Abstract:Objectives: To observe the clinical effectiveness and safety of Prof.WANG Qi's experiences(Tuomin Zhiti Decoction)in treating allergic rhinitis.Methods: This study was a case series study.We applied the diagnosis standard distributed in Lan Zhou Congress 2004 by Branch of Hals,Nasen and Ohrenheilkunde,Chinese Medical Association(CMA) for diagnosis and effectiveness evaluation.Results: This study totally included 52 cases,after taking Tuomin Zhiti Decoction for 4 weeks,6 weeks,8 weeks,the obvious effective rate were 57.69%,59.45% and 63.33%,the effective rate were 88.46%,89.18% and 90.00%.3 patients had stomachache,but after reducing the quantity of Small Centipeda Herb,their stomachache had disappeared.Conclusion: Prof.WANG Qi's experiences(Tuomin Zhiti Decoction) in treating allergic rhinitis is effective.

Abstract:Through the summarization of the drug usage characteristics and case analysis of Prof. Wu Wei - ping's experiences in the treatment of cough,it was discovered that based on the clinical feature of cough,Prof. Wu believed that the pathogenesis of cough was the upward perversion of lung qi. The treatment was emphasized on promoting qi activity,especially regulating liver and lung qi. In the paper,it was intro-duced that the modified xiaochaihu tang was used extensively in the treatment of cough. For cough due to in-vasion of exogenous pathogens,the treatment was to harmonize qi circulation and eliminating exogenous patho-gens. For cough due to internal injury,the treatment was to regulate qi activity,harmonize qi and blood and prevent from the invasion of pathogens. Xiaochaihu tang is good at regulating qi activity and smoothing liver and gallbladder. The clinical efficacy was significant.

Abstract:Chaihu Guizhi Ganjiang Tang syndrome is a controversial syndrome in Treatise on Febrile Diseases.Through studying related statements of the physicians of past generations and taking reference to typically clinical cases,we found that the pathogenesis of this decoction was as follows:shaoyang disease,stagnation of yang qi and dysfunction of the pivot;taiyin disease,deficiency of yang qi and dysfunction in transport and congestion of fluid-retention.The function was relieving shaoyang disorder,promoting qi and engendering fluid;warming yang to dissipate excessive fluid,warming spleen to expel cold;harmonizing yin and yang and eliminating pathogens to support healthy qi.And it was noted that the function in curing myxedema due to hypothyroidismis deserves further exploration.

Abstract:Rationale Kaixin San (KXS) is a prescription traditional Chinese medicine (TCM) with the effects of "tonifying the kidney and brain" and "improving memory". The volatile organic compounds (VOCs) in KXS could effectively improve senile dementia and depression, but only few studies have focused on the overall characterization of VOCs in KXS and the quantitative study of the main active components. Methods We have developed a strategy to correlate the results from headspace gas chromatography/ion mobility spectrometry (HS-GC/IMS) and headspace gas chromatography/mass spectrometry (HS-GC/MS) for the comprehensive characterization of VOCs in KXS and the quantitative analysis of the main pharmacodynamic substances. Results A totsal of 68 low molecular weight VOCs were identified in KXS by HS-GC/IMS at room temperature and atmospheric pressure; 117 VOCs were identified and 10 components (isocalamenediol, alpha-asarone, beta-asarone, methyl eugenol, isoeugenol methyl ether, camphor, anethol, 2,4-di-tert-butylphol, linalool, asarylaldehyde) as the quality markers of KXS based on HS-GC/MS. Conclusions This results from this study provide a foundation for quality control, pharmacodynamic mechanism research and further development of KXS, and provides more convincing data supporting the VOCs of other natural products.

