Abstract:Objective To investigate the diagnosis and treatment of pheochromocytomas in urinary bladder and review relative literatures.Methods The clinical data of 6 cases of bladder pheochroinocytoma were retrospectively analyzed.Bladder submucosal mass was detected by B-ultrasound in 6 cases, computerized tomography(CT)in 4,and cystoscopy in 3.All patients were treated with surgical operation. Results Two patients showed obvious fluctuation of blood pressure during operation.All patients were pathologically diagnosed as pheochroinocytoma postoperatively.Five patients were followed up {from 3 months to 7 years),and their blood pressure returned to normal.No patient had relapse and malignancy. Conclusion Typical hypertension during urination and hematuria constitute the chief symptoms of bladder pheochroinocytoma.B-ultrasound,CT,and cystoscopy are the main methods for the localization of the lesions.Partial cystectomy or excision of the tumor is the preferred management and postoperative long-term follow-up is necessary.

Abstract:Objective: To investigate oncological outcomes in patients with bladder cancer who underwent minimally invasive radical cystectomy (MIRC) or open radical cystectomy (ORC). Methods: We identified patients with bladder cancer who underwent radical cystectomy (RC) in 13 centers of the Chinese Bladder Cancer Consortium (CBCC). Perioperative outcomes were compared between MIRC and ORC. The influence of surgical approaches on overall survival (OS) and cancer-specific survival (CSS) in the entire study group and subgroups classified according to pathologic stage or lymph node (LN) status was assessed with the log-rank test. Multivariable Cox proportional hazard models were used to evaluate the association among OS, CSS and risk factors of interest. Results: Of 2 098 patients who underwent RC, 1 243 patients underwent MIRC (1 087 laparoscopic RC and 156 robotic-assisted RC, respectively), while 855 patients underwent ORC. No significant differences were noted in positive surgical margin rate and 90-day postoperative mortality rate. MIRC was associated with less estimated blood loss, more LN yield, higher rate of neobladder diversion, longer operative time, and longer length of hospital stay. There was no significant difference in OS and CSS according to surgical approaches (p=0.653, and 0.816, respectively). Subgroup analysis revealed that OS and CSS were not significantly different regardless of the status of extravesical involvement or LN involvement. Multivariable Cox regression analyses showed that the surgical approach was not a significant predictor of OS and CSS. Conclusions: Our study showed that MIRC was comparable to conventional ORC in terms of OS and CSS.

Abstract:Objective To investigate the clinical features,diagnosis and treatment of bladder pheochromocytoma.Methods The clinical data of 7 cases of bladder pheochromocytoma treated during April 2003 to April 2010 were retrospectively analyzed.The patients included 3 males and 4 females,with a mean age of 45.3(ranging 31~57) years.Hypertension was observed in 6 cases,of whom 5 had paroxysmal hypertension during micturition.Intermittent gross hematuria was founded in 3 cases.The positive rate of plasma MNs was 100.0%(7/7) in diagnosing bladder pheochromocytoma,and 24-hour urinary CA was 85.7%(6/7).B-Ultrasound,CT,MRI,131 I-MIBG and Cystoscopy were used to locate the tumor,with the positive rates of 85.7%(6/7),100.0%(6/6),66.7%(2/3),100.0%(5/5) and 75.0%(3/4) respectively.One case underwent radiotherapy,2 cases partial cystoscopy,and other 4 patients single excision of the tumor.Results Of the 7 cases,2 tumors were located in the anterior wall of the bladder,2 in the trigones,1 in the posterior wall,1 in the left lateral wall and 1 in the right lateral wall.The mean operation time was 131.2 min,ranging 110~190 min.The average volume of blood loss was 283 mL,ranging 100~1200 mL.One patient was diagnosed as malignant tumor,and the other 6 cases benign ones.During the follow-up of 5 months to 7 years,one patient suffered from recurrent tumor 3 years after initial operation and radical cystectomy was performed in the secondary surgery.Conclusions Pheochromocytoma of the urinary bladder is a rare neuroendocrine tumor which is hard to be accurately diagnosed.The typical symptoms include hypertension,hematuria,paroxysmal occurrence during urination and so on.Plasma CA and 24-hour urinary MNs are useful qualitative examinations.B-Ultrasound,CT,MRI,131I-MIBG and cystoscopy are useful to locate the lesions.Partial cystectomy or excision of the tumor is the preferred management and postoperative follow-up is indispensable.
Extra-adrenal Pheochromocytoma: Report of 34 Cases

