Bleeding management during and after operation in patients with cholelithiasis and liver cirrhosis

Abstract:Objective To summarize the experience in management of peri-and postoperative bleeding in patients with cholelithiasis and liver cirrhosis. Methods The clinical data of 45 patients with cholelithiasis and liver cirrhosis receiving operation in our hospital from January 2000 to July 2005 were retrospectively analyzed. Results Of the 45 patients, 38 were cured and 7 died (15. 6%). For the latter 7, 5 died of hepatic-renal failure and MODS and 2 mass bleeding of gallbladder bed. The 7 dead cases had the significant correlation with Child-Pugh classification: Child A accounting for 5. 6%, Child B for 16. 7% and Child C for 33. 3%. Amongst the 9 patients (20%) with postoperative bleeding, 6 had intraabdominal bleeding and 3 gastrointestinal bleeding. Six of the 9 bleeding patients died after the operation. Conclusions The perioperative management of cholelithiasis and liver cirrhosis should be strengthened to decrease the death rate. The cholelithiasis should be firstly managed upon emergency treatment It is safe to perform laparoscopy in the patients of Child A. The safest method for treatment of cholelithiasis and liver cirrhosis is devascularization and shunt first then bile duct operation.
