Abstract:BackgroundLeukemia caused by occupational risk is a problem that needs more attention and remains to be solved urgently, especially for acute lymphoid leukemia (ALL), acute myeloid leukemia (AML), and chronic lymphoid leukemia (CLL). However, there is a paucity of literature on this issue. We aimed to assess the global burden and trends of leukemia attributable to occupational risk from 1990 to 2019.MethodsThis observational trend study was based on the Global Burden of Disease (GBD) 2019 database, the global deaths, and disability-adjusted life years (DALYs), which were calculated to quantify the changing trend of leukemia attributable to occupational risk, were analyzed by age, year, geographical location, and socio-demographic index (SDI), and the corresponding estimated annual percentage change (EAPC) values were calculated.ResultsGlobal age-standardized DALYs and death rates of leukemia attributable to occupational risk presented significantly decline trends with EAPC [−0.38% (95% CI: −0.58 to −0.18%) for DALYs and −0.30% (95% CI: −0.45 to −0.146%) for death]. However, it was significantly increased in people aged 65–69 years [0.42% (95% CI: 0.30–0.55%) for DALYs and 0.38% (95% CI: 0.26–0.51%) for death]. At the same time, the age-standardized DALYs and death rates of ALL, AML, and CLL were presented a significantly increased trend with EAPCs [0.78% (95% CI: 0.65–0.91%), 0.87% (95% CI: 0.81–0.93%), and 0.66% (95% CI: 0.51–0.81%) for DALYs, respectively, and 0.75% (95% CI: 0.68–0.82%), 0.96% (95% CI: 0.91–1.01%), and 0.55% (95% CI: 0.43–0.68%) for death], respectively. The ALL, AML, and CLL were shown an upward trend in almost all age groups.ConclusionWe observed a substantial reduction in leukemia due to occupational risks between 1990 and 2019. However, the people aged 65–69 years and burdens of ALL, AML, and CLL had a significantly increased trend in almost all age groups. Thus, there remains an urgent need to accelerate efforts to reduce leukemia attributable to occupational risk-related death burden in this population and specific causes.
Associations of Parental Occupational Exposure to Extremely Low-Frequency Magnetic Fields with Childhood Leukemia Risk

Abstract:Previous studies on the association of parental occupational exposure to extremely low-frequency magnetic fields (ELF-MF) with childhood leukemia risk have produced inconsistent results. To reevaluate this association, we performed a meta-analysis by pooling 11 case-control and one cohort studies. The overall results showed that neither maternal nor paternal occupational exposure was associated with childhood leukemia risk. For paternal occupational exposure, significant association was found when pooling studies with small number of cases (OR=1.96; 95% CI, 1.03-3.74) or with lower quality score (OR=1.52; 95% CI, 1.07-2.15). However, this association was not confirmed when pooling studies with large number of cases or high quality score. In conclusion, our data indicate no association between parental occupational ELF-MF exposure and childhood leukemia risk, and the elevated OR under certain subgroup analysis is likely due to chance. Further studies with precise ELF-MF exposure assessment are suggested.
A Retrospective Cohort Study among Iron-Steel Workers in Anshan, China: Exposure Assessment.

Abstract:Although adequate assessment of exposure is needed in epidemiological studies among foundry workers, previous studies are often lacking in this aspect. We conducted a retrospective cohort study of a Chinese iron and steel company with a 14-yr follow up during 1980-1993. Exposure assessment was performed for a single job, i.e., the current job for the active worker and the longest job for the retired or deceased worker as of the end of the follow-up, which was allocated as the surrogate of lifetime job and was applied to a job-exposure matrix. Of the 147,062 cohort members, 52,394 males (43%) and 5,291 females (21%) were exposed to any of 15 hazardous factors such as dust, silica, PAHs (polycyclic aromatic hydrocarbons), CO (carbon monoxide) and heat. In 2,104 randomly selected samples, the exposure assessment of exposed workers based on a single job was found to be 12-14% lower than the real situation. This study suggests that the exposure assessment is valuable in evaluating the health effects among the foundry workers, despite some limitations such as underestimation of exposure assessment and the lack of data regarding smoking and drinking habits.
Occupations and breast cancer risk among Chinese women in urban Shanghai

