Abstract:Background: Acupuncture anesthesia originated from the pain relief and pain prevention theory in acupuncture and moxibustion. This technique is a new exploration of anesthesiology and an original achievement of China, representing a landmark combination of Traditional Chinese Medicine and Western medicine. Objectives: The aim of this historical vignette to introduce acupuncture anesthesia with its meaningful history, especially the use in neurosurgery to the public. Design: This historical vignette introduced the development, mechanism research, awake craniotomy, in order to analyze the utility of acupuncture anesthesia, its global impact, the current situation and future of acupuncture anesthesia. Conclusion: Acupuncture anesthesia was initiated in 1958, and, reflecting the historical background of China after the 1960s, the use of this technique spread widely throughout the country. Reaching other countries after 1971, acupuncture anesthesia had a significant influence, drawing attention from medical academia worldwide. Thus, acupuncture anesthesia has made a special contribution to the medical science of modern China.

Abstract:In the present paper, the author retrospects the historical developmental stages of acupuncture anesthesia-analgesia from 1) preliminary exploration:the creative application of acupuncture therapy to surgery with lower dosage or without anesthetics began in Shanghai and Xi'an in 1958; 2) clinical popularization:the acupuncture anesthesia-analgesia became popularized from 1965, peaked in 1970s' and declined from 1980s'; 3) mechanism study:from 1970 to 2000, a large quantity of research papers were published to confirm the validity of acupuncture analgesia (AA), and in 1997, NIH Consensus Conference for acupuncture was held, promoting the related research in the world; 4) focused research:from 2007 to now, the acupuncture anesthesia-analgesia was listed as one of the key research program in China; 5) theoretical summary:in view of the uncertainty of the scientific basis of the meridian system of traditional Chinese medicine, the nerve fibers distributed under the acupoint are now considered to be the indispensable components for AA, and the knowledge of modern neuroscience is competent to explain the underlying mechanism of AA; 6) returning to reality:AA has been accepted by the international scientific community, and the "acupuncture-assisted anesthesia", or "compound acupuncture and anesthetics anesthesia" has been used as the first choice for some types of surgical operation. The research on acupuncture anesthesia-analgesia greatly facilitates clinical application of acupuncture therapy in the world, and also promotes the development of acupuncture therapy itself and neuroscience. However, the transformation between theory and practice and their mutual interaction are endless endeavor. Thus, a careful review of the half-century-long practice of acupuncture anesthesia-analgesia is painstakingly necessary in order to clarifying the mechanisms, to raise the efficacy of clinical practice and to correct the possible errors referred to this concept.

Abstract:Pain is a kind of complex physiological and psychological symptom, which makes the person debilitated and uncomfortable. Some persistent pain is unbearable for the patients, reducing the quality of life and bringing considerable pressure to the individuals and society. Pain killers seem to be effective in analgesia for patients, but their safety and addiction are crucial issues. From the theory of traditional Chinese medicine (TCM), the blocked meridian is the main cause of pain, and effective acupuncture can play a positive analgesic effect. Acupuncture that can date back thousands of years is one of the ancient medical practices in China. Its safety and effectiveness are respected. Based on its superior safety and inferior side effects, it has been gradually recognized as a therapeutic intervention method for complementary medicine, which is also generally used to treat multiple pain diseases. It is shown by modern medical studies that neurotransmitters are the material basis for the acupuncture effect, and the effect of acupuncture analgesia is related to changes in neurotransmitters. However, the specific mechanism has not been elucidated. This review aims to comprehensively discuss the historical evolution of acupuncture analgesia, clinical research of acupuncture analgesia, comparison of acupuncture and drug therapy, the neurotransmitter mechanism of acupuncture analgesia, the effect of acupuncture manipulation on analgesia, and bibliometric analysis of acupuncture treatment for pain, to explore the superiority and related mechanism of acupuncture analgesia from different aspects, and to provide a more effective treatment for alleviating patients’ pain.

