Abstract:BackgroundLeukemia caused by occupational risk is a problem that needs more attention and remains to be solved urgently, especially for acute lymphoid leukemia (ALL), acute myeloid leukemia (AML), and chronic lymphoid leukemia (CLL). However, there is a paucity of literature on this issue. We aimed to assess the global burden and trends of leukemia attributable to occupational risk from 1990 to 2019.MethodsThis observational trend study was based on the Global Burden of Disease (GBD) 2019 database, the global deaths, and disability-adjusted life years (DALYs), which were calculated to quantify the changing trend of leukemia attributable to occupational risk, were analyzed by age, year, geographical location, and socio-demographic index (SDI), and the corresponding estimated annual percentage change (EAPC) values were calculated.ResultsGlobal age-standardized DALYs and death rates of leukemia attributable to occupational risk presented significantly decline trends with EAPC [−0.38% (95% CI: −0.58 to −0.18%) for DALYs and −0.30% (95% CI: −0.45 to −0.146%) for death]. However, it was significantly increased in people aged 65–69 years [0.42% (95% CI: 0.30–0.55%) for DALYs and 0.38% (95% CI: 0.26–0.51%) for death]. At the same time, the age-standardized DALYs and death rates of ALL, AML, and CLL were presented a significantly increased trend with EAPCs [0.78% (95% CI: 0.65–0.91%), 0.87% (95% CI: 0.81–0.93%), and 0.66% (95% CI: 0.51–0.81%) for DALYs, respectively, and 0.75% (95% CI: 0.68–0.82%), 0.96% (95% CI: 0.91–1.01%), and 0.55% (95% CI: 0.43–0.68%) for death], respectively. The ALL, AML, and CLL were shown an upward trend in almost all age groups.ConclusionWe observed a substantial reduction in leukemia due to occupational risks between 1990 and 2019. However, the people aged 65–69 years and burdens of ALL, AML, and CLL had a significantly increased trend in almost all age groups. Thus, there remains an urgent need to accelerate efforts to reduce leukemia attributable to occupational risk-related death burden in this population and specific causes.
Leukemia and occupational exposure to electromagnetic fields: review of epidemiologic surveys.

Abstract:Several recent surveys have presented data suggesting an increased risk of leukemia among men with occupational exposure to electromagnetic fields. Eleven pertinent data sets were compiled in order to assess the consistency of this pattern and to identify those occupations most deserving closer examination. Results for total leukemia show a modest excess risk for men in exposed occupations, with an enhanced risk elevation for acute leukemia and especially acute myelogenous leukemia. These studies are inherently limited regarding the effect of electromagnetic fields due to the absence of exposure characterization. Nonetheless, telegraph, radio, and radar operators, power and telephone linemen, and electrical and electronic engineers showed the most consistent results and warrant further study to ascertain potential occupational health hazards.

Lung cancer incidence among workers biologically monitored for occupational exposure to lead: a cohort study