Abstract:Background Although, an elevated risk of breast cancer has been suggested for a number of occupations, many earlier studies were limited by selection biases, the incomplete assessment of job histories, and the inability to control for confounding. Methods We examined the relationship between occupational history and breast cancer risk using data from a population-based case-control study of 1,458 cases and 1,556 age-matched controls (90% response rate) conducted in Shanghai, China. Unconditional logistic regression models were used to derive odds ratios (ORs) and 95% confidence intervals (95% CIs) of breast cancer risk associated with occupations and duration of employment adjusting for non-occupational risk factors. Results The following occupations were found to be associated with an increased risk of breast cancer: laboratory technicians (OR 9.94, 95% CI 1.20-82.37), telephone and telegraph operators (OR 4.63, 95% CI 1.85-11.59), leather and fur processors (OR 3.25, 95% CI 1.11-9.53), and glass-manufacturing workers (OR 2.08, 95% CI 1.14-3.82). A dose-response pattern for years of employment was observed for leather and fur processors (P = 0.02) and glass-manufacturing workers (P = 0.01). Stratified analyses also revealed dose-response relationships between the risk of breast cancer and years of employment as inspector and product analysts among pre-menopausal women (P = 0.02), and as farmers among post-menopausal women (P = 0.04). Conclusions This study found that several occupations are associated with an increased risk of breast cancer among women. Studies examining various occupational exposures in these high-risk occupations are warranted to identify carcinogens that may play a role in the increased breast cancer risk.
Occupational benzene exposure and cancer risk among Chinese men: A report from the Shanghai Men's Health Study

Abstract:Background: Benzene exposure has been associated with increased leukemia and other cancer risk; however, epidemiological evidence is inconsistent for the latter and confounding from smoking and alcohol was rarely adjusted. Methods: We investigated associations of occupational benzene exposure and risk of leukemia, lymphoma, myeloma, lung, stomach, liver, and kidney cancers in a population-based cohort of 61,377 men ages 40-74. A job-exposure matrix, constructed by industrial hygienists specifically for the study population, was used to derive cumulative benzene exposure from all jobs held. Cox regressions were performed to estimate adjusted hazard ratios (aHR) and 95% confidence intervals (CI) for benzene-cancer risk associations with adjustment for potential confounders. Results: Over 15-years of follow-up, 1,145 lung, 656 stomach, 445 liver, 243 kidney cancer cases, 100 leukemia, 124 lymphoma, and 46 myeloma cases were identified. Benzene exposure >550mg/m3 was associated with increased leukemia (aHR=2.3, 95%CI=1.1-4.5), lung (aHR=1.2, 95%CI=1.0-1.6), and stomach (aHR=1.4, 95%CI=1.0-1.9) cancer risk; benzene-exposure was associated with early cancer diagnosis age. The benzene-leukemia and -stomach cancer associations followed a linear dose-response pattern (Plinear=0.016 and 0.023), whereas benzene-lung cancer association was evident at higher exposure levels (Pnon-linear=0.027). Alcohol consumption modified the benzene-leukemia association (HR=3.0, 95%CI=1.1-8.3 for drinkers, aHR=0.9, 95%CI=0.4-2.0 for non-drinkers, Pinteraction=0.047). Conclusions: Benzene exposure was associated with increased leukemia, stomach, and lung cancer risk. Alcohol consumption may modify the benzene-leukemia association, although estimates are imprecise. Impact: Our study provides additional evidence that benzene exposure increases cancer risk beyond leukemia, information important for policymakers to develop programs to mitigate cancer risk among benzene-exposed workers.

[A Case-Control Study on Green Tea Consumption and the Risk of Adult Leukemia].