Abstract:Objective To review the diagnosis and treatment of extra-adrenal pheochromocytoma with a 34cases report. Methods Thirty-four cases of extra-adrenal pheochromocytoma were retrospectively analyzed. Hypertension was observed in 27 cases. Abdominal pain was seen in 10 patients and intermittent hematuria in 2 patients. Serum and urinary catecholamine and urinary VMA were measured in 34 cases. The level of serum or urinary catecholamine elevated in 20 cases and urine VMA elevated in 24 cases. Thirty-four cases had ultrasound examination,25 cases underwent CT scan and 6 cases underwent MER scan.Results Pheochromocytomas of 12 cases were located in the renal hilum, 2 in the lower pole of the left kidney, 1 in the posterior aspect of the inferior vena cava, 3 in the interaortocaval region, 2 in the anterior aspect of the abdominal aorta, 1 in the anterior of the right common iliac artery, 1 in the hilum of the liver, 1 in the posterior o{ the pancreas, 2 in the bladder wall, 1 in the posterior of the descending colon, and 8 cases of multifoci. Twenty-two cases of extraadrenal pheochromocytoma were benign and 12 cases were malignant. Thirty cases were followed up from 6 months to 13 years. Among 27 cases with hypertension, the blood pressure of 22 patients returned to normal and 5 cases were still hypertensive. Nine cases of malignant pheochromocytoma all had tumor recurrence or metastases at one year postoperatively. Six patients died during followed-up from 6 months to 3 years, including 3 cases died of cerebral hemorrhage and 3 cases of tumor metastases. Three cases got stable with 131Ⅰ-MIBG radiotheraphy.Conclusions The accurate detecting extra-adrenal pheochromocytoma is difficult. CT scan could be reliable in localizing the lesions. Surgical resection of the tumor could be the best therapy. Patients of malignant extra-adrenal pheochromocytoma may be treated with 131Ⅰ-MIBG after surgical therapy.

Abstract:Pheochromocytoma of the urinary bladder is often misdiagnosed as it is a rare tumor. In this report, we described a case with primary pheochromocytoma of the urinary bladder. We specifically conversed the diagnostic role of X-ray computed tomography and sonography to identify the location of tumor within urinary bladder compared to other malignant or benign tumors in the bladder, and exclude other ectopic pheochromocytoma. Histopathological report from bladder tissue biopsy was confirmative of extra adrenal pheochromocytoma of the urinary bladder finally. Importance in careful management of hypertensive crisis during cystoscope and partial cystectomy was addressed.

Abstract:Objective: To study the diagnosis and treatment of pheochromocytoma in urinary bladder. Methods: Six cases of bladder pheochromocytoma were studied. Four cases showed hypertension, 3 of which were paroxysmal hypertension during urination. Catecholamine (CA) was increased in a case, and vanillymandelic acid (VMA) was increased in 2 cases. Bladder sub- mucosal mass was detected by B-ultrasound in 5 cases (5/5), computerized tomography (CT) in 3 cases (3/3), cystoscopy in 5 cases (5/6). Four cases took α-receptor blocker for 2 weeks, 1 case took β-receptor blocker to decrease heart rate. All patients were treated with surgical operation including 4 partial cystectomies, 2 excavations. Results: Three cases had manifestations including headache, excessive perspiration and hypertension during cystoscopy. Four cases were confirmed before operation. Two cases showed hypertension during operation. All patients were pathologically diagnosed as pheochromocytoma post- operatively. In five cases followed up, blood pressure returned to normal. No patient had relapse and malignancy. Conclusions: Typical hypertension during urination comprised the main symptoms. We should highly suspect bladder pheochromocytoma if a submucosal mass was discovered with B-ultrasound, CT, 131I-MIBG (methyliodobenzylguanidine) and cystoscopy. The deter- mination of CA in urine is valuable for qualitative diagnosis. The preoperative management of controlling blood pressure and expansion of the blood volume are very important. Surgical operation is a good method for effective treatment. Postoperative long-time followed up is necessary.

