Abstract:Objective To compare clinical effectiveness of neurocombing (NC) and percutaneous radiofrequency thermocoagulation (RF) in the treatment of primary trigeminal neuralgia and evaluate its clinical significance.Methods Thirty seven patients with trigeminal neuralgia without neurovascular compression and undergoing neurocombing (NC group) and 37 trigeminal neuralgia patients obtaining percutaneous radiofrequency thermocoagulation (RF group) were given 3-year follow-up.Postoperative effectiveness rate,recurrence rate and occurrence rate of facial numbness were recorded and compared in the two groups of patients.Results In the NC group and RF group,postoperative 3-year effectiveness rate was 78.4％ (29/37) and 73.0％ (27/37) respectively.Postoperative 3-year recurrence rate was 21.6％ (8/37) and 27.0％ (10/37) respectively,as well as postoperative occurrence rate of facial numbness was 91.9％ (34/ 37) and 100％ (37/37).All intergroup differences were not statistically significant (P all ＞ 0.05).Conclusion NC and RF are both satisfactory treatment strategies for patients with primary trigeminal neuralgia.When taking into account the risk of craniotomy,RF is preferable to NC in some elderly patients with trigeminal neuralgia without neurovascular compression.

Abstract:Background Trigeminal neuralgia is a common clinical chronic neuropathic pain syndrome,the treatment for which includes pharmacological therapy,minimally invasive treatment,operation and so on.Minimally invasive procedures are mainly performed under the guidance of X-ray or CT.Ultrasonic technique is becoming a promising new tool in this field for its realtime visualization without X-ray exposure.Objective To summarize the relevant literatures of minimally invasive treatment of trigeminal neuralgia under ultrasound guidance,and provide a theoretical basis for its clinical application.Content To explore the anatomic landmark and localization,operating procedure,efficacy,indications and potential complications of ultrasound-guided trigeminal nerve block.Trend The efficacy and operating details of this approach should be further investigated.
Pulsed Radiofrequency for the Treatment of Trigeminal Neuralgia

Abstract:Purpose of review: Trigeminal neuralgia is a sudden, unilateral, stabbing pain in the distribution of one or more branches of the fifth cranial nerve, with an overall prevalence ranging between 0.03 and 0.3%. While conservative treatments may offer temporary relief, many patients experience chronic headaches associated with their neuralgia. Invasive treatments are available for patients with intractable neuralgia; however, they may cause permanent tissue damage and often do not provide relief. This article examines pulsed radiofrequency (PRF) ablation (RFA) of the trigeminal nerve as a minimally invasive procedure that offers a promising alternative to invasive procedures for relief of trigeminal neuralgia while minimizing tissue damage. Recent findings: Efficacy of PRF and RFA in treating trigeminal neuralgia has been studied before, but literature lacks large size studies. The results of this retrospective study indicate that PRF can be used as a safe and effective treatment for patients suffering from trigeminal neuralgia that is refractory to conservative measures.

Abstract:There is a lack of certainty regarding the etiology and pathophysiology of primary trigeminal neuralgia (PTN). This disease can be treated with several ways. Oral medical treatment is preferred firstly now, when it fails or severe adverse reactions cannot be tolerated by patients, surgical treatment becomes necessary, including microvascular decompression, radiofrequency thermocoagulation, gamma knife radio surgery, balloon compression, and so on. This multitude of treatments always puzzles the doctors and patients, which treatment fits which patients best. In this summary, the progresses of current treatments of PTN are reviewed.