Abstract:OBJECTIVE:To investigate whether green tea consumption can reduce the risk of adult leukemia.METHODS:A hospital-based matched case-control study was conducted in 2005 - 2006. We recruited 107 confirmed leukemia cases and 110 inpatient controls with orthopedic disease without leukemia or other malignancy matched on gender, age and hospitals that patients stayed. Related information were gathered on quantity, duration and frequency of tea consumption, demographic characteristics, exposure to radiation and occupational hazards, medications, using a validated questionnaire by face-to-face interview. Univariate and multivariate unconditional logistic regression analysis were used to estimate odds ratios (ORs) and associated 95% confidence intervals (CIs) with SPSS 11.5 software.RESULTS:Compared with non-tea-drinkers, the OR of those who consumed green tea was 0.58 (95% CI:0.34-1.00, P< 0.05) under univariate statistical analysis. The OR was 0.52 ( 95% CI: 0.28- 0.98, P = 0.04), using logistic regression to count for age, gender, residential area, smoking, level of education, exposure to radiation, benzene and organo-phosphorus. Compared with non-drinkers, the risk of adult leukemia declined with increasing quantity, duration, and frequency of green tea consumption. Tests for trend on dose-response was statistically significant (P < 0.01).CONCLUSION:A higher consumption of green tea seemed to be associated with a declined risk of adult leukemia. Tea consumption might be of help to human health planning projects.

Abstract:A case-control study involving interviews with 1,249 patients with lung cancer and 1,345 population-based controls was conducted in Shenyang, an industrial city in northeastern China, where mortality rates are high among men and women. Cigarette smoking was found to be the principal cause of lung cancer in this population, accounting for 55% of the lung cancers in males and 37% in females. The attributable risk percentage among females is high compared to elsewhere in China, largely because of a higher prevalence of smoking among women. After adjustment for smoking, there were also significant increases in lung cancer risk associated with several measures of exposure to air pollutants. Risks were twice as high among those who reported smoky outdoor environments, and increased in proportion to years of sleeping on beds heated by coal-burning stoves (kang), and to an overall index of indoor air pollution. Threefold increases in lung cancer risk were found among men who worked in the nonferrous smelting industry, where heavy exposures to inorganic arsenic have been reported. The associations with both smoking and indoor air pollution were stronger for squamous cell and small cell carcinomas than for adenocarcinoma of the lung. Risks due to smoking or air pollution were not greatly altered by adjustment for consumption of fresh vegetables or sources of beta carotene or retinol, prior chronic lung diseases, or education level. The findings suggest that smoking and environmental pollution combine to account for the elevated rates of lung cancer mortality in Shenyang.

Abstract:Foundry workers have increased mortality and morbidity risks from numerous causes, including various cancers. A retrospective Chinese iron-steel cohort study was conducted to examine the mortality effects of exposure to foundry work. Standardized mortality ratios (SMRs) and standardized rate ratios (SRRs) were calculated to evaluate mortality risks among male workers with exposure to 15 hazardous factors, adjusting for confounders. During 14 years of follow-up, 13,363 of 121,846 male workers died. SMR analysis showed a healthy worker effect in comparison with the general population. SRR analysis showed increased risks for all causes, all neoplasms, and others among the exposed workers compared with nonexposed blue-collar workers. Combined exposure to polycyclic aromatic hydrocarbons and two or more dusts increased the risks of lung cancer (SRR = 654; 95% CI: 113-3,780) and other malignancies. Foundry work has adverse health effects, including carcinogenic risks.
Retrospective occupational exposure assessment for case-control and case-series epidemiology studies based in Shanghai China

