Abstract:Objective: Since the first report on 1992 by Rukstalis and Chodak, laparoscopic retroperitoneal lymph node dissection (L-RLND) remains the most accurate method of assessing lymph node status in clinically stage I and II nonseminomatous germ cell tumors (NSGCT).1–4 Most of the RLND were performed trans peritoneal cavity. LeBlanc et al reported the L-RLND via retroperitoneal approach, which could avoid the interference to abdominal organs and get better exposure.4–6 We made some modifications to improve this technique and investigated the surgical techniques and clinical feasibility of the extraperitoneal L-RLND (EL-RLND). Materials and Methods: Three patients with primary testicular NSGCT on the right side received EL-RLND at 16, 28, and 31 days after orchiectomy. An incision of approximately 15 mm was made 2 cm medial to the right anterior superior iliac spine. The fascia of the external oblique muscles was cut. The internal oblique and transverse muscles were then bluntly divided. The retroperitoneal space was developed by inflating a preperitoneal balloon device with 900 mL of air. After removing the balloon device, a 10-mm laparoscopic trocar was placed at the existing surgical wound, which served as the camera port. Under laparoscopic observation, additional trocars were placed along the lateral border of the ipsilateral rectus abdominis or along the middle axillary line. The aorta, inferior vena cava, and surrounding adipose and lymphoid tissue were exposed. Dissection and ligation of testicular blood vessels and vas deferens were performed. Then, all para-aortic tissue lateral to the aorta and preaortic tissue cephalad to the inferior mesenteric artery was removed. The boundaries of dissection included renal vein (proximal), bifurcation of common iliac vessel (distal), ureter (lateral), and para-abdominal aorta/inferior vena cava area (medial). All tissue specimens were placed in an organ entrapment sack and removed. Results: The EL-RLND was performed in all the three patients. The average operating time was 180 min, the average intraoperative blood loss was 350 mL, the postoperative intestinal function recovery time was 2 days, and the average hospitalization time was 7 days. Follow-up revealed no evidence of recurrence or distant metastasis. Conclusions: EL-RLND that can be further applied is an operation for nonseminomatous testis cancer with minimal invasiveness, quick recovery. Because it is a difficult and complicated procedure performed near the large vessels, EL-RLND needs more experience on laparoscopic techniques to minimize complications.