Abstract:In this chapter, the clinical and experimental studies on acupuncture-drug balanced anesthesia will be reviewed. The history of acupuncture anesthesia in China started in 1958. After two decades of clinical practice and experimental investigation, the combination of acupuncture (usually electro-acupuncture, EA) with drugs has been successfully used to improve anesthesia for surgical operations and pain treatment since the 1980s, and this technique is now known as the acupuncture-drug balanced anesthesia in China. It is more advantageous to use acupuncture combined with selected drugs to produce anesthesia, because acupuncture could regulate the functions of multiple organs of the body in addition to analgesia Moreover, the underlying mechanism has been elucidated by investigating the potentiation effects of some commonly used drugs on acupuncture analgesia, in various animal models. Substantial evidence has shown that the combination of acupuncture with these drugs promotes the release of endogenous opioid peptides (EOP). In addition, these drugs potentiate EA-induced expression of EOP gene. Furthermore, several lines of evidence show that EA induces an increase in both binding affinity and density of the opioid receptors, while the combined use of acupuncture with drugs can further enhance the EA effect. Currently, clinical and bench studies on acupuncture-drug balanced anesthesia are still in progress to improve the clinical efficacy and to better understand the working mechanisms. We expect that new discoveries through the translational research will bring more benefits to patients.

Abstract:Acupuncture, as a healing art in traditional Chinese medicine, has been widely used to treat various diseases. In the history of acupuncture anesthesia, in the past decades, mechanisms of acupuncture analgesia has been widely investigated, and in recent years, acupuncture protection on organ functions has attracted great interest. This review summarized the research progress on mechanisms of acupuncture for analgesia and its protection against organ function injury in anesthesia, and its perspective of analgesia, immunomodulation, neuroendocrine regulation and multiple organ protection. The current evidence supports that acupuncture analgesia and its organ protection in anesthesia is associated with the integration of neuroendocrine-immune networks in the level of neurotransmitters, cytokines, hormones, neuronal ensembles, lymphocytes, and endocrine cells. Although the mechanisms of acupuncture analgesia and its organ protection are still not completely understood, basic as well as clinic researches on the mechanisms and applications of acupuncture and related techniques are being carried out.
Research trends from 1992 to 2022 of acupuncture anesthesia: a bibliometric analysis

Abstract:Background Acupuncture anesthesia is a significant technical development that originated in China in 1958 and was introduced to the West in the early 1970s. Due to its relative novelty, it has been the subject of intense scrutiny and contestation. Since the early 1970s, the use of acupuncture as a complementary treatment for opioid analgesics has been accepted. Research on acupuncture anesthesia has helped to reduce clinical opioid abuse. However, only a few articles have focused on previous publications that reflect the trend of the study, the main investigators, reciprocal collaboration, and other information in this field. In view of this, we utilized bibliographic analysis methods to objectively analyze current trends and research hotspots in this field, aiming to provide a foundation and reference for future studies. Methods The Web of Science database was searched for publications related to acupuncture anesthesia between 1992 and 2022. The CiteSpace and VOSviewer were used to analyze the annual publications, authors, Co-cited authors, and their countries (regions) and institutions, co-occurrence keywords, burst keywords, Co-citation references and Co-citation journals. Results A total of 746 eligible publications were retrieved from the database for the analysis, including 637 articles and 109 reviews. And the trend of annual publications continued to grow. Aashish J. Kumar, Daniel I. Sessler, Baoguo Wang, and Paul F. White published the most papers in this field (7), and all authors, had a very low centrality (<0.01). China (252) and the University of California System (21) were the most productive country (region) and institution, respectively, while the United States (0.62) and University of California System (0.16) had the highest centrality. After removing keywords related to the search strategy, the three most frequent were pain (115), electroacupuncture (109), and stimulation (91). The six most recent burst keywords were recovery, transcutaneous electrical acupoint stimulation, systematic review, quality, general anesthesia, and surgery. Wang et al.’s article had the highest co-citation count (20), whereas Zhang et al.’s articles had the highest centrality (0.25). The Journal of Anesthesia and Analgesia was the most influential one (408 co-citations). Conclusion This research provides valuable information for the study of acupuncture anesthesia. In recent years, frontier topics in acupuncture anesthesia research have been the promotion of perioperative rehabilitation, anesthesia management, and quality improvement.