Abstract:Objective:To summarize experience of the surgical and peri-operative treatment of pheochromocytoma,and to improve diagnostic and therapeutical levels. Methods:Retrospective analysis of 83 cases from 2004 to 2008.Total of 83 patients,47 cases of male,36 cases of female,age 28 to 79 years old,average age 49 ± 12 years old.Before the operation there were 75 patients(90.4%)who had symptom of high blood pressure,and 8 patients(9.6%) who hadn′t. Results:Fifteen cases of preoperative VMA test were normal,and 68 cases higher than normal range.Tumor diameter by CT examination varied from 3cm to 16.7cm.And the average value was 9.9 ± 6.9 cm.Before the surgery,all patients were treated with fluid expansion and vasodilative drugs. Medication time remained 3 days to 2 weeks according to the preoperative blood pressure. Intraoperative blood pressure fluctuations were significantly (>50 mmHg) in most cases. Blood pressure remained stable after the operation.And there was no case of perioperative death,no case of serious postoperative complications.All cases were diagnosed as pheochromocytoma by pathological examination,in which eight cases (9.6%) were malignant pheochromocytoma.Preoperative abnormal urinary VMA of 68 cases returned to normal value. Conclusion:On the premise of full preparation,the surgical treatment of pheochromocytoma is safe.Perioperative treatment of pheochromocytoma is crucial.
Comparsion between laparoscopic and open resection in pheochromocytoma：a review of 58 cases

Abstract:Objective We conducted a retrospective cohort study to determine whether laparoscopic resection of pheochromocytoma is a safe and effective therapy for the management of pheochromocytoma, compared with open resection. Methods We collected pertinent data on 58 patients underwent pheochromocytoma resections. Perioperative outcomes of 41 laparoscopic resections of pheochromocytoma were compared with 17 open resections. Results Time of postoperative stay was shorter in the laparoscopic group [(9±2) d vs (12±2) d, P<0.05)], and tumour size was smaller [(34±53) cm3 vs (197±397) cm3, P<0.05)]. The operation time was also shorter in laparoscopic group [(162±56) min vs (213±44) min, P<0.05)], the blood loss was less [(158±402) ml vs (1081±825) ml, P<0.05)]. There were no statistically significant differences in intraoperative hypertensive episodes, the need for antihypertensive and vasopressin agents between the two group. The number of blood transfusion and postoperative complications in laparoscopic group were 7 case and 1 case while there were 12 case and 4 case in open group. During the follow-up of 5 to 47 months, there were no tumor recurrences or metastasis. There were no perioperative mortalities in this series. Conclusions The laparoscopic removal of pheochromocytoma is safe and effective. And also can be used for ectopic pheochromocytoma.

Abstract:Bladder leiomyosarcoma is a unique mesenchymal tumour,accounting for less than 0.5% of all primary bladder malignancies.Bladder leiomyosarcoma used to be treated with radical surgeryin either old or young patients, often resulting in significantimpact on the patients’s quality of life after surgery. We reporton a case of bladder leiomyosarcoma in a 31-year-old femalewho was treated with partial cystectomy. Fortunately, no tumourmetastasis or relapse was observed during the 7-year follow-upperiod and the patient now has a good quality of life. We foundthat partial cystectomy may be an acceptable option to treat bladderleiomyosarcoma in the low MSKCC (Memorial Sloan-KetteringCancer Center) stage.
A Comparison of Laparoscopic and Open Adrenalectomy for Pheochromocytoma

