Correlation between different distribution patterns of cerebral microbleeds and Essen Stroke Risk Score in patients with acute ischemic cerebrovascular disease

Abstract:Objective To investigate the correlation between different distribution patterns of cerebral microbleeds (CMBs) and Essen Stroke Risk Score (ESRS) in patients with acute ischemic cerebrovascular disease.Methols The patients were selected from Nanjing Stroke Registry Program and those with acute cerebral infarction or transient ischemic attack treated in hospital were enrolled prospectively from April 2011 to December 2011.All patients underwent conventional MRI sequences and susceptibility weighted imaging scan.The baseline data were recorded and Essen score were performed.The correlation between Essen score and distribution parts of CMBs were analyzed.Results A total of 177 consecutive patients with acute cerebral infarction or transient ischemic attack were enrolled,77 (43.50%) of them were CMB positive,16 were purely lobar CMBs,and 61 were deep/infratentorial CMBs.There were significant differences in age (F =5.420,P =0.005),hypertension (x2 =13.069,P =0.001),stroke history (x2 =7.696,P =0.021),creatinine (F =4.127,P =0.018),fibrinogen (F =3.863,P =0.023),and ESRS (Z =21.008,P =0.000) among the CBM negative,purely lobar CMB and deep/infratentorial CMB groups.Multivariate logistic regression analysis showed that ESRS could independently predict deep/infratentorial CMBs (odd ratio [OR] 1.745,95% confidence interval [CI] 1.341-2.272; P=0.001),but it did not have significant independent correlation with purely lobar CMBs (OR 1.050,95% CI 0.692-1.591; P =0.819).Besides,serum fibrinogen level was independently associated with the purely lobar CMBs (OR 2.374,95% CI 1.047-5.383; P =0.038).Conclusions ESRS is independent predictors for the occurrence of deep/infratentorial CMBs in patients with acute ischemic cerebrovascular disease,and serum fibrinogen level is independently associated with the occurrence of lobar CMBs.