Abstract:Background: Although the use of acupuncture anesthesia for open heart surgery, which was introduced in China four decades ago, has declined in recent years, there is a renewed interest in it in contemporary China due to the escalating medical costs associated with open heart surgery. This study was aimed to determine whether a combined acupuncture-medicine anesthesia (CAMA) strategy reduces early postoperative morbidity and medical costs in patients undergoing open heart operation under cardiopulmonary bypass.Methods: From July 2006 to October 2010, CAMA was applied in 100 patients undergoing open heart surgery in comparison with another 100 patients under the conventional general anesthesia (GA). For all the CAMA patients, an abdominal breathing training program was practiced for the 3 consecutive days prior to operation. About 15 to 20 min prior to surgical incision, acupuncture needles were inserted into the bilateral points ZhongFu, LieQue, and XiMen. During operation, patients were kept on spontaneous breathing. Endotracheal intubation was not employed but only prepared as a standby. The narcotic drugs, fentanyl and midazolam, were intravenously injected but in very low doses as compared to GA. Open heart procedures were performed routinely in both groups.Results: Compared with the GA patients, the CAMA patients had a less usage of narcotic drugs (p<0.001), less postoperative pulmonary infection (p<0.05), shorter stay in intensive care unit (p<0.05), and a lower medical cost (P<0.05).Conclusions: A combined acupuncture-medicine anesthesia strategy reduces the postoperative morbidity and medical costs in patients undergoing open heart surgery under cardiopulmonary bypass. Abstract:Chronic pain is a debilitating and rather common health problem. The present shortage in analgesic drugs with a favorable spectrum but without remarkable side effects furthered the search for alternative therapeutic manipulations. Increasing evidence from both basic and clinical research on acupuncture, a main alternative therapy of traditional Chinese medicine, suggests that chronic pain is sensitive to acupuncture procedures. Clarification of the underlying mechanisms is a challenge of great theoretical and practical significance. The seminal hypothesis of Geoffrey Burnstock and the astounding findings of Maiken Nedergaard on the involvement of purinergic signaling in the beneficial effects of acupuncture fertilized the field and led to an intensification of research on acupurines. In this review, we will summarize the state-of-the-art situation and try to forecast how the field is likely to develop in the future.
Acupuncture in ancient China: How important was it really?

Abstract:Although acupuncture theory is a fundamental part of the Huangdi Neijing, the clinical application of the needle therapy in ancient China was always a limited one. From early times there have been warnings that acupuncture might do harm. In books like Zhang Zhongjing's Shanghanlun it plays only a marginal role. Among the 400 emperors in Chinese history, acupuncture was hardly ever applied. After Xu Dachun called acupuncture a "lost tradition" in 1757, the abolition of acupuncture and moxibustion from the Imperial Medical Academy in 1822 was a radical, but consequent act. When traditional Chinese medicine was revived after 1954, the "New Acupuncture" was completely different from what it had been in ancient China. The conclusion, however, is a positive one: The best time acupuncture ever had was not the Song dynasty or Yuan dynasty, but is now - and the future of acupuncture does not lie in old scripts, but in ourselves.

Numerous studies have explored the mechanism by which acupuncture works. The effects of acupuncture constitute a generalized systemic reaction with far-reaching consequences. Acupuncture has numerous clinical applications in obstetrics and dentistry, as well as use in the treatment of asthma, musculoskeletal disorders, various addictions, angina pectoris, peptic ulcer disease, functional bowel disorders, and acute bacillary dysentery.

Abstract:In this article the author tried to summarize more than 30 years' practice of acupuncture anesthesia(AA) in China,and to comment on the future development of this medical technique.(1) Acupuncture-induced analgesia is the key element of acupuncture anesthesia.(2) Acupuncture is able to reduce the pain produced by nociceptive inputs from surgical operations,but is theoretically impossible to use acupuncture for a toeal abolishment of surgical pain.(3) Quantitatively,acupuncture is able to reduce the amount of anesthetic drug by 40～50% in many surgical operations including the cranial and abdominal operations.(4) The beneficial points of AA in combination with drug anesthesia depend mainly on the reduction of anesthetic usage, resulting in a smoothening of the fluctuation of vital parameters during the surgery, acceleration of the post-operative recovery and shortening of the period of postoperative hospitalization,and partly on the possibility of surgeon-patient interaction and cooperation during the surgical procedure.(5) The difficulties of performing classical acupuncture manipulations have been solved since the availability of the Han's Acupoint Nerve Stimulator(HANS).(6) It is suggested to use acupuncture in combination with anesthetics,and this technique could be termed acupuncture-assisted anesthesia(AAA) which keeps most of the strong points of AA and puts the patient in a absolutely pain-free status.(7) From the hospital management point of view,there should be new regulations to encourage the anesthesiologists to use AAA whenever it is considered beneficial to the patient.

