Abstract:In the present paper, the author retrospects the historical developmental stages of acupuncture anesthesia-analgesia from 1) preliminary exploration:the creative application of acupuncture therapy to surgery with lower dosage or without anesthetics began in Shanghai and Xi'an in 1958; 2) clinical popularization:the acupuncture anesthesia-analgesia became popularized from 1965, peaked in 1970s' and declined from 1980s'; 3) mechanism study:from 1970 to 2000, a large quantity of research papers were published to confirm the validity of acupuncture analgesia (AA), and in 1997, NIH Consensus Conference for acupuncture was held, promoting the related research in the world; 4) focused research:from 2007 to now, the acupuncture anesthesia-analgesia was listed as one of the key research program in China; 5) theoretical summary:in view of the uncertainty of the scientific basis of the meridian system of traditional Chinese medicine, the nerve fibers distributed under the acupoint are now considered to be the indispensable components for AA, and the knowledge of modern neuroscience is competent to explain the underlying mechanism of AA; 6) returning to reality:AA has been accepted by the international scientific community, and the "acupuncture-assisted anesthesia", or "compound acupuncture and anesthetics anesthesia" has been used as the first choice for some types of surgical operation. The research on acupuncture anesthesia-analgesia greatly facilitates clinical application of acupuncture therapy in the world, and also promotes the development of acupuncture therapy itself and neuroscience. However, the transformation between theory and practice and their mutual interaction are endless endeavor. Thus, a careful review of the half-century-long practice of acupuncture anesthesia-analgesia is painstakingly necessary in order to clarifying the mechanisms, to raise the efficacy of clinical practice and to correct the possible errors referred to this concept.
Acupuncture Anesthesia for Open Heart Surgery in Contemporary China.

Abstract:Background: Although the use of acupuncture anesthesia for open heart surgery, which was introduced in China four decades ago, has declined in recent years, there is a renewed interest in it in contemporary China due to the escalating medical costs associated with open heart surgery. This study was aimed to determine whether a combined acupuncture-medicine anesthesia (CAMA) strategy reduces early postoperative morbidity and medical costs in patients undergoing open heart operation under cardiopulmonary bypass.Methods: From July 2006 to October 2010, CAMA was applied in 100 patients undergoing open heart surgery in comparison with another 100 patients under the conventional general anesthesia (GA). For all the CAMA patients, an abdominal breathing training program was practiced for the 3 consecutive days prior to operation. About 15 to 20 min prior to surgical incision, acupuncture needles were inserted into the bilateral points ZhongFu, LieQue, and XiMen. During operation, patients were kept on spontaneous breathing. Endotracheal intubation was not employed but only prepared as a standby. The narcotic drugs, fentanyl and midazolam, were intravenously injected but in very low doses as compared to GA. Open heart procedures were performed routinely in both groups.Results: Compared with the GA patients, the CAMA patients had a less usage of narcotic drugs (p<0.001), less postoperative pulmonary infection (p<0.05), shorter stay in intensive care unit (p<0.05), and a lower medical cost (P<0.05).Conclusions: A combined acupuncture-medicine anesthesia strategy reduces the postoperative morbidity and medical costs in patients undergoing open heart surgery under cardiopulmonary bypass. Abstract:In this chapter, the clinical and experimental studies on acupuncture-drug balanced anesthesia will be reviewed. The history of acupuncture anesthesia in China started in 1958. After two decades of clinical practice and experimental investigation, the combination of acupuncture (usually electro-acupuncture, EA) with drugs has been successfully used to improve anesthesia for surgical operations and pain treatment since the 1980s, and this technique is now known as the acupuncture-drug balanced anesthesia in China. It is more advantageous to use acupuncture combined with selected drugs to produce anesthesia, because acupuncture could regulate the functions of multiple organs of the body in addition to analgesia Moreover, the underlying mechanism has been elucidated by investigating the potentiation effects of some commonly used drugs on acupuncture analgesia, in various animal models. Substantial evidence has shown that the combination of acupuncture with these drugs promotes the release of endogenous opioid peptides (EOP). In addition, these drugs potentiate EA-induced expression of EOP gene. Furthermore, several lines of evidence show that EA induces an increase in both binding affinity and density of the opioid receptors, while the combined use of acupuncture with drugs can further enhance the EA effect. Currently, clinical and bench studies on acupuncture-drug balanced anesthesia are still in progress to improve the clinical efficacy and to better understand the working mechanisms. We expect that new discoveries through the translational research will bring more benefits to patients.