Abstract:Trigeminal neuralgia (TN) is a debilitating condition characterized by severe facial pain. Various surgical interventions are employed to manage this condition, including microvascular decompression (MVD), percutaneous radiofrequency rhizotomy (PRR), glycerol rhizotomy, percutaneous balloon compression (PBC), and stereotactic radiosurgery such as Gamma Knife radiosurgery (GKRS). This review synthesizes the outcomes of these interventions to provide an understanding of their efficacy and associated risks. MVD, known for its high initial relief rates, shows substantial long-term effectiveness, with recurrence rates varying based on patient demographics and comorbidities. GKRS offers significant pain relief with a favorable adverse event profile; however, recurrence rates increase over time, necessitating repeat procedures for sustained efficacy. PBC demonstrates high initial success, but pain recurrence is common, especially in patients with atypical TN. PRR provides immediate relief with a manageable recurrence rate and is particularly suitable for elderly patients and those with comorbidities. Glycerol rhizotomy, a cost-effective procedure, yields comparable outcomes to other interventions but requires careful patient selection. This review highlights the importance of tailored treatment approaches based on individual patient profiles, emphasizing the need for precise diagnostic criteria and careful patient selection to optimize outcomes. Long-term follow-up and the potential for repeat interventions are critical considerations in managing TN surgically.

Abstract:Trigeminal neuralgia (TN) is a unilateral, paroxysmal, sharp, shooting, or jabbing pain that occurs in the trigeminal nerve divisions, including the ophthalmic (V1), maxillary (V2), and mandibular (V3) nerves. Typically, an episode is triggered by anything touching the face or teeth. TN is a clinical diagnosis with no specific diagnostic test; it is determined by the patient's medical history and pain description. Imaging is necessary to exclude secondary causes. The precise reason for TN is uncertain, but it is commonly believed to result from vascular compression of the trigeminal nerve root, typically near its origin in the pons. There are numerous surgical and medical treatment options available. The most frequently applied medical treatment therapies are carbamazepine and oxcarbazepine. Surgical alternatives are reserved for patients who do not respond to medical treatment. Botulinum toxin A (BTX-A) has emerged as a novel and promising alternative to surgery for individuals whose pain is unresponsive to medication. Multiple studies have established the safety and usefulness of BTX-A in treating TN, with the most significant benefits occurring between six weeks and three months after the surgery. This article reviews various studies published in the last 10 years regarding the therapeutic use of BTX-A in TN. These studies include various observational, clinical, pilot, and animal studies.

Abstract:<h3 class="Heading">Purpose of Review</h3><p class="Para">Trigeminal neuralgia (TN) is characterized by recurrent attacks of lancinating facial pain in the dermatomal distribution of the trigeminal nerve. TN is rare, affecting 4 to 13 people per 100,000.</p><h3 class="Heading">Recent Findings</h3><p class="Para">Although there remains a debate surrounding the pathogenesis of TN, neurovascular compromise is the most currently accepted theory. Minimal stimulation caused by light touch, talking, or chewing can lead to debilitating pain and incapacitation of the patient. Pain may occur sporadically, though is primarily unilateral in onset. The diagnosis is typically determined clinically. Treatment options include medications, surgery, and complementary approaches.</p><h3 class="Heading">Summary</h3><p class="Para">Anti-epileptic and tricyclic antidepressant medications are first-line treatments. Surgical management of patients with TN may be indicated in those who have either failed medical treatment with at least three medications, suffer from intolerable side-effects, or have non-remitting symptoms. Surgical treatment is categorized as either destructive or non-destructive. Deep brain and motor cortex neuro-modulatory stimulation are off label emerging techniques which may offer relief to TN that is otherwise refractory to pharmacological management and surgery. Still, sufficient data has yet to be obtained and more studies are needed.</p>