Abstract:Compared with Dachaihu Tang in Synopsis of Golden Chamber（金匮要略）,Dachaihu Tang was only without Dahuang[Radix et Rhizoma Rhei,大黄]on the composition in Treatise on Febrile Diseases.The authors summarize the opinions and discussions of physicians of past generations on whether Dachaihu Tang includes Dahuang.By understanding the characteristic of syndrome and treatment of Dachaihu Tang in work of physicians of past generations,the authors conclude that adding Dahuang is used to treat excess of Yangming fu-viscera and constipation,dampness-heat amassing-steaming and sticky-stagnant stool,heat accumulation of stomach fu-viscera xuefen (血分)and abnormal state of mind,as well as hard,full and pain in epigastrium or rib by abdomen examination,etc.The main effect of Dachaihu Tang without Dahuang is reconciling Shaoyang,as well as conveying and dispersing liver and gallbladder.The indicating syndromes are diseases owing to liver and gallbladder stagnation and dysfunction of the pivot.

Abstract:This study explored the effects of a classical Chinese medicine formula- Xiao-Chai-Hu Tang(XCHT) on the model mice with D-galactosamine -induced liver injury. Sixty male imprinting control region (ICR) mice were used in the present study, and they were separated randomly into 6 groups: a normal control group (Group A, n=10), a model control (Group B, n=10), a positive control (Group C, n=10), a low dose of XCHT group (Group D, n=10), a medium dose of XCHT group (Group E, n=10), and a high dose of XCHT group (Group F, n=10). ELISA was used to detect the IL-6 and TNF-α levels in the serum. Real-time PCR was performed to assess the expression of FasmRNA, Fas-LmRNA, Bcl-2mRNA of the liver tissues. Western blotting was used to detect the Bax protein expression of the liver tissues. The serum IL-6 and TNF-α levels of Group B were significantly higher than the other groups (P<0.05). The expression of Fas mRNA, Fas-LmRNA, and Bax protein of the liver tissues of Group B were significantly higher than those of the other groups (P<0.05). The expression of Bcl-2 mRNA of the liver tissues of Group B was significantly lower than other groups (P<0.05). Both of XCHT and biphenyl dicarboxylate significantly decreased the serum IL-6 and TNF-α levels and FasmRNA, FasLmRNA, Bax protein expression and increased the Bcl-2 mRNA expression of the liver tissues of model mice (P<0.05). It may be through decreasing the serum IL-6 and TNF-α levels and FasmRNA, FasLmRNA, Bax protein expression and increasing the Bcl-2 mRNA expression of the liver tissues that XCHT significantly relieved the D-galactosamine -induced liver injury.

Abstract:Xiaochaihu (XCH) is a classic Chinese medicine formula. XCH tablet, XCH granule, XCH capsule, and XCH effervescent tablet are included in the Chinese Pharmacopoeia. In this review, the formula and quality standards of XCH preparations at home and abroad were compared. The differences in manufacturing process of XCH preparations are discussed. The progress of research on the qualitative identification, quantitative detection and fingerprint chromatogram/specific chromatogram of XCH preparations was reviewed. The characteristic components of Pinelliae Rhizoma Praeparatum Cum Zingibere Et Alumine and Jujubae Fructus was rarely analyzed for XCH preparations. It is suggested that the specificity of drug quality detection methods should be improved. Considering drug safety and drug efficacy, it is suggested to set the upper and lower limits of the content of saikosaponins. The standards for heavy metals and other limited items for XCH preparations are also suggested to be set.
Influence of modified Chaihu Plus Longgu Muli Tang on quality of life in patients with alcohol dependence

Abstract:Objective To investigate the influence of modified Chaihu Plus Longgu Muli Tang on quality of life in the patients with alcohol dependence by using a randomized controlled study. Methods The patients with alcohol dependence (n=56) were randomly divided into two groups, and treatment group was treated with granules formulation of Chaihu plus Longgu Muli Tang and control group was treated with placebo for 4 weeks. With a research tool of health status questionnaire (Short Form 36 Health Survey Questionnaire:SF-36), the influence of Chaihu plus Longgu Muli Tang on quality of life in patients with alcohol dependence was reviewed quantatively. Results After 4 weeks, through the comparison in SF-36 scoring data before and after treatment, scores of 6 dimensions of physical functioning (PF), role of physiology (RP), bodily pain (BP), general health (GH), vitality (VT) and mental health (MH) were all significantly higher in treatment group than those in control group, while comparison in dimensions of social functioning (SF) and role emotional (RE) showed there was no statistically significant difference. Conclusion Modified Chaihu Plus Longgu Muli Tang can improve the quality of life in patients with alcohol dependence.

