronal nitric oxide synthase, was found to be increased in the spinal cord of rat during the development of morphine antinociceptive tolerance. Activation of NMDA receptors and elevated level of intracellular calcium resulted in increased activities of nitric oxide synthase and stimulation of NMDA/NO cascade, in turn, exacerbated the development of tolerance. Coadministration of nitric oxide synthase inhibitor with morphine could prevent the development of tolerance to morphine-induced antinociception, even resulted in reversal of morphine antinociceptive tolerance, but the exact roles are still unclear. Additional studies need to be run to elucidate these in future.
Decreased PKG Transcription Mediated by PI3K/Akt/FoxO1 Pathway is Involved in the Development of Nitroglycerin Tolerance

Abstract:Phosphoinositide 3-kinase (PI3K)/Akt plays a pivotal role in the vascular response. The present study is to determine whether PI3K/Akt pathway in vascular smooth muscle cells is involved in nitroglycerin (NTG) tolerance and the underlying mechanism. Nitrate tolerance of porcine coronary arteries in vitro was induced by incubation of NTG (10(-5) M) for 24 h. Nitrate tolerance in vivo was obtained by subcutaneous injection of mice with NTG (20 mg kg(-1), tid, 3 days) and the aortas were used. Protein levels of total and phosphorylated Akt, forkhead box protein O1 (FoxO1), and cGMP-dependent protein kinase (PKG) were determined by western blot analysis. Isometric vessel tension was recorded by organ chamber technique. PKG mRNA was determined by real-time PCR. The cellular translocation of FoxO1 was observed by immunofluorescence. Reactive oxygen species (ROS) level was measured by DHE staining. The vascular relaxation to NTG was significantly inhibited in in vivo and in vitro NTG tolerant arteries. Meanwhile, the protein level of phosphorylated Akt at Ser473 was increased in the tolerant arteries. The attenuated relaxation and the augmented Akt-p were ameliorated by LY294002, a specific inhibitor of P13K. The protein and mRNA expression of PKG were significantly down-regulated in NTG tolerant arteries, which were reversed by LY294002. The level of phosphorylated FoxO1 at Ser256 and its translocation from the nucleus to the cytosol were both increased in NTG tolerance and were also inhibited by LY294002. ROS production was significantly increased in NTG tolerant arteries, which was not be affected by LY294002 but inhibited by N-acetyl-L-cysteine. In conclusion, the present study suggests that PI3K/Akt in vascular smooth muscle is involved in the development of NTG tolerance via inhibiting PKG transcription and the effect is mediated by FoxO1. Abstract:Nitrite(NIT) is an intermediate product of the nitrogen cycle, is not stable, commonly known as the "salt", mainly refers to the sodium nitrite(Na NO2) and potassium nitrite(KNO2), especially by sodium nitrite. Nitrite can be seen everywhere in the natural environment, and is widely used in industry, construction industry and food processing industry. This paper describes the properties of nitrite, and applications in food industry. Since 1970, nitrite in cooked meat products on the health effects caused concern. This paper describes the acute toxicity, chronic toxicity of nitrite(including teratogenicity and carcinogenicity), as well as the nitrite-poisoning events caused by eating meat products. Moreover, the authors summarize the detection methods of nitrite.

Abstract:It is now more than 35 years since endothelium derived relaxing factor was identified as nitric oxide (NO). The last few decades have seen an explosion around nitric oxide biochemistry, physiology and clinical translation. The science reveals that all chronic disease is associated with decreased blood flow to the affected organ which results in increased inflammation, oxidative stress and immune dysfunction. This is true for cardiovascular disease, neurological disease, kidney, lung, liver disorders and every other major disorder. Since nitric oxide controls and regulates blood flow, oxygen and nutrient delivery to every cell, tissue and organ in the body and also mitigates inflammation, oxidative stress and immune dysfunction, a focus on restoring nitric oxide production is an obvious therapeutic strategy for a number of poorly managed chronic diseases. Since dietary nitrate is a major contributor to endogenous nitric oxide production, it should be considered as a means of therapy and restoration of nitric oxide. This review will update on the current state of the science and effects of inorganic nitrate administered through the diet on several chronic conditions and reveal how much is needed. It is clear now that antiseptic mouthwash and use of antacids disrupt nitrate metabolism to nitric oxide leading to clinical symptoms of nitric oxide deficiency. Based on the science, nitrate should be considered an indispensable nutrient that should be accounted for in dietary guidelines.
