Analysis of Clnical Results and Complications of Cementless Total Hip Arthroplasty in Elderly

Abstract:Objective To analyze and summarize the clinical results and complications of cementless total hip arthroplasty in the elderly. Methods From February 2003 to August 2007, 28 patients (28 hips) underwent cementless total hip arthroplasty. The diagnosis included osteoarthritis in 16 cases,avascular necrosis of the femoral head in 7 cases,old femoral neck fractures in 3 cases,fresh acetabular fracture combine with femoral neck fracture in 1 case, and old acetabular fracture in 1 case. Clinical and radiographic evaluations were performed on every follow-up, and intraoperative and postoperative complications were recorded. Results All the patients were followed up with an average of 2.5 years (range 1~5.5 years).The mean preoperative Harris hip score was 42 (range 8 to 54),which were improved to 87 (range 66 to 97) at the latest follow-ups; The results were excellent in 9 cases,good in 16,fair in 2,and poor in 1. The excellent and good rate was 89.3%. Before operation, five (17.9%) patients suffered from moderate pain,19 (67.9%) severe and 4 (14.3%) very severe pain, while at the time of follow-up, 25 patients (89.3%) were free from pain, two (7.1%) had minimal pain and one (3.6%) slight pain. X-ray film showed that the acetabular cup abduction angle averaged 43.5°(range 38°to 56°)and the anterversion angle averaged 13.5°(range 4°to 20°), no socket demonstrated migration and loosening.Twenty-seven of the femoral components had radiographic evidence of bone ingrowth, and fibrous ingrowth in 1 case.Three of the 28 stems had subsidence of less than 2 mm.Postoperative complications included dislocation in 3 cases 3/28 (10.7%), superficial infection in 1 case and heterotrophic ossification in 2 cases. Conclusion In the short term, cementless fixation for total hip arthroplasty in the elderly can decrease pain and improve the joint function. But we must also know and recognize the complications in aged patients.