Abstract:Purpose To evaluate the feasibility of peritoneal laparoscopic excision of adrenal pheochromocytoma. Methods Peritoneal laparoscopic excision of adrenal pheochromocytoma was performed on 15 patients, the control group included 42 patients with adrenal pheochromocytoma who underwent open surgery.The postoperative outcomes were compared between the 2 groups. Results Fourteen peritoneal laparoscopic excisions of pheochromocytoma were successful, only 1 was converted to open surgery due to intraoperative bleeding. The mean largest dimension of the tumor was (3.4±1.8) cm (range,1.5 to 7.0) in laparoscopic surgery group vs (3.8±2.2) (1.5-10.5) cm in open surgery group (P0.05). The mean operative time was (94±34) min (range,75 to 170) in laparoscopic surgery group vs (131±38) (80-250) min in open surgery group (P0.01).The mean volume of blood loss was (45±36) (10-600) mL in laparoscopic surgery group vs (198±108) (50-2 000) mL in open surgery group (P0.01).Postoperative hospital stay was (6.2±1.1) (5-8) days in laparoscopic surgery group vs (9.3±1.3) (7-14) days in open surgery group (P0.01). All cases have been followed up 9 months to 3 years. Tumor recurrence was found in 1 case in open surgery group. Conclusions Based upon the experience and skills of the surgeons, peritoneal laparoscopic surgery for pheochromocytoma is feasible and safe, with no increase in the operation risks, and can be considered the preferred choice for pheochromocytoma.
Diagnosis and surgical treatment of small adrenal pheochromocytoma

Abstract:Objective To investigate the clinical features , diagnosis and key technique points of laparoscopic partial adrenalectomy for small adrenal pheochromocytoma . Methods From Oct.2006 to Jun. 2011, clinical data of 32 cases with small adrenal pheochromocytoma (≤3.0 cm) were collected and retro-spectively analyzed .Hypertension was observed in 12 patients, whereas 20 patients presented with adrenal incidentaloma .Thirteen patients had a left adrenal neoplasm , eighteen patients had a right adrenal tumor , while one patients had bilateral tumors .The positive rate of plasma-free metanephrines ( MNs) and 24-hours urinary catecholamine (CA) in diagnosing small renal pheochromocytomas was 92.6%(25/27) and 81.3%(26/32) respectively.The main localization diagnosis included ultrasonography , 131I-MIBG, and CT or MRI, with positive rates of 71.9%(23/32), 93.8%(15/16) and 96.9%(31/32) respectively.All the laparoscopic adrenalectomies were performed retroperitoneally .During the surgery , the internal part of the adrenal gland closing to the retroperitoneum was dissected first , and the whole adrenal gland was resected completely. Results Partial adrenalectomy was performed for 30 cases and radical adrenalectomy for 2 ca-ses.All operations were successful without perioperative or postoperative complications .The maximum diame-ter of tumor was 1.7±0.2 (1.0-3.0) cm.Histopathological results showed that all the cases were benign pheochromocytoma.The operative time was 82 (40-210) min.The estimated blood loss was 57 (20-180) ml.No patient required blood transfusion . Conclusions Plasma-free MNs, 24-hours urinary CA and VMA are important qualitative examinations in detection of adrenal pheochromocytoma .Ultrasonography , CT,MRI, and 131 I-MIBG are important in the localization of adrenal tumors .Retroperitoneal laparascopic partial adrenalectomy is the preferred choice in the management of small adrenal pheochromocytoma .Dissecting the internal part of the adrenal gland closing to the retroperitoneum first and exploring the whole adrenal tissue are the key technique points during the operations .
Application of partial cystectomy in bladder urothelial carcinoma

Abstract:Bladder urothelial carcinoma is one of the common urological tumors.For muscle invasive bladder cancer,radical cystectomy and chemotherapy is the current standard treatment.Radical cystectomy is the most complicated urological surgery,with a high risk of complications and secondary surgery,which also affected the quality of life after surgery.Partial cystectomy has been used as a once common surgery.Early a part of retrospective studies have caused a high recurrence rate because some patients were not suitable for partial cystectomy.If the surgical indications can be strictly controlled,the postoperative recurrence rate can be controlled.At present,the commonly used partial cystectomy includes laparoscopic partial cystectomy,robotic partial cystectomy and open partial cystectomy.Neoadjuvant therapy and adjuvant therapy are also the development direction of partial cystectomy.Partial cystectomy is an option for some patients who plan to perform radical cystectomy.This review will summarize the research progress of partial cystectomy for bladder urothelial carcinoma.
Clinical application of laparoscopic radical cystectomy