Abstract:To provide exposure information for epidemiology studies conducted in Shanghai from 2001 to 2008, we completed retrospective exposure assessments (EA) of benzene and other hazards. Interviewers administered questionnaires to subjects from Shanghai area hospitals. An initial exposure screening by EA staff members, blinded as to case-control status, stratified jobs into exposed, unexposed, or uncertain categories prior to review by a separate expert panel (EP). Resources for the EA included job/industry-specific questionnaire responses by subjects, short-term benzene area concentration measurements from a Shanghai regulatory agency database, Chinese literature for qualitative and short-term quantitative measurements, on-site investigations, summaries of technology changes, and selected task simulations with concurrent benzene concentration measurements. An EP in Shanghai completed semi-quantitative benzene exposure assignments, with categories of 0 to 4 corresponding to intensity ranges of none, <1, 1 to 10, >10 to 100, and >100 mg/m(3). For other hazards, sources included the EP's knowledge of the industries and Chinese and Western literature. For benzene, 20% of the EAs selected by a stratified random process were evaluated by two alternate methods. The study database of potential cases and controls included 18,857 jobs from the subjects' work histories. From 818 individuals initially screened as probably benzene exposed, 964 jobs underwent further review. From subjects with final diagnoses, 755 jobs qualified for inclusion in the final database for any study. For other exposures, the EA considered 17,893 jobs from 7654 subjects for possible exposures and were in the final study database. Of these, 2565 individuals had exposures of study interest from their 4909 exposed jobs. The prevalent exposures included agricultural chemicals, petroleum products, and metals. The EA involved extensive information assembly and exposure assignment by an EP and periodic reviews. The methods described went beyond those typically applied in past general population studies and may have provided improved information for the epidemiologic analyses. Occupational risk factors for breast cancer among women in Shanghai

Abstract:Although female breast cancer rates are lower in China than in Western countries, rates have been rising rapidly in China. This increase may be due to changes in established breast cancer risk factors, but it is possible that exposure to occupational and environmental carcinogens in Shanghai also have contributed to the rise in incidence. We used data collected by the Shanghai Cancer Registry and the Chinese Third National Census to study the risk of breast cancer by occupation and by occupational exposures. Standardized incidence ratios (SIRs) were used to compare observed cases to expected numbers of cases, based on the incidence rates for Shanghai and the number of women in each occupation according to the 1982 census. Statistically elevated SIRs for breast cancer were seen for a number of professional occupational categories, with the greatest risk seen among scientific research workers (SIR = 3.3). Administrative clerks, political and security personnel, and makers of rubber and plastics products also had significant excesses. Significant deficits of risk were seen for the categories of production and related workers, construction workers, and transportation equipment operators. For specific occupations, the highest SIRs were observed among doctors of Chinese-Western medicine (SIR = 14.7, 95% CI = 5.9-30.3) and doctors of Chinese medicine (SIR = 7.2, 95% CI = 4.4-11.4). We also found excesses among teachers at each level of education, librarians, clerical workers, electrical and electronic engineers, nurses, lab technicians, accountants and bookkeepers, rubber manufacturing products makers, weavers, and knitters. SIRs were significantly elevated for high probability of exposure to organic solvents (SIR = 1.4). For benzene exposure, we found significant excesses for overall exposure (SIR = 1.1) and for medium level of exposure (SIR = 1.3). There was no evidence of an association between risk and electromagnetic fields (EMF) exposure. Based on a small number of exposed, SIRs were elevated for both medium probability and high level of exposure to pesticides. The elevations in occupations reported here support some previous reports. Our finding of an increased risk associated with benzene also has been reported previously; the finding for organic solvents is new. However the literature on the risk of breast cancer related to occupational exposures is limited and there is no consistent body of literature for any of the exposures studied here. Further many comparisons were made and the problem of multiple hypothesis testing cannot be ignored in a survey such as ours. (C) 1998 Wiley-Liss, Inc.(dagger)

