Abstract:The health information systems of most medical institutions in China are isolated. Communications across these systems are generally realized through point-to-point interfaces at the database level, which tend to lack interoperability, extensibility, and security. In the resent study, we developed localized, document-oriented and data-focused clinical document architecture (CDA) templates based on health level 7 (HL7) CDA. Then, by combining these templates with the Service-oriented architecture for HL7 middleware, we accomplished interoperability across multiple heterogeneous systems. Modules of our system have been put into trial use in six medical institutions, including the Second University Hospital (main campus and regional campuses) of the School of Medicine, Zhejiang University and other collaborating hospitals.

Abstract:DICOM-e-mail is a secure teleradiology protocol released by German Radiology Society and has already been applied clinically. To improve the robustness of the system, the protocol has been upgraded by adding some mechanisms such as system feedback. Moreover, a test method is also implemented by sending erroneous mails to application software that supports DICOM-e-mail. Through the tests for two different DICOM-e-mail teleradiology application, the validity of the new protocol is proved and at the same time some bugs are found. The implementation of the latest protocol and the general error tests can help to make the whole teleradiology system a better robustness. The implementation of DICOM-e-mail protocol in our country is also discussed in this paper.

Abstract:With the pace of China's New Medical Reform, medical information systems have been rapidly developing, thus generating tremendous clinical data that contain large amount of information. Clinical data exchange is one of the most important foundations to make full use of clinical data. Under the whole background of Global Dolphin (an international data exchange system between China and Japan), this paper demonstrates a method of achieving standardized clinical data exchange based on Ensemble Integration Platform and using Health Level Seven International (HL7) as the data exchange standard. HL7 helps to standardize the data exchange process; Ensemble Platform contributes to the loose coupling of medical information systems and constructing a unified data management layer in logic, ultimately achieving the goal of sharing clinical data between a data center and medical institutions. ? Abstract:Nowadays, medical environment is integrated and complicated, involving large amounts of various medical data, such as images, waveforms and other digital data. For the interoperability of images and waveforms in imaging context, the images and waveforms usually need to be interchanged and stored using one standard. DICOM is the best choice, which is an international standard for the communication and storage of medical information. In this paper, we developed a DICOM middleware with capability of converting SCP-ECG, the European standard for resting ECGs, into DICOM ECGs. Then an ECG viewing component is implemented, which can parse and display SCP-ECG records and DICOM ECGs. The research results show that our work can realize seamless workflows in multi-vendor environment, contribute to the harmonization of ECG standards, and facilitate digital ECG applications

Abstract:It is widely accepted that the high quality healthcare services lie in the effectiveness and efficiency of health problem detection, innovative solution identification, and medical resource allocation, which in turn depend heavily on the proper collection, management and utilization of health information. As the main carrier of the health information, Electronic Medical Record (EMR) is a kind of electronic patient record that resides in a system specifically designed to support users through availability of complete and accurate data, practitioner reminders and alerts, clinical decision support systems, links to bodies of medical knowledge and other aids, which has experienced a booming development in China in last 10 years. Ministry of Public Health of China paid attention to EMR and a lot of laws and regulations were promulgated in the following few years. EMR systems in China faced many problems: effective quality control system, professional EMR system development and maintain talents, the costs and the relevant legal issues. But because EMR systems could improve care quality, safety, efficiency, and reduce health disparities, they are sure to evoke a radical revolution of clinical work in China.

Quantitative Imaging in Medicine and Surgery

Abstract:Following tremendous economic progress, society in China is also undergoing fundamental changes, as is the healthcare system. Currently the training of Chinese young doctors and their future work placement are all undergoing re-structuring. We compiled some thoughts and opinions on the topic of 'should clinicians in China engage in research?', and publish them as a special report in this issue of Quantitative Imaging in Medicine and Surgery (QIMS). The contributors included some editorial members of this journal, and a few personal friends. Besides a few minor linguistic corrections, opinions from the contributors have not been edited, as we want authors' to write their own independent views. However, it is possible there is a selection bias of the contributors of this paper; more likely those who are interested in the medical research are selected and therefore the views of the contributors may not be generalizable. To compare the structure and funding of China with other countries, authors from UK, The Netherlands, France, and USA are also invited.

Abstract:Online peer to peer medical consultation between doctors such as physicians and specialists in China has a broad market demand and has been continuously accepted. For some difficult diseases, electronic medical records with medical images are required to present to both sides at same time during the consultation so that both sides can manipulate the records interactively to understand the medical meanings of the records, especially images. Here, we presented design of a teleconsultation system integrated with a cloud-based collaborative image sharing network to provide online peer-to-peer medical consultation for difficult cases with multi-media medical records including DICOM images. The presented teleconsultation system provides bidirectional interactive manipulations on images presented to peer-to-peer sides and has been used for small lung nodule diagnosis services between Huadong hospital in Shanghai and Jiaxing First Hospital in Zhejiang Province through Internet.

