Abstract:Background Because the nonunion rate of conservative treatment was up to 1 5%, midshaft clavicle fractures with wedge-shaped fragments had been mainly treated with operation in recent years.Plate fixation was still one of the mainstream internal fixation methods,although a few scholars advocated various intramedullary fixations.For conventional plate and screw fixation techniques,one of the principles was to fix the free wedge-shaped fragments with lag screws as far as possible.We had ever improved the conventional techniques by intramedullary K-wire assistance in reduction,binding fragments with suture,and bridging plate fixation,which obtained good effects. But we found that the open reduction and bridging plate fixation increased the operation trauma because of the long incisions.We had made further improvement on this basis.From April 2011 to April 2014,26 patients of midshaft clavicular fracture with wedge-shaped fragments were treated by close reduction and MIPPO (minimal invasive percutaneous plate osteosynthesis)technique with three incisions.We performed this retrospective study to explore the operation methods and treatment effects of the midshaft clavicular fractures with wedge-shaped fragments using MIPPO technique.A group of 29 patients treated by conventional open reduction and internal fixation techniques,from March 2007 to November 2011,were also performed study as control group with complete follow-up data.Methods (1 )General information:All the cases were selected from the patients who were treated in our department from March 2007 to April 2014.Among them,26 patients were treated with three-incision MIPPO bridging technique,and 29 patients were treated with conventional open <br> reduction,lag screw fixation for fragments and neutralization or compression plate fixation.The mechanisms of injury were mainly fall damage.The two groups respectively had 2 cases of falling injury from height and 1 case with multiple fractures.According to Robinson classification,the fracture types were classified as follows:II B1 type was defined as the midshaft clavicular fracture with displacement and 1 wedged-shape fragment in the fracture site;IIB2 type was defined as the fracture with 2 or more free fragments in the facture site.The gender,age,dominant side,time from injury to operation and fracture type can be seen in table 1.(2)Operation methods:MIPPO group:Patients were on beach chair position under general anesthesia which had good muscular relaxation. The posterolateral side of the affected shoulder,upper arm and forearm were disinfected to guarantee that the surgeon can adequately move the shoulder during operation.The upward tilt tip of the proximal clavicular main fragment was obviously be palpated.A 2 cm incision was applied to expose the tip of the proximal fragments without periosteal dissection.A double-tip intramedullary K-wire with a diameter of 2 mm or 2.5 mm was inserted retrogradely into the medullary cavity of the proximal main fragment till the end of the K-wire was at the same level to the tip of the proximal fragment.The proximal tip of the K-wire pierced out of the skin in front of the proximal clavicle.The affected upper arm of the patient was elevated upward and tracted laterally by an assistant doctor in a way similar to the manipulative reduction to reduce the main fragment.At this point the operator could feel that the posterosuperior cortexes of both the distal and proximal main fragments were connected with palpation. Once the posterosuperior clavicular cortexes were matched, the intramedullary K-wire was inserted into the distal medullary cavity from the proximal medullary cavity till it pierced out of the clavicular posterior edge and was located at the posterior corner of acromion. Intraoperative fluoroscopy was conducted to confirm the reduction of the free fragments and the connection of the main fragments.The 10-hole anatomical locking plate (GE medical,US)was inserted percutaneously along the surface of the clavicle towards the proximal end through the middle incision.The proximal end of the plate was touched from the surface of the skin.A 2 cm incision was made on top of that to expose the proximal end of the plate.The plate was completely inserted to the proximal end till its distal end was located at the incision above the fracture site.Then the plate was moved towards the distal side along the surface of the clavicle until the fracture site was located right in the middle of the plate.The plate was touched at the distal end and then exposed with a 2 cm incision.After the plate was confirmed to be placed at the appropriate position,one locking screw was respectively driven in at both ends.Then other locking screws were inserted from lateral to middle at each of the distal and proximal ends.If the screw was obstructed,then the K-wire was pulled out gradually and the remaining screws were driven in.Three bicortical screws were inserted at each of the distal and proximal ends for fixation.The position of internal fixation was confirmed satisfactory under fluoroscopy.The incision was closed without the drainage tube.Conventional ORIF group:Cervical plexus block combined with brachial plexus block or general anesthesia was applied.The patient position was the same as that of the MIPPO group.The incision of clavicle surface was adopted to first expose the fracture site.The lag screw fixation was used between the larger free fragments and the main fragments to convert the clavicular shaft fracture with free fragments into simple fracture and then reduce it using Kocher clamp.The straight plate with 7-9 holes was fixed on the surface of clavicle after reduction and 3 screws were inserted respectively on each side.