Abstract:OBJECTIVES:The objectives are (1) to investigate and identify potential risk factors (personal characteristics, lifestyle and environmental factors) of acute myeloid leukemia (AML), and (2) to explore the relationships between potential risk factors and AML subtypes according to the World Health Organization (WHO) classification of myeloid neoplasms.MATERIALS AND METHODS:The investigation was a hospital-based case-control study consisting of 722 confirmed AML cases and 1444 individually gender-age-matched patient controls at 29 hospitals in Shanghai. A 17-page questionnaire was used to obtain information on: demographics, medical history, family history, lifestyle risk factors, employment history, residential history, and environmental and occupational exposures. Certain occupations of interest triggered a second questionnaire, which was occupation-specific and asked for more details about jobs, tasks, materials used and work environment. Risk estimates (odds ratios and 95% confidence intervals) were calculated using conditional logistic regression models.RESULTS:Several potential risk factors of AML (all subtypes combined) and individual subtypes were identified; including low-level education, body mass index (BMI), blood transfusion, smoking, alcohol consumption, home or workplace renovation, living on a farm, planting crops, raising livestock or animals, employment as farm workers or in the agricultural industry, and exposures to insecticides or fertilizers. Some risk factors applied to all or several subtypes (such as low-level education and living on a farm), while others were limited to one or two specific subtypes (such as home/office renovation and acute promyelocytic leukemia). An inverse association was found between BMI and overall AML or the sub-category "AML not otherwise categorized", whereas a positive association between BMI and the subtype acute promyelocytic leukemia was detected. An unexpected finding was the association between the use of traditional Chinese medicines and a reduced risk of AML in general as well as several major subtypes.CONCLUSIONS:The study identified a number of risk factors for AML in general as well as for some specific subtypes. Some of the risk factors were subtype-specific. The difference in risk by subtype underscores the importance of investigating the etiologic commonality and heterogeneity of AML by subtype in epidemiologic research.
[Risk of Lung Cancer among Iron and Steel Workers in Anshan, China--case-control Study].

Abstract:A case-control interview study on 610 lung cancer patients and 959 controls was conducted among male workers in Anshan Iron-steel Complex. After adjusting for non-occupational risk factors, such as smoking, pulmonary disease, family history of cancer and the consumption of fruit, risks for lung cancer were significantly higher in workers engaged in smelting and rolling (OR = 1.5, 95% CI = 1.1-2.2), in the fire-resistant brick factory (OR = 2.9, 95% CI = 1.4-5.9), in general loading (OR = 2.5 95% CI = 1.0-6.1), and in coking (OR = 3.4, 95% CI = 1.4-8.5) for 15 or more years. Significant dose-response was observed for exposure to total dust and B[a]P, but not for specific chemical components of dust. The lung cancer risk increased 40% in iron and steel workers with long term occupational exposure.
Occupational exposure to benzene and risk of non‐Hodgkin lymphoma in an extended follow‐up of two population‐based prospective cohorts of Chinese men and women

Abstract:What's new? The association between occupational benzene exposure and risk of non‐Hodgkin lymphoma (NHL) is suggestive but remains inconclusive. Occupational benzene exposure was historically high in China. This study, using a novel approach combining a benzene‐specific job‐exposure matrix with historical benzene measurements, evaluated NHL risk in association with occupational exposure among men and women in Shanghai. Occupational benzene exposure was found to be significantly associated with NHL risk. In addition, increasing duration and cumulative benzene exposure exhibited exposure‐response associations. The findings support a plausible association between occupational benzene exposure and risk of NHL. The carcinogenicity of benzene was reevaluated by the International Agency for Research on Cancer in 2017, with the Working Group reaffirming positive yet inconclusive associations with non‐Hodgkin lymphoma (NHL). To extend our previous observation of a significant exposure‐response for cumulative occupational benzene exposure and NHL risk among Chinese women in a population‐based cohort in Shanghai, we extended follow‐up of this cohort and pooled the data with a similarly designed population‐based cohort of men in Shanghai. Cumulative exposure estimates were derived for 134,449 participants in the pooled analysis by combining ordinal job‐exposure matrix intensity ratings with quantitative benzene measurements from an inspection database of Shanghai factories. Associations between benzene exposure metrics and NHL (n = 363 cases including multiple myeloma [MM]) were assessed using Cox proportional hazard models. Ever occupational exposure to benzene in the pooled population was associated with NHL risk (HR = 1.5, 95% CI = 1.2–2.0), and exposure‐response relationships were observed for increasing duration (ptrend = .003) and cumulative exposure (ptrend = .003). Associations with ever exposure, duration, and cumulative exposure were similar for NHL with and without MM in the case definition, including lifetime cumulative exposures in the highest quartile (HR = 1.6, 95% CI = 1.1–2.4 with MM included; HR = 1.7, 95% CI = 1.1–2.7 with MM excluded). An elevated risk of the chronic lymphocytic leukemia subtype was suggested in the pooled analyses (HR for ever vs. never exposure = 2.3, 95% CI = 0.9–5.6). These observations provide additional support for a plausible association between occupational benzene exposure and risk of NHL.