Abstract:The enormous number of studies performed at the Nuclear Medicine Department of University of Arkansas for Medical Sciences (UAMS) generates a huge amount PET/CT images daily. A DICOM workstation had been used as "mini-PACS" to route all studies, which is historically proven to be slow due to various reasons. However, replacing the workstation with a commercial PACS server is not only cost inefficient; and more often, the PACS vendors are reluctant to take responsibility for the final integration of these components. Therefore, in this paper, we propose an alternative imaging archiving and communication system called Enterprise Imaging Repository (EIR). EIR consists of two distinguished components: an image processing daemon and a user friendly web interface. EIR not only reduces the overall waiting time of transferring a study from the modalities to radiologists' workstations, but also provides a more preferable presentation.

Abstract:AIM: To address issues in interoperability between different fundus image systems, we proposed a web eyepicture archiving and communication system (PACS) framework in conformance with digital imaging and communication in medicine (DICOM) and health level 7 (HL7) protocol to realize fundus images and reports sharing and communication through internet.METHODS: Firstly, a telemedicine-based eye care work flow was established based on integrating the healthcare enterprise (IHE) Eye Care technical framework. Then, a browser/server architecture eye-PACS system was established in conformance with the web access to DICOM persistent object (WADO) protocol, which contains three tiers.RESULTS: In any client system installed with web browser, clinicians could log in the eye-PACS to observe fundus images and reports. Multipurpose internet mail extensions (MIME) type of a structured report is saved as pdf/html with reference link to relevant fundus image using the WADO syntax could provide enough information for clinicians. Some functions provided by open-source Oviyam could be used to query, zoom, move, measure, view DICOM fundus images.CONCLUSION: Such web eye-PACS in compliance to WADO protocol could be used to store and communicate fundus images and reports, therefore is of great significance for teleophthalmology.

Abstract:The biomedical industry is developing at an unprecedented speed and the establishment and improvement of a digital infrastructure for biomedical research and development (R&D) is crucial for biotechnology innovation and Health China construction. This study analyzes the implication, classification, and systematic framework of the digital infrastructure for biomedical R&D in the new era. It summarizes the strategic layouts and development paths of the digital infrastructures for biomedical R&D in the United States, the European Union, and China, and examines the gap between China and European and American countries in terms of international influence and authority, development model design and efficiency, sustainability, and management organization construction. Moreover, it presents the challenges faced by the digital infrastructure of China regarding top-level design, management, and governance. Furthermore, we suggest that the following aspects should be strengthened: top-level framework design, coordination of biomedical and healthcare resources, clustered construction and governance of the platform, and capacity building and operationguarantee of the open platform.

Abstract:Image acquisition plays an important role in PACS. In this paper, we compare some currently used methods of image acquisition, and select the acquisition method through DICOM interface in network environment. After describing the concepts of DICOM network and the DICOM message exchange, we summarize the key technique for DICOM image acquisition.Our software is introduced as one solution for image acquisition from digital medical imaging systems. The software is written in Windows Socket API and provides such services as verification, storage, query and retrieve defined by DICOM standard, and acts as the acquisition server for images sent from imaging equipment. Up to now, our software works well with different equipment under different conditions.

Abstract:The digital imaging and communications in medicine (DICOM) protocol is the leading standard for image data management in healthcare. Imaging biomarkers and image-based surrogate endpoints in clinical trials and medical registries require DICOM viewer software with advanced functionality for visualization and interfaces for integration. In this paper, a comprehensive evaluation of 28 DICOM viewers is performed. The evaluation criteria are obtained from application scenarios in clinical research rather than patient care. They include (i) platform, (ii) interface, (iii) support, (iv) two-dimensional (2D), and (v) three-dimensional (3D) viewing. On the average, 4.48 and 1.43 of overall 8 2D and 5 3D image viewing criteria are satisfied, respectively. Suitable DICOM interfaces for central viewing in hospitals are provided by GingkoCADx, MIPAV, and OsiriX Lite. The viewers ImageJ, MicroView, MIPAV, and OsiriX Lite offer all included 3D-rendering features for advanced viewing. Interfaces needed for decentral viewing in web-based systems are offered by Oviyam, Weasis, and Xero. Focusing on open source components, MIPAV is the best candidate for 3D imaging as well as DICOM communication. Weasis is superior for workflow optimization in clinical trials. Our evaluation shows that advanced visualization and suitable interfaces can also be found in the open source field and not only in commercial products.

Abstract:This paper introduces a new application of PACS in our hospital. Through the integration of PACS, HIS and RIS, digital transformation is made in every step. The functional modules of Body Parts Examined in DICOM is set and good link between PACS and DR is made.