(3)Post-operative rehabilitation and follow-ups:The passive activities of the shoulder joint,mainly including external rotation and abduction,were initiated at the second day after surgery.The abduction angle is not more than 90°.Three sets of exercises respectively distributed in the morning,afternoon and evening were carried out every day with 10 times in each set while the affected limb was put in a sling for protection during the rest of the time.After 4 weeks,the sling was removed and the shoulder was allowed for simple daily life use, but the affected limb was limited to lift heavy obj ects and hyperabduct.1 2 weeks later,the affected limb was restored for complete work.The fracture healing of heavy manual workers should be confirmed by fluoroscopy. The X-ray examinations were <br> performed in the 4th,8th,12th,and 16th week.During follow-ups Constant Score System was applied to assess their shoulder functions and the complications were recorded.(4 ) Statistical methods:SPSS 19.0 software was used.The measurement data were indicated as means ± standard deviations.χ2 test and Fisher′s exact probability were performed in the comparison of the measurement data between two groups.If the measurement data of the two groups met the normal distribution,then the Independent-Sample t test was used for the comparison;otherwise, the Wilcoxon rank sum test was adopted;a p value less than 0.05 was considered as significant difference. Results After an average of 1 5 months (6~3 1 months)follow-ups,all the fractures in the MIPPO group obtained bony union.One case of nonunion occurred in the conventional ORIF group with plate breakage and loss of reduction.The patient underwent reoperation.The two groups had no significant difference in the failure rate.The operation time of the MIPPO group was 73.5 minutes in average with the mean intraoperative blood loss of 3 1 .9 ml.The MIPPO group was obviously better than the conventional ORIF group in the respects of operation time and blood loss.The time of fracture healing was 13.7 weeks in average and had no significant difference when compared to the conventional ORIF group.The Constant scores had no significant difference between two the groups either. Some patients in the two groups accepted second operation to remove implants although there was no complication.Two lean patients of the MIPPO group had obvious skin uplift and strongly urged the second surgeries of implants removal.Conclusions This study suggested that for the midshaft clavicle fractures with free wedged-shape fragments,the satisfactory functional reduction of free fragments can be obtained completely through close reduction.Compared to the conventional rigid internal fixation technique with lag screws for wedge-shaped fragments,it is unnecessary to pursuit anatomical reduction.MIPPO technique has the advantages of simple operation,small trauma and satisfactory treatment effects,reduces the operation time and increases the healing rate.

Abstract:Background The humeral shaft fracture is a common type of fractures in adults,accounting for about 2％-3％of all fractures in the body.The humeral shaft fractures with severe displacement and combined injury need surgical treatments.In recent decades,with the development of internal fixation technology and fixation material design,more choices have been made for surgical methods,and controversy has also appeared.At present,the two most commonly used surgical methods are closed reduction intramedullary nail fixation and open reduction plate internal fixation.Both methods have their own advantages and disadvantages.The advantage of antegrade intramedullary internal fixation is minimal invasive,thus the blood supply around the periosteum and the fracture is well protected.and the fracture end is less stimulated;the disadvantage is that the rotator cuff would be cut when the nail is inserted,and patients with limited shoulder activity and the shoulder pain were reported.The advantages of the plate fixation are that the anatomical reduction can be obtained under direct vision,and the fracture can be firmly and stably fixed;the disadvantages are that the trauma is large with more blood supply damage,and there is a risk of radial nerve injury.At present,the advantages and disadvantages of the two surgical methods are always debated.The previous RCT studies have relatively low level of evidence,and several meta-analytical studies have different conclusions,thus there is no clear conclusion now.Methods 1.General information.Inclusion criteria:Patients with humeral shaft fractures who underwent internal fixation surgery in the Department of Trauma and Orthopaedics of Peking University People's Hospital from August 2013 to October 2017.Exclusion criteria:Open fracture,pathological fracture,old fracture,radial nerve injury,severe multiple injury,abnormal function of shoulder and elbow joint of the affected limb before the injury.A total of 56 patients met the standard,including 35 males and 21 females,aged 18-70 years.with an average age of 42 years.There were 11 cases of traffic accidents,36 cases of falls,and 9 cases of falls from height.The fractures were classified by the AO clasification:19 cases were type A,24 cases were type B,and 13 cases were type C.Locking compression plate fixations were performed in 31 cases(AO company or Biomed company),and antegrade magnetic navigated interlocking intramedullary nails were in 25 cases(Smith and Nephew Company).2.Surgical methods:All patients underwent general anesthesia with tracheal intubation combined with brachial plexus block analgesia.