Risk Factors for Acute Myeloid Leukemia in Adults: a Hospital-Based Case-Control Study

Abstract:Objective To determine risk factors of environment and lifestyles involved in acute hematology departments of 29 secondary-care and tertiary-care hospitals in Shanghai. There were 206 hospitalized patients with de novo AML during June 2003 to April 2007 meeting the diagnostic criteria for AML by the World Health Organization, excluding those with definite history of radiotherapy or chemotherapy, and 412 age- and gender-matched controls with non-hematological other diseases from the same hospitals. All the participants were interviewed face-to-face using a standard structured questionnaire,including personal medical history, family medical history, history of medications, traditional Chinese herb medicines, exposure to occupational hazards, lifestyle factors, etc. in the past five years prior to onset of the disease. Conditional univariate and multivariate logistic regression analyses were performed to identify risk factors for AML. Results Logistic regression analysis showed that taking traditional Chinese herb medicine might be a protective factor for AML, with an odds ratio (OR) of 0. 39 and its 95% confidence interval (CI) of 0. 16-0. 98. Potential risk factors for AML included history of diabetes and taking hypoglycemic agents ( OR: 1.39, 95% CI: 0.67 -2. 87), occupational exposure to metal ( OR: 1.73, 95% CI: 0. 77 -3. 91 ), glue, paint and other coating materials, etc. ( OR: 1.72, 95% CI: 0. 93 - 3.2), raising animals (OR: 1.33, 95% CI: 0. 79 -2. 24). No association of living within a distance of 100 meters from high voltage power transmission lines, home renovation and decoration in the past year, hair dyeing, smoking,alcohol drinking with AML in adults was found. Conclusions Factors mentioned above such as occupational exposure to metal, glue, paint and other coating materials potentially increase risk for AML in adults,however taking traditional Chinese herb medicine is possibly a protective factor for it.
Global burden of leukemia attributable to occupational exposure to formaldehyde from 1990 to 2019

Abstract:The objective of this study was to evaluate the worldwide burden of leukemia owing to occupational exposure to formaldehyde (OEF) from 1990 to 2019. Data on leukemia due to OEF were obtained from the Global Burden of Disease Study (GBD) 2019. By region, age, sex, and disease subtype, the numbers and age-standardized rates (ASRs) associated with deaths, years of life lost (YLLs), years lived with disability (YLDs), and disability-adjusted life years (DALYs) were analyzed. Annual average percentage change (AAPC) was used to estimate disease burden trends from 1990 to 2019. To measure the risk of leukemia due to OEF, the population attributable fraction (PAF) was introduced. From 1990 to 2019, the number of deaths, DALYs, YLLs, and YLDs for leukemia caused by OEF increased by 44%, 34%, 33%, and 124%, respectively. Regarding the change in ASRs, the age-standardized YLDs (ASYLDs) rate of leukemia due to OEF, which was 38.03% (AAPC = 1.17 [95% confidence interval [CI] 1.11, 1.23]), indicated an increased trend. But the age-standardized mortality rate (ASMR), age-standardized DALY (ASDALY) rate, and age-standardized YLL (ASYLL) rate showed decline trends, with - 11.90% (AAPC = - 0.41 [95% CI - 0.45, - 0.37]), - 14.19% (AAPC = - 0.5 [95% CI - 0.55, - 0.45]), and - 14.97% (AAPC = - 0.53 [95% CI - 0.58, - 0.48]), respectively. In terms of PAFs, there were increasing trends in PAFs of age-standardized deaths, ASDALYs, ASYLLs, and ASYLDs for leukemia caused by OEF, with 20.15% (95% uncertainty interval [UI] 11.76%, 30.25%), 36.28% (95% UI 21.46%, 53.42%), 51.91% (95% UI 35.05%, 72.07%), and 36.34% (95% UI 21.58%, 53.63%), respectively. Across the socio-demographic index (SDI) regions, the leukemia burden caused by OEF was concentrated in middle and high-middle SDI regions. Besides, OEF poses a more serious risk for acute leukemia among the leukemia subtype. Globally, leukemia caused by OEF remains a public health burden. Policies must be developed to avoid the burden of leukemia caused by OEF.
Leukemia Incidence by Occupation and Industry: A Cohort Study of 2.3 Million Workers from Ontario, Canada

