Detection of cognitive impairment with multiple sclerosis and it's influencing factors

Abstract:Objective To understand the characteristics of cognitive impairment with multiple sclerosis(MS),and further explore the affecting factors of cognitive impairment.MethodsRepeatable battery for the assessment of neuropsychological status(RBANS) were employed to assess cognitive function,Self-rating depression scale(SDS)and self-rating anxiety scale(SAS) were employed to assess mood status in 27 MS patients and 27 normal controls.Expanded disability status of the disability(EDSS) was employed to evaluate clinical disablement in patients' group.Two-tailed testing was used for analysis of two-sample test,Mann-Whitney U test was used for analysis of non-parametric statistical hypothesis,and Pearson correlation coefficient was used for analysis of correlation between the samples.ResultsCompared with control group,RBANS scores were significantly reduced in MS group(74.7 ± 17.6 and 98.0 ± 15.5,P = 0.0013).Reduced items included immediate memory,Visuospatial,Language,attention and delayed memory(P values were 0.0056,0.0001,0.0040,0.0008 and 0.0001 respectively).Analysis of the RBANS scores suggested that patients in low score group had a higher EDSS scores(5.3 ± 1.5,P < 0.001) and a high proportion of SPMS type(6/7),while patients in high score group had a lower EDSS scores(2.2 ± 2.1,P < 0.001) and a high proportion of RRMS type(12/13,P < 0.05).Correlation studies showed that RBANS score was correlated with EDSS,level of education,and SDS(Pearson correlation coefficient are-0.61,-0.42 and 0.54,P < 0.01),but not correlated with with gender,age,disease duration,relapse frequency,SAS score and the lesion sites of involvement in T2WI.ConclusionsThe severity of cognitive impairment may be related to the clinical classification of MS,physical disability status and Mood disorders.