Abstract:Objective To evaluate the surgical technique and clinical efficacy of laparoscopic radical cystectomy for bladder cancer.Methods We analysed 28 cases of bladder cancer,with age ranging from 58 to 73 years(mean 68.5 years).The pathological reports of 26 cases showed transitional cell carcinoma Ⅱ-Ⅲ grads;and 2 cases were squamous carcinoma.We performed laparoscopic radical cystectomy on 15 cases plus reconstruction of bladder with ileum,on 10 cases plus rectum pouch,and on 3 cases plus ureterostomy,and we watched the operation time,the blood loss,the intestine function and the complication.Results All operations were successful,with the operation time being 7-10 hours,the blood loss 600-1 500 mL,mean 850 mL.The intestine function recovered about 72hrs after operation.All ureteral catheters were removed 2 weeks after surgery.Urethral catheter was removed 3 weeks after surgery,the patients were able to normosthenuria by abdominal pressure,and the IVU were normonal.Conclusion Laparoscopic radical cystectomy for invasive bladder cancer is a safe and efficient method,with advantages of good operating field,miniinvasion,less celiac complications,short hospital stay and rapid recovery.It's worth to be widespread.
Pure laparoscopic radical cystectomy with orthotopic ileal neobladder: Report of 15 cases

Abstract:Objective To investigate the clinical feasibility of pure laparoscopic radical cystectomy with orthotopic ileal neobladder and summarize the surgical techniques. Methods Fifteen patients with bladder cancer underwent laparoscopic radical cystectomy with orthotopic ileal neobladder. Results Operation time was 341±47.3(275-440) minutes and the blood loss was 740±336(300-1400) ml. Postoperative pathologic results confirmed that all patients had bladder transitional cell carcinoma. The number of lymph nodes was 16±4.4(10-27). No positive surgical margin was found in all patients. The recovery time of intestinal function was 3.8±0.9(3-6) days. The single-J catheters were removed two weeks after operation. No severe complication occurred except for one case of intestinal fistula. The postoperative hospitalization time was 25±4.1 (21-33) days. Patients were followed up for 6-15 months, within which no recurrence or metastasis was found. Conclusions Pure laparoscopic radical cystectomy with orthotopic ileal neobladder can reduce surgical trauma, expose the anatomical structure more clearly and help patients postoperative recovery. Due to the difficulty of the surgical procedures, the surgeon must have mastered laparoscopic technique.
Application of Partial Prostate Capsule Sparing in Radical Cystectomyand In Situ Ileal Neobladder for Bladder Cancer

Abstract:Objective:To compare the effects of radical removal of the bladder and modified cystectomy with partial prostate capsule sparing, and to examine the standard procedure of orthotopic bladder replacement for bladder cancer. Methods:Data of 45 cases with radical removal of the bladder because of bladder cancer, during a period between January 2000 and January 2006, were collected. All patients were male, with a mean age of 59 years. Among the cases, traditional radical removal of the total bladder was conducted in 26 cases (traditional treatment group, TT group) and in situ ileal cystectomy with partial prostate capsule sparing was conducted in 19 cases (modified treatment group, MT group). The surgical results and quality of life were analyzed. Results:The results of pathological report were as follows:T2aN0M0 was found in 26 cases, T2bN0M0 was found in 16 cases, and T3aN0M0 was found in 3 cases. The mean follow-up period of the patients was 39 months. There were 19 cases with disease-free survival in the MT group, and there were 23 in the TT group. Metastasis was observed in 1 case and mortality in 2. In the MT and the TT group, the rate of urinary continence during the day was 100% and 73%, and that during the night was 10% and 50%, respectively. Conservation of erectile function occurred in 81% and 14.3% of the cases in the two groups, respectively. In all patients, capacity of the urinary vesicle ranged from 350 to 480 ml, filling pressure was 13 to 25 cm H2O, and the residual urinary volume was 16 to 60 ml. Conclusion:Radical cystectomy with partial prostatic capsule sparing and orthotopic neobladder in selected patients can improve sexual function and urinary continence.

