Posterior-only approach hemivertebra resection and transpedicular fixation guided by surgical CT navigation for juvenile congenital scoliosis

Abstract:Objective To evaluate the accurate rate of insertion and the short-term curative effect of posterior-only hemivertebra resection and transpedicular fixation for children with congenital scoliosis guided by surgical CT navigation intraoperatively. Methods Ten patients suffered from single fully segmented hemivertebrae with an average age of 4.5 years (ranged from 3 to 6 years). Among them,hemivertebrae were located at T10 in 1 case,T11 in 2 cases,T12 in 3 cases,L2 in 2 cases,L3 in 1 case,and L4 in 1 case. All pedicle screws insertion and hemivertebral resection were performed guided by surgical CT navigation. Cobb's angles of deformed segment curve were measured respectively pre-and post-operatively. Results All the 48 screws for 10 cases were inserted exactly. No penetration of pedicle into internal walls occurred. In coronal plane,preoperative mean Cobb's angle was 40.5° and postoperative mean Cobb's angle reduced to 8.3° with 78.5% correction. In sagital plane,preoperative mean Cobb's angle was 29.3° and it reduced to 8.3° postoperatively in 6 thoracic hemivertebrae. Preoperative mean Cobb's angle was 1.5° in kyphosis and postoperative mean Cobb's angle recovered to 13.0° at lordosis in 4 lumbar hemivertebra. No implants failure and correction lost were found during the mean follow-up of 5 months. All of compensatory curves were nearly the normal. Conclusion The use of surgical CT navigation during operation can make screws insertion more accurate and safer in the courses of hemivertebrae resection and transpedicular fixation to correct juvenile congenital scoliosis.
