Abstract:[Objective]To explore the relationship of lipid metabolism and serum hs-CRP levels with subclinical hypothyroidism patients with type2 diabetes mellitus (T2DM).[Methods] A total of 427 patients with type-2 diabetes mellitus treated in our hospital from January 2015 to May 2017 were selected for the study.Of these,130 cases with subclinical hypothyroidism were treated as the observation group and 297 cases with normal thyroid function as the control group.Levels of serum total cholesterol (TC),triglyceride (TG),low density lipoprotein cholesterol (LDL-C),high density lipoprotein cholesterol (HDL-C),apolipoprotein A (ApoA),apolipoprotein B (ApoB),and hsCRP and the incidence of vascular complications were compared between the two groups.Patients in the observation group were further divided into the 4.5～10.0μU/mL group A1 and the ＞ 10μU/mL group A2,according to different levels of thyrotropin (TSH).The differences of clinical indexes between the two groups was compared.Related factors influencing TSH levels were analyzed by multivariate stepwise regression analysis.The correlation between serum TSH with hs-CRP level and lipid metabolism indexes was analyzed by Pearson correlation analysis.[Results]Serum TC,TG,LDL-C,ApoA,ApoB,hs-CRP levels and the incidence of cardiovascular disease in the observation group were significantly higher than those in the control group,while HDL-C level was significantly lower than that in the control group.The differences were statistically significant between the two groups (P ＜0.05).The levels of TC,TG,LDL-C,HDL C,ApoB and hsCRP between the two different TSH level sub-groups A1 and A2 of the observation group showed statistically significant difference (P ＜0.05).Multiple stepwise regression analysis showed that TC,TG,LDL-C and hs-CRP were independent risk factors for TSH in T2DM patients.Pearson correlation analysis showed that TSH was positively correlated with TC,TG,LDL-C and ApoB (r =0.823,0.675,0.702 and 0.363,respectively,P ＜0.05).Hs-CRP was positively correlated with TC,TG,LDL-C (r =0.385,0.372,0.456,respectively,P ＜0.05) and negatively correlated with HDL-C (r =-0.383,P ＜ 0.05).[Conclusion] Subclinical hypothyroidism patients with type 2 diabetes mellitus is closely related to the levels of blood lipid metabolism and inflammatory stimulation.Thus,it increases the risk of cardiovascular disease in T2DM patients.
