Abstract:Based on the experiences of community care development in Britain,this paper aims to analyze the meanings,elements,barriers of home-care system of elderly in China.There are many differences in the characteristics of the meanings,in particular in the field of elderly social service.Researcher conducted qualitative date by using comparative analysis of literature and programs to answer the proposed aims of this research.There are valuable lessons that we can learn from the British experiences for developing community home-care system of elders in China,in particular its series of laws,reliable source of funding and professionals involved.Finally,based on the above discussions,the author reflects on the community development difficulties faced by China and offers some suggestions for the future.

Abstract:Establishing the system of community-based home care service for the elderly is one of the principal pathways to solve the problem of social provision for the aged. Based on the analysis of background problem and research status quo, this article establishes the system of elderly care service in aspects of supply mode and support system of community endowment service. According to the pension service, it builds a connection between the aged and providers, and sets up four support “elements”for community platform to implement service operation which includes facilities construction, organization management, capital operation and information platform. It provides ideas for establishment and consummation of community home endowment service system.

Abstract:With the rapid population aging in China, healthy elderly care has become a consensus of the whole society. As an important part of healthcare system, community health service plays a vital role in health maintenance and promotion for the elderly, and it is an extremely important foundation for realizing healthy endowment. In this article we summarize the status quo of home-based care for the aged, in which the community health service plays an important role, and also make suggestions for further improvement.

Abstract:With the increasingly serious population aging, economic system transformation, and social transformation, the elderly have an increasingly strong demand for the elderly care service industry, but the development of China's elderly care service industry started late, and there are still many problems. For how to build a better elderly care service model, under the current situation of shortage of elderly care resources, community elderly care may become a solution. By analyzing the status quo, existing problems and development trends of home-based care services in Hangzhou's community, and learning from foreign experience, it explores the establishment of a "integrated physical, medical, and nursing care" community health management model for the elderly and provides a perfect solution for the development of Hangzhou's community elderly care services countermeasures and suggestions for mode operation. Based on the evaluation, problem analysis, and empirical research on the current situation of home care services in Hangzhou communities, this paper draws on relatively mature community care practices, proposes to build a community elderly health management model that combines medical care and care, and improves and innovates the overall design and medical care of the elderly service model measures for the health management of the elderly in the integrated community. The experimental results of this study show that, according to the demand for elderly care services, elderly people in need of care account for 86.6%, 79.5%, and 68.4% of the elderly population, especially in the areas of medical care, life care, and housekeeping services.

Research on the Quality Enhancement of Community Home-Based Elderly Care Services: Dimensional Differences and Realistic Approaches

Abstract:. Community home-based elderly care services, which are family-based, community-based and supplemented by institutions, have become the optimal path to alleviate the pressure of ageing, and are the focus of China's so-cialised elderly care system, with the quality of services directly affecting the quality of life of the elderly in their twilight years, and have become a livelihood issue of the greatest concern to all sectors of society. Based on the SERVQUAL model, we divided the community home-based elderly care service into five types, refined 25 evaluation indicators and determined the dimensions, and conducted research on 502 elderly people in Harbin City. The research found that there is a significant difference between service quality perception and expectation, the service quality level of reliability and responsiveness dimension is higher, and the service quality level of tangibility, assurance, empathy dimension is lower. Accordingly, a number of feasible recommendations have been put forward to effectively meet the urgent needs of the elderly for elderly care services.

Abstract:Aiming at the serious contradiction between the rapidly rising, and diversified requirement of social elderly-care, and the lowly developing aging issue as wel as the insufficiency of bed number of elderly facilities in current China, taking “community” as the research vector, based on the concept of “Medical-Nursing”, with the goal of strengthening the elderly care abilities of both elderly facility and community household, the paper transits the connotation of “home-based care of the aged”, discussing the feasibility of establishing the “Community Elderly-Care Nursing Unit” Model; clarifying the constitute and research emphasis point of the model from levels of urban structure, community space, and architectural programming, laying the foundation of further studies.

Abstract:The mode of community home-based care with combination of medical care and elderly care is im-portant for promoting healthy aging. But now the services contents provided by community health service agencies were not clear, related system of policies and regulations were imperfect, human resources were insufficient and net-work of service resource was not perfect. All of these factors caused the service cannot met old people medical de-mands and the use for community health services is inadequate. So we should proposesupply-side reform on communi-ty health service to meet the medical demands for old people and even to stimulate new demand.

Abstract:Since 2000, China has entered a critical period of development. With the rapid development, there are many social problems, among which the most intractable one is the increasing aging of the population. Aging is a very difficult challenge for our country. Due to the problems of low popularity, backward and imperfect infrastructure, poor quality and low efficiency of service personnel, home-based elderly care service centers in China are generally “virtual”, which makes it difficult for relevant care institutions to play a bigger role in China’s arduous task of caring for the elderly. As a new model of elderly care service, “Internet plus” home-based elderly care service can solve the problem of virtual care based on technological innovation, further integrate online and offline resources, promote the integration of medical care and nursing, and effectively solve the problem of aging in China.

