Abstract:As an important basis for medical equipment resource allocation, cost-beneift analysis obtained more and more attention from government management departments and hospital management. In this paper, the method of informatization technology was used to make the cost-benefit analysis so as to monitor the equipment assets, operating costs and revenue in real time. It also provided decision-making basis for department cost control and master planning for medical equipment, and improved the level of ifne management for medical equipment.

Abstract:Objective:To understand the rationality of the utilization of large medical equipments in hospitals and to identify the influencing factors.Methods:Data was collected from the discharged patients' records in 2009 from different levels of hospitals in different provinces.Experts reviewed the records and filled in the questionnaires.Results: The level of hospital played a great role in the disease mix.The differences in the rationality of CT,MRI,DSA utilization between hospital levels and among provinces were statistically significant.Conclusion: Rate of reasonable utilization of CT,MRI and DSA was higher in tertiary hospitals than that in secondary hospitals.Service capacity building should be strengthened in secondary hospitals.Government Department of health should also increase monitoring for the optimized rationality of the utilization of equipments.
Common issues and improvements on clinical trial data inspection of medical device

Abstract:Objective:The authenticity and accuracy of medical device related clinical trial data is crucial for the efficacy and safety of tested products. This article analyzed issues identified during previous data inspections of medical device clinical trials in our hospital, summarized experiences and findings, and proposed solutions.Methods:According to the " Medical Device Clinical Trial Inspection Points and Judgment Principles" , this study retrospectively analyzed the data inspection of medical device clinical trials in our hospital since 2016, summarized and analyzed the common issues identified.Results:A total number of six data inspections on medical device clinical trials were carried out in our hospital, during which 30 findings were identified. These findings include 4 items of pre-trial preparation, 2 items of patient rights protection, 7 items of trial processes, 12 items of records and reporting, as well as 5 items of experimental medical device management.Conclusions:The completeness, accuracy and consistency of clinical trial records in clinical trials of devices in our hospital have a lot space for improvement. And there is still room for improvement in the compliance to the protocol and the management of medical devices during the trial process. Based on the common findings, our drug clinical trial center will strengthen training and quality control, improve informatization and centralized management, and emphasize the importance of records. Through that the accuracy and authenticity of trial data for medical devices, and the credibility and objectiveness of trial results would be assured.

Abstract:Objective:To analyze the current situation of allocation and utilization of major medical equipment in China and make policy recommendations.Methods:Literature review and field survey methods were used to achieve the purpose.Results:The utilization of major medical equipment in the inpatients was higher than that in the outpatients.The service amount of CT was the biggest and service amount of major medical equipment decreased from east to west.Utilization efficiency varied among sample provinces(city),and positive detection rate was generally high.Excessive use and under use coexisted.Macro distribution and utilization management need to be improved.Conclusion:The macro-governance policy on major medical equipment should be optimized during the new health system reform.Some mechanisms for major medical equipment should be developed and improved,including distribution,investment,monitoring and evaluation.

Abstract:Objective To study the management status of implantable medical devices in one city, to find the problems existing, and raise suggests to improve the management. Methods Quantitative studies with strata randomized sampling of hospitals, and interview with responsible persons in the hospitals. Results In the 7 key record points, the highest rate of the point with record is 99.68%, the lowest is 22.22%, while blank record items are common in all key points. Conclusions The management status of implantable medical devices in the city is unsatisfactory, further efforts such as legislation, supervision, safety consciousness and scientific management techniques should be put.

Abstract:Objective To investigate the misuse or improperly use situation of statistical methods on medical postgraduates' dissertations.Methods Cross-sectional study was used for the 192 dissertations of Zhejiang University which submitted in last 5 years,dissertation was selected by multistage stratified random sampling.Results Misuse or improperly use rate of the 192 dissertations in research design,statistical description,statistical inference,result expression/interpretation and the overall were 76.6％,70.3％,8.9％,50.0％ and 94.8％;chi-square test for the misuse or improperly use rate showed that submit time (x2 =19.86,P =0.001) was significant in research design,college (x2 =5.67,P =0.017)、supervisor title (x2 =7.59,P =0.006) and submit time (x2 =14.14,P =0.015) were significant in statistical description,college (x2 =8.75,P =0.003) was significant in result expression/interpretation,college,supervisor title,submit time and degree were not significant in statistical inference and the overall.Conclusions Misuse or improperly use rate is quite high in medical postgraduates' dissertations,education of medical statistics should be paid more attention to strengthen the statistical knowledge and skills of postgraduate.

Abstract:Objective To improve the quality of medical device clinical trial.Methods To analyze medical device clinical trials conducted in our hospital from 2006 to 2010,and proposed the strategy to strengthen medical device clinical trial management.Results Most of the medical devices clinical trials were class Ⅲ medical devices clinical validation in our hospital,Surgical departments undertook most of the clinical trials.Until July 2013,49 trials were done only 56％ of undertake the project.96％ of completed trials were approved for medical device registration certificate.Conclusions The quality of clinical trials must be improved,andthe quality control process in medical device clinical trial should be reinforced.

