Abstract:Objective:To explore the risks factors of podocyte lesion in patients with type 2 diabetic nephropathy(DN),and the relationship between podocyte lesion and the progression of renal function.Methodology:A retrospective study was performed in twenty four patients with biopsy-proven DN,they were followed for at least 3 years.Podocyte density was detected with immunofluorescence double-staining using anti-GLEPP1,a marker of podocyte,and anti-type Ⅳcollagen a3 chain.The stained sections were examined by the confocal microscopy and the density of podocyte per glomerulus was calculated using specific software.The rate of decline in creatinine clearance rate(ml/min)over at least 3 years was quantified through Cockroft-Gault formula.Results:(1)Comparing with normal control subjects,significantly decreased podocyte density per glomerulus was observed in patients with DN.(2)Reduced podocyte density was strongly associated with body mass index and increased total cholesterol(r=-0.486,P0.05;r=-0.438,P0.05).Bad serum glucose control for a long time(glycosylated hemoglobin7%)contributed to a lower podocyte density compared with subjects under better glucose control(glycosylated hemoglobin 7%)(P0.05,ANOVA).(3)There was a strong correlation between the reduced podocyte density and the degree of proteinuria(r=-0.529,P0.01).Furthermore,stepwise linear regression showed that podocyte density was the strongest predictor of proteinuria in patients with DN.(4)In the subsequent longitudinal study,although all the subjects were under satisfactory metabolic control,there were significant differences in proteinuria and podocyte density at the time of biopsy between DN patients with or without progressive renal failure.(5)Reduced podocyte density was strongly related to the rate of creatinine clearance decline(r=0.424,P0.05).In addition,durations of diabetes(r=-0.614,P0.05)and degree of proteinuria(r=-0.668,P0.01)also correlated with the rate of decline in renal function.Conclusion:Podocyte density reduction was observed in patients with DN.The changes of podocyte may contribute to increasing proteinuria,and be significantly related to the progression of renal function in patients with DN.Metabolic disorders may be partly responsible for the development of podocyte lesions in DN.
