Correlative factors on multiple gestation and pregnancy in in vitro fertilization and embryo transfer

Abstract:To observe the influence of the number of transferred good quality embryos, total follicles, total good quality embryos, female age and treatment cycles on the pregnancy rate (PR) and multiple gestation rate (MGR) in IVF-ET. Retrospective cohort study. In this retrospective study, a total of 4672 patients in fresh cycle between Jan. 2004 and Nov. 2006 were analyzed. According to the transferred good quality embryos, the patients are firstly devided into 3 groups: <2, 2 and 3. And then the patients are further sub-divided according to the female age (<35 or ≥35) and treatment cycles (<3 or ≥3). Pregnancy rate and multiple gestation rate are compared among different groups using chi-square test. In these patients the total pregnancy rate is 43.64% and multiple gestation rate is 28.59%. We found that in women <35 with treatment cycles less than 3 times, transferred <2, 2 and 3 good quality embryos resulted in increased multiple gestation rates (19.88% (32/161),31.49% (409/1299) and 44.37% (63/142), respectively) but the pregnancy rates (PRs) showed no significant difference when transferred 2 or 3 good quality embryos (51.69% (1299/2513), 54.41% (142/261), respectively). The PRs of these two groups are higher than patients transferred less than 2 good quality embryos (29.65% (161/543)). In women ≥35 years old with treatment less than 3 times, the MGRs in patients transferred <2 and 2 good quality embryos groups showed no difference (20.00% (8/40),20.69% (18/87), respectively) but the MGR in patients transferred 3 good quality embryos (42.33% (69/163)) is significantly higher than those two groups. Although the PR in patients transferred 3 good quality embryos is slightly higher than in patients transferred 2 good quality embryos (42.34% (163/385) and 34.66% (87/251) respectively), there is no significant differences. The PRs in these two groups are significant higher than in patients transferred <2 good quality embryos (20.41% (40/196)). The PRs and MGRs become stable in patients with a number of total follicle counts up to 9 and the PRs become stable while the MGRs still goes high when the number of total good quality embryos is up to 3 . In women with the treatment cycles less than 3 times and regardless of the age, transferring 2 good quality embryos resulted in similar pregnancy rates when compared to transferring 3 good quality embryos but will significantly reduced multiple gestation rates.
