Abstract:Objective To investigate the community health care needs of patients with advanced cancer,so as to provide evidence for better community palliative care.Methods A questionnaire survey was used to investigate 113 advanced cancer patients in 6 communities in Shanghai.Results There is an increasing need for palliative care service in patients with advanced cancer.Community was the main place of cancer caring.The patients' needs covered a wide range of aspects and varied in different situations,including physiological,psychological,psychiatric,social and cultural aspects.Conclusions Palliative care team consisting of members of multidisciplinary professionals should be set up as the a basic unit in community cancer caring.
Medical Workers' Perceptions on What Palliative Home Care Service Content Need to Be Provided in the Communities

Abstract:Objective To explore what palliative home care services need to be provided for advanced cancer patients and their families.Methods Sixty-four primary health care workers and 57 medical workers from oncology ward of tertiary hospitals were investigated by a self-designing survey about what palliative home care services need to be provided for advanced cancer patients and their families in Shanghai.The average ages of responders from communities and hospital were 39.02±10.66 and 29.82±5.36.The differences on ages and work-times and education were significant(P0.01).Results Responders were predominantly femal.All subjects support actively developing palliative home care service;Subjects from hospitals knew well about palliative care ones from communities knew a little(χ2=54.55,P0.01),but acquaintances on community health care were just the reverse(χ2=21.31,P0.01;More than 79 percent of subjects thought it was important to develop 28 of all 31 items,but they disagreed on the 7 items from physical care field(P0.05).Conclusion In principle,the palliative home care service including 31 items is necessary to be practiced,indicating that the necessity of integration of the opinions from community health care workers and oncologists cancer nurses should be considered.

Abstract:Hospitals have played a leading role in providing palliative care in Taiwan as its care model has developed over the past few decades. However, earlier local studies in Taiwan showed that terminal patients prefer to die at home, highlighting the need to promote community-based palliative care instead of hospital-based care. Along with this shift, how community nurses provide palliative home care merits further exploration. This qualitative descriptive study aims to understand (1) how community nurses implement community-based palliative care, (2) what preparations are needed, and (3) what challenges they may face. Purposive sampling was used for recruiting nurses. We conducted one-on-one, in-depth, semi-structured interviews. Interview recordings were transcribed verbatim and analyzed using thematic analysis. Eight community nurses with a range of experience in palliative home care were interviewed. Four major themes emerged: (1) Opportunities, (2) Qualifications, (3) Support, and (4) Commitments. Psychological preparedness, well-developed professional capabilities, external assistance, and peer support motivate community nurses to offer community-based palliative care. As the requests for palliative home care services increase, community nurses play a critical role in palliative home care. Although the sample size is small and the findings retrieved from a small number of experiences might not be generalized to every region, the study results could inform future experience-sharing and workshop sessions to train more nurses for community-based care, expanding service coverage, and providing optimal palliative care.

Abstract:Objective? To explore the working characteristics and difficulties of hospice nurses and provide reference for improvement of palliative care practice. Methods? By purposive sampling, 10 nurses from a cancer specialist hospital and a community hospital in Beijing were selected and interviewed by semi-structured in-depth interviewing method and the interview data was analyzed by Colaizzi method. Results? The palliative nurses need patience and detailed mind, sympathy, compassion and empathy; the working characteristics of nursing included establishing trust, all the way accompany; palliative nursing and nurses have mutual influences: while getting the self-improved and sense of accomplishment, nurses' psychological burden was heavy and needed psychological adjustment; the palliative nurses faced difficulties such as unsuccessful communication with patients and their family members, lacking in relevant knowledge, misunderstanding from the colleagues and short of medical resources. Conclusions? Hospitals should strengthen the emotional adjustment support for the palliative nurses and to relieve the nurses from stress and negative affect; the government and hospitals should strengthen the propaganda of palliative care so that the citizens can better understand and accept palliative care.