Abstract:Objective Earlier studies have reported increased risks of lung, kidney and brain cancers for exposure to lead. The International Agency for Research on Cancer (IARC) Working Group evaluated inorganic lead and its compounds probably carcinogenic to humans. This study aimed to assess the association between blood lead level in occupational exposure and risk of lung cancer. Methods The study was based on the follow-up of lung cancer incidence during 1973–2014 among 20 729 employees biologically monitored for their occupational lead exposure in 1973–1983. Duration of employment in the monitored work was assessed using records from the Finnish Centre for Pensions; and potential confounding by other occupational carcinogens using longitudinal information on the occupation in censuses and the Finnish National Job-Exposure Matrix (FINJEM). Occupation- and gender-specific prevalence of regular tobacco smoking and the socioeconomic status were also utilized in the adjustments for potential confounding. Results Positive trends were found for the elevated blood lead levels on the lung cancer risk. Among employees with the duration of employment of ≥60 months, the relative risk (RR) of lung cancer was 1.72 [95% confidence interval (CI) 1.28–2.31] for mean blood lead 1.0–1.9 μmol/L and RR 2.63 (95% CI 1.71–4.05) for mean blood lead ≥2.0 μmol/L, compared with mean lead <0.5 μmol/L. The studied potential confounders did not explain the findings on the increased risk for lead exposure. Conclusions The current study lends support to the findings that exposure to lead increases lung cancer risk. Abstract:Background Worldwide, lung cancer is the second leading cause of cancer death in women. The present study explored associations between occupational exposures that are prevalent among women, and lung cancer. Methods Data from 10 case–control studies of lung cancer from Europe, Canada, and New Zealand conducted between 1988 and 2008 were combined. Lifetime occupational history and information on nonoccupational factors including smoking were available for 3040 incident lung cancer cases and 4187 controls. We linked each reported job to the Canadian Job‐Exposure Matrix (CANJEM), which provided estimates of probability, intensity, and frequency of exposure to each selected agent in each job. For this analysis, we selected 15 agents (cleaning agents, biocides, cotton dust, synthetic fibers, formaldehyde, cooking fumes, organic solvents, cellulose, polycyclic aromatic hydrocarbons from petroleum, ammonia, metallic dust, alkanes C18+, iron compounds, isopropanol, and calcium carbonate) that had lifetime exposure prevalence of at least 5% in the combined study population. For each agent, we estimated lung cancer risk in each study center for ever‐exposure, by duration of exposure, and by cumulative exposure, using separate logistic regression models adjusted for smoking and other covariates. We then estimated the meta‐odds ratios using random‐effects meta‐analysis. Results and Conclusions None of the agents assessed showed consistent and compelling associations with lung cancer among women. The following agents showed elevated odds ratio in some analyses: metallic dust, iron compounds, isopropanol, and organic solvents. Future research into occupational lung cancer risk factors among women should prioritize these agents.

Global burden of leukemia attributable to occupational exposure to formaldehyde from 1990 to 2019

Abstract:The objective of this study was to evaluate the worldwide burden of leukemia owing to occupational exposure to formaldehyde (OEF) from 1990 to 2019. Data on leukemia due to OEF were obtained from the Global Burden of Disease Study (GBD) 2019. By region, age, sex, and disease subtype, the numbers and age-standardized rates (ASRs) associated with deaths, years of life lost (YLLs), years lived with disability (YLDs), and disability-adjusted life years (DALYs) were analyzed. Annual average percentage change (AAPC) was used to estimate disease burden trends from 1990 to 2019. To measure the risk of leukemia due to OEF, the population attributable fraction (PAF) was introduced. From 1990 to 2019, the number of deaths, DALYs, YLLs, and YLDs for leukemia caused by OEF increased by 44%, 34%, 33%, and 124%, respectively. Regarding the change in ASRs, the age-standardized YLDs (ASYLDs) rate of leukemia due to OEF, which was 38.03% (AAPC = 1.17 [95% confidence interval [CI] 1.11, 1.23]), indicated an increased trend. But the age-standardized mortality rate (ASMR), age-standardized DALY (ASDALY) rate, and age-standardized YLL (ASYLL) rate showed decline trends, with - 11.90% (AAPC = - 0.41 [95% CI - 0.45, - 0.37]), - 14.19% (AAPC = - 0.5 [95% CI - 0.55, - 0.45]), and - 14.97% (AAPC = - 0.53 [95% CI - 0.58, - 0.48]), respectively. In terms of PAFs, there were increasing trends in PAFs of age-standardized deaths, ASDALYs, ASYLLs, and ASYLDs for leukemia caused by OEF, with 20.15% (95% uncertainty interval [UI] 11.76%, 30.25%), 36.28% (95% UI 21.46%, 53.42%), 51.91% (95% UI 35.05%, 72.07%), and 36.34% (95% UI 21.58%, 53.63%), respectively. Across the socio-demographic index (SDI) regions, the leukemia burden caused by OEF was concentrated in middle and high-middle SDI regions. Besides, OEF poses a more serious risk for acute leukemia among the leukemia subtype. Globally, leukemia caused by OEF remains a public health burden. Policies must be developed to avoid the burden of leukemia caused by OEF.
Strawman Concept for A Virtual Solar Observatory