Abstract:For at least 2,500 years, acupuncture has been an integral part of traditional Chinese medicine. However, recently as more people in western countries are diagnosed with chronic disease poorly treated with modern medical therapies, many are turning to acupuncture and other forms of alternative medical treatments. Based on the theory of harmonious flowing qi being the basis of good health, acupuncture focuses on restoring qi by manipulation of the complementary and opposing elements of yin and yang. However, in the modern medical community we struggle to with the concept of qi, given a lack of anatomic and histological evidence supporting its existence. However, with the surge in public interest in acupuncture, the scientific community begun heavy investigation of acupuncture's efficacy, as well as the physiologic basis behind it. Thus far, evidence supports the use of acupuncture in post-operative nausea and vomiting, postoperative dental pain, chronic pain conditions such as lower back pain, and possibly also such psychologic conditions as addiction. It is possible that by affecting afferent nerve signaling, acupuncture may influence the release of endogenous opioids to promote pain relief. This effect may be augmented by release of ACTH and cortisol, as well as through down-regulation of signaling through pain fibers. When treating patients who may utilize alternative forms of medicine, it is important that medical practitioners be educated in regards to the basic fundamental beliefs behind acupuncture, as well as the scientific evidence supporting its use and revealing its efficacy. The purpose of this review is to give western trained physicians exposure to history, basic knowledge and its clinical applications of acupuncture to accommodate accelerating interests in acupuncture in modern society.
Global trends of acupuncture clinical research on analgesia from 2010 to 2023: a bibliometric and visualization analysis

Abstract:Objective: Acupuncture, acknowledged as a potent non-pharmacological therapy, is frequently employed to alleviate pain. Despite its widespread use, there has been a lack of overarching bibliometric analysis of clinical research on acupuncture analgesia. We aimed to summarize current patterns, hotspots, and development trends in this field through bibliometric analysis. Methods: This study evaluates academic publications retrieved from the Web of Science database (2010.01–2023.09) concerning acupuncture analgesia in clinical settings. All primary and secondary studies on humans were included. To track global developmental trends, we employed several software for analyzing annual publication volumes, countries/regions, institutions, authors, cited authors, journals, cited journals, references, and keywords and to draw collaborative networks and reference co-citation network maps. Results: The final search encompassed 7,190 relevant studies, including 1,263 randomized controlled trials (RCTs) and 1,293 systematic reviews and meta-analyses. The results indicated a gradual increase in the number of annual publications on acupuncture analgesia in clinical practice. Among countries and institutions, China (2,139) and Chengdu University of Traditional Chinese Medicine (258) ranked first. Liang FR (89 articles) was the most prolific author, while MacPherson H (604) was the most cited author. MEDICINE (455) was the most productive journal, and Pain (2,473/0.20) ranked first in both the frequency and centrality of cited journals. Notably, the most frequently cited reference was a systematic review of individual patient data on acupuncture carried out for chronic pain that was published by Vickers Andrew J in 2012 (156). Burst analysis identified frontier research areas for 2010–2020, encompassing network meta-analysis, case reports, dry needling, lumbar disc herniation, cancer, post-herpetic neuralgia, insomnia, and bibliometric analysis. Conclusion: This study outlines current trends and potential future research hotspots in clinical acupuncture analgesia over the past decade. Findings emphasize the necessity for enhanced international collaboration to improve research output and translation.

Abstract:Applying a stimulating current to acupoints through acupuncture needles-known as electroacupuncture-has the potential to produce analgesic effects in human subjects and experimental animals. When acupuncture was applied in a rat model, adenosine 5-triphosphate disodium in the extracellular space was broken down into adenosine, which in turn inhibited pain transmission by means of an adenosine A1 receptor-dependent process. Direct injection of an adenosine A1 receptor agonist enhanced the analgesic effect of acupuncture. The analgesic effect of acupuncture appears to be mediated by activation of A1 receptors located on ascending nerves. In neuropathic pain, there is upregulation of P2X purinoceptor 3 （P2X3） receptor expression in dorsal root ganglion neurons. Conversely, the onset of mechanical hyperalgesia was diminished and established hyperalgesia was significantly reversed when P2X3 receptor expression was downregulated. The pathways upon which electroacupuncture appear to act are interwoven with pain pathways, and electroacupuncture stimuli converge with impulses originating from painful areas. Electroacupuncture may act via purinergic A1 and P2X3 receptors simultaneously to induce an analgesic effect on neuropathic pain.

