Pre-operative celecoxib administration in orthopaedics surgery

Abstract:Objective To evaluate the effeets of pre-operative celecoxib analgesia in patients undergoing orthopaedic surgery.Methods Eighty patients scheduled for elective lower limbs surgery were randomly given placebo or celecoxib 200mg one hour before incision and 12,24 h after the surgery. The operation was performed under combined spinal and extradural anesthesia. After the operation, pain intensity was measured using visual-analog scale (VAS),and analgesic requirements, side effects and levels of PGE_2 in blood serum were compared. Results There were no marked differences in the VAS pain scores at the beginning of PCA,1,12,24,48h after the operation between two groups.Compared with placebo group,the patients of celecoxib group had significantly lower VAS pain scores at 2, 4, 8h after the operation(P<0.01).A significantly lower total consumption of tramadol, as compared with placebo group, was observed at 48h after operation in patients of celecoxib group (P<0.05). It was also found that the side effects were significantly lower in celecoxib group than those in placebo group. The serum concentration of PGE_2 did not differ significantly between two groups at the end of operation. However, in patients of celecoxib group, a significantly lower serum concentration of PGE_2, as compared with that in placebo group, was observed at 24h after operation(P<0.01). Conclusion Pre-operative celecoxib in patients undorgoing orthopaedic surgery can reduce postoperative opioid consumption and side effects and attenuate inflammatory response.