Abstract:Objective To evaluate the surgical technique and clinical efficacy of laparoscopic radical total cystec- tomy plus reconstruction of bladder with ileum for bladder cancer.Methods:We analysed 12 cases of bladder canc- er,with age from 58 to 73 years(mean 68.5 years),pathological reports were transitional cell carcinomaⅡ～Ⅲgrads,2 cases were squamous carcinoma.We perform laparoseopie radical total cysteetomy plus reconstruction of bladder with ileum,watched the operation time,the blood loss,the intestine function and the complication.Results: All operation were successful,the operation time was 7～10 hours,the blood loss was 600～1500 ml,mean 850ml, the intestine function recovered about 72 hours after operation,all ureteral catheter were removed at 2 weeks after surgery,urethral catheters were removed at 3 weeks after surgery and normosthenuria by abdominal pressure,theⅣU were normonal.Conclusions:I.aparoscopic radical total cystectomy plus reconstruction of bladder with ileum for invasive bladder cancer is a safe,efficient method,with advantages of good operating field,reliable hemosta- sis,mini-invasion,less celiac complications,hospital stay and rapid recovery,worth to be extended.
Preoperative 3D Planning and Intraoperative Navigation in Robot-Assisted Laparoscopic Surgery for Complex Pheochromocytoma

Abstract:Objective: To evaluate the value of preoperative 3D planning and intraoperative navigation in robot-assisted laparoscopic surgery for complex pheochromocytoma. Study design: Observational study. Place and Duration of the Study: Department of Urology, The Affiliated Hospital of Hebei University, from July 2022 to July 2023. Methodology: Sixty patients with complex pheochromocytoma were divided into the study group (n = 30) and the control group (n = 30) according to the treatment method. The study group was treated with robot-assisted laparoscopic surgery, with preoperative 3D printing-based planning, and intraoperative 3D navigation, and the control group underwent conventional retroperitoneal robot-assisted laparoscopic surgery. Surgical-related indicators and intraoperative and postoperative complication rates between the two groups were compared. Results: The operation time, postoperative hospital stay, and drainage tube retention time in the study group were significantly shorter than in the control group (p <0.001), and the intraoperative blood loss in the study group was significantly less than in the control group (p <0.001). The study group exhibited a significantly lower incidence of intraoperative haemodynamic instability (HI), use of vasoactive drugs, and postoperative ICU transfer rate than the control group (p <0.05). The incidence of surgical complications in the study group was significantly lower than in the control group (p = 0.04). The non-recurrence rate in the study and control groups after 1-year follow-up was 96.7% and 90%, respectively (χ2 = 2.77, p = 0.10). Conclusion: Robot-assisted surgery, performed through renal surface, has the advantages of less bleeding, shorter surgical time, faster recovery, fewer complications related to chromaffin cell inflammation and postoperative complications, and is safe and effective in treating complex pheochromocytoma. Surgical treatment of 192 cases with pheochromocytoma/paraganglion