Abstract:Purpose of review: Postherpetic neuralgia is an annoying pain that mainly affects older people. In order to give patients more options, this review summarizes the pharmacological and interventional treatments for postherpetic neuralgia and updates the research on the efficacy, thereby providing doctors with more treatment options. The adverse effects and effective doses of its various treatments are also presented so that the therapy can be prescribed according to their concrete physical conditions. In a word, this review is dedicated to providing a comprehensive overview of the treatment options for postherpetic neuralgia and offering patients more choices. Recent findings: Combinational therapy is more excellent than monotherapy. The local anesthesia and gabapentin comprised outstanding compatibility. In addition, two therapeutic tools for PHN patients, especially for the intractable ones, electroacupuncture (EA), and osteopathic manipulative treatment (OMT), show their efficacy and become potential options to alleviate pain. In terms of treatment, guidelines recommend patients use tricyclic antidepressants (TCAs), gabapentin, pregabalin, and 5% lidocaine patches as the first-line medications, and gabapentin is investigated most, especially the gabapentin enacarbil (GEn). And drug efficacy can be limited by adverse effects and tolerated doses. Interventional treatments, with their invasiveness and operational difficulty, are usually considered for intractable patients. Combinational therapies may be used when a single therapy cannot achieve the desired effect. Therapies such as OMT and EA have also been proposed to palliate pain in some cases, and future directions of treatment may be investigated in Chinese medicine and acupuncture.

Abstract:Trigeminal neuralgia is a syndrome of unilateral, paroxysmal, stabbing facial pain, originating from the trigeminal nerve. Careful history of typical symptoms is crucial for diagnosis. Most cases are caused by vascular compression of the trigeminal root adjacent to the pons leading to focal demyelination and ephaptic axonal transmission. Brain imaging is required to exclude secondary causes. Many medical and surgical treatments are available. Most patients respond well to pharmacotherapy; carbamazepine and oxcarbazepine are first line therapy, while lamotrigine and baclofen are considered second line treatments. Other drugs such as topiramate, levetiracetam, gabapentin, pregabalin, and botulinum toxin-A are alternative treatments. Surgical options are available if medications are no longer effective or tolerated. Microvascular decompression, gamma knife radiosurgery, and percutaneous rhizotomies are most promising surgical alternatives. This paper reviews the medical and surgical therapeutic options for the treatment of trigeminal neuralgia, based on available evidence and guidelines.

Therapeutic Efficacy and Safety of Botulinum Toxin Type A in Trigeminal Neuralgia: a Systematic Review

Abstract:Trigeminal neuralgia is a common disorder caused mainly by compression of the trigeminal nerve root by an overlying blood vessel. Pharmacotherapy and surgery are ineffective or unsuitable in many patients. Therefore, other therapeutic modalities have been tried, including injection of botulinum toxin type A (BTX-A). This study aims to systematically review the therapeutic efficacy and safety of BTX-A in trigeminal neuralgia. PubMed, EMBASE, Cochrane Library Clinical Trials and Web of Science from January 1966 to March 2013 were searched with the terms of “botulinum toxin” AND “trigeminal neuralgia”, and references of related articles were traced. Data on the efficacy and safety of BTX-A in this disorder were extracted and analyzed by at least 2 reviewers. Data for individual studies were reported, and pooled data were analyzed if appropriate. Five prospective studies and one double-blind, randomized, placebo-controlled study were identified. Response was achieved in approximately 70-100% of patients, and the mean pain intensity and frequency were reduced by approximately 60-100% at 4 weeks after treatment in most studies. Major adverse events were not reported. Available studies show BTX-A may be effective in treatment of trigeminal neuralgia. However, well-designed randomized, controlled, double-blinded trial is still lacking. Future BTX-A treatment studies on optimal dose, duration of the therapeutic efficacy, common AEs, and the time and indications for repeat injection would be promising.

Abstract:Trigeminal neuralgia (TN) is a chronic pain condition that affects the trigeminal nerve, the largest of the cranial nerves. It is characterized by severe, sudden, and recurrent facial pain, often triggered by light touch or a breeze. Treatment options for TN include medication, nerve blocks, and surgery, but radiofrequency ablation (RFA) has emerged as a promising alternative. RFA is a minimally invasive procedure that uses heat energy to destroy the small portion of the trigeminal nerve responsible for the pain. The procedure is performed under local anesthesia and can be done as an outpatient procedure. RFA has been shown to provide long-term pain relief for TN patients with a low complication rate. However, RFA is not suitable for all TN patients and may not be effective for those with multiple pain sites. Despite these limitations, RFA is a valuable option for TN patients who are not responding to other treatments. Furthermore, RFA is a good alternative for a patient unsuitable for surgery. Further research is needed to fully understand the long-term effectiveness of RFA and identify the best candidates for the procedure.
BLOCKING THERAPY OF TRIGEMINAL NEURALGIA:A LITERATURE REVIEW