Abstract:We propose here an architecture and feature set for a prototype Virtual Solar Observatory (VSO). The prototype will not include all the features that might eventually become part of the VSO, nor even all the features that are concurrently under development elsewhere that should become parts of the VSOs functionality. It is possible to approach the design of such a system in at least two different ways. In one (top-down), all possible features and uses of a system are studied, and the best solution for as many as possible is proposed. This is the approach taken by the European Grid of Solar Observations (EGSO). Alternately, one can approach a system design from the bottom up, and ask what the essential element or elements of the design have to be in order to have a functioning and useful system. The VSO study group decided, after examining different approaches to abstracting the procedures for solar data identification and access, to build the smallest box possible around that problem, rather than attempting to draw a box around all possible aspects of a VSO. Abstract:BackgroundAgricultural workers may be exposed to potential carcinogens including pesticides, sensitizing agents and solar radiation. Previous studies indicate increased risks of hematopoietic cancers and decreased risks at other sites, possibly due to differences in lifestyle or risk behaviours. We present findings from CanCHEC (Canadian Census Health and Environment Cohort), the largest national population-based cohort of agricultural workers.MethodsStatistics Canada created the cohort using deterministic and probabilistic linkage of the 1991 Canadian Long Form Census to National Cancer Registry records for 1992–2010. Self-reported occupations were coded using the Standard Occupational Classification (1991) system. Analyses were restricted to employed persons aged 25–74 years at baseline (N = 2,051,315), with follow-up until December 31, 2010. Hazard ratios (HR) and 95% confidence intervals (CI) were modeled using Cox proportional hazards for all workers in agricultural occupations (n = 70,570; 70.8% male), stratified by sex, and adjusted for age at cohort entry, province of residence, and highest level of education.ResultsA total of 9515 incident cancer cases (7295 in males) occurred in agricultural workers. Among men, increased risks were observed for non-Hodgkin lymphoma (HR = 1.10, 95% CI = 1.00–1.21), prostate (HR = 1.11, 95% CI = 1.06–1.16), melanoma (HR = 1.15, 95% CI = 1.02–1.31), and lip cancer (HR = 2.14, 95% CI = 1.70–2.70). Decreased risks in males were observed for lung, larynx, and liver cancers. Among female agricultural workers there was an increased risk of pancreatic cancer (HR = 1.36, 95% CI = 1.07–1.72). Increased risks of melanoma (HR = 1.79, 95% CI = 1.17–2.73), leukemia (HR = 2.01, 95% CI = 1.24–3.25) and multiple myeloma (HR = 2.25, 95% CI = 1.16–4.37) were observed in a subset of female crop farmers.ConclusionsExposure to pesticides may have contributed to increased risks of hematopoietic cancers, while increased risks of lip cancer and melanoma may be attributed to sun exposure. The array of decreased risks suggests reduced smoking and alcohol consumption in this occupational group compared to the general population.