Abstract:With the introduction of various subjects, such as clinical epidemiology and evidence-based medicine, the qualities and levels of Traditional Chinese Herbal Medicine (TCHM) in China improved substantially, and the processes of internationalization of Traditional Chinese Medicine (TCM) are further accelerated. Since, a variety of drug products in China have been approved for marketing in other countries, and approximately 10 products have submitted the IND application to FDA of United States, of which various Chinese herbal preparations such as compound Danshen dripping pills, Xingling granules, and HMPL-004 have been approved to be investigated in phase III clinical trials. In general, multi-center studies of TCHM are increasing with years, but most of the studies are performed in some certain country, and the actual international multi-center clinical trials are very rare. Number of SCI literatures on multi-center clinical trials of TCHM that published in the recent decade also showed increasing tendency with years, despite the evident reduction in the past 2 years due to the influence of COVID-19 pandemic. Of the multi-center clinical trials of TCHM that performed by mainland China and other oversees regions, except for Taiwan, China, nearly 70% were focused on classic Chinese medicinal formulae and Chinese patent medicine, while the other 30% were on dietary supplements and plant extracts. Facing the future, the "human experience" has attracted close attentions from researchers throughout the world. Effectively utilizing the historic "human experience" is an important method to vitalize potential of original scientific and technological resources of TCHM. Performing multi-center clinical trials with high qualities is still an essential method for TCHM in accessing the mainstream medicine market. In addition, it is also required to further improve the evaluation techniques and methods that not only meet the international standards but also meet the characteristics of TCHM. Furthermore, we should also focus on the TCHM specific clinical values and scientific reports.

Analysis of main constituents and mechanisms underlying antidepressant-like effects of Xiaochaihutang in mice

Abstract:Ethnopharmacological relevance: Xiaochaihutang (XCHT), a famous Chinese herbal formula which consists of seven Chinese herbs, has been used clinically in depressive disorders in China. Our previous studies have demonstrated that XCHT improved depressive-like behavior in several animal models of depression. However, therapeutic basis of XCHT on depression are challenging, due to the complex active constituents of XCHT and the unclear pharmacological mechanism of action. Materials and methods: To provide further insights into therapeutic basis of XCHT, the core in compatibility of XCHT on antidepressant therapy was assessed by the method of orthogonal array design. The comparative evaluations on antidepressant effects of XCHT and its core in compatibility were executed by tail suspension test (TST), forced swim test (FST), novelty suppressed feeding test (NSFT), reserpine-induced hypothermia and palpebral ptosis. Moreover, the potential mechanism was explored by investigating levels of monoamine neurotransmitters in hypothalamus and striatum and neurogenesis in hippocampus. Chemical profile of active constituents in plasma after oral administration of the core in compatibility of XCHT was revealed by ultra performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS). Results: The results of orthogonal array design experiment showed that Huangqin (Radix scutellariae), Renshen (Ginseng) and Gancao (Radix glycyrrhizae), defined as HRG, might be the core in compatibility of XCHT on antidepressant therapy. In accordance with XCHT, oral administration of HRG for 15 days significantly reduced immobility duration in TST and FST without affecting locomotor activity. Both HRG and XCHT increased immobility latency in FST, decreased the latency in NSFT, reversed reserpine-induced hypothermia and palpebral ptosis. Moreover, both HRG and XCHT significantly increased levels of 5-HT and DA in hypothalamus. In addition, HRG could remarkably increase Ki-67 and doublecortin (DCX) positive cells in hippocampus. A total 25 active constituents in plasma, including 14 prototype components and 11 metabolites, were identified by UPLC-MS/MS after oral administration of HRG. Conclusion: The present results reveal that HRG is supposed to be the core in compatibility of XCHT on antidepressant therapy. In accordance with XCHT, HRG exerts significant antidepressant-like effects, which are likely attributed to regulating serotonergic and dopaminergic systems and increasing hippocampal neurogenesis. The constituents identified in plasma after oral administration of HRG may be the potential active ingredients for the treatment of depression.