(1)antegrade magnetic navigated interlocking intramedullary nail:The patient took the beach chair position,a vertical straight incision was made on the lateral side of the shoulder for about 4 to 5 cm,the guide needle was inserted at 1 cm medial to the greater tuberosity of the humerus and 0.5 em behind the biceps groove,parallel to the axial direction of the medullary cavity.Reduction was achieved under X-ray,then the intramedullary nail was inserted,the locking screws were guided magnetically.(2)Locking compression plate:The anterior lateral incision was made in the proximal and middle fractures,and the posterior incision was taken in the distal fracture to find and protect the radial nerve;the fraeture was cleared and aligned,and then the Kirschner wires were temporarily fixed.The prebent AO or Biomet locking compression plate(LCP)was placed,and then the cortical bone screws and the locking screws were inserted to fix the fracture depend on the situation.3.Postoperative treatment:The postoperative forearm sling suspension protection lasted 3 weeks.From the second day after surgery,the patients could perform passive shoulder and elbow joint function exercises within the allowable range of pain.Active shoulder and elbow joint exercises were performed 3 weeks after surgery.The affected limb should avoid carrying heavy objects within 2 months.The X-ray was reviewed monthly for 4 months after surgery to understand the fracture healing.X-ray examination was performed once every 3 months after the fourth month.and once every 6 months after 1 year.The operation time,postoperative complications,fracture healing time,and range of motion of the shoulder and elbow joints were recorded.The shoulder and elbow function scores were evaluated 1 year after surgery.The shoulder joints were scored by the University of California,Los Angeles(UCLA)based on pain(10 points),activity(10 points),the range of forward flexion(5 points),forward flexion strength(5 points)and patient satisfaction(5 points).The scorewas divided into excellent(34-35 points),good(29-33 points),and poor(＜29 points).The elbow joint function used the Mayo score,which was based on pain(45 points),range of joint activity(20 points),degree of stability(10 points),and daily function(25 points).It was divided into excellent(90 points or more),Good(89-75 points),fair(60-74 points),poor(＜60 points).Results All the 56 patients were followed up for 10 to 26 months,with an average of(18.7±3.6)months.The operation time was about 45-110 minutes and the average operation time was(74±14)minutes.Two cases of superficial wound infection occurred,and finally recovered well after debridement and dressing change.One case of intraoperative iatrogenic radial nerve injury was treated with nerve electrical stimulation combined with neurotrophic drugs.The wrist and thumb extension functions were completely restored 6 weeks after operation.The fractures reached clinical union at 8-24 weeks,with an average of(13±4)weeks.There was no delayed union or nonunion.The union rate reached 100％.The shoulder abduction was 95°-170°,with an average of(151±11)°;and forward flexion was 100°-175°,with an average of(153±12)°.The elbow joint extension was 0°-10°,with an average of(5±2)°;the flexion was 122°-145°,with an average of(131±5)°.In the follow-up of all patients.the shoulder function was evaluated using the UCLA score:53 cases were excellent and 3 cases were good.According to the Mayo score,the elbow joint functions were excellent in 55 cases and was good in 1 case.Comparing the intramedullary nail group with the LCP group,there was no significant difference in gender ratio,age,operation time,length of hospital stays,and fracture types between the two groups,so the two groups were comparable.There was no infection or nerve injury in the intramedullary nail group.There were 2 cases of superficial wound infection and 1 ease of nerve injury in the LCP group,but there was no significant difference between the groups.The aeromion impingement ratio of intramedullary nail group was significantly higher than that of LCP group.The range of shoulder flexion and abduction activity was significantly lower than that of LCP group.There was no significant difference in the other prognosis indicators.Conclusions Locking compression plate fixation group had a higher incidence of wound infection and a higher incidence of radial nerve injury,but there was no significant difference compared with the intramedullary nail fixation group.The incidence of postoperative acromion impact ratio was higher in patients with intramedullary nail,and the mobility of the shoulder joint was also significantly decreased.Therefore,this study found that the use of intramedullary nails and LCP had their own advantages and disadvantages,the overall safety and effectiveness of both were high.All patients could obtain excellent shoulder and elbow joint function.Therefore,the surgeon should take the strength and avoid weakness when selecting the fixation method,for example the surgeon should minimize the damage to the rotator cuff when using the intramedullary nail and improve the surgical technique to properly handle the intramedullary needle cap to avoid aeromion impact.Large-scale prospective randomized controlled trials are still needed to validate the result.
