Efficacy of emergency percutaneous coronary intervention for acute myocardial infarction patients with bifurcation lesion

Abstract:Objective To evaluate the efficacy of emergency percutaneous coronary artery intervention(PCI)for acute myocardial infarction(AMI)patients with bifurcation lesion.Methods A total of 1209 AMI patients who underwent emergency coronary angiography(CAG)and PCI within 12 hours of onset in the Department of Cardiology of Shenyang Northern Hospital from January 2004 to June 2008 had been analyzed.There were 1037 patients with acute ST-elevation myocardial infarction(STEMI)and 172 patients with acute non-ST-segment elevation myocardial infarction(NSTEMI)were involved.These patients were divided into bifurcation lesion(BF)group(n=247,20.4%)and nonbifurcation lesion(NBF)group(n=962,79.6%)according to coronary infarct-related artery(IRA).The coronary angiographic characteristics,procedural success rates and the rate of major adverse cardiac events(MACE)at 1 year after PCI were assessed.Results The differences of mean age,sex,smoking history,hypertension,diabetes,hypercholesteremia,old myocardial infarction(OMI)and the percentage of New York Heart Association(NYHA)heart function class Ⅲ to Ⅳ were statistically insignificant between the two groups.IRA of patients were all treated with drug eluting stents(DES).Among the BF patients,220 received only stenting of the main branch(89.1%),102 used guide wire for side branch protection(41.3%),and 27 patients received stenting of both main branch and side branch.There were no significant differences in the instant success rate of PCI between the BF and the NBF groups(96.8% vs.98.2%,χ2=2.10,P=0.15).There was no significant difference between the two groups in occurrence of MACE during hospitalization(4.9% vs.3.2%,χ2=1.53,P=0.22)and 1 year later(14.2% vs.12.5%,χ2=0.51,P=0.48).Conclusions Emergency PCI has higher success when used in AMI patients with bifurcation lesion,and can achieve similar acute and long-term clinical results when used in AMI patients with nonbifurcation lesion.