Abstract:Objective:To review the advance of medicine blocking therapy for neuralgia pain by evidence-based medicine methods.Methods:Literature search was conducted on the following databases:PubMed,CBM,CNKI,Wanfang data,with the keywords of trigeminal neuralgia,blocking and medicine.Articles about blocking therapy of neuralgia were found and analysed.Results:26 articles among 196 ones were enrolled finally according to the criteria of enrolled elimination and literature quality.The frequently-used medicines of blocking treatment include absolute ethyl alcohol,glycerin,Vit B12,some antibiotics,chlorpromazine and TCM.All of these medicines were mostly effective and safe.Conclusion:For these patients who were not suitable for a surgery because of age or systemic diseases,blocking therapy using certain drugs was a better choice,but more clinical evidences were needed to support this conclusion.
Selective Percutaneous Radiofrequency Thermocoagulation for the Treatment of Trigeminal Neuralgia: 1860 Cases Report

Abstract:Objective Selective percutaneous radiofrequency thermocoagulation were performed on 1860 cases with trigeminal neuralgia. X-ray 3D-CT and navigation oval foramen locations were used in 22 19 and 1 patient respectively on intractable trigeminal neuralgia. Methods Supraorbital foramen infraorbital foramen and oval foramen radiofrequency thermocoagulation were performed on 1860 cases of trigeminal neuralgia. X-ray 3D-CT and navigation oval foramen locations were used in 42 cases intractable location so that the direction and position could be defined for trigeminal neuralgia. Result Pain alleviation immediately but no serious complication occurred in all of the patients. The effective rate is 96.3%. Conclusion Selective percutaneous radiofrequency thermocoagulation is an effective way for the treatment of trigeminal neuralgia. X-ray 3D-CT and navigation oval foramen locations can raise the successful rate of puncture enhance the safety and reduce the incidence rate of complication.

Abstract:Introduction Medically refractory cases of trigeminal neuralgia often require treatment escalation. Surgical options include microvascular decompression and percutaneous ablation. This paper provides a bibliometric analysis of the most influential papers on trigeminal neuralgia surgical management. Methods Web of Science was queried to identify the top 100 cited articles concerning trigeminal neuralgia surgical treatment. Search term used: "ALL=(("trigeminal neuralgia" or "tic douloureux" or "Fothergill's disease" or "Trifacial neuralgia")AND("surgical treatment" or "surgical management" or "surgery" or "neurosurgery")NOT("radiosurgery" or "gamma knife"))." Extracted variables included first and senior author name, journal, publication year, institution, and surgical modality. Results Our bibliometric search yielded 2,104 studies, with 41,502 citations overall. Within the top 100 articles, Zakrzewska had the most first author papers (n=5), and Burchiel had the most senior author papers (n=6). The Massachusetts General Hospital was the most represented institution (n=5). The United States was the most represented country (51%). Microvascular decompression was the most studied surgical strategy (51%), followed by percutaneous radiofrequency coagulation (9%), balloon/nerve compression (7%), and glycerol rhizolysis (7%). Some studies assessed multiple treatment modalities (22%). Types of studies included retrospective articles (58%), prospective articles (26%), reviews (10%), anatomic studies (2%), and basic science (1%). Neurosurgery (35%) and the Journal of Neurosurgery (33%) were the most represented journals. Conclusion The current literature consists of retrospective reviews and mostly describes microvascular decompression for trigeminal neuralgia. Future studies should include further characterization of other surgical modalities such as percutaneous radiofrequency thermocoagulation, glycerol injection, and balloon compression.