Abstract:Xiaoyao Wan (XYW) is a prescription medicine of traditional Chinese medicine (TCM) with the effects of "soothing the liver and relieving depression," and "strengthening spleen and nourishing blood". XYW has been widely concerned in the treatment of depression and has become one of the commonly used classic formulas in clinical practice. However, the pharmacodynamic substance basis and the quality control studies of XYW are hitherto quite limited. Here, we aim to fully utilize an advanced ultra - performance liquid chromatography-quadrupole - Orbitrap mass spectrometry (UPLC-Q-Orbitrap-MS), headspace-solid phase microextraction-gas chromatography-mass spectrometry (HS-SPME-GC-MS) and headspace-gas chromatography-ion mobility spectrometry (HS-GC-IMS) technique to deep characterization of the pharmacological substance basis and quantitatively evaluate the quality of XYW. Firstly, 299 compounds were identified or tentatively characterized, including 198 non-volatile organic compounds (n-VOCs) and 101 volatile organic compounds (VOCs). Secondly, principal component analysis (PCA) and hierarchical cluster analysis (HCA) was used to analyze quality differences in XYW at different manufacturers. Thirdly, a parallel reaction monitoring (PRM) method was established and validated to quantify the fourteen major effective substances in different manufacturers of XYW, which were chosen as the benchmarked substances to evaluate the quality of XYW. In conclusion, this study shows that the strategy provides a useful method for quality control of TCM and offers a practical workflow for exploring the quality consistency of TCM.

Abstract:Objective: To evaluate the therapeutic effect of Chaihu Taoran Tang(CTT) on the AIMS scores in patients with tardive dyskinesia(TD) and the possible mechanisms.Methods: Eligible patients were randomized to receive the CTT or placebo in a double-blinded study lasting 12 weeks.The therapeutic effects were assessed by using abnormal involuntary movement scale(AIMS).The mechanism was investigated by comparing the activities of plasma superoxide dismutase(SOD),catalase(CAT) and glutathione peroxidase(Gpx) as well as maleic dialdehyde(MDA) level before and after the treatment.Results:CTT effectively reduced the severity of dyskinesia;the AIMS scores decreased over 50% in 15 patients while only 2 patients got worse in CTT group.CTT increased the activities of SOD[from baseline(88.9±19.2) to(95.8±26.1)×103U·L-1],Gpx[from(112.9±21.7) to(123.1±29.6)×103U·L-1] and CAT[from(1.6±1.5) to(3.1±2.4) ×103U·L-1],and decreased the level of MDA[from baseline(14.1±7.8) to(9.4±4.7) mmol·L-1] significantly(P<0.05).Conclusion:CTT,a prescription of traditional Chinese Medicine,is markedly effective in reducing the severity of dyskinesia in TD patients,and the oxidative damage might be pathophysiological key factor of TD.

Abstract:Ethnopharmacological relevance: Tangchong (ཐང་ཕྲོམ།), a term in the Tibetan language, encompasses a diverse group of plants belonging to different genera of the Solanaceae family. These plants have been utilized in traditional Tibetan medicine for centuries and are currently still employed to treat a variety of ailments, including acute and severe abdominal pain, intestinal obstruction, epilepsy, ascariasis, lung abscess, and other diseases. The therapeutic properties of tangchong are attributed to the presence of tropane alkaloids (TAs), although recent research has also revealed their toxicity. Aim of this review: The purpose of this review is to provide a comprehensive analysis of the traditional uses of tangchong, as well as the phytochemical and pharmacological studies conducted on this plant. The review aims to offer a critical update on the current state of knowledge of tangchong and to identify new opportunities for exploring its therapeutic potential. Materials and methods: To gather information on tangchong, we conducted a thorough search of several scientific databases, including Google Scholar, Web of Science, Scifinder, Baidu Scholar, PubMed, and CNKI. Results: This review summarizes the traditional usage of 11 plants, 168 chemical components, including alkaloids, flavonoids, steroids, amides, coumarins, etc., describes the pharmacological activities of these medicinal plants and their mechanisms of action, provides an elaboration of toxicology, and provides new plant-derived drugs and herbal preparations from tangchong. Conclusions: Clarity regarding the source of the drug is crucial to ensure the safety and efficacy of clinical applications of Tibetan medicine. In this paper, we provide a comprehensive review of tangchong, including its traditional medicinal uses, chemical composition, pharmacology, toxicology, and quality control measures. Our analysis highlights the significant value of tangchong as an ethnomedicinal resource, with pharmacological activities primarily linked to its alkaloid content. Furthermore, this review serves as a valuable reference for future research on alkaloid-containing ethnomedicines.