Abstract:Although several occupational exposures have been linked to the risk of COPD; limited data exists on sex-specific differences. This study aimed to identify at-risk occupations and sex differences for COPD risk. Cases were identified in a large surveillance system established through the linkage of former compensation claimants' data (non-COPD claims) to physician visits, ambulatory care data, and hospital inpatient data (1983–2020). Cox proportional hazard models were used to estimate hazard ratios (HRs) and corresponding 95% confidence intervals (CI) for occupation groups (occupation at time of claim), stratified by sex. HRs were indirectly adjusted for cigarette smoking using another population dataset. A total of 29,445 male and 14,693 female incident cases of COPD were identified. Increased risks were observed in both sexes for construction (HR male 1.15, 95% CI 1.12–1.19; HR female 1.54, 95% CI 1.29–1.83) transport/equipment operating (HR male 1.32, 95% CI 1.28–1.37; HR female 1.53, 95% CI 1.40–1.68) farming (HR male 1.23, 95% CI 1.15–1.32; HR female 1.19, 95% CI 1.04–1.37) and janitors/cleaners (HR male 1.31, 95% CI 1.24–1.37; HR female 1.40, 95% CI 1.31–1.49). Increased risks were observed for females employed as chefs and cooks (HR 1.44, 95% CI 1.31–1.58), bartenders (HR 1.38, 95% CI 1.05–1.81), and those working in food/beverage preparation (HR 1.34, 95% CI 1.24–1.45) among other occupations. This study demonstrates elevated risk of COPD among both male and female workers potentially exposed to vapours, gases, dusts, and fumes, highlighting the need for occupational surveillance of COPD.

Abstract:Leukemia and lymphoma are the two most common forms of hematologic malignancy, and their etiology is largely unknown. Pathophysiological mechanisms suggest a possible association with air pollution, but little empirical evidence is available. We aimed to investigate the association between long-term residential exposure to outdoor air pollution and risk of leukemia and lymphoma. We pooled data from four cohorts from three European countries as part of the "Effects of Low-level Air Pollution: a Study in Europe" (ELAPSE) collaboration. We used Europe-wide land use regression models to assess annual mean concentrations of fine particulate matter (PM2.5), nitrogen dioxide (NO2), black carbon (BC) and ozone (O3) at residences. We also estimated concentrations of PM2.5 elemental components: copper (Cu), iron (Fe), zinc (Zn); sulfur (S); nickel (Ni), vanadium (V), silicon (Si) and potassium (K). We applied Cox proportional hazards models to investigate the associations. Among the study population of 247,436 individuals, 760 leukemia and 1122 lymphoma cases were diagnosed during 4,656,140 person-years of follow-up. The results showed a leukemia hazard ratio (HR) of 1.13 (95% confidence intervals [CI]: 1.01-1.26) per 10 μg/m3 NO2, which was robust in two-pollutant models and consistent across the four cohorts and according to smoking status. Sex-specific analyses suggested that this association was confined to the male population. Further, the results showed increased lymphoma HRs for PM2.5 (HR = 1.16; 95% CI: 1.02-1.34) and potassium content of PM2.5, which were consistent in two-pollutant models and according to sex. Our results suggest that air pollution at the residence may be associated with adult leukemia and lymphoma.
A systematic review and meta-analysis of haematological malignancies in residents living near petrochemical facilities

Abstract:Abstract Background The petrochemical industry is a major source of hazardous and toxic air pollutants that are recognised to have mutagenic and carcinogenic properties. A wealth of occupational epidemiology literature exists around the petrochemical industry, with adverse haematological effects identified in employees exposed to ‘low’ concentrations of aromatic hydrocarbons (benzene, toluene, ethylbenzene, and xylene). Releases from the petrochemical industry are also thought to increase the risk of cancer incidence in fenceline communities. However, this emerging and at times inconclusive evidence base remains fragmented. The present study’s aim was to conduct a systematic review and meta-analysis of epidemiological studies investigating the association between incidences of haematological malignancy and residential exposure to the petrochemical industry. Methods Epidemiological studies reporting the risk of haematological malignancies (Leukaemia, Hodgkin’s lymphoma, Non-Hodgkin’s lymphoma, and Multiple myeloma) were included where the following criteria were met: (i) Cancer incidence is diagnosed by a medical professional and coded in accordance to the International Classification of Diseases; (ii) A clear definition of fenceline communities is provided, indicating the proximity between exposed residents and petrochemical activities; and (iii) Exposure is representative of normal operating conditions, not emergency events. Two investigators independently extracted information on study characteristics and outcomes in accordance with PRISMA and MOOSE guidelines. Relative risks and their 95% confidence intervals were pooled across studies for the four categories of haematological malignancy, using a random effects meta-analysis. Results The systematic review identified 16 unique studies, which collectively record the incidence of haematological malignancies across 187,585 residents living close to a petrochemical operation. Residents from fenceline communities, less than 5 km from a petrochemical facility (refinery or manufacturer of commercial chemicals), had a 30% higher risk of developing Leukaemia than residents from communities with no petrochemical activity. Meanwhile, the association between exposure and rarer forms of haematological malignancy remains uncertain, with further research required. Conclusions The risk of developing Leukaemia appears higher in individuals living near a petrochemical facility. This highlights the need for further policy to regulate the release of carcinogens by industry. Graphical abstract