Abstract:Trigeminal neuralgia (TN) is a neuropathic pain disorder that affects the trigeminal nerve distribution area. Pharmacological therapy has remained the first-line treatment for TN. If pharmacological intervention is not effective, surgical treatments including radiosurgery (mainly gamma knife therapy), radiofrequency (RF) of the Gasserian ganglion, and microvascular decompression have been utilized. RF is one of the most common procedures used to treat TN. Two RF approaches are commonly utilized: conventional radiofrequency (CRF) and pulsed radiofrequency (PRF). Both methods have been used to successfully treat TN; however, each procedure has distinct advantages and disadvantages. This article summarizes the current relevant literature to compare the treatment of TN with CRF vs. PRF. We discuss the treatment indications, operative methods, and complications of each treatment strategy. Most of the patients treated with CRF had a satisfactory outcome, whereas most PRF patients still had significant pain after the procedure. The application of a higher voltage can improve the curative effect of PRF, and its complications are less than CRF. Both CRF and PRF can be used to treat TN, and the former was more effective. Although the complications of CRF are more than those of PRF, most of them were short-lived and lacked sequelae. The pain relief rate in PRF-treated patients was significantly lower compared to CRF-treated patients. Furthermore, recurring pain was more commonly observed in PRF-treated patients. Therefore, CRF may be the preferred treatment option for TN, whereas treatment with PRF requires further study.

Abstract:Trigeminal neuralgia is a very painful neurological condition with severe, stimulus-evoked, short-lasting stabbing pain attacks in the face. The past decade has offered new insights into trigeminal neuralgia symptomatology, pathophysiology, and treatment, leading to a change in the classification of the condition. An accurate diagnosis is crucial because neuroimaging interpretation and clinical management differ among the various forms of facial pain. MRI using specific sequences should be a part of the diagnostic workup to detect a possible neurovascular contact and exclude secondary causes. Demonstration of a neurovascular contact should not be used to confirm a diagnosis but rather to facilitate surgical decision making. Carbamazepine and oxcarbazepine are drugs of first choice for long-term treatment, whereas microvascular decompression is the first-line surgery in medically refractory patients. Advances in neuroimaging techniques and animal models will provide further insight into the causes of trigeminal neuralgia and its pathophysiology. Development of more efficacious treatment options is highly warranted.

Abstract:Tumor-related trigeminal neuralgia (TN) is a deeply debilitating condition that severely impacts patient quality of life. Two principal treatment methods in use are open surgical resection of the causative tumor or the use of stereotactic radiosurgery (SRS). In this letter, we aim to evaluate the use of both treatment methods and highlight that in patients with commensurate anatomy, open surgical resection continues to provide greater rates of symptomatic relief, lower rates of recurrence, and complication compared to stereotactic radiosurgery.
Clinical study on trigeminal neuralgia treatment with minimally invasive multipoint radiofrequency thermal coagulation

Abstract:Objective To observe the clinical efficacy of multipoint radiofrequency thermal coagulation on trigeminal neuralgia management. Methods 180 patients diagnosed as primary trigeminal neuralgia were included in this study. The patients were randomly divided into three groups, 60 patients in treatment group, 60 patients in the first control group and 60 patients in second control group respectively. Multipoint radiofrequency ablation therapy was utilized in the treat-ment group, radiofrequency thermal coagulation therapy on trigeminal ganglion was applied in the first control group and patients in second control group were treated by nerve block therapy. Then the curative effect of the three groups after the treatment was compared and analyzed. Results To the curative effects, the VAS score showed insignificant difference between treatment group and the first control group ( P>0. 05) , while an significant difference between treatment group and the second control group ( P<0. 05) . To the complications concerned, higher occurrence was found in the first con-trol group. There was no significant difference between the treatment group and the first control group in the recurrence rate ( P>0. 05) . Conclusion The minimally invasive multipoint radiofrequency ablation on the treatment of trigeminal neuralgia has significant effect with fewer complications and higher patients safety.
