Expanded transpedicular eggshell technique for the treatment of severe idiopathic scoliosis with curves over 100°

Abstract:Objective To investigate the safety and clinical curative effect of expanded transpedicular eggshell technique for adolescent idiopathic scoliosis with curves over 100°. Methods A total of 24 consecutive severe and rigid idiopathic scoliosis cases with curves over 100° (10 males and 14 females) underwent expanded transpedicular eggshell procedures from October 2003 to November 2006. Standing roentgenograph of total spine,MRI and 3D reconstructional CT scan were performed in all the patients preoperatively. The mean scoliotic Cobb's angle was 114°(ranged from 102° to 145°)and kyphotic Cobb's angle was 98°(ranged from 78° to 130°). The operation time,intraoperative blood loss,length of spine shortened intraoperatively,the postoperative data of deformity correction rate and trunk balance were documented. Results All the patients underwent expanded transpedicular eggshell procedures safely,and the average operation time was 4.5 h. The mean length of spine shortened intraoperatively was 32 mm,and the blood loss volume ranged from 1300 to 3950 mL (with an average of 1800 mL). The mean postoperative scoliotic Cobb's angle was 45° (ranged from 35° to 62°),and the mean postoperative kyphotic Cobb's angle was 28°(ranged from 20°to 45°). The correction rate for scoliosis and kyphosis was 61% and 71%,respectively. One case with transient monoplegia lower extremities paresis and 3 cases with nerves root pain occured,and all the patients recovered without special treatment. One patient suffered cerebrospinal fluid leak owning to the dura sac tear. All the patients had an average 28-month follow-up (ranged from 12 to 50 months). Bone fusion was achieved at the osteotomy site,and no loss of correction occurred in any case. No imbalance,instrumental failure and pseudarthrosis was found over the follow-up. Conclusion Expanded transpedicular eggshell technique is a safe and effective method to correct severe idiopathic scoliosis with Cobb's angle over 100°.
