Diagnosis of fetal cardiac and pulmonary abnormalities by MRI

Abstract:Objective To explore the diagnostic value of MRI in fetal cardiac and pulmonary abnormalities. Methods Thirty pregnant women, aged 21 - 43 years, mean age 28 years, with gestational age from 16 to 36 weeks(mean 27 weeks) were studied with a 1.5 T superconductive MR unit within 24 to 48 hours after ultrasound(US) studies. The imaging protocol included 2-dimensional fast imaging employing steady-state acquisition(2D FIESTA), single-shot fast spin echo (SSFSE) and not-gated real-time steadystate acquisition sequences in the axial, frontal, and sagittal planes relative to the fetal brain, thorax, abdomen, especially hearts and lungs. Prenatal US and MRI findings were compared with postnatal diagnoses(16 fetuses), autopsy or dead body image(14 fetuses). Postnatal evaluations included US, MRI, computed tomography and surgery. Results In 30 fetuses with cardiac and pulmonary abnormalities, there were abnormalities of lungs(n = 20) and hearts(n = 10). For all cases, the diagnosis established by MRI were correct when compared with postnatal diagnosis or autopsy. There were 3 cases misdiagnosis by prenatal US examination. In 2 cases, MRI correctly provided additional information to the US-determined diagnosis. Conclusion Prenatal MRI is effective in the assessment of fetal pulmonary abnormalities. It could be better than US in diagnosing and identifying fetal pulmonary abnormalities and offer information additional to that provided with US. It now can offer diagnostic information in fetal cardiac anomalies, but not better than US. The MRI and US in the prenatal diagnosis may improve diagnostic accuracy of fetal cardiac and pulmonary abnormalities.