Abstract:Although a significant body of evidence has attributed certain occupational exposures with leukemia, such as benzene, formaldehyde, 1,3-butadiene and ionizing radiation, more research is needed to identify work environments at increased risk for this disease. Our study aimed to identify occupational and industry groups associated with an elevated incidence of leukemia using a diverse cohort of workers' compensation claimants from Ontario, Canada. A total of 2,363,818 workers in the Occupational Disease Surveillance System (ODSS) cohort, with claims between 1983-2019, were followed for malignant leukemia diagnoses up to 31 December 2019. We used a Cox proportional-hazards model to estimate the relative incidence of leukemia in specific occupation and industry groups. After adjusting for age and birth year, males in protective services (HR = 1.17, 95% CI = 1.02-1.35), metal machining (HR = 1.23, 95% CI = 1.07-1.41), transport (HR = 1.15, 95% CI = 1.06-1.25), and mining occupations (HR = 1.28, 95% CI = 1.02-1.60) had elevated risks of leukemia compared to other workers in the ODSS, with comparable findings by industry. Among female workers, slight risk elevations were observed among product fabricating, assembling, and repairing occupations, with other increased risks seen in furniture and fixture manufacturing, storage, and retail industries. These findings underscore the need for exposure-based studies to better understand occupational hazards in these settings.

Abstract:OBJECTIVE:Studies had suggested that risk of leukemia might be associated with occupational or residential exposures to electromagnetic fields and varied at distance to and level of the exposure or type of occupations. Through pooled analyses, etiologic insight on the associations between exposure and disease might be explained. METHODS:We carried out a Meta-analysis based on primary data (1980 - 2010) from 9 studies related to the electric and magnetic fields exposure and acute myeloid leukemia in adults to assess whether the combined results, adjusted for potential confounding, would indicate an association between them. RESULTS:In this study the overall estimated OR value was 1.24 (95%CI: 1.11 - 1.37). The odds ratios for exposure categories of 0.1 - 0.2 µT, ≥ 0.2 µT, compared with < 0.1 µT, were 1.17 (95%CI: 0.98 - 1.39) and 1.51 (95%CI: 1.15 - 1.98), respectively. CONCLUSION:Through employing the alternate cut points, stratification by level of exposure or distance and the relation on different ways of exposure, there appeared consistent evidence of increased risk between acute myeloid leukemia in adults and the extremely low frequency-electromagnetic to field exposure.
Incidence and mortality of leukemia among permanent residents in Luwan district of Shanghai during 2004-2011

Abstract:OBJECTIVE Leukemia is a kind of hematopoietic system malignancy with poor prognosis and high mortality, and the incidence of leukemia has risen in China in recent years.This study is to analyze the incidence and mortality of leukemia among permanent residents in Luwan district of Shanghai from 2004 to 2011 for providing references for prevention and intervention of leukemia.METHODS The data of leukemia in permanent residents of Luwan district werecollected from the database of cancer registration and management system in Shanghai.The incidence and mortality of leukemia were calculated and normalized.The temporal trend in the incidence and mortality of leukemia was assessed.RESULTS A total of 204 cases of leukemia were diagnosed from 2004 to 2011, accounting for 1.96% of the total patients with malignant diseases.The average annual crude incidence rate of leukemia was 8.15/105, age-standardized incidence rate by Chinese standard population (ASIRC) was 5.16/105 and age-standardized incidence rate by world standard population (ASIRW) was 5.84/105.Average annual crude incidence rates of males and females were 9.69/105 and 6.64/105, ASIRC were 6.39/105 and 4.00/105 and ASIRW were 7.41/105 and 4.38/105 respectively.The crude incidence rate of males was significantly higher than that of females.A total of 149 cases died of leukemia during this period, accounting for 2.25% of the total deaths with malignant diseases.The average annual crude mortality rate of leukemia was 5.95/105, and age-standardized mortality rate by Chinese standard population (ASMRC) and by world standard population (ASMRW) were 3.33/105 and 3.88/105.Average annual crude mortality rates of males and females were 7.27/105 and 4.66/105 respectively, ASMRC were 4.50/105 and 2.25/105, and ASMRW were 5.18/105 and 2.70/105 respectively.The crude mortality rate of males was significantly higher than that of females.There were considerable fluctuations of leukemia incidence and mortality from 2004 to 2011.There was a small peak of leukemia incidence and mortality among children younger than 10 years old.The incidence and mortality of leukemia reached the highest point over the age of 65.CONCLUSIONS The occurrence and death of leukemia happened at almost all age categories among permanent residents in Luwan district of Shanghai during 2004-2011.It is important to further investigate the etiology of leukemia, so as to identify effective prevention and control strategies.