Research trends from 1992 to 2022 of acupuncture anesthesia: a bibliometric analysis

Abstract:Background Acupuncture anesthesia is a significant technical development that originated in China in 1958 and was introduced to the West in the early 1970s. Due to its relative novelty, it has been the subject of intense scrutiny and contestation. Since the early 1970s, the use of acupuncture as a complementary treatment for opioid analgesics has been accepted. Research on acupuncture anesthesia has helped to reduce clinical opioid abuse. However, only a few articles have focused on previous publications that reflect the trend of the study, the main investigators, reciprocal collaboration, and other information in this field. In view of this, we utilized bibliographic analysis methods to objectively analyze current trends and research hotspots in this field, aiming to provide a foundation and reference for future studies. Methods The Web of Science database was searched for publications related to acupuncture anesthesia between 1992 and 2022. The CiteSpace and VOSviewer were used to analyze the annual publications, authors, Co-cited authors, and their countries (regions) and institutions, co-occurrence keywords, burst keywords, Co-citation references and Co-citation journals. Results A total of 746 eligible publications were retrieved from the database for the analysis, including 637 articles and 109 reviews. And the trend of annual publications continued to grow. Aashish J. Kumar, Daniel I. Sessler, Baoguo Wang, and Paul F. White published the most papers in this field (7), and all authors, had a very low centrality (<0.01). China (252) and the University of California System (21) were the most productive country (region) and institution, respectively, while the United States (0.62) and University of California System (0.16) had the highest centrality. After removing keywords related to the search strategy, the three most frequent were pain (115), electroacupuncture (109), and stimulation (91). The six most recent burst keywords were recovery, transcutaneous electrical acupoint stimulation, systematic review, quality, general anesthesia, and surgery. Wang et al.’s article had the highest co-citation count (20), whereas Zhang et al.’s articles had the highest centrality (0.25). The Journal of Anesthesia and Analgesia was the most influential one (408 co-citations). Conclusion This research provides valuable information for the study of acupuncture anesthesia. In recent years, frontier topics in acupuncture anesthesia research have been the promotion of perioperative rehabilitation, anesthesia management, and quality improvement.

Abstract:In this article the author tried to summarize more than 30 years' practice of acupuncture anesthesia(AA) in China,and to comment on the future development of this medical technique.(1) Acupuncture-induced analgesia is the key element of acupuncture anesthesia.(2) Acupuncture is able to reduce the pain produced by nociceptive inputs from surgical operations,but is theoretically impossible to use acupuncture for a toeal abolishment of surgical pain.(3) Quantitatively,acupuncture is able to reduce the amount of anesthetic drug by 40～50% in many surgical operations including the cranial and abdominal operations.(4) The beneficial points of AA in combination with drug anesthesia depend mainly on the reduction of anesthetic usage, resulting in a smoothening of the fluctuation of vital parameters during the surgery, acceleration of the post-operative recovery and shortening of the period of postoperative hospitalization,and partly on the possibility of surgeon-patient interaction and cooperation during the surgical procedure.(5) The difficulties of performing classical acupuncture manipulations have been solved since the availability of the Han's Acupoint Nerve Stimulator(HANS).(6) It is suggested to use acupuncture in combination with anesthetics,and this technique could be termed acupuncture-assisted anesthesia(AAA) which keeps most of the strong points of AA and puts the patient in a absolutely pain-free status.(7) From the hospital management point of view,there should be new regulations to encourage the anesthesiologists to use AAA whenever it is considered beneficial to the patient.