It put forward that one should change one's idea in constructing palliative nursing team in China, and one should vigorously generalize palliative nursing education. Abstract:Objectives: This study aimed to obtain an overview of the current research status of palliative care in Mainland China and identify research directions for future studies by characterizing palliative care studies conducted among patients with any life-limiting illness in Mainland China and published in a peer-reviewed journal before November 2016. Methods: A review guide with 7 categories was initially developed based on existing international palliative care definitions and guidelines through content analysis. Ten databases were used to identify relevant studies from the inception of online cataloging to November 2016. Studies conducted in Mainland China and their research topic that fell within one of the categories of the review guide were included for further analysis. Descriptive analysis was adopted to summarize the relevant findings. Results: 54 studies found to be relevant were included for the analysis. Three studies on “palliative care education and training” (category 1) asserted that education and training programs were scant in Mainland China and that only 1 program had been devised within the health-care context of Shanghai. Five studies on “palliative care screening and timely identification” (category 2) highlighted the absence of early screening criteria or checklists and referral procedures for palliative care. Thirty-one studies on “palliative care needs assessment (n = 12/31) and implementation (n = 19/31)” (category 3) were identified, and various methodological flaws were observed in most of these included studies. Twelve studies on “advanced decision-making” (category 4) were identified, all of which focused on investigating the attitudes of patients with cancer, their families, and/or health-care professionals toward advanced decision-making only. The percentage of patients, family members, and health-care professionals who held positive attitudes toward advanced decision-making were varied and suboptimal, particularly for family members (51.4%-58.0%). Five studies on “caring for patients at the end of life” (category 5) were identified, and the experience of health-care professionals in caring for those patients was explored. No studies relating to “death and bereavement care” (Category 6) and “psychological support for palliative care providers” (Category 7) were identified. Conclusion: The current research status of palliative care in Mainland China remains at an early stage with minimal palliative care services used. Although several knowledge gaps were identified, the first step, which should be addressed, is assessing the palliative care needs. An appropriate and ongoing needs assessment could provide important information for constructing comprehensive education and training programs of palliative care, identifying prognostic factors of timely palliative care referral, and developing evidence-based and tailored palliative care services.

Abstract:OBJECTIVE: Although there has been an increasing emphasis on assisting nurses with providing quality hospice care to patients and family members, few studies have explored the challenges that oncology nurses face when delivering hospice care in the Chinese cultural context. The objective of this study was to elucidate the challenges for oncology nurses in providing hospice care for terminally ill cancer patients in mainland China.METHODS: A descriptive qualitative study with purposive sampling using audio-recorded fact-to-face interviews. A total of 13 hospice nurses from four hospitals in Beijing, mainland China, participated in this study. Data collection was from April to June 2019, and thematic analysis method was used to analyze the data.RESULTS: Challenges identified by hospice nurses in providing hospice care for terminally ill cancer patients included: (1) public misperception on hospice care, (2) lack of financial support, (3) fear of medical disputes and legal action, (4) shortage of human resources, (5) insufficient specialization and lack of "hierarchy" training on hospice care, (6) inexperience in communication skills, and (7) lack of self-care and stress management skills.CONCLUSIONS: It is imperative and critical for the government, health-care institutions, and hospice care providers to clearly understand the challenges that currently exist in providing hospice nursing. Joint efforts are needed to overcome those challenges, which might result in qualified hospice nurses and provide evidence for further development of hospice care in mainland China.

Abstract:Hospice and palliative care have been demonstrated to improve quality of life, shorten hospital stays, and save costs. As a developing country, faced with a rapidly aging population and an increasing number of patients with life-threatening illnesses such as cancer, China has made great progress in promoting hospice and palliative care during the past few decades. A trained nurse can play a significant role in promoting quality hospice and palliative care by providing care, coordinating a multidisciplinary team, calling for policy support, and raising public awareness. It is critical for nurses to clearly understand the development and challenges that currently exist in the country. With this awareness, nurses will improve their ability to recognize and address patients' and families' needs, resulting in quality, dignified, and individualized end-of-life care.

Abstract:Hospice care emerged in mainland China in 1988, 30 years ago, but its development since that time has been slow. In 1994, the Ministry of Health listed hospice care into the Catalogue of Diagnostic and Therapeutic Subjects of Medical Institutions,<sup><a class="ref showRefEvent2" href="#">1</a></sup> but the public's awareness of hospice and palliative care and the supply of it are still limited. Large numbers of people still die in acute hospitals. In recent years, hospice and palliative care has witnessed rapid development: in February 2017, the State Health Planning Commission issued a guideline<sup><a class="ref showRefEvent2" href="#">2</a></sup> for hospice care, which began to promote the work of hospice care nationwide. In May of 2019, the second batch of national pilot work on hospice care were chosen, 71 regions including whole area of Shanghai are being involved in developing local hospice care service.

