The clinical study of embolism and segmental wall motion abnormalities in patients undergoing total hip replacement

Abstract:Objective To observe the severity and incidences of embolism and segmental wall motion abnormalities(SWMAs) at various stages in patients undergoing total hip replacement.Methods Thirty patients undergoing total hip replacement under general anesthesia participated in this study. The severity and incidences of embolism and SWMAs at various stages in surgery were observed using transesophageal echocardiography. Results The incidences of embolism were much higher during reaming and fixing the acetabulum,reaming of the femur and insertion of cemented femoral components than those before incision(P<0.01).The larger emboli (>2mm) were only observed during insertion of femoral components. SWMAs were seen in 14 patients when femoral components were inserted,which was significantly different from that before incision(P<0.01). SWMAs began shortly (within 30-60s) after the arrival of intracardiac emboli. None of the patients had failure of lung and heart,and all recovered uneventfully. Conclusion The insertion of femoral component was associated with the largest number of emboli and the most distinguished SWMAs in surgery. Almost all patients undergoing total hip replacement had evidence of intracardiac emboli,but fatal embolism was rare.