Abstract:This study was aimed to investigate the relationship between HPLC fingerprint of three kinds of agents and the effect of Shu-Gan-Jie-Yu,and to provide both theory and data basis for an innovative approach of quality control of traditional Chinese medicine(TCM).HPLC was introduced to analyze fingerprint of three kinds of agents,namely the traditional pill,herbal decoction and new formulation.Complex etiology model was used to study the pharmacological effects.Correlation between fingerprint and effect of Shu-Gan-Jie-Yu were identified using grey relational analysis.The results showed that chemical components representing a contribution to pharmacy dynamics were ordered by representative spectrum: new formulation > herbal decoction > traditional pill.It was concluded that the process of new agent is reasonable and the quality is stable.The evaluation of Xiao-Yao formula has an important value on the research and development of classic formula.
Disscussion on Clinical Effects of Duhuo - Jisheng Tang on Rheumatoid Arthritis

Abstract:Objective:To observe the effects of Duhuo-Jisheng Tang on the treatment of Rheumatoid Arthritis.Methods:Atotal of 70 patients with rheumatoid arthritis were randomly divided into two group,the 35 patients in the control group were give tripterygium wilfordii,and the 35 patients in the treating group were additionally given Duhuo-Jisheng Tang.The course of treatment lasted for 1 months.Results:After the treatment,there were obvious improvements in joint ache,swelling,tendmess,jointmovable degree moring rigidity and ESR,CRP.The curative effect rate in the treatment group was higher than that in the control group(P0.05).Conclusion:Lt is indicated that Duhuo-Jisheng Tang has a significant therapeutic effect on rheumatoid arthritic.

METHODS:In retrospective survey,the application of TCM injections in our hospital during 2011-2013 was analyzed statistically in respects of consumption sum and its annual growth,constituent ratio of consumption sum,consumption sum,DDDs,sorting ratio and DDC of single type of TCM injection. RESULTS:The types of TCM injections was stable during 2011-2013,involving 27 types at most;and total amount of 2012 and 2013 increased by 24.78% and 11.26%,compared with last year. The proportion of consumption sum of TCM injection in total consumption sum were 7.96% in 2011,7.97% in 2012 and 7.98% in 2013,respectively. The top 2 TCM injection in the list of consumption sum were drugs for promoting blood circulation to remove blood stasis and drugs for clearing away heat and detoxicating. Top 2 single type in the list of consumption sum were Shuxuening injection and Danhong injection.By the number of drug type,TCM injection were mainly used in department of internal medicine;by consumption sum and DDDs,TCM injection were mainly used in paediatrics department,general surgery department and emergency department. CONCLUSIONS:TCM injections are widely used in the clinic. The doctors should pay attention to the safety of TCM injection use.

Abstract:Application experience in humans, a summary of the clinical practice of traditional Chinese medicine(TCM), serves as an important data source for evaluating the safety, effectiveness, and clinical value of drugs in the development of new Chinese medicine. The collected data serving as the evaluation evidence through statistical analysis are critical to the research on the application experience in humans. This article summarized and analyzed the data characteristics and statistical methodology of application experience of Chinese medicine in humans, and concluded the data types, outcome evaluation, bias evaluation, confounding factors, and missing values. Furthermore, the article emphasized the importance of data analysis of application experience of Chinese medicine in humans for TCM evidence and put forward the current difficulties, such as low data quality and large internal bias, lack of individualized data processing methods, and lack of methods for "disease-syndrome combination" data. We believe that with the development of methodology, the application experience of Chinese medicine in humans can strongly support the development of new drugs in TCM.
