Abstract:Objective To explore the long-term effects of risk-based stratification treatment of children with medulloblastoma and elucidate the relationship between risk factors,treatment effects and prognosis.Methods Patients admitted from January 2010 to June 2015 with primary medulloblastoma confirmed by postoperative histopathology were stratified into low-risk or high-risk group.They were stratified on the basis of risk factors,i.e.age,Chang et al's staging system (no metastasis M0,with metastasis M +),complete/near total tumor resection and postoperative residue tumor (tumor residue ≥ 1.5 cm).pathological type (classical medulloblastoma CMB,desmoplastic medulloblastoma DMB,medulloblastoma with extensive nodularity MBEN,large cell medulloblastoma LCMB and anaplatic medulloblastoma AMB).The low-risk group (＞3 years,M0,complete/near total tumor resection,CMB,DMB/MBEN) received postoperative 24～30 Gy craniospinal irradiation followed by a boost of 54～56 Gy to tumor bed in posterior fossa and then 8 cycles of sequential maintenance chemotherapy.In the high-risk group (≤3 years,M +,tumor residue ≥1.5 em,AMB/LCMB),children aged ＞3 years received two-cycles of postoperative induced chemotherapy,followed by 36 Gy craniospinal irradiation and then a boost of 54～56Gy to tumor bed in posterior fossa,followed by 4 cycles of maintenance chemotherapy after irradiation.Children aged ≤3 years received 3 cycles of postoperative induced chemotherapy followed by 4 cycles of maintenance chemotherapy.Results Among 115 children with histologically confirmed primary medulloblastoma,there were 77 boys and 38 girls with a male-to-female ratio of 2.02 ∶ 1.Eight cases were aged under 3 years and 107 cases older than 3.Therefore the median age was 6.75 years (15 months the youngest and 15 years the oldest).The lowrisk group had 60 children (accounting for 52.2％ of total) while 55 children (accounting for 47.8％) were put in high-risk group.The median follow-up period was 25 (6-60) months.The 3-5 year overall survival (OS) was 85.5％ in low-risk group versus 67.7％ in high-risk group.And significant difference existed between two groups (P＜0.05).In addition,3-5 year disease-free survival (DFS) was 76.8％ in low-risk group while 47.3％ in high-risk group.Two groups showed significant intergroup difference (P＜0.05).Among 31 recurrent cases (27.0％),twoof them were aged ≤3 years.The median time of postoperative recurrence was 12 months (4 weeks to 48 months).Among them,2 obtained complete remission and 13 patients showed partial remission.Another 16 patients died,including 1 patient aged ≤ 3 years.One case suffered secondary intracranial high-grade glioma.Conclusions For diagnosis and therapy stratified on the basis of risk-factors for children with medulloblastoma,survival ratio and quality of life may be improved.The long-term survival of such children has correlation with risk factors.