Abstract:In recent years, the number of cancer deaths per year in China has been in the forefront of all kinds of diseases, and a large number of people at the end of life have put forward the need for hospice care. Hospice care is a mode of providing comprehensive care for terminal-stage patients, which is a part of integrated oncology nursing. This article reviews the definition and content of hospice care, emphatically focusing on the roles and thinking of nurses, to help nurses play their professional roles and perform their duties better in hospice care for patients with cancer.

Abstract:Abstract Background Hospital facilities in China are experiencing increased strain on existing systems and medical resources, necessitating the use of home-based hospice and palliative care (HBHPC). HBHPC primarily relies on community nurses and related medical personnel. Understanding the challenges that community nurses face when providing this form of care is urgently needed to optimize the design and delivery of HBHPC. Our study aimed to gain insight into community nurses’ challenges when providing HBHPC for patients. Methods We performed a descriptive qualitative study using a phenomenology approach. Purposive sampling was used to recruit 13 nurses from two community health service centers in Jinan, Shandong Province, China. A thematic analysis was applied to identify themes from the transcribed data. Results Three major themes emerged: 1) Community nurses’ inadequate self-preparation for providing HBHPC; 2) Patients and their families’ non-collaboration in HBHPC; 3) Community health service career disadvantages. Many negative experiences can be attributed to institutional barriers. Conclusion Community nurses faced multifaceted challenges in home care settings. This study could provide a framework for guiding the improvement of interventional variables in the provision of HBHPC. Future research should involve developing effective methods of improving community nurses’ job motivation and community health service institutions’ incentive systems, as well as increasing advocacy around HBHPC.

A qualitative study of caring in hospice wards in Shanghai

Abstract:Despite the large number of people with cancer who need hospice care, utilization rates at community hospice service in Shanghai are low. This study aimed to review the care provided in these hospice wards and examine the difficulties in delivering service. Fourteen hospice care providers were interviewed, and the data analyzed using qualitative content analysis. Three themes emerged from the interviews: (i) characteristics of hospice patients, mostly cancer patients at the end-of-life stage (within 3-months of life expectancy) who were admitted without awareness of their condition; (ii) components of care, from physicians and nurses who focused mainly on physical problems as psycho-spiritual care stagnated at a superficial level; and (iii) difficulties in care, including a lack of patients as well as inadequacies in service, along with suggestions for the future. Efforts at improvement should be made in both the health care system and society. To address the lack of patients, doctors in secondary/tertiary care hospitals need improved skills in discussing end-of-life care, while the general public needs better awareness and acceptance of hospice care. Solutions should respect the perspective of patients and their families, while hospice care providers must also enhance their skills of communication and psycho-spiritual care.

Abstract:Objective To investigate major physical and mental reactions in family members of patients with advanced cancer in communities , the reasons and their needs , in order to improve community hospice care.Methods From April to June in 2014, we selected 16 family members of patients with advanced cancer who received hospice care in Desheng Community Health Service Center of Xicheng District using purposive sampling method , and we conducted in -depth interviews with the subjects and made sound records.Results Severe physical and mental reactions appeared in all 16 subjects, including fear, sadness, hate to part, anxiety, sense of being lost , helplessness and the regret and self -blame after patients′death.The reasons were the pursuit of peace of mind , inadequate physical strength in nursing , the dissatisfaction with the nursing of employed nursing workers , increasing financial burden , inability of domestic care and the coexistence of worry and concern.Support and help should be provided , including mental relief , the help and guidance from experienced persons and community hospice care service.Conclusion Family members of patients with advanced cancer in communities have serious physical and mental reactions and are in bad need of help , thus we should give more attention and care to them.Relevant education and hospice care service publicity should be enhanced , and the self-protection ability of family members of patients with advanced cancer should be cultivated , in order to reduce their side reactions.