Abstract:Background: Benzene exposure has been associated with increased leukemia and other cancer risk; however, epidemiological evidence is inconsistent for the latter and confounding from smoking and alcohol was rarely adjusted. Methods: We investigated associations of occupational benzene exposure and risk of leukemia, lymphoma, myeloma, lung, stomach, liver, and kidney cancers in a population-based cohort of 61,377 men ages 40-74. A job-exposure matrix, constructed by industrial hygienists specifically for the study population, was used to derive cumulative benzene exposure from all jobs held. Cox regressions were performed to estimate adjusted hazard ratios (aHR) and 95% confidence intervals (CI) for benzene-cancer risk associations with adjustment for potential confounders. Results: Over 15-years of follow-up, 1,145 lung, 656 stomach, 445 liver, 243 kidney cancer cases, 100 leukemia, 124 lymphoma, and 46 myeloma cases were identified. Benzene exposure >550mg/m3 was associated with increased leukemia (aHR=2.3, 95%CI=1.1-4.5), lung (aHR=1.2, 95%CI=1.0-1.6), and stomach (aHR=1.4, 95%CI=1.0-1.9) cancer risk; benzene-exposure was associated with early cancer diagnosis age. The benzene-leukemia and -stomach cancer associations followed a linear dose-response pattern (Plinear=0.016 and 0.023), whereas benzene-lung cancer association was evident at higher exposure levels (Pnon-linear=0.027). Alcohol consumption modified the benzene-leukemia association (HR=3.0, 95%CI=1.1-8.3 for drinkers, aHR=0.9, 95%CI=0.4-2.0 for non-drinkers, Pinteraction=0.047). Conclusions: Benzene exposure was associated with increased leukemia, stomach, and lung cancer risk. Alcohol consumption may modify the benzene-leukemia association, although estimates are imprecise. Impact: Our study provides additional evidence that benzene exposure increases cancer risk beyond leukemia, information important for policymakers to develop programs to mitigate cancer risk among benzene-exposed workers.

Abstract:OBJECTIVES:To examine the relation between occupation and cumulative lead exposure-assessed by measuring bone lead-in a community-dwelling population. METHOD:We measured bone lead concentration with K-shell X-Ray Fluorescence in 1320 men in the Normative Aging Study. We categorized job titles into 14 broad US Census Bureau categories. We used ordinary least squares regression to estimate bone lead by job categories adjusted for other predictors. RESULTS:Service workers, construction, and extractive craft workers and installation, maintenance, and repair craft workers had the highest bone lead concentrations. Including occupations significantly improved the overall model (P < 0.001) and reduced by 15% to 81% the association between bone lead and education categories. CONCLUSION:Occupation significantly predicts cumulative lead exposure in a community-dwelling population and accounts for a large proportion of the association between education and bone lead.
Cancer Mortality of Workers Exposed to Lead:A Retrospective Cohort Study in a Smeltery

