A Follow-Up Study of Bowel-Control and Anorectal Manometry after Endorectal Pull-Through(soave)procedure for Hirschsprung's Disease

Abstract:Objective To appraise the bowel movement and anal continence in patients with Hirschsprung’s Disease(HD) after endorectal pull-through(Soave) procedures. Methods 63 patients(aged 4 days to 12 year) with HD underwent endorectal pull-through(Soave) procedure. Patients were followed-up with a period of 6 months to 4.5 years and the operative related complications including constipation, continence and anorectal manometry. Results Forty-seven patients completed the study with a mean follow-up. Among all the 47 patients, the assessment of anal continence was graded as good in 15, fair in 26 and bad 6. Two patients developed continuous constipation and eight patients had postoperative enteroclitis. Rectoanal inhibitory reflex(RAIR)were recovered in two patients. The rectal persure in the 47 followed-up patients had no different when compared with normal group, but the anal canal high pressrue zone length of the 6 patients whose anal continence graded as bad was shorten. Conclusions Endorectal pull-through (Soave) procedure performed on infants with Hirschsprung’s Disease can get a good bowel control outcome. Stooling disorders in some cases are probably related to injury of internal anal sphincter. Anorectal manometry may be one of the most important method in appraise the stooling patterns and sphincter function.