Abstract:OBJECTIVE:Due to the sparse data on benzene exposure and myelodysplastic syndrome (MDS) subtypes, we studied this relationship in patients from 29 hospitals in Shanghai, China. METHODS:We recruited 604 cases of MDS and 1193 controls matched on age, sex, and admission date. We interviewed subjects for information on workplace and lifestyle exposures, and developed semi-quantitative exposure estimates. RESULTS:Benzene exposure showed a direct exposure-response pattern with refractory cytopenia with multilineage dysplasia, a less certain association with refractory cytopenia with unilineage dysplasia, and no association with other MDS subtypes. A different pattern was observed with farm residence and smoking, which was primarily related to refractory anemias. CONCLUSIONS:This research demonstrates the importance of MDS subtype specification for more robust etiologic insights. Our data suggests that subtypes with non-erythroid dysplasia are associated with benzene exposure.

Abstract:The objectives were (1) to investigate potential environmental and occupational risk factors of non-Hodgkin lymphoid neoplasms (NHLN), and (2) to explore the relationships between risk factors and NHLN subtypes according to the World Health Organization (WHO) classification. The investigation was a hospital-based case–control study consisting of 649 newly diagnosed NHLN cases (August 2003 through January 2008) and 1298 individually gender-age-matched patient controls at 25 hospitals in Shanghai. A 17-page questionnaire was used to obtain information on demographics, medical history, family history, lifestyle risk factors, employment history, residential history, and occupational and non-occupational exposures. Certain occupations of interest triggered a second questionnaire, which was occupation-specific and asked for more details about jobs, tasks, materials used and work environment. Exposure assessments were based on the questionnaires, on-site workplace investigations, data published in the Chinese literature, historical exposure measurements maintained by government health agencies, and expert opinions of a panel of local scientists who were familiar with workplaces in Shanghai. Risk estimates (odds ratios and 95% confidence intervals) of individual risk factors were calculated using conditional logistic regression models. A number of potential environmental and occupational risk factors were associated with an increased risk of NHLN (all subtypes combined) and/or individual subtypes; including home/workplace renovation, living on a farm, planting crops, raising livestock or animals, farm workers, fabric sewing and cutting workers, welders and sheet metal workers, masonry and plastering workers, product and chemical testing workers, toy manufacturing, agriculture industry, and beauty salon. Exposures associated with an increased risk of NHLN (all subtypes combined) and/or individual subtypes included benzene, solvents, petroleum fuels, metals, insecticides, herbicides, fertilizers, and glues and adhesives. Multivariate models were used to adjust for potential confounding exposures, and several potential risk factors were subsequently eliminated. The results of the investigation indicated that some risk factors applied to all or most subtypes (e.g., insecticides and overall NHLN and subtypes of B-cell lymphoid neoplasms), while others to specific subtypes only (e.g., benzene and follicular lymphoma). Thus, some risk factors were subtype-specific. The difference in risk by subtype underscores the importance of the etiologic commonality and heterogeneity of NHLN subtypes.
