The progress of relationships between ghrelin and osteoporosis

Abstract:Ghrelin,a 28-amino acid brain-gut peptide that mediates multiple biological effects of growth hormone (GH),is the endogenous ligand of the growth hormone secretagogue(GHS) receptor(GHSR).People used to believe that osteopenia caused by anorexia nervosa(AN) and gastrectomy was related to the malabsorption of vitamins and minerals.In fact,there is emerging evidence indicating a direct involvement of ghrelin in bone metabolism,which make it a candidate hormone of secondary osteoporosis.It has been demonstrated in many clinical studies that bone mineral density (BMD) or bone mineral mass(BMC) is associated with initial ghrelin levels.Osteoblasts are found to respond to ghrelin by increased proliferation,differentiation and mineralization in vitro.However,ghrelin inhibits early osteogenic differentiation of bone mesenchymal stem cells(MSC),and stimulates its adipogenic differentiation from preadipocytes.Among the adolescents with anorexia nervosa(AN),elevated ghrelin levels,leading an acquired GH resistant state and higher adrenocorticotropin secretion,may contribute to systemic bone loss and capacity failure for attainment of peak bone mass.