Abstract:Objective To determine the mortality of cancer and relative risk of long-term lead exposure workers and to study the relation between occupational lead exposure and cancer. Methods A cohort study including 6 971 male workers employed for at least one year before Jan 1,1985 and still alive at this time,and a subcohort of 3 344 of them worked at lead-exposed workshop was conducted.The study was followed up from Jan 1,1985 to Dec 31,1997.Standardized mortality ratios(SMR) and relative risks(RR) were calculated.Cumulative exposure dose was used for a dose-response analysis. Results For 6 971lead exposure workers,SMR and its 95%CI for lung cancer were 128.0(101.1～159.7).SMR for lung cancer was 463.7(P<0.01) among lead exposure workers employed after Jan 1,1965.SMR for nasopharyngeal cancer was 408.0(P<0.01),lung cancer 156.6(P<0.01),among workers with lead-exposed history and length of more than 20 years.RR and 95%CI for neoplasms(in total),malignant neoplasms(in total),oesophageal cancer,stomach cancer,liver and biliary tract cancer,lung cancer,bladder cancer were 3.25(2.43～3.89),3.28(2.54～4.32),3.57(1.08～10.47),2.93(1.15～4.87),4.58(2.03～9.97),8.58(4.82～15.11),6.66(2.83～13.01) respectively.RR for malignant neoplasms at medium (0.1～0.5 mg*m-3*a-1) and high(0.5～ mg*m-3*a-1) cumulative exposure doses were 1.62(P<0.05) and 1.70(P<0.05) respectively and there was an increasing trend of RR as the doses raised.RR for lung cancer at medium and high exposure doses were 2.55 (P<0.05) and 2.31(P<0.05) respectively. Conclusions The increased death risks may relate to lead exposure.However,further study under controlling the harmful factors of other occupational exposure and non-occupational exposure is necessary.

Abstract:Objectives: 1,3-Butadiene (BD) exposure's link to leukemia is under regulatory scrutiny. The assessment methods for BD exposure risks have evolved from early animal and limited human studies to advanced exposure-response modelling with comprehensive quantitative data. The objec- tive of this study is to explore the nuances of exposure-response modelling, investigating how various statistical methods have influenced the quan- tification of exposure-response relationships. Material and methods: Although this study was not conducted as a formal systematic review, a search was performed in Medline/Pubmed to identify all human studies on leukemia risk assessment for BD exposure. This search included articles written in English. The electronic search spanned from inception of records until July 23, 2023, using the search term: "butadiene AND (leukaemia OR leukemia OR myeloid OR lymphoid)" and was restricted to human species. Focusing on the synthetic styrene-butadiene rubber (SBR) industry cohort study conducted by the University of Alabama at Birmingham, USA, this review evaluates various statistical models and factors influenc- ing exposure-response modelling. Results: Peak exposures to BD may be more influential in the dose-response relationship than cumulative or long-term exposure. The authors recommend utilizing β-coefficients derived from the latest SBR study update, employing Cox proportional hazard modelling, non-lagged and non-transformed cumulative BD exposure, and adjusting for age and peak BD exposure. The study reveals that statistical model selection has a limited impact on the calculated dose-response effects. The significant variation in estimated cancer mortality values arises from additional assumptions needed for metrics like the excess leukemia risk or the occupational BD effective concentration. Conclusions: In con- clusion, this study provides insights into exposure-response modelling for BD exposure and leukemia mortality, highlighting the importance of peak exposures. The recommended statistical approach offers a reliable basis for regulatory risk assessment and public health population metrics. Int J Occup Med Environ Health. 2024;37(3):300-10.
Targeting of workplace inspections for lead