Abstract:The acupuncture has been practiced in China for more than 3000 years and was spread to Europe and American from the sixteenth to the nineteenth century. The history of acupuncture research was initiated in the eighteenth century and developed rapidly since then. In the past, physicians tried hard to apply acupuncture into clinical practice, while scientists were focused on the possible characteristics of acupoints and meridians. In the modern time, scientists have strived hard to evaluate the real effectiveness of acupuncture and the underlying physiological and biological mechanisms of acupuncture. Reviewing research history from past to present, we are delighted to witness this wonderful development. Accumulated evidences that acupuncture is beneficial in various conditions significantly enhanced our understanding the mechanisms of acupuncture treatment. However, there is still no conclusive evidence in acupuncture clinical studies. The clinical research still needs great improving, while the basic research results need to be appropriately transformed into clinical outcomes. Based on current achievements, we believe that although the challenges and difficulties exist, a more collaborative, innovative, and integrated approach will help us to achieve further progress in future acupuncture research.

Abstract:For at least 2,500 years, acupuncture has been an integral part of traditional Chinese medicine. However, recently as more people in western countries are diagnosed with chronic disease poorly treated with modern medical therapies, many are turning to acupuncture and other forms of alternative medical treatments. Based on the theory of harmonious flowing qi being the basis of good health, acupuncture focuses on restoring qi by manipulation of the complementary and opposing elements of yin and yang. However, in the modern medical community we struggle to with the concept of qi, given a lack of anatomic and histological evidence supporting its existence. However, with the surge in public interest in acupuncture, the scientific community begun heavy investigation of acupuncture's efficacy, as well as the physiologic basis behind it. Thus far, evidence supports the use of acupuncture in post-operative nausea and vomiting, postoperative dental pain, chronic pain conditions such as lower back pain, and possibly also such psychologic conditions as addiction. It is possible that by affecting afferent nerve signaling, acupuncture may influence the release of endogenous opioids to promote pain relief. This effect may be augmented by release of ACTH and cortisol, as well as through down-regulation of signaling through pain fibers. When treating patients who may utilize alternative forms of medicine, it is important that medical practitioners be educated in regards to the basic fundamental beliefs behind acupuncture, as well as the scientific evidence supporting its use and revealing its efficacy. The purpose of this review is to give western trained physicians exposure to history, basic knowledge and its clinical applications of acupuncture to accommodate accelerating interests in acupuncture in modern society.

Abstract:Acupuncture, as a healing art in traditional Chinese medicine, has been widely used to treat various diseases. In the history of acupuncture anesthesia, in the past decades, mechanisms of acupuncture analgesia has been widely investigated, and in recent years, acupuncture protection on organ functions has attracted great interest. This review summarized the research progress on mechanisms of acupuncture for analgesia and its protection against organ function injury in anesthesia, and its perspective of analgesia, immunomodulation, neuroendocrine regulation and multiple organ protection. The current evidence supports that acupuncture analgesia and its organ protection in anesthesia is associated with the integration of neuroendocrine-immune networks in the level of neurotransmitters, cytokines, hormones, neuronal ensembles, lymphocytes, and endocrine cells. Although the mechanisms of acupuncture analgesia and its organ protection are still not completely understood, basic as well as clinic researches on the mechanisms and applications of acupuncture and related techniques are being carried out.
Nixon and Scheel in China: Acupuncture and Anesthesia in West and East Germany in the 1970s and 1980s

