Experimental study on brain injury after deep hypothermic circulatory arrest determined by diffusion weighted imaging

Abstract:Objective: In order to reduce neurological complications after cardiac surgery,we want to develop and evaluate a imaging method for early assessment of brain injury after deep hypothermia circulatory arrest(DHCA).Methods: Animal model of DHCA were established with 12 healthy adult miniature male pigs which were randomly divided into two groups,with six in each group.The experimental group underwent DHCA,while the control group were performed anesthesia only.All animals were alive after operation.All animals were underwent diffusion-weighted magnetic resonance imaging(DWI),T1WI,T2WI;on the first preoperative day and first postoperative day,respectively.Fluoro jade-c staining and Nissl staining were performed to detect the histological and pathological changes.TUNEL assay was carried out to detect apoptosis.The results of DWI were compared with that of histological and pathological assays to verify the effectiveness of DWI in brain injury early assessment after DHCA.Results: In control group,no new focal brain lesions were detected by postoperative DWI,T1WI,T2WI.In experimental group,new focal brain lesions were detected by postoperative DWI but not by T1WI and T2WI.Denatured neurons were also detected by Fluoro jade-c staining,TUNEL and Nissl staining in the same brain lesions and showed consistent results with DWI.Conclusion: DWI can assess brain injury after DHCA,furthermore,DWI could found brain lesions that were not detected by T1WI,T2WI.