Abstract:The prevention of occupational lead poisoning requires identification of worksites with ongoing excessive lead exposures. The utility of different sources of surveillance data in identifying worksites was evaluated by comparing a list of companies inspected by the Occupational Safety and Health Administration (OSHA) for lead with 1) Ohio Bureau of Workers' Compensation (BWC) claims for lead poisoning, and 2) the New York Health Department's Heavy Metal Registry (NYHMR) reports of individuals with elevated blood lead levels. For the period 1981 through 1985, the NYHMR identified 179 companies with at least one employee having an elevated blood lead level. Of the 134 OSHA inspections conducted in New York during the same time period, 23 (17%) companies were identified by the NYHMR. In Ohio from 1979 through 1985, 50 companies had workers' compensation claims filed against them involving documented elevated blood lead levels. OSHA inspected 306 companies; 23 (7.5%) were identified by the BWC. In both states, companies inspected by OSHA were concentrated in larger industries with traditional, well-recognized lead hazards (e.g., primary metal and fabricated metals). Companies identified by compensation claims and laboratory reports tended to be in industries dominated by smaller establishments where lead is not a primary part of the industrial process (e.g., automotive repair and construction). Sources of surveillance data, such as workers' compensation claims and laboratory reports, identify worksites that tend not to be routinely inspected by OSHA and which need intervention to prevent excessive lead exposure. To maximize the impact of public health resources devoted to the elimination of occupational lead poisoning, follow-up efforts at companies identified by state health departments and workers' compensation systems offer an important opportunity to complement OSHA's inspection efforts.
Population-based study of amyotrophic lateral sclerosis and occupational lead exposure in Denmark

Abstract:Objectives: Previous research has indicated links between lead (Pb) exposure and increased risk of neurodegenerative disorders, including amyotrophic lateral sclerosis (ALS). In this study, we evaluated the association between occupational Pb exposures and ALS. Methods: ALS cases were ascertained through the Danish National Patient Registry from 1982 to 2013 and age and sex-matched to 100 controls. Using complete employment history since 1964 from the Danish Pension Fund, cumulative Pb exposure was estimated for each subject via a Danish job exposure matrix. Associations were evaluated using conditional logistic regression analyses and stratified by sex. Results: For men with >50% probability of exposure, there was an increase in odds of ALS for exposures in the 60th percentile or higher during any time 5 years prior to diagnosis (aOR: 1.35; 95% CI 1.04 to 1.76) and 10 years prior to diagnosis (aOR: 1.33; 95% CI 1.03 to 1.72). No significant associations were observed in women, and there were no linear trends seen for Pb exposures for either sex. Conclusions: Our study indicates an association between consistently higher occupational Pb exposures and ALS. These findings support those of previously reported associations between ALS and specific occupations that commonly experience Pb exposure.

Abstract:A a common chronic illness among children and youth, affects one school-aged child in 11, or two to three students in a class of 30. Asthma accounts for 14 million lost school days each year. Students with asthma miss instruction while in the health room and may not be fully alert due to lack of sleep from night time asthma symptoms. Schools often lose funding for student absences. Schools are responsible for ensuring the health of all students while at school. Students with asthma may experience serious or life-threatening episodes at school, so schools need to know the students who have the illness and what staff can do to prevent asthma episodes or to provide quick intervention. How can schools deter mine who has asthma? School health coordinators, school nurses, and school medical consultants often face this question: “ Doctor Jones spoke with the principal and strongly urged us to provide asthma screening for all our students, but should we do this when our resources are limited? What should we do?”

Abstract:Objective Study carcinogenicity of inorganic lead, classified as ‘probably carcinogenic’ to humans by the International Agency for Research on Cancer (brain, lung, kidney and stomach). Methods We conducted internal and external analyses for cancer incidence in two cohorts of 29 874 lead-exposed workers with past blood lead data (Finland, n=20 752, Great Britain=9122), with 6790 incident cancers. Exposure was maximum measured blood lead. Results The combined cohort had a median maximum blood lead of 29 μg/dL, a mean first blood lead test of 1977, and was 87% male. Significant (p 40 μg/dL) showed a significant excess for lung cancer in both countries combined, and significant excesses in Finland for brain and lung cancer. The Great Britain data were limited by small numbers for some cancers, and limited variation in exposure. Conclusions We found strong positive incidence trends with increasing blood lead level, for several outcomes in internal analysis. Two of these, lung and brain cancer, were sites of a priori interest.