Abstract:Objectives To evaluate the surgical treatment of pheochromocytoma/ paraganglion.Methods 167 cases of adrenal pheochromocytoma and 25 cases of paraganglion surgical treated between from January 2003 to October 2011 were retrospective analyzed.Hypertension was observed in 141 cases.The positive rate of plasmafree MNs and 24-hour urine CA was 92.8％ (142/153) and 90.1％ (137/152) respectively.The accuracy rate of location of B-Ultrasound,CT,MRI and 1311-MIBG was 83.9％ (125/149) 、95.5 ％ (106/111) 、98.1％ (53/54) 和 73.1％ (71/97).Results All of patients received operations.18 patients were treated with laparoscope.For 149 cases of patients,108 cases were chosen by lumbodorsal incision,4 by abdominothoracic incision,30 by superior abdominal L-shape incision,and 7 by inverse splayed incision.25 cases of paraganglion were treated with open surgery.8 cases were chosen by superior median abdominal incision,5 by inferior median abdominal incision,and 12 by superior abdominal L-shape incision.All specimens were proved to be paraganglion.All patients have been followed up for 7 months to 8 years,during which 11 cases had tumor recurrence and 9 underwent re-operation.5 cases died postoperatively.Conclusions Surgical treatment of pheochromocytoma/paraganglion depends on tumor's size,location and relation with the surrounding tissues.Ultrasonography,CT,MRI and 1311-MIBG can be used to formulate the surgical proposed and approach.The sufficient preparations and proper homeostasis during the operation are the keys of the success of the surgery.
Diagnosis and treatment of pheochromocytoma: a single center case report of 142 cases

Abstract:Objective To review the experience in diagnosis and treatment of pheochromocytoma in a single center. Methods A total number of 142/145 pheochromocytoma cases treated surgically in our institute from August 2002 to February 2010 were retrospectively reviewed. The mean diameter of tumor was 5.9 cm (1.3- 18. 0 cm). The majority of the tumors (92.9%) were adrenal pheochromocytomas. Ninety-eight patients (69.0 % ) presented initially with hypertension, whereas 44 patients (31%)presented with adrenal incidentaloma. A specific anti-hypertensive pre-surgery preparation with phenoxybenzamine or doxazosine mesylate was started over 1 week before the operation.Of the 142 patients, 91 accepted open surgery, 54 accepted laparoscopic surgery, of which, 5 converted from laparoscopic surgery to open surgery. Results Histopathological results showed that all the cases were pheochromocytoma, while 83 cases were benign, 23 cases were malignant and 37 cases were suspected malignant. Sudden rising of blood pressure during operation was related to the preoperative serum level of catecholamine. Eighty-seven of 98 patients with preoperative hypertension had normal postoperative blood pressure; the remaining 11 patients reduced the dosage of anti-hypertension medication postoperatively. During the follow-up of 3-96 months (median 46 months), 10 of 91patients had a recurrence or metastasis. Six patients died of recurrences or metastasis within 5 years.Conclusions The procedures of qualitative and locative diagnosis of phechromocytoma include clinical manifestations, biochemical tests and imaging investigation. Surgical excision is the fundamental treatment for cure. Patients with high serum level of catecholamine tend to have a sudden rising of blood pressure during operation. Preoperative management is extremely important for the safety of the patient. Intensive follow up is necessary.
Pediatric laparoscopic partial cystectomy for the treatment of benign bladder tumors and urachal cysts

Abstract:Objective: To investigate the feasibility and efficacy of carrying out pediatric laparoscopic partial cystectomies (LPC) when treating benign bladder tumors and urachal cysts. Methods: Retrospectivey analyzing 4 clinical cases involving children with bladder tumors, which were collected from October 2017 to December 2018. In these clinical cases, there were 3 male children and 1 female child, aged from 4.5 to 9.4 years old, with an average age of 6.5 years. An intraperitoneal laparoscopic partial cystectomy was performed in the treatment of 3 of these patients with benign bladder tumors and in 1 patient with an urachal cyst. The surgical procedures included a partial cystectomy and a complete intracavitary bladder suture. Results: All 4 cases were successful and no operation was transferred to opensurgery. The operation time was 100-120 min, with an average time of 108 min. The intraoperative blood loss was 10-20 ml, with an average loss of 15 ml. 6 h after the operation, the patients still maintained a fluid diet and 1 case of hematuria had occurred, with the catheter removed 12 days after the operation. No postoperative urine leakage, intestinal adhesion or intestinal obstruction occurred, and the average postoperative hospitalization time was 14 days. Conclusion: Laparoscopic partial cystectomy is a safe and feasible method to be used for the treatment of benign bladder tumors and urachal cysts. It presents the advantages of being minimally invasive, and having a quick recovery and short hospitalization time. It is an alternative surgical method for the treatment of pediatric benign bladder tumors.