Abstract:Background: Acupuncture anesthesia was used instead of intubation anesthesia in the 1970s and 1980s in West Germany and West Berlin. In East Germany acupuncture played no decisive role. Sources: Different articles and papers in journals, in daily press, statements of contemporary witnesses, films, records in archives. Results: As in other Western countries, acupuncture was hugely popular in the Federal Republic of Germany in the 1970s. Chief triggers were the state visits to China of the American President Richard Nixon in 1972 and shortly thereafter of West German Foreign Minister Walter Scheel and his wife Mildred, an x-ray technician. During that period observation of an operation under acupuncture anesthesia was an obligatory element in the agenda of a foreign delegation. Following this showcasing, acupuncture was widely adopted in Western surgery as an alternative to the previously exclusive employment of intubation anesthesia. While the alternative method was soon abandoned in the frontline city of West Berlin, it continued to prevail in other West German cities, e.g. Gießen and Munich. Following the Chinese example, the acupuncture effect was normally enhanced electrically. In accordance with the animosity between the USSR and the People's Republic of China, exchange between the German Democratic Republic and China was very restricted through the late 1980s. This made it easier for East German acupuncture sceptics to reject the procedure and brand it as unscientific. Those who advocated it were in a precarious position. Conclusions: Acupuncture was lauded in the West as ancient savvy destined to complement science-oriented medicine. However, the cultural transfer which accompanied the spread of acupuncture was flawed by misunderstanding and misguidance. Acupuncture anesthesia instead of intubation was not practiced in the GDR. Acceptance of acupuncture in East German medicine failed to make any headway until the last few years of the country's existence.

Abstract:High quality and safe cardiac anesthesia is a prerequisite for success in cardiac care. Cardiac surgery has developed rapidly over recent years in China. Because of language barriers, the current status of cardiac anesthesia in China is not well known to Western countries. To assess practice patterns, volume, workforce, and training requirements of Chinese cardiovascular anesthesiologists, we surveyed 92 major cardiovascular centers in China regarding their anesthesia practice, monitoring techniques, resources, staffing, and work hours. We aim to provide a review of the history, new developments, and a current cross section of cardiac anesthesia practice patterns in China. The goal is to allow Western readers to understand the unique achievements and challenges in Chinese cardiovascular anesthesiology, thus promoting further communications with Chinese cardiovascular anesthesiologists.

[Further Clinical Study of Acupuncture Anesthesia for Supratentorial Craniocerebral Operations].

Abstract:OBJECTIVE:To improve the successful rate of acupuncture anesthesia for supratentorial craniocerebral operations.METHODS:On the basis of previous study of acupuncture anesthesia for craniocerebral operations, 72 cases of front fossa and temporo-parieto-occipital region lesions were operated on by acupuncture anesthesia with new modified acupoints and adjuvants from 1993 to 1998.RESULTS:One hundred percent of successful rate was achieved in all 72 cases by the new methods.CONCLUSION:Acupuncture anesthesia has distinct advantage and should be widely used as one of the effective anesthesia for supratentorial craniocerebral operations.

Abstract:Through literature retrial, the authors aimed at exploring the organ protective effects of acupuncture assisted anesthesia. Acupuncture anesthesia is invented by Chinese medical practitioners according to functions of acupuncture to relieve the pain and regulate phsyological functions of the organs. It can be applied before surgery for depressive prevention. And it can also be adopted during the operation to reduce the dosage of anesthetic drugs. It also has the post-operative effects of relieving pain, preventing nausea and vomiting, protecting the functions of the brain, cardiac muscle, lung, kidney, liver, stomach and intestines. It benefits the micro-circulation and stress rea-ction as well. Further researches should be extencled to other key problems, for example clinical prognsis of acupunctue assited anesthesia.

Abstract:Pain is a kind of complex physiological and psychological symptom, which makes the person debilitated and uncomfortable. Some persistent pain is unbearable for the patients, reducing the quality of life and bringing considerable pressure to the individuals and society. Pain killers seem to be effective in analgesia for patients, but their safety and addiction are crucial issues. From the theory of traditional Chinese medicine (TCM), the blocked meridian is the main cause of pain, and effective acupuncture can play a positive analgesic effect. Acupuncture that can date back thousands of years is one of the ancient medical practices in China. Its safety and effectiveness are respected. Based on its superior safety and inferior side effects, it has been gradually recognized as a therapeutic intervention method for complementary medicine, which is also generally used to treat multiple pain diseases. It is shown by modern medical studies that neurotransmitters are the material basis for the acupuncture effect, and the effect of acupuncture analgesia is related to changes in neurotransmitters. However, the specific mechanism has not been elucidated. This review aims to comprehensively discuss the historical evolution of acupuncture analgesia, clinical research of acupuncture analgesia, comparison of acupuncture and drug therapy, the neurotransmitter mechanism of acupuncture analgesia, the effect of acupuncture manipulation on analgesia, and bibliometric analysis of acupuncture treatment for pain, to explore the superiority and related mechanism of acupuncture analgesia from different aspects, and to provide a more effective treatment for alleviating patients’ pain.

