Advantages of robot-assisted laparoscopic surgery in urinary bladder and prostate:an initial clinical experience with 4 cases

Abstract:Objective To assess the feasibility and safety of robot-assisted laparoscopic radical cystectomy(RLRC) with double U-shaped ileal neobladder and robot-assisted laparoscopic radical prostatectomy (RLRP).Methods One patient with bladder carcinoma underwent RLRC with double U-shaped ileal neobladder and three patients with prostate carcinoma underwent RLRP using the da Vinci robotic surgical system during March 2010 to October 2010.All patients had histologically-confirmed adenocarcinoma of prostate or bladder biopsy and a negative bone scan.Peri-operative and early surgical outcome data were collected prospectively and functional recovery was assessed by an independent oncology nurse clinician.Results The operative time of three patients undergoing RLRP was 200 - 270 min,including 45 min of the preoperative set up time of the robot.The perioperative blood loss was 300-800 mL,and the blood transfusion volume was 0~400 mL.In the prostate carcinoma the left and right drainage tubes were removed on the 5th and 6th postoperative day,respectively.The operation time for the patient bladder carcinoma was 330 min,with the time for total bladder resection being 165 min and the time for orthotopic neobladder being 120 min.The intraoperative blood loss was 800 mL and the blood transfusion volume being 300 mL.The ureteral catheters were removed on the 10th postoperative day,and the urethral catheter was removed on the 21th postoperative day.The postoperative process was uneventful and the automatic micturition was restored.Conclusions The da-Vinci robotic surgical system has the advantage of miniinvasiveness, and it can be used for patients with high risk factors.The system has a potential to be used widely, but the risks should be noted.