Abstract:The elderly often have a variety of chronic diseases at the same time. The chronic diseases have high prevalence, slow progress and long duration. There are many people with both dementia and disability. So the elderly need full range of health care model (acute care, mid-term care, long-term care). Implement the entire health care for the elderly is also one of the measures that deal with the 2016 national strategy which puts forward "healthy China 2030" and "ensure people's health comprehensively during the whole process". This paper gives an overview of the current care mode of the elderly. It illustrates the problems in the care mode of the elderly in China. And based on the experience of the introduction of foreign advanced model departments in the department of geriatrics of West China Hospital, Sichuan University, this article evaluates how to bring in and localize the advanced ideas and models of care for the elderly, and introduce the Acute Care for the Elderly, the unit of comorbdity management and function promotion in frail senior and the Palliative Care Unite of West China Hospital. So that it is expected to play a certain role in the reform of the care mode of the elderly in China.

Abstract:Objective:To survey the experiences and perception of caregivers on home care service for community-dwelling elderly.Methods:From August to December 2021, individual semi-structured in-depth interviews were conducted with caregivers of the elderly from five community health centers in Beijing Xicheng and Miyun districts selected by purposive sampling.Results:Nine caregivers,2 males and 7 females aged 49-76 (62.8±9.4) years were interviewed in this study. The nursing care period was 3-48 (17±13) years. Four themes were extracted from the interview data: older people′s own condition was the primary factor influencing the home care needs; medical needs for home care were determined by the caregivers′ factors; community health service was important for meet home medical needs; and supporting of home care services for the elderly need to be strengthened.Conclusion:The experience of elderly home caregivers in using community home medical services is affected by many factors, and in the future, the content of home medical services should be improved, and the skills training of community elderly home caregivers should be strengthened, so as to improve the quality of life at home and promote the health literacy of the elderly.

Abstract:China entered an aging society in 1999, and so far, Beijing has entered the moderately aging society. Under the circumstances, home care has become the primary choice for the elderly in Beijing. Based on the relevant literature, this paper studies the existing policies and practical difficulties of home-based elderly care service in Beijing. The paper recognizes good effects of the current policies and further analyzes the problems to be improved from the aspects of policy follow-up of market segmentation, policy coordination, and policy dynamic adjustment. Based on the research, the paper suggests that the government should coordinate the development of industries and institutions by doing a good job in the market segmentation. The government need to improve the matched policies to reflect the comprehensive effect of the policy system. All sectors of society need to advocate a new view of supporting the elderly to create a good policy environment.

Abstract:The problem of aging has appeared in some countries, and the problem of providing for the aged has become one of the key issues. In recent years, aging has also become a social problem in China, but the current pension model can not meet the diversified and personalized needs of the elderly in China. In this paper, the intelligent community home-based elderly care model has become the forefront of academic research and a new topic of elderly care practice. On this basis, this paper sorts out the research context of the home-based elderly care model in smart communities, and uses Maslow's Hierarchical Needs Theory (MHNT) and Welfare Pluralism Theory (WPT) as research methods to analyze the needs of the elderly and the choice of welfare providers. In addition, this paper uses the method of system dynamics to establish a model to analyze the supply and demand balance of different pension models, and demonstrates the advancement of community home-based pension model. Finally, through the interview and investigation of the staff and managers of the smart community home-based elderly care model in Meilin community, Dongzhi County, Anhui Province, China, this paper summarizes the existing elderly care model in the community, and finds some deficiencies, such as the lack of full-time personnel, the inadequate use of new technologies and the lack of financial support. According to this, the paper puts forward corresponding countermeasures: (1) the problems of personnel can be solved by strengthening the training of service personnel, improving the system of volunteers and community service personnel working together, etc;(2) The problem of technology application can be solved by cooperating with external enterprises or other organizational units in research and development; (3In terms of financial support, it is necessary to broaden the channels of obtaining funds, seek the cooperation of enterprises, and adopt the innovative pension service mode of charging for providing high-end services and using the income for public welfare free basic services to effectively raise funds. Through the above research, the authors can put forward solutions to the pension problems faced by Meilin community, integrate relevant resources, and reduce the social burden.

Abstract:The city of Guiyang,which has stepped into an aging population society,began to explore the service of home- based care for the aged in community since 2007. Up to now,it has been six years,the service of home-based care for the aged in community has covered the whole city,and system of home- based for the aged has been basically completed. Based on the analysis of present situation of the service of home- based care for the aged in community in the city of Guiyang and the existing problems,the paper suggests that it is imperative to encourage the whole society to work together to set up the diverse pension services,improve the quality of the service staffs,admit the participation of social work services,improve the infrastructure in the community,improve the effective demand for service,so as to further promote the service quality and achieve the social harmonious development.
Attitude and perception towards ways of aged-care and home medical services among community-dwelling elderly: a qualitative study

Abstract:Objective:To understand the attitude and perception of the community-dwelling elderly on the provision of ways of aged-care and home care services by primary health care institutions.Methods:The focus group interviews were conducted from July to December 2021 among 40 home-bound older adults aged≥65 years, selected by purposive sampling method from 5 community health service centers in Beijing Xicheng District and Miyun District, and 4 eligible participants from each community. The topics of the interview were home care and home medical services. The recorded data were transcribed, coded, and analyzed. The themes of interview contents were extracted, summarized and refined with NVivo12 software.Results:Among 40 participants, 20 were males and 20 were females with an average age of (70.95±4.62) years (65 to 83). Three themes were extracted from the interviews: the mode of provision for aged care is influenced by various factors; the role of community medical services for home-based care of the elderly is limited; the contents of community home medical services need to be expanded.Conclusions:The home-bound elderly have a need for home medical services provided by the community, which are limited at present. The implementation of telemedicine and increasing service coordination in the community will be conducive to convenient home medical services for the home-bound elderly.