Abstract:It has long been accepted that acupuncture, puncturing and scraping needles at certain points on the body, can have analgesic and anesthetic effects, as well as therapeutic effects in the treatment of various diseases. This therapy, including acupuncture anesthesia, has drawn the attention of many investigators and become a research subject of international interest around the world. Numerous studies have demonstrated that the nervous system, neurotransmitters, endogenous substances and Jingluo (meridians) may respond to needling stimulation and electrical acupuncture. An abundance of information has now accumulated concerning the neurobiological mechanisms of acupuncture, in relation to both neural pathways and neurotransmitters/hormonal factors that mediate autonomic regulation, pain relief and other therapeutics. Early studies demonstrated that the analgesic effects of electroacupuncture (EA) are mediated by opioid peptides in the periaqueductal gray. Recent evidence shows that nitric oxide plays an important role in mediating the cardiovascular responses to EA stimulation through the gracile nucleus-thalamic pathway. Other substances, including serotonin, catecholamines, inorganic chemicals and amino acids such as glutamate and α-aminobutyric acid (GABA), are proposed to mediate certain cardiovascular and analgesic effects of acupuncture, but at present their role is poorly understood. The increased interest in acupuncture health care has led to an ever-growing number of investigators pursuing research in the processes of the sense of needling touch, transduction of needling stimulation signals, stimulation parameters and placebos. In this Review, the evidence and understanding of the neurobiological processes of acupuncture research have been summarized with an emphasis on recent developments of nitric oxide mediating acupuncture signals through the dorsal medulla-thalamic pathways.

Abstract:Background The utilization of acupuncture as a therapeutic intervention for the management of postoperative nausea and vomiting has experienced a notable increase in its popularity, and the purpose of this analysis is to provide a comprehensive understanding of the level of concern within the academic discipline and the main contributors and their partnership, as well as to identify research focal points and trends. Methods A comprehensive search was carried out to identify relevant articles on the topic of acupuncture therapy for PONV in the Web of Science and China National Knowledge Internet. The search spanned from January 1, 2011, to June 6, 2023. The annual publications were count to see the degree of scholarly attention devoted to the discipline and how it has changed over time. A statistical analysis of article distribution across various journals was conducted to serve a rough indicator for assessing the quality of articles. And a bibliometric analysis was conducted using the software CiteSpace to visually analyze various aspects of the literature. Analyze authors, institutions and countries to identify the main contributors and their collaborative relationship; and analyze keywords and references to explore research hotspots and trends. Results This study examined a comprehensive collection of 819 articles focused on acupuncture therapy for PONV, demonstrating a varying upward trend in the quantity of publications. Notably, the most productive author and institution were identified as Zheng Man and Guangzhou University of Traditional Chinese Medicine, respectively. While China had the highest number of publications, the United States held a greater prominence in this specific field. Collaboration among contributors was found to be weak. High-frequency keywords in the publications included "transcutaneous electrical acupoint stimulation," "electroacupuncture," "pain," and so forth. The literature with the highest citation count pertained to " Stimulation of the wrist acupuncture point PC6 for preventing postoperative nausea and vomiting ," while the article with the highest centrality was " Consensus Guidelines for the Management of Postoperative Nausea and Vomiting ." Several large clusters obtained from the references are also included"postoperative pain,""transcutaneous electrical acupoint stimulation". Nothing pertaining to mechanism study was found in the analysis results. Conclusion The utilization of acupuncture for the treatment of postoperative nausea and vomiting has been gaining increasing recognition, although there remains room for improvement in the quality of research conducted in this area. Chinese authors and institutions have emerged as significant contributors to this field, while the United States has demonstrated greater proficiency in fostering collaborative efforts. It is imperative to enhance collaboration among these contributors. The current focal points of acupuncture for PONV encompass pain management, electroacupuncture, auricular acupuncture, and transcutaneous electrical acupoint stimulation. Additionally, TEA and enhanced recovery after surgery have been identified as the forefronts of research in this particular domain. In addition, there is still much room for research in the aspect of mechanism and insurance coverage. This study provides an in-depth perspective on acupuncture for PONV, which offers reference material for clinicians with rational choice of therapeutic scheme, educators with hot topics, and researchers with valuable research directions.