Abstract:Objective: By analyzing the utilization of major medical equipment in sample hospitals, this paper aims to provide recommendations for the appropriate use of major medical equipment in China. Methods: The sample inpatient medical records were obtained by field investigation, and were evaluated by experts according to the principle of clinical audit. Epidata3.1 was used for data input and SPSS16.0 for statistical analysis. Results: 86.47%, 90.49% and 97% of CT, MRI and DSA, respectively, were used appropriately. The overuse rate of CT was 4.23% while the underuse rate was 9.30%. MRI had an underuse rate of 8.89% while the overuse rate was 0.62%. All inappropriate cases for DSA usage were underuse. Logistic regression showed that the patients’ age and department had a certain influence on the appropriate use of equipment. Conclusion: In general, the usage of major medical equipment was appropriate and issues with the providers was an important reason for inappropriate use. Health administrative departments should make reasonable criteria for evaluating the appropriate use of major medical equipment.
Improvement of Medical Device "Made in China" Based on Ten-Year Data of the Shanghai Medical Device Market Survey

Abstract:Objective To promote the satisfaction of after-sales service and find out the gap between domestic and imported manufactures through the annual survey of after-sales service satisfaction and the market share of domestic medical equipment in Shanghai, and to provide reference for improving the market share of domestic medical equipment. Methods Shanghai medical device management and quality control center had carried out the survey on the mainstream medical device market in Shanghai since 2007, and analyzed the recent 10 year's data. Results In the past 10 years, the market share and service satisfaction of domestic medical equipment had been continuously improved. The average satisfaction score had risen from 3.4 to 4.2. In the 2016 market survey, the domestic brands achieved the top 5 spot results for the first time in the quality of after-sales service, and the market share of five domestic products reached 10%. Conclusion The market competitiveness of domestic medical equipment continues to improve. However, the market share of high-end medical equipment has not yet exceeded 15%. We still need to further tap the deep needs of users, master core technologies, improve product quality, and join hands in technological innovations to enhance the strength of "Made in China".
[Better medical devices regulations for better health care: enlightenment for medical devices regulatory reform in China, from experiences of the E.U. and the U.S.A]

Abstract:The expansion of applications of medical devices has attracted the increased attention of government regulatory bodies around the world to the safety and effectiveness of these products. Most developed countries, such as the United States and European Union, have developed well-established regulatory systems for medical devices, which have also consistently been amended to accommodate the changing requirements of safety and the trend of globalization.The current "Regulations for the Supervision and Administration of Medical Device (China)", established in 2000, has brought about great improvements for the safety and effectiveness of products, safeguarding public health. But there are still, at present, a lot of counterfeit and poor quality devices and device-related adverse events for lack of powerful post -market and in-use regulatory controls for products. It is therefore very urgent for the Chinese government to reform its medical device administration and management. This research paper analyses and compares the different requirements and executions of medical devices regulations in the EU, the US and China, to draw some experiences of the EU and US regimes that are very useful to China's regulatory reform. It is suggested that when developing a new scheme of medical devices regulatory reform in China, two prominent aspects have to be considered by policy makers and regulators. Firstly, the global trend of medical devices regulations has to be taken into account. Secondly, the experiences learned from the EU and US systems should be applied to the Chinese regulatory reform in combination with the concrete practice of China.

Abstract:Objective To introduce experience of major medical equipment management of foreign countries and provide reference for the perfection of major medical equipment management in China.Methods Information related to major medical equipment management were collected by browsing web sites of ministry of health or other in charge departments of several development countries and searching related information through Google Scholar search engine.Results The market access system of the Netherlands and Japan,etc,the monitoring and warning system for marketing equipments of EU,the recall mechanism for defective equipments of Canada,and the Certificate of Need System(CON) of the United States are all effective management experience for major medical equipment.Conclusion Management experience of market access system,marketing equipment monitoring and warning system,recall mechanism,as well as government's mandatory review and regulation of equipment configuration procedures can be the reference for the improvement of major medical equipment management in the new round of health reform in China.

Abstract:This article analyses the current situation and development of Chinese medical device testing institutes from the perspectives of the two most important functions - testing functions and medical device standardization functions. Areas Covered: The objective of the Chinese government regulations for medical device industry is to ensure the safety and effectiveness of medical devices for Chinese patients. To support the regulation system, the Chinese government has established medical device testing institutes at different levels for example, the national, provincial, and municipal levels. These testing institutes also play an important role in technical support during medical device premarket registration and post market surveillance, they are also the vital practitioners of Chinese medical device standardization. Expert Commentary: Chinese medical device testing institutes are technical departments established by government, and serve the regulatory functions of government agency. In recent years, with the rapid development of medical device industry as well as constantly increasing international and domestic medical device market, the importance of medical device testing institute is more prominent, However, there are still some problems unsolved, such as their overall capacity remains to be improved, construction of standardization is to be strengthened, etc.
