Predictive Value of Left Ventricular Endocardial Pacing in Percutaneous Transluminal Septal Myocardial Ablation of Hypertrophic Obstructive Cardiomyopathy

Abstract:Objective To study the prognostic value of left ventricular endocardial pacing in percutaneous transluminal septal myocardial ablation(PTSMA)in patients with hypertrophic obstructive cardiomyopathy(HOCM).Methods Fifteen HOCM patients with resting left ventricular outflow gradient(LVOTG)≥30 mmHg and/or stress LVOTG≥50 mmHg were included in this study.Left ventricular endocardial pacing was attempted in the patients 20 minutes before PTSMA.Heamodynamic parameters and LVOTG were monitored during operation and follow up echocardiography was performed 1 month after the operation.Results The mean chickness of the ventricular septals decreased from 22.07±7.85 mm before PTSMA to 17.39±5.79 mm 1 month afterwards(P0.01).The resting LVOTG dropped from 56.02±32.86 mmHg to 15.23±6.67 mmHg(P0.001)and the stress LVOTG dropped from 74.87±32.52 mmHg to 20.53±12.28 mmHg(P0.001)after 1 month.Among the 15 patients,left ventricular endocardial pacing was performed successfully in 10 patients and in whom successful PTSMA was achieved.PTSMA was failed in 3 out of the 5 patients whose application of left rentricular endocardial pacing was failed as well.The resting LVOTG dropped from 50.40±39.41 mmHg before pacing to 30.30±29.85 mmHg at 10 mins after pacing while stress LVOTG dropped from 122.53±52.52 mmHg to 57.40 mmHg 40.79 mmHg(both P0.05).By crosstabs Kappa analysis,the healing effect of PTSMA was correlated with the effect of left ventricular endocardial pacing in terms of reduction of LVOTG(correlation coefficient:Kappa=0.842,P=0.001).Conclusion The reduction of LVOTG after left ventricular endocardial pacing in HOCM patients may be a predictor of acute LVOTG reduction after PTSMA.