Purpose: To analyze the general information and identify the research hotspots and trends of acupuncture in perioperative medicine in the last 10 years by bibliometric analysis. Methods: We searched the Web of Science Core Collection for publications on acupuncture in perioperative medicine from 2013 to 2023. The articles and reviews were collected with no language restriction. CiteSpace and VOSviewer software were used for bibliometric and visual analysis of relevant literature. Results: A total of 814 bibliographic records were retrieved. Overall, the annual number of publications showed an increasing trend. China and its institutions were in a leading position regarding the publication number. With comparatively more scientific collaboration with China, the USA ranked second. Shanghai University of Traditional Chinese Medicine was the most prolific institution. Ha, In-Hyuk had the most publications, and Han JS and Lee A were the most cited authors. Medicine was the most popular journal and Journal of Clinical Oncology had the highest impact factor. "Acupuncture", "electroacupuncture" and "postoperative pain" were the top three keywords. The most popular topics were postoperative pain, postoperative ileus, and postoperative nausea and vomiting according to the keywords and references. And the clusters of postoperative cognitive dysfunction, anxiety, and breast cancer attracted relatively more attention recently. Conclusion: This study summarized the research status, hotspots, and trends of acupuncture in perioperative medicine in the past decade, which may aid researchers in better understanding this field. The research hotspots primarily focused on postoperative pain management and postoperative gastrointestinal function. The research of acupuncture for postoperative cognitive dysfunction, cancer-related surgery, and psychological states were the main frontiers topics and may be the focus in the future. Abstract:Acupuncture has been used in China for more than 3,000 years. Although the clinical application of acupuncture is very popular, its mechanisms of action are still unclear. Beginning in the middle of the 20th century, a boost of research on acupuncture emerged in China, and I was one of the researchers involved in this episode. With the help of modern neuroscience, the mechanisms of acupuncture for pain management have been unraveled at least partly. This article describes my decision as a young medical graduate to devote my life to research on acupuncture and pain medicine; it has since been my life's journey-one full of challenges and happiness, pitfalls and achievements.

Abstract:Acupuncture and electroacupuncture effects are mediated through a variety of neural and neurochemical mechanisms. Research done in the early 1970s first elucidated the mechanisms for the effect of acupuncture anesthesia. Further experiments showed that this effect can be transferred from one rabbit to another by cerebrospinal fluid (CSF) transfusion. Further investigation explored the role of classic central neurotransmitters in the mediation of acupuncture analgesia, including catecholamines and serotonin. The availability of rat models for electroacupuncture, using the tail flick latency as a bioassay, allowed further experiments to be done to explain the basis for the effects. Differential release of central nervous system (CNS) opioid peptides by electroacupuncture has been noted, with 2-Hz electroacupuncture triggering release of enkephalins and β-endorphins, and 100-Hz stimulation selectively increased the release of dynorphin in the spinal cord. A combination of both frequencies allows synergistic interaction among the three endogenous opioid peptides and a powerful analgesic effect. In addition, multiple acupuncture treatments with the optimal time spacing may result in an accumulation of electroacupuncture effect. A bimodal distribution of analgesic effect can be noted if a large group of rats is given electroacupuncture ("low responders" and "high responders"). The mechanism of low response is at least twofold: a low rate of release of opioid peptides in the CNS, and a high rate of release of CCK-8, which exerts potent anti-opioid effects. A newly discovered anti-opioid peptide is orphanin FQ, which has also been linked to negative feedback control of electroacupuncture stimulation. Further research will serve to elucidate the mechanisms of acupuncture treatment in opiate detoxification, and how different disease entities may respond differently to electroacupuncture.