Abstract:The acupuncture has been practiced in China for more than 3000 years and was spread to Europe and American from the sixteenth to the nineteenth century. The history of acupuncture research was initiated in the eighteenth century and developed rapidly since then. In the past, physicians tried hard to apply acupuncture into clinical practice, while scientists were focused on the possible characteristics of acupoints and meridians. In the modern time, scientists have strived hard to evaluate the real effectiveness of acupuncture and the underlying physiological and biological mechanisms of acupuncture. Reviewing research history from past to present, we are delighted to witness this wonderful development. Accumulated evidences that acupuncture is beneficial in various conditions significantly enhanced our understanding the mechanisms of acupuncture treatment. However, there is still no conclusive evidence in acupuncture clinical studies. The clinical research still needs great improving, while the basic research results need to be appropriately transformed into clinical outcomes. Based on current achievements, we believe that although the challenges and difficulties exist, a more collaborative, innovative, and integrated approach will help us to achieve further progress in future acupuncture research.

Nixon and Scheel in China: Acupuncture and Anesthesia in West and East Germany in the 1970s and 1980s

Abstract:Background: Acupuncture anesthesia was used instead of intubation anesthesia in the 1970s and 1980s in West Germany and West Berlin. In East Germany acupuncture played no decisive role. Sources: Different articles and papers in journals, in daily press, statements of contemporary witnesses, films, records in archives. Results: As in other Western countries, acupuncture was hugely popular in the Federal Republic of Germany in the 1970s. Chief triggers were the state visits to China of the American President Richard Nixon in 1972 and shortly thereafter of West German Foreign Minister Walter Scheel and his wife Mildred, an x-ray technician. During that period observation of an operation under acupuncture anesthesia was an obligatory element in the agenda of a foreign delegation. Following this showcasing, acupuncture was widely adopted in Western surgery as an alternative to the previously exclusive employment of intubation anesthesia. While the alternative method was soon abandoned in the frontline city of West Berlin, it continued to prevail in other West German cities, e.g. Gießen and Munich. Following the Chinese example, the acupuncture effect was normally enhanced electrically. In accordance with the animosity between the USSR and the People's Republic of China, exchange between the German Democratic Republic and China was very restricted through the late 1980s. This made it easier for East German acupuncture sceptics to reject the procedure and brand it as unscientific. Those who advocated it were in a precarious position. Conclusions: Acupuncture was lauded in the West as ancient savvy destined to complement science-oriented medicine. However, the cultural transfer which accompanied the spread of acupuncture was flawed by misunderstanding and misguidance. Acupuncture anesthesia instead of intubation was not practiced in the GDR. Acceptance of acupuncture in East German medicine failed to make any headway until the last few years of the country's existence.
Research trends on acupuncture for neuropathic pain: A bibliometric analysis from 1979 to 2023

Abstract:Background: Acupuncture has drawn increasing attention as a complementary and alternative therapy for neuropathic pain (NP). The present study aimed to summarize the current status and research trends on acupuncture for NP over the past several decades. Methods: The publications on acupuncture for NP in the database of Web of Science Core Collection from 1979 to 2023 were searched. VOSviewer (1.6.15) and CiteSpace software (5.5.R2) were applied to identify active authors, journals, countries and institutions, co-cited references and hot keywords. Results: A total of 642 publications were finally included, and the quantitative trend of annual publications on acupuncture for NP have shown overall upward from 1979 to 2023. Peoples R China was the most productive and influential country, while Kyung Hee University from South Korea was both the first in publications and citations. Fang JQ ranked the first productive author and Han JS was the first 1 among the co-cited authors. The first productive journal was Evidence-based Complementary and Alternative Medicine, while the first co-cited journal was Pain. The high-frequency keywords were divided into 9 clusters, and the frontier topic focused on "Chronic pain". Conclusion: This present study visually showed the research status and trends of acupuncture for NP from 1979 to 2023 on the basis of bibliometric analysis, which may in some way help researcher discovery and explore some new research directions and ideas in the future.

Abstract:Acupuncture has been used in China for more than 3,000 years. Although the clinical application of acupuncture is very popular, its mechanisms of action are still unclear. Beginning in the middle of the 20th century, a boost of research on acupuncture emerged in China, and I was one of the researchers involved in this episode. With the help of modern neuroscience, the mechanisms of acupuncture for pain management have been unraveled at least partly. This article describes my decision as a young medical graduate to devote my life to research on acupuncture and pain medicine; it has since been my life's journey-one full of challenges and happiness, pitfalls and achievements.
